Abstract

Pada mayoritas kasus anak - anak membutuhkan anestesi umum untuk prosedur mata dan operasi. Tehnik anestesi yaitu mencegah dan menjaga terjadinya peningkatan tekanan intra okuler bola mata, yaitu dengan smoth induction dan smoth intubation, alternatif lain adalah dengan menggunakan Laringeal Mask Airway (LMA) untuk prosedur yang sederhana. Oculocardiac refleks dapat terangsang selama operasi mata dan risiko menyebabkan efek berbahaya bradycardi, yang dapat dicegah dengan premedikasi dengan agen antikolinergik. Mual pasca operasi dan muntah sering terjadi setelah operasi mata pada anak-anak dan mungkin pemberian profilaksis antiemetik. Pada kasus operasi tertentu mungkin sangat nyeri seperti evisceration, enucleation dan operasi vitroretinal pertimbangan pemberian analgetik dengan opioid seperti fentanyl dapat dipertimbangkan.

Abstract

Background: Post anaesthesia shivering is a autonom mechanism compensation against fall body temperature to increased heat body production. Hypothermia during operation is primary cause post anaesthetic shivering. Shivering can increased cardiac activity, intraocular pressure, intracranial pressure and create postoperative discomfort condition. Objective: The aim of this study is compare effectivity of pethidin 0,5 mg/kg, tramadol 1 mg/kg and ketamin 0,5 mg/kg as a prophylactic drug of shivering after laparascopy surgery with general anesthesia. Methods: This study is double blind randomized controlled trial. The subject were 193 patient, male or female with age 18-60, physical status of ASA I and II, who undergo of laparascopy surgery with general anesthesia. Subject were divided into three group. Group P with pethidin 0,5 mg/kg, group T with tramadol 1 mg/kg, group K with ketamin 0,5 mg/kg, drug were diluted with 10 mL normal saline. Prophylactic drugs given when superficial skin closure. Results: Is done measurement systolic blood pressure, diastolic blood pressure, heart rate and body temperature. Measurement done before procedure anaesthesia, during anesthesia and at the recovery room. Conclusion : Shivering assessment began after terminating inhalation anaesthesia agen.

Abstract

Seperti pada dewasa, teknik regional anestesi pada pediatrik kini makin popular digunakan oleh ahli anestesikarena keuntungannya. Namun demikian selalu ada risiko dan kemungkinan timbulnya komplikasi dari setiap tindakan yang dilakukan, termasuk tindakan anestesi regional pada pediatrik. Insidensi komplikasi anestesi regional pada pediatrik tidak banyak, dan kalaupun terjadi komplikasi adalah minor. Komplikasi bisa diakibatkan dari identifikasi ruang saraf, alat, obat, teknis tindakan anestesi regionalnya dan komplikasi lainnya.Walaupun tidak banyak kejadian komplikasi regional anestesi yang dilaporkan pada pediatrik, dan bukanlah komplikasi yang fatal, teknik regional anestesi pada pediatrik harus dilakukan dengan lebih hatihati, pertimbangan risiko dan keuntungannya untuk menghindari terjadinya komplikasi, terlebih karena kebanyakan komplikasi dapat dihindari dengan mempelajari teknik yang benar, menggunakan peralatan yang sesuai, dan sangat menerapkan prinsip keamanan pada pasien dengan baik.

Abstract

Postoperative acute pain is an unavoidable problem in postoperative conditions. Effective relief of pain is of the most importance to treating patients undergoing surgery. Effective pain management has significant physiological benefits. Manyways for pain management, althought opioid is general pain management and Regional block is a popular techniques in paediatric pain management recently, wether for surgery or need of adequate analgesic condition.

Abstract

Background. PONV is one of the side effects that often occur after general anesthesia, occurs in the first 24 hours post surgery and occurs in as many as 30 - 70 % of hospitalized patients. PONV greatly avoided by patients and anesthesiologists. Objective. To compare the efficacy of ondansetron 4mg IV-dexamethasone 8mg IV combination with granisetron 3mg IV for preventing of postoperative nausea and vomiting (PONV) in high risk patient in gastrointestinal tract operation. Methods. The research design was randomized control trial (RCT) study with the total of 80 patients, aged 18-60 years old, physical status ASA I and II who underdue gastrointestinal tract operation recruited. Data analysis is done by determined PONV score, side effect, rescue antiemetic needed, incidence and severity of PONV, fentanyl and neostigmine added, fluid status and duration of operation. Results. Incidence PONV of ondansetron-dexamethasone group is 12,5%. Incidence PONV of granisetron group is 10%. There was no significant difference of incidence PONV between these groups. In this study there was no side effect between these groups. Conclusions. Efficacy of ondansetron 4mg IV-dexamethasone 8mg IV combination with granisetron 3mg IV for preventing of postoperative nausea and vomiting (PONV) in high risk patient in gastrointestinal tract operation was no significant difference.

Abstract

A 12-day-old male neonate diagnosed with atresia oesophagus type C was planned to undergo decompression gastrostomy and feeding jejunostomi surgery. The patient’s mother complained that her child always vomit when given breast milk and said the child’s stomach was bloated. Patient was born from G1P1A0 mother with a history of aterm pregnancy, history of routine ANC, birth through normal delivery in midwife with clear amniotic fluid and APGAR SCORE 7/9. Birth weight 2382 grams and body weight is now 2282 grams. Anesthesia management performed under general anesthesia with conscious intubation technique ET No. 2.5 uncuff facilitate with 5mg ketamine. Maintenance anesthesia with O2, air, and sevoflurane. hemodynamics are stable during surgery. The operation lasts for 45 minutes. After the surgery is completed the patient is again treated at the NICU.

Abstract

Acute pain can be found in patient undergoing surgery. Postoperative pain scale is variable from mild to moderate-severe. There are difficulties in assessing pain scale in pediatric, not like adult, pediatric patient can’t say what they feel. Management of postoperative pain, especially in moderate-severe scale needs a multimodal approach to overcome it. The aim of multimodal strategy is to optimize analgetic effect and reduce the adverse effect of the drugs.

Abstract

Sindromaa Mowat-Wilson adalah penyakit genetik yang disebabkan oleh mutasi heterozigot atau delesi dari gen ZEB2 dan ditandai oleh ciri-ciri klinis yang khas. Malformasi kongenital yang khas dari sindroma ini memerlukan prosedur diagnostik dan pembedahan dini yang memerlukan anestesi umum, tetapi sedikit informasi tentang manajemen anestesi pada pasien dengan sindroma tersebut. Sindromaa Mowat-Wilson melibatkan kelainan sistem ganda. Kelainan yang paling sering ditemukan adalah deformitas wajah, keterbelakangan mental, dan penyakit Hisprung. Kami melaporkan manajemen anestesi pada kasussindroma Mowat-Wilson dengan jalan nafas yang sulit yang menjalani prosedur biopsi rectum et causa penyakit hisprung.

Abstract

Pain is a complex phenomenon that occurs due to the transmission of nociceptive stimuli from the peripheral nervous system through the spinal cord to the cerebral cortex. Given the complexity of the pain mechanism, effective treatment of pain requires the use of multimodal therapies that target multiple sites along the pain pathways. Opioid receptors decrease the release of excitatory neurotransmitters from terminals carrying nociceptive stimuli.The use of opioids in the management of acute pain in children must be considered when the intensity and the duration of the pain does not respond to the other classes of analgesic drugs. The increase in the knowledge of physiologic and pharmacologic of these medications has reduced the fear of their application in the pediatric field and helped in the recognition and management of side effects, where excessive sedation and severe respiratory depression are the most serious adverse effects of opioids.

Abstract

Latar Belakang: Neurotransmiter GABA dan NMDA berperan penting pada sel otak yang masih mengalami perkembangan saraf. Secara umum, agen anestesi bekerja pada kedua neurotransmiter tersebut. Midazolam menghambat neurotransmisi dengan cara potensiasi reseptor GABA. Paparan pada reseptor GABA mengganggu perkembangan sel saraf dan memicu terjadinya proses apoptosis yang ditandai dengan peningkatan gen pro apotosis seperti Bax, Bak dan Bim.Metode: Penelitian ini merupakan penelitian experimental control trial, sampel dibagi menjadi 3 kelompok. 27 tikus dibagi menjadi kontrol NaCl 0,9% (n= 9), midazolam 10 mg/kgbb (n= 9), midazolam 40 mg/kgbb (n= 9) disuntikkan secara intraperitoneal. Kemudian otak tikus di setiap kelompok diambil dengan potong beku. Ekspresi gen proapoptosis mRNA Bax kemudian diukur dengan PCR.Hasil: Pemberian midazolam dengan dosis 10 mg/kgbb dan midazolam dosis 40 mg/kgbb terjadi peningkatan ekspresi gen-Bax dan bernilai signifikan < 0,05 jika dibandingkan kelompok kontrol.Kesimpulan : Terdapat peningkatan signifikan ekspresi gen bax pada pemberian midazolam dosis 10 mg/kgbb dan 40 mg/kgbb pada otak tikus wistar yang masih mengalami perkembangan saraf.
