Abstract

SD Negeri 01 Ciambar, Sukabumi, West Java, is one of the schools that has quite a large number of students with diverse social and economic backgrounds. Based on initial observations, many students lack awareness of the importance of maintaining dental and oral health. In addition, teachers also need to improve their understanding in guiding students to be able to apply good habits in maintaining dental and oral hygiene. The method used in this activity consists of several stages as follows: observation and identification of problems with initial observations carried out to determine the condition of students' dental and oral health and their level of understanding of dental care and interviews with teachers were conducted to determine the extent of their role in educating students about dental and oral health. Overall, this program succeeded in increasing the awareness and behavior of students and teachers of SDN 01 Ciambar in maintaining dental and oral health. It is hoped that with ongoing education, these good habits can become part of a healthy lifestyle culture in the SDN 01 Ciambar school environment. This program proves that participatory-based education and training supported by interesting methods can increase the awareness and habits of students and teachers in maintaining dental and oral health. The sustainability of the program can be improved with the support of school facilities, routine check-ups by medical personnel, and parental involvement in building a healthy lifestyle culture from an early age.

Abstract

Background: Caries is the most common oral disease. Streptococcus mutans is the main microorganism in caries etiology due to its ability to form biofilm. Biofilm can be eliminated using mouthwash. Elephant ginger (Zingiber officinale var. officinale) can be developed as a herbal mouthwash because it is able to inhibit S. mutans. This study aimed to analyze pH and antibiofilm effect in vitro of elephant ginger mouthwash formulations on S. mutans. Method: Elephant ginger was macerated with 96% ethanol, then formulated into 2.5%, 5%, 10%, and 15% mouthwash formulations, underwent pH measurement for 28 days, and their antibiofilm effect on S. mutans were measured using microplate reader for 1h (therapeutic assay) and 24h (preventive assay). Result: All formulations showed pH values ranging from 6.42-6.87, changing significantly within 28 days. All formulations were able to reduce S. mutans biofilm adherence for 1h better than commercialized herbal mouthwash and similar to 0.1% CHX. Furthermore, 5% and 10% mouthwash formulations showed similar effectivity to 0.1% CHX and commercialized herbal mouthwash in inhibiting S. mutans biofilm formation for 24h, while 15% mouthwash formulation was more effective than 0.1% CHX and similar to commercialized herbal mouthwash, and 2.5% mouthwash formulation was less effective than 0.1% CHX and commercialized herbal mouthwash. Conclusion: Elephant ginger mouthwash formulations showed pH values that were likely to decrease for 28 days and had the potential as an herbal mouthwash due to its antibiofilm effect on S. mutans.

Abstract

Perawatan saluran akar J-shaped pada premolar pertama mandibula: laporan kasus ABSTRAKPendahuluan: Klinisi harus waspada dengan adanya kelainan pada anatomi internal saluran akar. Pemilihan instrumentasi yang tepat diperlukan untuk preparasi saluran akar melengkung. Kesalahan iatrogenik pada saluran akar melengkung dapat terjadi seperti ledge, separasi instrumen, penyumbatan saluran akar, perforasi, dan transportasi apikal tear-drop. Laporan kasus ini bertujuan untuk menyajikan perawatan endodontik dengan bentuk anatomi saluran akar berbentuk J-shaped. Laporan kasus: Pasien perempuan berusia 52 tahun memiliki keluhan tidak nyaman saat makan. Gigi #34 didiagnosis sebagai nekrosis pulpa. Radiografi awal mengungkapkan lengkungan akar (J-shaped). Preparasi saluran akar dimulai dengan file #8 yang telah dilengkungkan sebagai file awal dan dilanjutkan dengan file rotari gold NiTi (M3-Pro Gold, UDG, Cina) hingga #25/.06 mengikuti kelengkungan saluran akar. Saluran akar J-Shaped dalam klasifikasi Schneider menunjukkan severly curve (29°). Manajemennya dimulai dengan akses lurus ke saluran akar, preparasi dengan k-file kecil dan precurved #08, #10, #15 dengan gerakan push and pull, dilanjutkan dengan file rotary berurutan, bahan kelasi (Ethylenediaminetetraacetic acid/EDTA) digunakan, dan irigasi berulang dengan Natrium hipoklorit (NaOCl). Apikal patensi dilakukan selama cleaning dan shaping untuk menjaga panjang kerja dan mencegah penyumbatan saluran akar. Simpulan: Variasi anatomi saluran akar, teknik instrumentasi, dan obturasi saluran akar penting dipahami guna mencapai keberhasilan perawatan saluran akar pada saluran akar J-Shaped. Kata kunciJ-shaped canal, perawatan saluran akar, glide path, gold nickel-titanium Root canal treatment of a J-shaped canal mandibular first premolar: a case report ABSTRACTIntroduction: Clinicians should be aware of internal anatomical variations. Appropriate selection of instrumentation is essential for preparing curved canals. Iatrogenic errors in curved canals may occur, such as ledge formation, instrument separation, canal blockage, perforation, and tear-shaped apical transportation. The aim of this case report is to present the challenging endodontic management of a mandibular first premolar with a J-shaped canal. Case report: A 52 year-old female patient presented with discomfort while eating. Tooth #34 was diagnosed with pulp necrosis. A pre-operative radiograph revealed root curvature consistent with a J-shaped configuration. Root canal preparation was initiated with precurved file #8 (Dentsply, Switzerland) as initial file, followed by gold NiTi rotary instrumentation (M3-Pro Gold, UDG, China) up to #25/.06, respecting the canal curvature. According to Schneider’s classification, the J-shaped canal showed a severe curvature (29°). Management began with straight access into the canal then prepared with small and precurved k-file #08, #10, #15 with push and pull motion. This was followed by rotary file sequencing. A chelating agent (EDTA) and copious irrigation with sodium hypochlorite were employed. Apical patency was maintained throughout cleaning and shaping to preserve the working length and prevent canal blockage. Conclusion: Knowledge of root canal anatomy, together with appropriate instrumentation materials and techniques, is crucial to achieving successful root canal treatment.  KeywordsJ-shaped canal, endodontic treatment, glide path, gold nickel-titanium

Abstract

Vital pulp therapy (VPT) is a crucial dental intervention designed to maintain the vitality of the dental pulp, particularly in carious lesions or trauma. The evolution of biomaterials in VPT has significantly affected dental practices by providing less invasive alternatives that safeguard tooth vitality and functionality. Continued exploration of the properties and effects of these materials is crucial for enhancing treatment strategies and optimizing dental care outcomes. Bibliometric analysis allowed the assessment of trends in the biomaterials used for VPT. Data mining was performed using the Scopus database and Boolean expressions. Data extraction and analysis were conducted using VOSviewer version 1.6.20., VOSviewer thesaurus, and Microsoft Excel. A total of 856 documents were identified, and the United States had the largest number of documents and citations (110 documents, 6,938 citations). The top three sources were the Journal of Endodontics, International Journal of Endodontics, and Dental Materials Journal. Mineral trioxide aggregate (MTA) is still the dominant biomaterial used in VPT to this day. MTA can accelerate dentinogenesis in VPT. The prominence of “MTA” and “VPT” as keywords highlights research on prevalence, causes, prevention, and evaluation, emphasizing the need to investigate biomaterial-driven healing, regeneration, stem cell activity, and gene expression.

Abstract

Introduction: Most commonly used vital pulp therapy material is calcium hydroxide (Ca(OH)2) but it has several disadvantages. Previous studies found that nano-hydroxyapatite might induce reparative dentin with no tunnel defect and adding collagen can improve hydroxyapatite mechanical properties. The collagen can also increase pulp cell proliferation and differentiation. The addition of Epigallocatechin-3-Gallate (EGCG) to collagen gel can be beneficial in reducing pulp inflammation. Objective: The purpose of this study is to synthesize and analyze the physical characteristics of the nano-Hydroxyapatite-collagen-Epigallocatechin-3-Gallate hydrogel composite. Methods: Nano-hydroxyapatite from chicken egg shells, 0,2 g/mL collagen type I, and 10 mmol/L EGCG each dissolved in 2 mL deionized water with various ratio of nano-hydroxyapatite:collagen, 40:60 (group 1), 50:50 (group 2), and 60:40 (group 3). Each solution was mixed together then 2% HPMC was added into the mixture. In present study, we observed hydrogel composite pH value, gelation time, and injectability analysis. Results: The pH value in 1 hour stirring for each group was 8,05 ± 0,05, 8,43 ± 0,02, and 8,71 ± 0,04. All samples had 1 hour gelation time. Injectability for each group was 90,67%, 91,93%, and 95,03%. Conclusions: The nanohydroxyapatite, collagen and EGCG hydrogel composite has a potential physical characterization to be used as a vital pulp therapy material based on its pH value and injectable ability but further study should be considered in determining ideal gelation time. ABSTRAK Pendahuluan: Bahan perawatan pulpa vital yang paling umum digunakan adalah kalsium hidroksida (Ca(OH)2), tetapi bahan tersebut memiliki beberapa kekurangan. Penelitian sebelumnya menunjukkan nano-hidroksiapatit mampu merangsang pembentukan dentin reparatif tanpa tunnel defect dan penambahan kolagen mampu meningkatkan sifat mekanik hidroksiapatit. Penambahan Epigallocatechin-3-Gallate (ECGC) pada gel kolagen bermanfaat dalam mengurangi respon inflamasi pulpa. Tujuan: Penelitian ini dilakukan untuk mensintesis dan melakukan uji karakteristik fisik komposit hidrogel nano-hidroksiapatit-kolagen-Epigallocatechin-3-Gallate. Metode: Nano-hidroksiapatit dari cangkang telur ayam dilarutkan dengan 0,2 g/mL kolagen tipe I, dan 10 mmol/L EGCG masing-masing ke dalam air deionisasi dengan perbandingan rasio nano-hidroksiapatit dan kolagen, 40:60 (kelompok 1), 50:50 (kelompok 2), dan 60:40 (kelompok 3). Seluruh larutan dicampurkan dan ditambahkan HPMC 2%, lalu dilakukan uji karakteristik gel berupa uji pH, waktu gelasi, dan uji injektabilitas. Hasil: Nilai pH 1 jam setelah pengadukan secara berurutan adalah 8,05 ± 0,05, 8,43 ± 0,02, dan 8,71 ± 0,04 dengan waktu gelasi selama 1 jam. Injektabilitas secara berurutan adalah 90,67%, 91,93%, dan 95,03%. Kesimpulan: Bahan komposit hidrogel nano-hidroksiapatit, kolagen, dan EGCG memiliki karakteristik fisik yang berpotensi untuk digunakan sebagai bahan terapi pulpa vital jika dilihat dari nilai pH dan kemampuan injektabilitas yang diperoleh.
