Abstract

This study aims to analyze the quality control of tempeh chips products in Batam using the Failure Mode and Effect Analysis (FMEA) and Root Cause Analysis (RCA) approaches. Tempeh chips are processed food products that have significant economic value for MSMEs in Batam, but often face various challenges related to quality control. The research method used is a combined qualitative and quantitative approach with a sample of 3 MSMEs producing tempeh chips in Batam. The results of the study identified several major quality problems in the tempeh chips production process, including non-uniform thickness, damaged packaging, soft texture, imperfect packaging seals, and weight contents that do not match the label. Through the application of FMEA, this study calculates the Risk Priority Number (RPN) for each failure mode, identifying five critical problems with the highest RPN values. Furthermore, using RCA and 5-Why analysis so that the root cause of each problem can be identified based on human, machine, method, material, and environmental factors. Based on the results of the analysis, this study recommends a series of comprehensive quality control recommendations, including the development of Standard Operating Procedures (SOPs), quality monitoring systems, employee training programs, and the implementation of quality control checkpoints. This research provides a significant contribution to the development of the tempeh chips industry in Batam and can be a reference for similar research in other areas.

Abstract

Education is very important for the progress of the country, education cannot be separated from an assessment. Assessment must be based on the existing rubric to be clear, structured, and transparent. One of the existing assessments is based on analytic, holistic, and perceptual scales. The purpose of this study was to establish an assessment rubric in the algebra course because the course was only seen from a quantitative assessment, but in this study, it was assessed based on attitude, and character and then made criteria based on independent assignments and structured assignments and the results would later be tested differently by using the sample t-test. test, namely the independent sample t-test, the results of the data are not normally distributed, it is better to use the Wilxoson test and sign t-test. The result is that there is no difference between independent tasks and structured tasks seen from the significant table and the t-test table.

Abstract

Material inventory is a fundamental aspect of the company. The smoothness of the production process is not only based on the level of effectiveness in the production process, but also on other processes related to material inventory. If materials are in short supply, the production process will experience disruption which results in financial and non-financial losses. If there is too much material inventory, then the material storage costs borne by the company will be greater. The aim of this research is to maximize material supplies and determine the right time to reorder raw materials. The methods used are EOQ, ROP, and ITO. Data collection was carried out quantitatively at a fish canned food company. Based on the calculation results, it was found that the economic level of purchasing main raw materials using the EOQ method approach was 25 kg per order and the recommended reorder amount was 10 kg per order, while for safety sticks the recommended amount was 4.4 kg/day.

Abstract

PT XYZ, as a logistics service provider, faces complex operational risks such as incorrect data input in the system, limited supply availability, and sudden customer demand. These risks lead to delivery delays, inaccurate information flow, and decreased operational efficiency. This study aims to identify and analyze operational risks and develop appropriate mitigation strategies. The methods used include the Supply Chain Operations Reference (SCOR) model to map the supply chain processes (plan, source, make, deliver, and return), and the House of Risk (HOR) Phase 1 and 2 to identify the relationship between risk events and risk agents, determine risk priorities based on the Aggregate Risk Potential (ARP), and formulate mitigation strategies using the Effectiveness to Difficulty Ratio (ETD). The results show that there are 16 risk events and 17 risk agents, with 3 risk agents identified as top priorities. A total of 11 mitigation strategies were proposed, including the implementation of an ERP system, integration of an Early Warning System, and strengthening of human resource competencies. These strategies are expected to reduce recurring risks and support the sustainable improvement of PT XYZ’s operational performance.

Abstract

This study explores the impact of integrating Failure Mode and Effect Analysis (FMEA) and Root Cause Analysis (RCA) on marine hose quality at PT Yokohama Industrial Products Manufacturing Indonesia. Driven by a defect rate that exceeded the 5% threshold (5.13% from June 2024 to May 2025), the research employed a descriptive quantitative design, using observation, documentation, and interviews. Among 937 defective units, surface cracks (468 cases, 39.8%), joint leaks (281 cases, 23.4%), and pipe deformations (188 cases, 15%) were most prevalent. FMEA assigned Risk Priority Numbers (RPNs), identifying cracking (RPN = 270), leakage (240), and deformation (144) as top risks. RCA with the 5 Whys revealed root causes: irregular vulcanization machine calibration, substandard raw materials, and inconsistent temperature control. Following corrective measures scheduled calibrations, raw material verification, and standardized vulcanization processes the defect rate dropped significantly. Over three months, defective units fell from 243 to 97 (a 40% reduction), and RPNs decreased: cracking to 120 (−55.6%), leakage to 90 (−62.5%), and deformation to 60 (−58.3%). These results confirm that combining FMEA and RCA effectively reduces failure risk, enhances product quality, and improves manufacturing efficiency.

Abstract

This community service program was implemented with the main objective of implementing an effective and sustainable waste management and sorting system in Batam City. The methods used in implementing this program include a participatory approach through a series of focus group discussions, waste sorting training, and the establishment of a community-based waste management system in the pilot village, namely Batu Aji Village. As demonstrated by the fact that the majority of residents are now actively sorting waste at the household level, the primary outcomes of this community service program demonstrate a notable improvement in community awareness and waste sorting skills. The amount of mixed garbage in the intervention region has decreased because to the organic-inorganic sorting system's adoption, which has also helped to lower the amount of waste that ends up in landfills. The community's economy benefits from this scheme as well, as waste banks and recycling initiatives generate more revenue. Keywords— Waste Management, Sorting, Community, Batam

Abstract

This study evaluates quality control in leather shoe Small and Medium-sized Enterprises (SMEs) in Batam City using Failure Mode and Effects Analysis (FMEA) and Root Cause Analysis (RCA) methods. The background addresses product quality challenges affecting SME reputation and competitiveness. The research aims to identify potential failure modes, calculate Risk Priority Numbers (RPN), determine root causes, and formulate improvement recommendations. The methodology involves a hypothetical case study, applying FMEA to identify failure modes (Severity, Occurrence, Detection) and RCA (5 Whys, Fishbone Diagram) for root causes. FMEA results highlight "untidy/broken stitches" (RPN 360) and "cutting pattern errors" (RPN 336) as top priorities. RCA identified root causes such as insufficient operator training, suboptimal raw material procurement policies, inadequate machine maintenance, operator fatigue, and dull cutting tools. The study concludes by successfully identifying critical areas and root causes of quality issues. Recommendations include enhancing training, standardizing processes, preventive maintenance, and investing in precise equipment for SMEs.
