Abstract

Background: Tinea capitis is a type of dermatophytosis (skin disease) caused by the infection of dermatophytes. It is generally caused by the dermatophyte species of Microsporum and Trichophyton. It has been known to be able to spread human to human and animal to human. It is also almost commonly found in the prepubertal age and very rare among the adults of either gender. This study aims to determine the prevalence of Tinea capitis in children by age, sex, clinical manifestation, laboratory tests, and treatments.Methods: A cross-sectional observational study from the medical records of patients with Tinea capitis was conducted at Dermato-Venereology Outpatient Clinic RSUP Sanglah from 2014 to 2017. A total of 5 research variables were identified for this study: age, sex, clinical manifestation, laboratory tests, and treatment. The data was then analyzed and interpreted and further presented in forms of description, tables and graphs in this study by using SPSS version 17 for WindowsResults: A total of 32 patients were identified as having Tinea capitis from the period of 2014–2017. There was a slightly higher prevalence in the male gender (56.25%) compared with female gender (43.75%). The highest prevalence of the disease was found in the age groups of 1 – 5 and 6 – 10 years old with 43.75% respectively. Majority of the patients showed the presence of grey patch on their scalp on examination (53.13%). The dermatophyte Trichophyton rubrum had the highest isolation in comparison with other dermatophytes at 25.00%. The patients were almost all treated with oral Griseofulvin.Conclusion: The majority diagnosis of Tinea capitis was found in group 1-5 and 6-10 years old and had a grey patch appearance, whereas Trichophyton rubrum as the highest isolation in comparison with other dermatophytes

Abstract

Introduction: Sexually transmitted infections (STIs) is still a major health issue nowadays. Several studies have shown an increased incidence of STIs in the group of man who has sex with man (MSM) and bisexuals when compared to heterosexual men or women. This is inseparable from risky sexual behaviour practiced by this population, so it is not impossible for individual in this population to be suffering from more than one STI.Case description: We report one case of secondary syphilis with fourth stage HIV infection accompanied with tuberculous lymphadenitis in a bisexual man.Conclusion: Management of each specific condition have to in accordance with treatment standards guideline. Long term evaluation regarding patient compliance, laboratory serology test, and further education regarding the disease and transmission are mandatory.

Abstract

Background: Herpes zoster ophthalmicus occurs in 10-20% of cases of herpes zoster. Herpes zoster ophthalmicus occurs when the human herpes virus type 3 reactivates the first division of the trigeminal nerve, which is also called the ophthalmic division. In some patients, it may show ocular involvement.Case: A 60 years old male patient came with swelling and wounds on the left forehead and around the left eye. Initially appear as watery spots and accompanied by pain. The pain felt like being stabbed, hot like burning sensation. The patient had chickenpox as a child. In the dermatological status of the superior palpebra, frontalis and left parietalis, multiple erythema patches were seen with multiple erosions. It appears that the lesion is unilaterally distributed according to the dermatome of the trigeminal cranial nerve of the left ophthalmic branch. Gram examination on erosive lesions showed the presence of 1-5 leukocytes / large field of view with Gram positive cocci and no Gram negative rods. The patient was diagnosed with herpes zoster ophthalmicus left with secondary infection. Consultation to ophthalmology division revealed left ocular herpes zoster ophthalmic (blepharoconjunctivitis), corneal erosions, iridoplegia and mature senile cataracts. The patient discharged on the fifth day. On the 11th day, complaints of watery eyes, swelling and eye discharge were gone.Conclusion: A case of herpes zoster ophthalmicus in old age with ocular complications and secondary infection was reported. For erosional lesions, open compresses was applied with NaCl and sodium fusidate cream. Patients respond well to therapy. Latar Belakang: Herpes zoster oftalmikus terjadi pada 10-20% dari kasus herpes zoster. Herpes zoster oftalmikus terjadi ketika human herpes virus tipe 3 mengalami reaktivasi pada divisi pertama dari saraf trigeminalis, yang disebut juga divisi oftalmik. Pada beberapa pasien, dapat menunjukkan keterlibatan okular.Laporan Kasus: Pasien laki-laki, usia 60 tahun datang dengan keluhan bengkak dan luka pada dahi kiri serta area sekitar mata kiri. Awalnya muncul berupa bintik-bintik berair dan disertai nyeri. Nyeri dirasakan seperti ditusuk-tusuk, panas seperti terbakar, dan mata terasa seperti tertarik. Pasien pernah menderita penyakit cacar air pada saat kanak-kanak. Pada status dermatologis palpebra superior, frontalis dan parietalis sinistra tampak patch eritema multipel dan pada beberapa tempat disertai erosi multipel. Tampak lesi terdistribusi secara unilateral sesuai dermatom nervus kranial trigeminus cabang oftalmikus sinistra. Pemeriksaan Gram pada lesi erosi menunjukkan adanya leukosit 1-5/lapang pandang besar, ditemukan adanya kokus Gram positif dan tidak didapatkan batang Gram negatif. Pasien didiagnosis dengan herpes zoster oftalmikus sinistra disertai infeksi sekunder. Konsultasi pada bagian mata didapatkan okuli sinistra herpes zoster optalmika (blefarokonjungtivitis), erosi kornea, iridoplegia dan katarak sinilis matur. Pasien dipulangkan pada hari kelima perawatan. Pada hari ke 11, Keluhan mata berair, bengkak dan kotoran mata sudah tidak ada.Simpulan: Telah dilaporkan sebuah kasus herpes zoster oftalmikus pada usia tua dengan komplikasi okular dan didapatkan juga infeksi sekunder. Pada lesi erosi diberikan kompres terbuka dengan NaCl dan krim natrium fusidat. Pasien memberikan respon yang baik dengan terapi.

Abstract

Introduction: Scleroderma is a rare autoimmune disease of connective tissue characterized by extensive fibrosis, inflammation, and vasculopathy. It classified as limited cutaneous and diffuse based on the degree of skin involvement. The management is still a challenge since it has a high morbidity and mortality rate. This case report aims to understand scleroderma and provide appropriate treatment that can improve the patient's prognosis.Case report: A 55-year-old female came with complaints of stiff skin on the forearms, lower legs and face since approximately eight months ago, initially on the right and left forearms, then to face, right and left lower legs. On physical examination, there was sclerosis of the skin on the face, forearms and lower legs, and salt and pepper appearance with Rodnan score were 26. The result from histopathological examination were sceleroderma. The patient was diagnosed with limited cutaneous scleroderma. The management consisted of methotrexate, folic acid, sulpha ferrous, vitamins B1, B6, B12 intraorally and 10% urea cream topically. She also was informed to practice moving the hands and fingers slowly.Conclusion: The diagnosis of limited cutaneous scleroderma is based on history, physical examination and investigations. It is still a life-threatening disease, however multidisciplinary management with early detection and treatment of complications can improve the prognosis.

Abstract

Background: Leprosy is a chronic granulomatous infectious disease caused by Mycobacterium leprae (M. leprae) that primarily infects Schwann cells in the peripheral nerves, leading to nerve damage and the development of disabilities. In 2018, Indonesia was the third country with the most leprosy cases in the world. Erythema nodosum leprosum (ENL), also known as type II leprosy reaction, is a severe immune-mediated complication of multibacillary leprosy. Purpose: To report a case of borderline lepromatous leprosy with severe ENL. Case: A 49-year-old Balinese man presented with multiple tender erythematous skin nodules all over his body, fever, arthralgia, bilateral cervical lymphadenopathy, and sensory loss for the past week. The acid-fast bacilli bacteriological examination showed a positive result. The patient was diagnosed with borderline lepromatous (BL) leprosy with severe ENL and was treated with multibacillary multidrug therapy (MB MDT), methylprednisolone, and other symptomatic medications. After 1 month of treatment, there was an improvement in skin lesions. The MB-MDT treatment was continued and methylprednisolone was planned to be tapered down gradually. Discussion: Approximately 20-50% of all leprosy patients show leprosy reactions in the course of the disease. The goals of treatment for severe ENL are to control inflammation, reduce pain, treat neuritis to prevent nerve dysfunction and contractures, and prevent recurring ENL. The prognosis of leprosy with ENL reactions depends on the severity of the occurring leprosy reaction; early diagnosis and prompt treatment; and patient compliance with treatment. Conclusion: Early diagnosis and treatment are essential to avoid deformities in leprosy patients.

Abstract

Background: Staphylococcus aureus is an major human pathogen. Most individuals get Staphylococcus aureus infections with varying degrees of malignancy, such as superficial pyoderma, at some point in their lives. Antibiotics are often used to treat Staphylococcus aureus infections, however they may not be utilised or prescribed correctly, resulting in resistance. Alkaloids, flavonoids, tannins, essential oils, terpenoids, and curcuminoids are some of the active components in turmeric that have antibacterial properties. Objective: The goal of this study was to see how efficient the most effective turmeric extract (Curcuma domestica Val.) was in stopping Staphylococcus aureus from growing. Methods: This is an experimental research with a post-test control group. A maceration process employing 96 percent ethanol as a solvent was used to extract Singaraja turmeric. The extract concentrations employed in this research were 15%, 30%, 50%, 75%, and 100%; the positive control was vancomycin, and the negative control was 96 percent ethanol. Using the disc spreading approach, the test was conducted four times. The Kruskal-Wallis test was used to examine the findings of the research. Results: For the average, the inhibition zones were 7 mm, 7.25 mm, 7.5 mm, 7 mm, and 7 mm in diameter at each concentration. This research obtained significance figure by 0.001 (p<0.05). This suggests that the development of Staphylococcus aureus is influenced by varying amounts of turmeric ethanol extract. Conclusions: Turmeric extract (Curcuma domestica Val.) inhibits the growth of Staphylococcus aureus at all concentrations, with 50% of the extracts displaying the largest inhibition zone width.

Abstract

Dermatitis seboroik (DS) adalah penyakit kulit kronis yang ditandai rasa gatal dan skuama pada area kulit dominan kelenjar sebasea. Patogenesis DS diduga melibatkan multifaktor dengan produksi kelenjar minyak berlebih dan kolonisasi Malassezia menjadi yang utama. Dermatitis seboroik tidak memiliki predileksi usia, dengan predominansi seks pada pria. Gejala klinis DS berupa patch eritema dengan skuama kekuningan. Penelitian ini menggunakan metode deskriptif retrospektif untuk melihat profil jumlah kasus, seks, usia, lokasi lesi, dan jenis pengobatan Dermatitis Seboroik di Poliklinik Dermatologi dan Venereologi RSUP Prof IGNG Ngoerah periode Juli 2019 - Juli 2022. Hasil penelitian ini menampilkan dari 43 kasus DS (0,38%) usia tersering pada 45-65 tahun (41,86%), seks pria (51,17%), lokasi lesi wajah (65,11%), serta pengobatan kortikosteroid dan antijamur topikal yang dikombinasi (67,45%) yang tersering digunakan.

Abstract

Radiasi solar ultraviolet (SUV) dapat membawa dampak baik sekaligus dampak buruk bagi kesehatan jika terpapar pada kulit dalam rentang waktu tertentu. Negara Indonesia memiliki letak yang berdekatan dengan garis ekuator sehingga intensitas SUV (UVI) cenderung tinggi setiap tahunnya, peningkatan UVI juga diketahui sejajar dengan peningkatan dampak buruk paparannya pada kulit. Penelitian di Bali menunjukan prevalensi kanker kulit sebagai dampak terburuk paparan SUV lebih tinggi pada para pekerja outdoor dengan usia lanjut, padahal mekanisme terjadinya kanker terjadi akibat paparan kronis sejak usia muda. Karena itu, penelitian ini bertujuan untuk mengetahui tingkat pengetahuan terhadap paparan SUV pada kulit dari Mahasiswa Agroekoteknologi Universitas Udayana Angkatan 2019, yang berisiko karena ke depannya bekerja di lahan outdoor. Penelitian ini dilakukan untuk mencari tahu distribusi data dari tingkat pengetahuan Mahasiswa berdasarkan variabel sosiodemografi jenis kelamin dan usia Mahasiswa. Metode penelitian yang dipakai adalah studi deskriptif dengan desain penelitian potong lintang (cross-sectional) dan pengambilan sampel dilakukan secara total sampling dengan kriteria inklusi dan eksklusi. Instrumen penelitian yang ada menggunakan kuesioner oleh studi Suryantari, dkk. (2019). Jumlah sampel yang didapat sebesar 102 Mahasiswa. Data yang didapat diolah dan disajikan dalam bentuk proporsi menggunakan SPSS v.26. Berdasarkan hasil penelitian, proporsi tingkat pengetahuan baik (mean ± standar deviasi) dari jenis kelamin laki-laki didapati sebanyak sebanyak 92,82% (12,048 ± 2,284), jenis kelamin perempuan 96,67% (12,900 ± 1,674), usia 18-19 tahun 96,3% (12,519 ± 1,718), Usia 20-22 tahun 94,67% (12,56 ± 2,081). Penelitian ini menyimpulkan bahwa Mahasiswa Agroekoteknologi Universitas Udayana Angkatan 2019 memiliki pengetahuan yang baik akan paparan SUV pada kulit. Abstract

Psoriatic arthritis (PsA) presents a significant challenge due to its high prevalence and morbidity rates. Psoriasis vulgaris affects an estimated 0.19-2% of the global population, with approximately 30% of these individuals progressing to experience psoriatic arthritis. Methods: This is a retrospective study using medical records of psoriatic arthritis patients at the Dermatology and Venereology Clinic. Result and Discussion: A total of 168 psoriasis patients were assessed at the Dermatology and Venereology Clinic of Prof. I.G.N.G Ngoerah Hospital from 2021 to 2023. Among them, 58 cases were also diagnosed with Psoriatic Arthritis (PsA), accounting for 35.1% of the sample. Of these, 33 patients were male (67.3%) and 16 were female (32.7%). The highest prevalence was observed in the 40-59 age group (51%). Additionally, 77.5% of patients had a Body Surface Area (BSA) greater than 10%, 61.2% had a Psoriasis Area Severity Index (PASI) score of severe psoriasis (>10), and 49.0% had a Dermatology Life Quality Index (DLQI) score indicating moderate impact (6-10). Similarly, 49.0% of patients had a moderate Disease Activity Index for Psoriatic Arthritis (DAPSA) score (>4-14). The study revealed a PsA prevalence of 35.1%, which surpasses global averages, likely due to the hospital's role as a tertiary referral center for severe psoriasis cases. Conclusion: The prevalence of Psoriatic Arthritis (PsA) among patients treated is still high, particularly among male patients aged 40-59 years who have severe psoriasis based on BSA (Body Surface Area), PASI (Psoriasis Area Severity Index), and DLQI (Dermatology Life Quality Index) criteria.

Abstract

Background: Pustular psoriasis in children is one of the clinical variants of psoriasis. It is classified into generalized pustular psoriasis (GPP) and localized pustular psoriasis. The aetiology of psoriasis in children has not been known but is believed to be multifactorial. Nail psoriasis rarely occurs in children who suffer from skin psoriasis, with an incidence lower than that reported in adults. The diagnosis is generally made from the clinical and histological examination. The choice of therapy depends on the severity of the disease. Until now, there have been no specific guidelines for the management of psoriasis in children. Case report: A-15-years-old Balinese girl presenting with erythema, confluent scaly plaques over the trunk and extremities with pustules localized on the lower extremity. She had a history of fever before the lesions appear. Right third digital nails examination showed subungual hyperkeratosis and onycholysis. Positive auspitz sign and karsvlek phenomena were found. Biopsy result suitable for psoriasis. The patient got improvement after treated with methotrexate tablet orally and desoximetasone cream topically within four weeks without any side effect. Conclusion: Combination therapy with methotrexate tablet and desoximetasone cream give an effective result. However, the safety and side effects of methotrexate in children still need further monitoring.
