Abstract

Lipomas are the most common soft tissue tumor neoplasms. Their range from benign lipomas to high grade liposarcomas. Liposarcoma happens when the neoplasm of a lipoma is malignant. There are various diagnostic methods that can be used to evaluate lipomas, one of which is Fine Needle Aspiration Biopsy (FNAB). FNAB is the initial diagnosis to determine the diagnosis of lipoma soft tissue tumors. Histopathology is the definitive diagnosis or the gold standard for diagnosing lipoma soft tissue tumors. This research aims to determine the value of the Fine Needle Aspiration Biopsy diagnostic test against histopathological tests as the gold standard in lipoma soft tissue tumor patients. This research is a retrospective with a descriptive observational design. This research involved taking medical records of patients with lipoma soft tissue tumors who were examined by FNAB at RSPAL dr. Ramelan, Surabaya for the period August 2019 - December 2021. The results of the FNAB examination were then matched against the histopathological examination data as the gold standard. Data processing using 2x2 cross tabulation to calculate the diagnostic test which includes sensitivity, specificity, positive predictive value, negative predictive value, and accuracy against the gold standard. The results of the Fine Needle Aspiration Biopsy diagnostic test for histopathology in soft tissue lipoma tumors obtained sensitivity of 100.00%, specificity 95.83%, positive predictive value (NRP) 87.5%, negative predictive value (NRN) 100.00%, and 96.77% accuracy. Fine Needle Aspiration Biopsy (FNAB) has good diagnostic value for diagnosing lipoma soft tissue tumors.

Abstract

Many laboratory installations have not implemented quality assurance periodically due to the high cost of purchasing control materials. This study aims to determine the difference in Variant Index Score value of homemade lyophilized serum with commercial parameters of SGOT, SGPT, creatinine, BUN, glucose, uric acid, triglycerides, and cholesterol. The research was conducted in January–May 2023 at the Surabaya Health Center laboratory, the reference laboratory, and the Ubaya Technobiology Faculty using descriptive comparative research with a quantitative approach. The test material used was homemade lyophilized serum, with the research variable being the VIS value of lyophilized serum. The results of the analysis showed a comparison of the VIS value of homemade and commercial serum based on the participant's target values of SGOT, SGPT, creatinine, BUN, glucose, triglycerides, and cholesterol parameters obtained a significance value of 00,759; 0,983; 0,160; 0,994; 0,413; 0,797; 0,318; and 0,262, and based on the true value target values of SGOT, SGPT, creatinine, BUN, glucose, uric acid, triglycerides, and cholesterol parameters obtained a significance value of 0,70; 0,973; It is concluded that there is no significant difference in VIS value between homemade and commercial lyophilized serum, so homemade lyophilized serum can be used as an option for control material to replace commercial serum for laboratory quality assurance.

Abstract

The aim of this study was to evaluate the compatibility of platelet histogram results with IP Message on the hematology analyzer Sysmex XN-1000 for the platelet morphology of peripheral blood smears of thrombocytopenia patients. This was a quantitative descriptive study of 54 samples taken using the saturation sampling technique at Grati Pasuruan Hospital from April 1–30 2023. The results showed that the histograms that often appeared were abnormal heights on the PU Flag and the IP Messages that often appeared were “PLT Clumps?”, “PLT Abn Distribution”, and “Thrombocytopenia”, with a compatibility level of platelet morphology on peripheral blood smear examination reaching 90.7%. The study concluded that there was a high compatibility between the platelet histogram results accompanied by the IP Message and the platelet morphology of the peripheral blood smear of thrombocytopenia patients, which could improve the accuracy and efficiency of platelet count examination.

Abstract

Quality assurance is important to ensure that the quality system runs properly and guarantees the results of laboratory tests, one of which is the inspection of control materials. The control material requirement is to have long stability, blood-like characteristics and low price. From these conditions, commercial control materials have a longer stability than fresh blood but are expensive. In practice, commercial control materials are still used even after the expiry date of the materials has passed. This is because the availability of materials is often delayed, the procurement of materials takes a long time and the price is relatively expensive. Therefore, this study aims to determine the stability of commercial whole blood control stored at 2-8oC. This research is an experimental analytic study with a cross sectional research design. The sample in this study was commercial whole blood control with 3 levels, namely normal, low abnormally and abnormally high levels which were stored at 2-8oC and checked every week for 10 weeks. Based on the test results obtained p-value for each parameter at all levels on the parameters of the number of erythrocytes, leukocyte count, hematocrit, hemoglobin and platelet count. Based on the results, it can be concluded that there is no change in the stability of whole blood control with storage for 10 weeks at a temperature of 2-8oC on the parameters of abnormally low and normal leukocyte count levels, abnormally low and abnormally high hematocrit levels, and all levels on the parameters of leukocyte count, hemoglobin and platelet count. Meanwhile, in the normal level of hematocrit parameters and abnormally high leukocyte count, there is a change in stability.

Abstract

The Erythrocyte sedimentation rate is a routine blood test performed to monitor the course of the disease where high values ​​are likely to be associated with the presence of inflammation. ICSH (International Council for Standardization in Hematology) recommends the Westergren method as a method for examining LED with several developments in terms of the anticoagulants and diluents used. Anticoagulants that can be used are Sodium Citrate 3,8% anticoagulant and EDTA (Ethylene Diamine Tetra-Acetid-Acid) anticoagulant. This study aims to determine the differences in the erythrocyte sedimentation rate of the westergren method using EDTA anticoagulants and sodium citrate 3.8%.This study uses primary data collection techniques with comparative research type. The research was conducted at the Health Analyst Laboratory of Poltekkes Kemenkes Surabaya, in January-April 2021. The sample of the study was 50 menstruating women who had met several criteria, who would be examined for LED values ​​using the Westergren method using EDTA anticoagulants in purple vacuum tubes and sodium citrate 3,8 % on a black vacuum tube.The results showed that the mean of sedimentation rate with EDTA anticoagulant was 27.66 mm/hour and 22.04 mm/hour for sodium citrate 3,8% anticoagulant. The statistical test was carried out by using the paired sample t-test which showed the sig (p) value = 0.000 0.05, so it could be concluded that there was a difference between the EDTA anticoagulant LED and the sodium citrate anticoagulant LED. Thus the EDTA anticoagulant can still be used as an anticoagulant in examining the erythrocyte sedimentation rate

Abstract

Acute Myocardial Infarction (AMI) is a heart disease condition due to heart muscle cells not getting enough blood flow and oxygen due to atherosclerosis of the coronary arteries. When the condition of atherosclerosis gets worse, it will cause an inflammatory reaction, causing an increase in neutrophils. Inflammation due to atherosclerosis will worsen blood flow to the heart, resulting in heart muscle necrosis which causes an increase in biochemical markers, namely cardiac troponin. The aim of this research is to determine the relationship between the number of neutrophil cells and troponin I levels in patients with Acute Myocardial Infarction.This type of research is a cross-sectional analytic observational with a retrospective approach. This research was conducted on patients with Acute Myocardial Infarction at the RSPAL dr. Ramelan Surabaya who performed a complete blood count and troponin I level with a period of January 1st 2020 – May 3rd 2021, taken by purposive sampling. The results showed that male respondents were 29 patients (63%) while female respondents were 17 patients (37%). Respondents aged 65 years amounted to 28 (61%) people, while respondents aged 65 years were 18 (39%) people. The results of the hypothesis test have a sig value of 0.000 < (0.05), so there is a relationship between the number of Neutrophil cells and Troponin I level in patients with Acute Myocardial Infarction. In hypothesis testing, the correlation coefficient value is 0.594, which means that there is a moderate relationship with a positive direction.

Abstract

Anemia of pregnancy is a condition of the body with hemoglobin levels in the blood <11g% in the 1st and 3rd trimesters. The erythrocyte index is used as a screening test to diagnose anemia based on its morphology. Ret-He is a new parameter that functions to assess iron in the body. This study aims to analyze the relationship between red blood cell index and reticulocyte hemoglobin (Ret-He) levels in pregnant women with iron deficiency anemia. The study design was cross-sectional involving 15 pregnant women at Wangaya Hospital, Denpasar. The measurement results data were then analyzed using the Pearson correlation test. The results showed that the mean MCV = 70.04 fL, average MCH = 24.05 pg, average MCHC = 27.48 g/dL, average Ret-He = 23.64 pg. The results of statistical analysis on the correlation between Ret-He and MCV, MCH and MCHC, showed p values were 0.004 for MCV, 0.001 for MCH and 0.793 for MCHC respectively. The conclusion of this study is that there is a relationship between Ret-He and MCV and MCH.Keywords: erythrocyte index; Ret-He; iron deficiency anaemia ABSTRAK Anemia kehamilan adalah kondisi tubuh dengan kadar hemoglobin dalam darah <11g% pada trimester 1 dan 3. Indeks eritrosit digunakan sebagai pemeriksaan penyaring untuk mendiagnosis terjadinya anemia berdasarkan morfologinya. Ret-He merupakan suatu parameter terbaru yang berfungsi menilai zat besi dalam tubuh. Penelitian ini bertujuan untuk menganalisis hubungan antara indeks eritrosit dengan kadar retikulosit hemoglobin (Ret-He) pada ibu hamil dengan anemia defisiensi besi. Rancangan penelitian ini cross-sectional yang melibatkan 15 ibu hamil di RSUD Wangaya, Denpasar. Data hasil pengukuran selanjutnya dianalisis menggunakan uji korelasi Pearson. Hasil penelitian menunjukkan bahwa rerata MCV = 70,04 fL, rerata MCH = 24,05 pg, rerata MCHC = 27,48 g/dL, rerata Ret-He = 23,64 pg. Hasil analisis statistik pada korelasi antara Ret-He dengan MCV, MCH dan MCHC, menunjukkan nilai p masing-masing adalah 0,004 untuk  MCV, 0,001 untuk MCH dan 0,793 untuk MCHC. Kesimpulan penelitian ini adalah ada hubungan antara Ret-He dengan MCV dan MCH.Kata kunci: indeks eritrosit; Ret-He; anemia defisiensi besi

Abstract

Background: Congenital hypothyroidism (CH) is a condition of thyroid hormone deficiency that occurs at birth. The TSH (thyroid-stimulating hormone) test is crucial for diagnosing hypothyroidism. CH is known to cause prolonged unconjugated hyperbilirubinemia. Therefore, this study aimed to analyze the relationship between TSH and indirect bilirubin levels in neonates suspected of having jaundice. Methods: This is a non-experimental, retrospective study conducted at Lombok Dua Dua Lontar Mother and Child Hospital in Surabaya. The study involved data collection on neonates aged 2–7 days suspected of jaundice, whose TSH and indirect bilirubin levels were measured between November 2022 to April 2024. Results: Among 100 neonates, 62% were aged 2-4 days, while 38% were aged 5-7 days. The majority were male (56%), with female comprising 44%. Of the 100 neonates, only 1 (1%) had borderline TSH levels, while 99% had normal TSH levels. Hyperbilirubinemia was observed in 94% of the neonates, while 6% had normal indirect bilirubin levels. Statistical analysis using the Spearman correlation showed no significant link between TSH and indirect bilirubin levels (p = 0.802). Conclusions: While this study did not find a clear connection between TSH and indirect bilirubin levels in neonates suspected of having jaundice, one case of borderline TSH was identified. This neonate required referral to pediatric endocrinology, as untreated congenital hypothyroidism can lead to mental retardation. Despite limited research linking TSH and bilirubin levels in jaundiced neonates, routine screening for congenital hypothyroidism using TSH testing should be reconsidered. Future studies could benefit from focusing on specific causes of neonatal jaundice to help narrow down research questions in this area.

Abstract

Subclinical inflammation and almost every indication of systemic inflammation, which is defined by elevated levels of inflammatory markers in the blood, are seen in patients with diabetes mellitus (DM). Understanding the relationship between inflammatory markers and glycemic control is essential for creating comprehensive management strategies to avoid diabetes-related complications. The aims of this study were to determine the correlation between erythrocyte sedimentation rate (ESR) and high-sensitivity C-reactive protein (hs-CRP) and glycated hemoglobin (HbA1c) in DM patients. This was an analytical observational study with a cross-sectional design that was conducted on 35 DM patients from the DM prolanis group at the Bangkalan Regency First-Level Health Facility. HbA1c and hs-CRP levels were measured using the fluorescence immunoassay (FIA) method, while ESR was assessed using the Westergren method. Statistical analysis was performed using the Spearman correlation test. Regarding the biomarkers, all respondents (100%) had HbA1c levels >8.0%, 43% had elevated hs-CRP levels associated with cardiovascular risk, and 80% had abnormal ESR values. A significant relationship was found between ESR and HbA1c (p = 0.019; r = 0.394), but no significant relationship was observed between hs-CRP and HbA1c (p = 0.351; r = 0.163). The relationship between inflammation and glycemic control in DM can be understood through the correlation of ESR, hs-CRP, and HbA1c. These findings offer practical value, particularly in monitoring disease progression and managing diabetes-related complications. Future research should investigate the effects of interventions aimed at reducing inflammation, e.g lifestyle modifications or anti-inflammatory medications, on ESR, hs-CRP, and HbA1c levels.

Abstract

Control materials are used as indicators of monitoring the performance of laboratory examinations so that the results issued are valid. Method of assessing the implementation of External Quality Assurance (EQAS) used Variance Index Score (VIS). This study aims to determine difference between pooled sera, homemade and commercial lyophilizates on AST and ALT parameters based on VIS. This research used comparative descriptive with quantitative approach in December 2023-May 2024. Homemade lyophilizates performed at UBAYA technobiology laboratory, determination of true value performed at 2 reference laboratories and clinical chemistry laboratory Poltekkes Surabaya, and determination of VIS performed at 10 primary level clinical laboratories in Surabaya Raya which determined by purposive sampling. The statistical analysis results showed no difference between homemade and commercial control materials based on VIS to the true value and all participating laboratories, so it was concluded that homemade control materials could be used as a substitute in implementing EQAS.
