Abstract

Introduction: Maintaining vascular access is a complex process that involves various clinical factors, the competence of healthcare staff—particularly nurses—and the products used in treatment. Administering drug and fluid therapy through the intravenous route is intricate and carries significant risks of complications. Therefore, the Vessel Health and Preservation (VHP) model approach is crucial for administering effective therapy. To date, no studies have been conducted on comprehensive vascular access management in Indonesia. This study aimed to describe vascular access management in inpatient care using the VHP Model.Methods: Using an accidental sampling technique, the study employed a descriptive design, with 224 nurses from two private hospitals in Indonesia serving as respondents. The instrument was an observation sheet, and data collection was conducted on 224 vascular access installations in patients, based on observations made in March 2024. The data were analyzed using descriptive statistics.Results: Vascular access management, including the four quadrants in the VHP model, is critical to apply because phlebitis complications still occurred in 17 (7.59%) of 224 cases, and only 181 (80.80%) of intravenous access insertions were successful on the first puncture.Conclusion: These results indicate the need for ongoing nurse education and training in vascular access management. The complexity of administering drug and fluid therapy through the intravenous route, with a reasonably high risk of complications, requires an approach model that can be used in administering therapy through the intravenous route.

Abstract

ABSTRAK Diabetes melitus adalah gangguan metabolik kronis dan saat ini menjadi beban dalam sistem layanan kesehatan. Pendekatan multifaset diperlukan dalam pencegahan dan penanganan diabetes. Pemberdayaan masyarakat merupakan proses yang memungkinkan masyarakat untuk meningkatkan kontrol atas kehidupan mereka. Adanya pemberdayaan masyarakat diharapkan mampu mengatasi faktor risiko diabetes terutama faktor yang dapat dimodifikasi seperti diet, aktivitas fisik, kebiasaan merokok, dan konsumsi alkohol. Kegiatan ini bertujuan untuk menggambarkan status kesehatan peserta didik meliputi Indeks Massa Tubuh (IMT), tekanan darah, dan kadar glukosa darah sewaktu, asam urat, dan kolesterol, serta memberdayakan komunitas dengan meningkatkan pengetahuan melalui pendidikan kesehatan mengenai faktor risiko dan pencegahan diabetes. Kegiatan Pengabdian kepada Masyarakat ini dilakukan pada 10 Maret 2024. Metode pembelajaran adalah ceramah, diskusi, demonstrasi dan return-demonstration. Media pembelajaran yang digunakan adalah PowerPoint dan alat peraga berupa kertas koran. Hasil dari kegiatan ini adalah mayoritas peserta memiliki IMT normal (41,71%) dan 35,58% mengalami overweight serta obesitas sebanyak 17,17%. Mayoritas peserta memiliki tekanan darah normal (60,73%), namun terdapat kasus pra-hipertensi dan hipertensi. Selain itu, mayoritas peserta memiliki kadar glukosa darah (52,14%) dan kadar kolesterol (69,32%) yang tidak normal. Intervensi kesehatan diperlukan untuk mengelola faktor risiko ini dengan pemberdayaan masyarakat dalam memodifikasi gaya hidup, melakukan pendidikan kesehatan secara berkelanjutan kepada masyarakat dan pemantauan rutin. Kata Kunci: Diabetes Melitus, Pendidikan Kesehatan, Pemberdayaan Masyarakat  ABSTRACT Diabetes mellitus is a chronic metabolic disorder and currently imposes a burden on the healthcare system. A multifaceted approach is needed to prevent and treat diabetes. Community empowerment is a process that enables people to increase control over their lives. Empowering the community is expected to help overcome diabetes risk factors, especially modifiable factors such as diet, physical activity, smoking habits, and alcohol consumption. This activity aimed to describe the health status, including BMI, blood pressure, blood glucose levels, uric acid, and cholesterol, and empowering the community by increasing knowledge through health education regarding risk factors and prevention of diabetes. This activity was conducted on March 10, 2024. The learning methods included lectures, discussions, demonstrations, and return-demonstrations. The learning media used were PowerPoint presentations and teaching aids in the form of newspapers. The results of this activity showed that the majority had a normal BMI (41.71%), while others were overweight (35.58%) or obese (17.17%). Most had normal blood pressure (60.73%), but some cases of pre-hypertension and hypertension were observed. Most had abnormal blood glucose levels (52.14%) and cholesterol levels (69.32%). Health interventions are needed to manage these risk factors through community empowerment, ongoing health education, regular monitoring, and lifestyle modifications. Keywords: Diabetes Mellitus, Community Empowerment, Health Education

Abstract

Diabetes Melitus (DM) tipe 2 merupakan penyakit gangguan metabolik ditandai dengan hiperglikemia akibat resistensi insulin sehingga sel beta pankreas tidak dapat berfungsi melakukan sekresi insulin dengan baik. Indonesia termasuk peringkat kelima di dunia dengan jumlah penderita DM tertinggi yaitu 19.5 juta kasus. Setiap tahun kasus penderita DM dan komplikasi semakin meningkat, salah satunya disebabkan oleh kurangnya pengetahuan dan kepatuhan penderita dalam melakukan manajemen nutrisi seperti jenis makanan, jumlah makanan, dan jadwal makanan yang menyebabkan kadar gula tidak terkontrol dan mempercepat terjadinya komplikasi. Tujuan penelitian ini adalah untuk mengidentifikasi gambaran pengetahuan dan kepatuhan mengenai manajemen nutrisi pada penderita DM tipe 2 di Wilayah Puskesmas Kelapa Dua. Metode Penelitian menggunakan metode kuantitatif dengan desain penelitian analisis deskriptif. Teknik sampling menggunakan nonprobability sampling dengan pendekatan accidental sampling dengan jumlah sampel sebanyak 54 responden. Uji statistik dengan analisa univariat. Hasil penelitian variabel pengetahuan menunjukkan sebanyak 41 responden (75,91%) memiliki pengetahuan baik terhadap manajemen nutrisi diabetic, sebanyak 44 responden (81,48%) patuh terhadap manajemen nutrisi diabetik.

Abstract

Type 2 diabetes mellitus (T2DM) management requires effective self-care, which can be strengthened through structured education. The Five Pillars of Diabetes Self-Care including nutrition, physical activity, medication adherence, blood glucose monitoring, and foot care provide a comprehensive framework for patient with T2DM. This study evaluated the effects of an educational program based on these five pillars on the knowledge and self-care behaviors of patients with T2DM. A quasi-experimental one-group pretest–posttest design was used. A total of 24 patients with T2DM from the Jalan Emas Community Health Center in Tangerang were recruited using accidental sampling. The intervention consisted of four weekly interactive educational sessions that covered all five pillars. Valid and reliable instruments (the Five Pillars Knowledge Questionnaire and the Summary of Diabetes Self-Care Activities/ SDSCA) were used for data collection. Data were analyzed using the Wilcoxon Signed-Rank Test. Post-intervention analysis showed significant improvements in knowledge related to nutrition management (p = 0.023), blood glucose monitoring (p = 0.008), and foot care (p = 0.003). Overall self-care behavior also increased significantly (p = 0.001). However, no significant improvements were observed in knowledge related to physical activity (p = 0.317) and antidiabetic medication (p = 0.053). The Five Pillars Diabetes Education program was effective in improving key areas of knowledge and self-care behavior among patients with T2DM. These findings support the adoption of structured, comprehensive, and practical educational interventions, while also emphasizing the need for more targeted strategies to enhance physical activity engagement and medication adherence.
