Abstract

Moral education is the core of the Islamic education system, which cannot be separated from scientific, spiritual, and social aspects. This article examines the contribution of Imam Al-Ghazali's thoughts in shaping a value-based Islamic education system and explores the challenges and opportunities for its implementation in the contemporary era. This study employs a qualitative method with a literature review approach, focusing on Al-Ghazali’s major works such as Ihya’ ‘Ulumuddin and Ayyuha al-Walad, as well as secondary literature from various related studies. The main findings indicate that Al-Ghazali places moral education as the foundation of the entire educational process, with tazkiyatun nafs as the central goal. He integrates knowledge and morality into a complementary value system and emphasizes the importance of the teacher’s role as a moral model. However, the implementation of this concept faces various obstacles, ranging from the dominance of cognitive curricula to the lack of spiritual guidance for educators. On the other hand, there are significant opportunities through the integration of values into the curriculum, the use of technology to strengthen character education, and collaboration with the community. This article recommends the renewal of Islamic education policies oriented toward values and spirituality as a response to the current moral crisis.

Abstract

Introduction: Surgical interventions during pregnancy present a complex clinical dichotomy requiring the balance of maternal physiological stability with fetal safety. Trauma necessitating ear reconstruction typically requires general anesthesia, which carries elevated risks of difficult airway management, aspiration, and potential neurotoxicity in the developing fetus. The "Ring Block" technique offers a regional alternative, yet evidence regarding its safety profile specifically in second-trimester trauma remains limited. Case presentation: We present the case of a 41-year-old multigravida at 14 weeks of gestation (ASA II) presenting with a complex traumatic laceration of the right auricle following a motor vehicle accident. Given the patient’s advanced maternal age and the risks associated with general anesthesia, including hemodynamic fluctuation and teratogenicity concerns, an awake ear reconstruction was planned. We utilized a landmark-based Ear Ring Block using 12 mL of 2% Lidocaine. The procedure achieved complete surgical anesthesia with a V-pattern and inverted V-pattern injection trajectory. Intraoperative monitoring revealed hemodynamic stability with no fluctuations in mean arterial pressure or fetal heart rate abnormalities. The patient reported a visual analog scale score of zero intraoperatively and was discharged 12 hours after surgery without complications. Conclusion: The Ear Ring Block represents a superior anesthetic modality for auricular trauma in pregnant patients. It effectively mitigates the physiological risks of general anesthesia while providing profound analgesia and hemodynamic stability. This technique should be considered a primary anesthetic strategy for auricular reconstruction in the obstetric population.

Abstract

Thermal Tensioning is one of the methods use to minimise the occurrence of residual stress and unwanted distorsion during welding. One of the thermal tensioning methods is Static Thermal Tensioning (STT), which involves countering thermal stress during welding. The STT method requires an active cooling behind the welding line. Several properties are required for the coolant material, such as thermal conductivity, heat resistance, and good flexibility. To achieve the desired properties, a mixture of different materials was used, commonly called a composite. The purpose of this study is to know the effect of filler metal composition and type on the heat resistance of silica rubber matric composites. This study used 2 independent variables: the filler composition in composites with variations of K1 (60%) and K2 (70%), and the type of filler metal, aluminium and copper powder with variation J1 (100%-0%), J2 (75%-25%), J3 (50%-50%), J4 (25%-75%), and J5 (0-100%). The testing process used Thermogravimetry Analysis (TGA) method with replication 2 times. The results of this study showed the specimens with K1;J1 composition, containing 60% pure aluminium filler metal, showed the best heat resistance with a final weight of 88,25%. While specimens with K2;J5 composition, containing 70% pure copper filler metal, showed the worst heat resistance with a final weight of 74,25%.

Abstract

Introduction: Maternal cardiac disease, specifically right-sided valvular lesions exacerbated by pulmonary hypertension, remains a primary driver of maternal mortality. The physiological demands of pregnancy act as a cardiovascular stress test, often leading to decompensation in patients with underlying pathology. This case describes the management of a triple-pathology parturient. Case presentation: A 37-year-old female (G2P1A0) at 34 weeks’ gestation presented with NYHA Class IV symptoms, including progressive dyspnea and orthopnea. Echocardiography revealed severe tricuspid regurgitation (regurgitant volume 112 mL), right ventricular dilatation, and a high probability of pulmonary hypertension with a mean pulmonary arterial pressure of 50.39 mmHg and a systolic pulmonary arterial pressure of 79.32 mmHg. Systemic neurofibromatosis added concerns regarding neuraxial anatomy and airway management. An emergency Cesarean Section was performed under a graded epidural technique using 0.375 percent Levobupivacaine and 50 mcg Fentanyl, administered in 3 mL increments every 5 minutes. Hemodynamic stability was maintained through strict fluid restriction of 300 mL and titrated vasopressors. Conclusion: A carefully titrated graded epidural provides superior stability in the hostile hemodynamics of right heart failure by allowing a slow, compensatory sympathetic blockade. Early multidisciplinary coordination is essential for success in complex cardio-obstetric cases.

Abstract

Background: Eclampsia complicated by severe thrombocytopenia presents a critical anesthetic challenge, often representing an absolute contraindication to neuraxial anesthesia. When alternative approaches are mandated, General Anesthesia with Rapid Sequence Induction or a modified rapid sequence approach is crucial for high-risk obstetric emergencies. Case presentation: A 16-year-old primigravida at 35 weeks gestation presented with eclampsia, acute fetal distress, and severe thrombocytopenia. Initial vitals showed a blood pressure of 150/98 mmHg. Laboratory findings confirmed severe preeclampsia with a critical platelet count of 42,000/µL. An airway assessment revealed a Mallampati class II airway with mild pharyngolaryngeal edema. Due to the high risk of spinal epidural hematoma, neuraxial anesthesia was contraindicated. An emergency cesarean section was performed using a modified rapid sequence induction. Following preoxygenation, induction was achieved with fentanyl 100 µg, midazolam 3 mg, and propofol 100 mg. Due to institutional unavailability of rocuronium, atracurium 30 mg was utilized. After a 2.5-minute onset interval utilizing apneic oxygenation and continuous cricoid pressure, the patient was successfully intubated on the first attempt with a Macintosh size 3 blade. The intraoperative course was hemodynamically stable. Conclusion: A modified rapid sequence induction utilizing atracurium provides an effective alternative for airway control and physiological stability in eclamptic adolescents with coagulopathy, particularly in resource-limited settings where standard rapid-acting non-depolarizing agents are unavailable.
