Abstract

Program pendampingan Self Help Group (SHG) telah digunakan sebagai intervensi psiko-sosial untuk meningkatkan kepatuhan pengobatan dan menurunkan viral load pada Orang dengan HIV/AIDS (ODHA). Namun, efektivitas SHG dalam meningkatkan kepatuhan minum antiretroviral (ARV) dan menurunkan tingkat viral load masih menunjukkan variasi hasil antar studi. Oleh karena itu, dilakukan systematic review untuk menganalisis pengaruh program SHG terhadap tingkat viral load dan kepatuhan minum ARV pada ODHA.Kajian ini menggunakan desain systematic review dengan panduan PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses). Literatur dicari melalui empat basis data: Scopus (n=34), ScienceDirect (n=3.945), ProQuest (n=5), dan Sage Journal (n=5), dengan total hasil pencarian awal sebanyak 12.365 artikel. Kata kunci yang digunakan: ("Self Help Group" OR "Support Group") AND ("Viral Load" OR "HIV Suppression") AND ("ARV Adherence" OR "Medication Compliance") AND ("PLHIV" OR "HIV Patients"). Seleksi dilakukan berdasarkan inklusi studi intervensi terhadap ODHA yang menilai dampak SHG terhadap kepatuhan dan viral load. Dari hasil penyaringan, diperoleh sejumlah artikel yang memenuhi kriteria dan menunjukkan bahwa keterlibatan dalam SHG secara konsisten berkorelasi positif dengan peningkatan kepatuhan minum ARV serta penurunan viral load. Beberapa studi juga menunjukkan peningkatan dukungan sosial, pemahaman penyakit, dan pemberdayaan individu sebagai mediator utama. Program SHG terbukti berpengaruh positif terhadap peningkatan hasil klinis ODHA, khususnya dalam aspek kepatuhan pengobatan dan kontrol virologis. SHG direkomendasikan sebagai intervensi tambahan dalam layanan HIV terintegrasi di berbagai setting layanan kesehatan. Penerapan SHG juga mendukung manajemen rumah sakit dalam meningkatkan kualitas pelayanan komprehensif serta memperkuat sistem dukungan pasien dalam continuum of care

Abstract

Background: The use of e-health is now widely used as a new intervention e-health can be applications in smart phones, smart watches, or wearables, as well as telemedicine, interactive video and web-based interventions. Purpose: Submitting the results of exploration related to the impact of e-health use in increasing physical activity and changes in BMI in adults with obesity. Method: Literature reviews on several databases such as clinical key nursing, sage journal, and Pubmed using PRISMA methods. Results: This systematic review study proves that eHealth has an influence on physical activity and BMI in adults with obesity. From the 7 articles that used e-health as an intervention in changing physical activity and BMI showed significant results against changes in physical activity, BMI despite using different types of eHealth, such as virtual reality, smartphone applications/wearable devices and telehealth. Conclusion: The use of e-health can be recommended as part of a body weight management strategy as it has a good impact over time and needs to be integrated with nursing interventions so that nurses can monitor patient progress and adjust recommendations based on the data collected.

Abstract

Background: Integrated primary care (IPC) is recognized as essential for strengthening health systems. However, Indonesia currently lacks a validated instrument to assess stakeholder support, which is a critical determinant of successful IPC implementation. This study evaluated the content validity, internal structure, and reliability of the newly developed Stakeholder Support Questionnaire for Integrated Primary Care (SSQ-IPC), grounded in Freeman’s Stakeholder Theory. Methods: A cross-sectional psychometric validation was conducted with 570 primary healthcare nurses recruited through stratified random sampling across Indonesia. The initial 18-item instrument, encompassing four constructs, underwent content validation by a panel of seven experts. Data analysis included calculating the Content Validity Index (CVI), conducting exploratory factor analysis (EFA), confirmatory factor analysis (CFA), and second-order CFA. Model fit was assessed using multiple fit indices, while construct validity and reliability were evaluated through factor loadings, Average Variance Extracted (AVE), Cronbach’s alpha, and McDonald’s omega. Results: EFA supported a four-factor structure. First-order CFA indicated acceptable model fit (CFI = 0.933, TLI = 0.921, SRMR = 0.037), with all factor loadings statistically significant (p<.001) and strong indicator reliability. Convergent validity was established with AVE values ranging from 0.650 to 0.869, and internal consistency was high across constructs (α= 0.877–0.978; ω = 0.884–0.973). Second-order CFA supported a hierarchical model, with all four constructs significantly loading onto a higher-order factor. Conclusion: The SSQ-IPC demonstrates strong evidence of validity and reliability, supporting its multidimensional and hierarchical structure.
