Abstract

Abstract

Introduction: Recessive dystrophic epidermolysis bullosa (RDEB) is a rare bullous disease caused by mutations in the COL7A1 gene encoding anchoring fibril collagen that maintains skin integrity. Case: Male, 67-year-old, presented with blisters and wound on various parts of the body, worsening in the recent year. They had been known to easily appear after trauma since. Vesicles-bullae, erosions-excoriations, milia, and atrophic scars were found on the axillae, groins, and limbs, as well as hyperkeratosis and anonychia of the hands and. Histopathological examination showed subepidermal cleft, milia and fibrosis, without immunoglobulin deposits on immunofluorescence. Discussion: Contrary to classical RDEB, blisters in mitis variant do not always cause impairment. The pronounced symptoms in advanced age might be mistaken for epidermolysis bullosa acquisita. Negative immunoglobulin deposition is the diagnostic key, confirming that blister does not result from an autoimmune process, but rather, from trauma to the abnormal skin, more so by flattening of the rete ridges in the elderly. In the absence of definitive therapy, treatment is symptomatic. Conclusion: The diagnosis RDEB-mitis might be missed in geriatric. Correct diagnosis has implication on management because not only does RDEB-mitis require no immunosuppressant, measures to minimize blister formation is more important to save patient from complication.

Abstract

Hydroa vacciniforme (HV) is a rare photodermatoses with the most frequent onset in childhood. The exact pathogenesis of HV is still not known. This disease is a spectrum with almost similar clinical symptoms, but with a very different prognosis. Classic HV has the best prognosis, but affects the quality of life due to scar formation, while hydroa vacciniforme-like lymphoma (HVLL) is the variant with the worst prognosis. HV needs to be considered as a differential diagnosis, especially in childhood-onset photodermatoses. Classical HV must be distinguishes from hydroa vacciniforme-like lymphoproliferative disorders (HVLPD) because it is related to prognosis. There is still no consensus or guidelines on diagnosis and management for HV.

Abstract

Manifestasinya bervariasi, namun manifestasi eritema multiforme (EM) bulosa jarang dilaporkan.

Abstract

Abstract

Introduction: Green nail syndrome is a common bacterial infection of the nails and is characterized by green discolouration of the nail plate caused by Pseudomonas aeruginosa. It frequently occurs in people who are exposed to constant water and due to mechanical trauma, especially in elderly patients. Case Illustration: We reported 2 cases treated with a combination therapy of oral ciprofloxacin, 1% acetic acid dressing, gentamicin ointment and showed a significant clinical improvement. There are two cases: a 67-year-old man and a 64-year-old woman complained of black-greenish discolouration on his right big toenail, which has been accompanied by changes in the shape of the nail for 3 months. Both patients have a history of frequent and prolonged contact with water—dermoscopy results in greenish-black discolouration in the nail plate.  The culture results of both patients, Pseudomonas aeruginosa was found. Combination treatment showed significant clinical improvement in both patients. Discussion: Dressing of 1% acetic acid was useful as an adjuvant to improve the ointment penetration, inhibiting the formation of Pseudomonas aeruginosa biofilms. Gentamicin and ciprofloxacin are effective against aerobic gram-negative bacterial infections, such as Pseudomonas aeruginosa. Conclusion: The use of ciprofloxacin is recommended, especially in elderly patients. Combination therapy with oral ciprofloxacin, 1% acetic acid dressing, and gentamicin ointment is an effective option for patients with green nail syndrome

Abstract

BACKGROUND Pressure injury develops due to sustained pressure at the bony prominence of the skin and tissues. Geriatric patients often have multiple comorbidities, predisposing them to pressure injury. Data on the characteristics of the geriatric with pressure injuries are still limited. This study aimed to report the characteristics of geriatric patients with pressure injuries admitted at a tertiary hospital in Indonesia. METHODS This cross-sectional study used medical records of geriatric patients admitted with all stages of pressure injuries consulted to the Department of Dermatology and Venereology at a tertiary hospital in Indonesia between January 2017 and April 2021. Pressure injuries were classified based on the 2019 National Pressure Injury Advisory Panel guideline. RESULTS 39 patients presented with varied pressure injury characteristics. The sacral region was the most reported site (36%), with stage 2 pressure injury being the most commonly found stage in the patients (74%). Interestingly, 22% of the patients had pressure injuries found on the atypical sites due to prolonged surgery or the pressure of medical devices. More than half of the patients used conventional dressings (51.3%). Immobility was found in 35.9% of the patients due to being bedridden. CONCLUSIONS No characteristics were found as a significant risk factor for pressure injury formation during or outside the admission period. However, a history of surgery might be related to pressure injury formation during admission.

Abstract

Background: The skin undergoes degenerative and metabolic changes with aging, increasing susceptibility to dermatological conditions. Xerosis cutis, characterized by rough, scaly skin on the lower legs, is the most common cause of pruritus in elderly patients, with a prevalence of 38–85%. Treating xerosis cutis is challenging due to its multifactorial causes, especially in the elderly. Moisturizers remain the primary and most effective treatment, providing convenience and efficacy. This study aimed to evaluate the effects of QV Intensive Moisturizing Cleanser™️ and QV Intensive Cream™️ (Ego Pharmaceuticals, Australia), which contain humectants, emollients, and occlusives. Case Illustration: This observational case series involved six elderly subjects (>60 years) with xerosis cutis. Exclusion criteria included medications that cause dry skin, use of topical moisturizers within 2 weeks, or oral supplements (hyaluronic acid, vitamin C, and ceramide) within 4 weeks before the study. Over four weeks, subjects used only the specified cleanser and moisturizer on affected skin. Improvement was assessed subjectively using the Smoothness, Redness, Roughness, Cracking (SRRC) score, the Dermatology Life Quality Index (DLQI), and the 5D Pruritus Scale. Objective measures included transepidermal water loss (TEWL) via Tewameter® TM300 and skin capacitance (SCap) via Corneometer® CM825 (Courage+Khazaka Electronic GmbH, Germany). Discussion: All subjects showed improvement in SRRC, 5D Pruritus Scale, DLQI, TEWL, and SCap after four weeks of cleanser and moisturizer use, with no side effects or irritation observed. Conclusion: Cleansers and moisturizers containing humectants, emollients, and occlusives were found to be safe and effective for treating xerosis cutis in geriatric patients.
