Abstract

Introduction : Post surgical pain is an important problem in the process of operation, so that a strong analgesia is needed in the post-surgery. Already popular opioid used as an analgesic tramadol hydrochloride one, but use a single dose require large doses resulting in severe side effects. Preemptive method is one method to produce a satisfactory analgesia. In this study aimed to compare the combination of tramadol hydrochloride 2.5mg / kg / IV and oral clonidine 75•g for pain management after orthopedic surgery extremities.Methods : After the approval of the ethics committee of FK-USU, 45 samples were collected, men and women, from the age of 18- 40 years with ASA physical status 1-2, 1-4 preoperative VAS, who underwent elective orthopedic extremity surgery with general anesthesia Adam Malik Hospital. Samples were then randomly divided into 2 groups with 23 subjects each. This study used a randomized controlled clinical trial double-blind. Group A received tramadol hydrochloride 2.5mg / kg / IV IV and 75 mg oral clonidine. Group B received tramadol hydrochloride 2.5mg / kg / IV and placebo. Systolic and diastolic blood pressure, pulse frequency VAS values were recorded and measured. All data were analyzed using chi-square test and the crosstab. Results : VAS value in hours and hours to 6 to 8 significant difference clinically and statistically significant in both groups, with P = 0:02 on the clock to 6 and P = 0.00 at 8 hours.Conclusion : The result showed that a combination of tramadol hydrochloride with oral clonidine produces better analgesic than tramadol hydrochloride.Keywords : tramadol hydrochloride; klonidine; visual analogue score (VAS)

Abstract

This research presents a novel approach utilizing Fuzzy Logic Mamdani-based simulation implemented through MATLAB and SolidWorks for sorting Solanum Lycopersicum fruits to achieve premium fruit quality. By integrating advanced computational tools, the simulation emulates intricate sorting processes, enabling precise decision-making akin to human judgment. This innovative methodology offers a promising avenue for improving fruit sorting efficiency and product quality in agricultural industries. This simulation system uses loadcell sensors, color sensors and PIR sensors as input. These inputs are used to control three servos and one DC motor. The fuzification process is carried out on both the input and output sides. Based on the results of the fuzification process, eighty-one rules were obtained and a fuzzy inference process was carried out using the intersection method. After carrying out the fuzzy inference process, it is continued with the defuzzification process. The result of the defuzzification value on the quality output is 7324.99 in the high set and the result of the simulation of the quality output value is 7250 in the high set. Thus, the accuracy level of the simulation's results is 98.98%. The simulation results show that the system is able to sort low, medium and high quality Solanum lycopersicum fruit.

Abstract

Abstract: Dementia is a progressive and degenerative clinical syndrome, that affects memory, thinking ability, behaviour, emotions and personality that interfere with daily life activities. The aetiology of dementia is linked to the involvement of vascular, inflammatory, oxidative stress and neurotoxicity that causes neurodegenerative brain pathologies that can contribute to cognitive impairment and dementia. Multidomain interventions targeting multiple risk factors for effective prevention including dietary supplements from traditional plants. Nigella sativa (NS) is a traditional plant native to the Mediterranean Area and Southwest Asia that has long been used to maintain human health. Nigella Sativa has a significant content of thymoquinone (TQ) as a bioactive element of 30-48% of the total NS compounds. Several in vitro and in vivo preclinical studies and clinical trials that are still limited to non-dementia humans show that TQ has a positive effect on cognitive impairment through oxidative stress control mechanisms, vascular health, anti-inflammatory, maintenance of nerve cell integrity and improvement of neurotrophic regulatory factors which are promising in preventing cognitive decline. Nigella sativa serves as a potential dietary supplement to prevent the progression of Alzheimer's disease but requires long-term investigation and follow-up research to test safety and toxicity and compare its effectiveness at different stages of dementia such as Alzheimer's disease.

Abstract

Abstract: Myopia was found to be a common cause of headaches, and about 28% of the patients studied had myopia. This study was conducted with the aim of finding out the relationship between myopia and headache in students with the target students of the Faculty of Medicine, Universitas Muhammadiyah Sumatera Utara. This study is an analytical observational research with a cross-sectional approach involving 72 research subjects of students of the Faculty of Medicine, Universitas Muhammadiyah Sumatera Utara, and using an online questionnaire with a Google form application to obtain primary data.  Of the group of students who suffered from mild myopia, 23.5% suffered from mild headaches, 61.8% had moderate headaches, and 14.7% had severe headaches. Meanwhile, in the group of students who suffered from moderate myopia, 6.9% suffered from mild headaches, 58.6% moderate headaches and 34.5% severe headaches. For the group of students who suffer from severe myopia, no one experiences mild or moderate headaches, but there are 100% suffer from severe headaches. The Chi Square test shows the sum of 0.000. It can be concluded that myopia is related to headaches in medical students of the Universitas Muhammadiyah Sumatera Utara.

Abstract

Human beings were created by Allah with complex and complex structures and functions. The more we try to dig about the brain, it will never end. Therefore, everything that Allah made every part of us is a sign of Allah's greatness that should be our contemplation together. There is a term that states that understanding how the brain works is the main way to uncover the very essence of human beings. Character (morals or behavior) is the most important thing that must be owned by every human being and we must observe and analyze that the brain is the center of human character. The brain is part of the composition of the central nervous system that works as a center for controlling human activities in the form of thinking, moving, behaving, feeling something, remembering, and so on. Without the brain, humans will not live, like inanimate objects, without any use and potential. If we talk about interactions with the Qur'an that humans do such as reading, memorizing, listening, meditating or contemplating the meaning of the Qur'an, then this will affect the function directly or indirectly even the structure and function of the brain itself microscopically cellular can be affected. Many studies have been conducted to prove how the effects of the Qur'an from listening, reading, memorizing the Qur'an to brain function, especially in terms of brain waves, mental health, and so on. Health effects can be obtained very much before we interact with the Qur'an. If we analyze simply the stages of a person's activity before interacting with the Qur'an also indirectly has a positive impact on the body.

Abstract

This study aimed to compare the onset and duration of analgesia in spinal anesthesia for cesarean section using hyperbaric bupivacaine combined with fentanyl or with meperidine. A double-blind randomized controlled trial was conducted involving sixty patients undergoing either elective or emergency cesarean section at Jejaring Hospital and H. Adam Malik General Hospital Medan. Patients were randomly assigned to receive hyperbaric bupivacaine with fentanyl or hyperbaric bupivacaine with meperidine, and the data were analyzed using appropriate statistical methods to examine differences between the groups. The findings revealed that the combination of bupivacaine with fentanyl produced a faster onset of analgesia compared to bupivacaine with meperidine, whereas the combination with meperidine provided a longer duration of analgesia. Both combinations were effective in achieving intraoperative and postoperative pain control, each offering distinct advantages. The study concludes that the choice of spinal anesthesia combination can be tailored to the clinical needs of patients, with fentanyl being preferable for rapid onset and meperidine offering prolonged analgesia.

Abstract

Perdarahan intrakranial spontan pada terapi antikoagulan rivaroxaban mulai sering ditemukan, apalagi bila dikombinasi dengan antiplatelet clopidogrel maupun fibrinolitik lumbrokinase merupakan hal yang sering ditemukan pada penderita trombosis. Perdarahan intrakranial spontan terbanyak adalah perdarahan intracerebral. Perdarahan spontan epidural (EDH) akut merupakan hal yang jarang ditemukan, biasanya terjadi karena ada penyakit yang mendasarinya. Penatalaksanaan pada kasus ini berdasarkan patofisiologinya dan melibatkan multidisiplin ilmu lainnya. Seorang laki-laki, 26 tahun berat badan 80 kg yang didiagnosa deep vein thrombosis (DVT) mengalami penurunan kesadaran mendadak, pupil anisokor 4 mm/1mm ketika sedang beraktivitas. Tidak dijumpai riwayat cedera kepala. Setelah diresusitasi didapatkan hasil head CT-scan dengan EDH temporoparietal dextra 50 cc, dilakukan dekompresi craniektomi dan evakuasi EDH. Setelah 10 jam pasca operasi terjadi gejolak hemodinamik dan dilakukan head CT-scan ulang dan didapatkan EDH 80 cc dan minimal perdarahan intracerebral. Dilakukan redo craniektomi. Pasca operasi dirawat di ICU dengan koreksi faktor koagulasi. Pasien kembali komposmentis GCS 15 dengan gejala sisa hemiparese sinistra sementara.Acute Spontaneus Epidural Hemorrhage due to Combination Therapy Rivaroxaban, Clopidogrel and LumbrokinaseSpontaneous intracranial hemorrhage on anticoagulant therapy rivaroxaban lately often found, especially when combined with clopidogrel antiplatelet or fibrinolytic lumbrokinase is often found in patients with thrombosis. Spontaneous intracranial hemorrhage is most widely occured is intracerebral hemmorrhage. Spontaneous epidural hemorrhage (EDH) acute is a uncommon, usually occur because there is an underlying disease. Treatment on the case based on patophysiology and involves a multidisciplinary peer other sciences. A young man, 26 years old weight 80 kg which was diagnosed with deep vein thrombosis (DVT) awareness of sudden decline, the pupil anisokor 4 mm/1mm while activity. No head trauma history. After resuscitation, head Ct-scan with EDH temporoparietal dextra 50 cc, carried out the evacuation EDH and decompression craniektomi. After 10 hours of post-operative haemodynamic turmoil happened and done a head ct-scan and obtained EDH 80 cc and minimal intracerebral hemorrhage. Do redo craniektomi. Post-operative hospitalized in ICU with correction factor for coagulation. The patient recovers conciousness into composmentis GCS 15 with sequelae hemiparese sinistra temporary.
