Abstract

Usually, a modern industry requires theproduction keep going on with the least interrupt as possible,butfrequently, the unscheduled maintenance will waste money in a large amount which is allocated in amodern industry needs an enormous investment. Machines with good condition generally produce a littlerelative of vibration but with increase in service life, the vibration of machine will increase. In somecondition, the increase in quite drastic vibration probablymay occur. Thisphenomenon is an indicator thatthe machine must have an immediate repair. This vibration spectrum analysis is tried to conductedfor thetransmission system of differential gear. By means to use the alternative tools beside DSA (Dynamic SignalAnalyzer), that is CREATIVE SOUNDBLASTER AUDIGY LS5.1 and MATLAB 6.5 whichhave Fast FourierTransform facility and so, can to know the real condition gear by time domain andfrequency domain. Abstract

Kreatinin serum memiliki banyak keterbatasan dalam mendiagnosis cedera ginjal akut (CGA) terutama dalam ruang lingkup unit perawatan intensif sehingga kurang sensitif untuk menggambarkan tingkat disfungsi ginjal pada pasien sakit kritis. Dari sekian banyak penanda biologis baru yang tersedia, terdapat 4 penanda biologis yang saat ini secara luas digunakan para klinisi di seluruh dunia untuk mendeteksi CGA, antara lain neutrophil gelatinase-associated lipocalin (NGAL), cystatin C, KIM-1, dan interleukin-18. Tujuan penelitian ini adalah mengetahui sensitivitas dan spesifisitas cystatin C dan kreatinin serum dalam mendiagnosis cedera ginjal akut pada pasien sepsis yang dirawat di ruang rawat intensif. Jenis penelitian ini adalah uji diagnostik dengan jumlah sampel 24 pasien dan dikerjakan di ruang rawat intensif Rumah Sakit H. Adam Malik (RSHAM) periode Februari–Maret 2014. Populasi dalam penelitian ini adalah semua pasien dewasa dengan sepsis, sepsis berat, dan syok sepsis di ruang rawat intensif. Uji statistik dilakukan dengan metode receiving operator characteristics (ROC) melalui perangkat lunak SPSS 17. Hasil penelitian ini menunjukkan bahwa cystatin C serum lebih superior dibanding dengan kreatinin serum dalam mendeteksi CGA pada pasien sepsis di ruang rawat intensif. Cystatin C memiliki sensitivitas, nilai prediksi positif, nilai prediksi negatif, dan area under curve -receiving operator characteristics (AUC-ROC) yang lebih tinggi dibanding dengan kreatinin serum. Namun, dari nilai spesifisitas dijumpai nilai yang sama pada kedua penanda biologis. Cystatin C serum memiliki nilai cut-off 1,03 mg/L, sedangkan kreatinin serum mempunyai nilai cut-off 1,0 mg/dL. Simpulan, cystatin C dapat dijadikan penanda biologis alternatif untuk deteksi CGA pada pasien sepsis di RRI (Ruang Rawat Intensif) dengan nilai diagnostik yang lebih baik.Kata kunci: Cedera ginjal akut, cystatin C serum, kreatinin serumSensitivity and Specificity of Serum Cystatin C and Creatinine for Diagnosing Acute Kidney Injury in Sepsis Patients Treated in Intensive Care Unit at H. Adam Malik Hospital MedanSerum creatinine has many limitations when being used to diagnose acute kidney injury (AKI), especially in the scope of intensive care unit due to its low sensitivity to depict the kidney dysfunctional level of critically-ill patients. Out of numerous new available biological markers, four biological markers are widely used all over the world to detect AKI, e.g. neutrophil gelatinase-associated lipocalin (NGAL), cystatin C, KIM-1, and interleukin-18. The purpose of this study was to determine the sensitivity and specificity of serum cystatin C and creatinine in establishing the diagnosis of acute kidney injury in sepsis patients treated in intensive care room. This study was a diagnostic test to 24 patients and conducted in intensive care room of H. Adam Malik Hospital during the period of February–March 2014. Population in this study is all adult patients with sepsis, severe sepsis, and septic shock in the intensive care room. A statistical test was conducted using receiving operator characteristics (ROC) method using SPSS 17 software. The result of this study showed that serum cystatin C was more superior than serum creatinine in detecting acute kidney injury in sepsis patients treated in intensive care room. In this study, cystatin C had higher sensitivity, positive prediction score, negative prediction score, and area under curve -receiving operator characteristics (AUC-ROC) than serum creatinine. As of specificity, there was no significant difference between these two biological markers. Serum cystatin C had a cut-off point of 1.03 mg/dL, and serum creatinine had 1.0 mg/dL. In conclusion, cystatin C can be an alternative biological marker to detect AKI in sepsis patients treated in intensive care room with a better diagnostic value.Key words: Acute kidney injury, serum creatinine, serum cystatin C DOI: 10.15851/jap.v4n2.819

Abstract

Perubahan mendadak laju filtrasi glomerulus (LFG) pada pasien sakit kritis dengan sepsis tidak diikuti secara paralel dengan perubahan kreatinin serum. Tujuan dari penelitian ini adalah membandingkan kegunaan dari cystatin C serum dan kreatinin serum sebagai penanda biologis fungsi ginjal pada pasien sepsis di ruang rawat intensif (RRI). Sebuah studi cross-sectional dilakukan pada pasien dewasa usia 18–65 tahun di RRI RSUP Haji Adam Malik. Kreatinin serum, cystatin C serum dan creatinin clearance (CrCl) 24 jam urin diobservasi pada 24 pasien sepsis. CrCl 24 jam urin yang disesuaikan dengan luas permukaan tubuh digunakan sebagai “baku emas” untuk menentukan LFG. Kreatinin serum, cystatin C serum dan CrCl 24 jam urin (nilai rata-rata ± standar deviasi [range]) adalah 1,53 ± 1,13 mg/dL (0,3–4,2 mg/dl), 1,71 ± 1,1 mg/L (0,6–4,48 mg/L), dan 66,33 ± 37,77 ml/min/1,73 m2 (4–137 mL/min/1,73 m2). 17 dari total 24 pasien mengalami CGA. Cystatin C serum memilki nilai sensitivitas dan spesifisitas sebesar 82,4% dan 85,7%. Sedangkan kreatinin serum memiliki nilai sensitivitas dan spesifisitas sebesar 52,9% dan 85,7%. Cystatin C secara diagnostik lebih superior dibandingkan kreatinin serum dengan area under the curve (AUC) 0,874 untuk cystatin C serum dan 0,785 untuk kreatinin serum. Cystatin C serum dengan nilai cutt-off 1,03 mg/L dan kreatinin serum dengan cutt-off 1,0 mg/dL memiliki sensitivitas dan spesifisitas yang sama yaitu 82,4% dan 85,7%. Cystatin C adalah penanda biologis yang akurat dalam mendeteksi perubahan akut pada LFG, dan terbukti lebih superior dibandingkan kreatinin serum dalam mendiagnosa CGA pada pasien sakit kritis. The aim of the present study was to compare the utility of serum cystatin C and serum creatinin as a marker of renal function in these patients.A cross-sectional study was conducted in adult patients among 18-65 years in the intensive care unit Haji Adam Malik hospital. Serum creatinine, serum cystatin C and 24-hour creatinine clearance (CrCl) were observed in 24 critically ill patients with sepsis. Twenty-four-hour body surface adjusted CrCl was used as a control because it is the ‘gold standard’ for determining GFR.Serum creatinine, serum cystatin C and CrCl (mean ± standard deviation [range]) were 1.53 ± 1.13 mg/dL (0.3–4.2 mg/dl), 1.71 ± 1.1 mg/l (0.6–4.8 mg/l), and 66.33 ± 37.77 mL/min per 1.73 m2 (4–137 mL/min per 1.73 m2), respectively. Of the total 24 patients, 17 patients had AKI. Serum cystatin C has a sensitivity of 82,4% and spesificity value of 85,7%. Serum creatinin has a sensitivity and spesificity value of 52,9% and 85,7%. Cystatin C was diagnostically superior to creatinine (area under the curve [AUC] for cystatin C 0.874 and for creatinine 0,785. Serum cystatin C with cutt-off value 1,03 mg/L and serum creatinin with cutt-off value 1,0 mg/dl has the same sensitivity and spesificity of 82,4% and 85,7%, respectively. Cystatin C is an accurate marker of subtle changes in GFR, and it may be superior to creatinine when assessing this parameter in clinical practice in critically ill patients. Abstract

Literasi ekologi sosial Islam adalah interaksi manusia dengan lingkungan alam, teknologi, dan sosial yang didasarkan pada prinsip dasar Islam. Rekonstruksi literasi ekologi sosial Islam yang bisa direkonstruksi adalah prinsip dasar Islam yang menegaskan posisi manusia sebagai “pemimpin” yang diberi “amanah” untuk mengelola “bumi” atau “lingkungan alam dan sumber daya alam” sebaik-baiknya. Rekonstruksi literasi ekologis inilah yang kemudian akan diaktualisasikan pada masyarakat. Proses aktualisasi adalah kegiatan aktual dalam menanamkan kesadaran ekologi sosial Islam pada masyarakat yang mana dilakukan dalam ruang sosial keluarga, masyarakat, dan sekolah yang diorganisasi oleh negara melalui kebijakan dan peraturan per undang-undangan. Dengan proses rekonstruksi dan aktualisasi yang terstruktur ini, maka negara akan aktif membangun kesadaran ekologis sosial Islam dengan aktif dan terstruktur dengan baik guna mewujudkan basis kesadaran, ilmu pengetahuan, dan tata nilai ekologi sosial Islam pada masyarakat.

Abstract

TK Masyithoh 25 Sokaraja library has an important role in developing the reading interest of their students. The activity which is developed is library literacy. Through this library literacy TK Masyithoh 25 Sokaraja is able to develop children's interest in reading. The library literacy activities which developed are the activities of borrowing and returning books, story telling activities for children, and literacy activities of reading aloud. The three activities in turn programmed are followed by students of TK Masyithoh 25 Sokaraja. Through these three activities, students of TK Masyithoh 25 Sokaraja can increase their reading interest which is characterized by: the child's needs for reading books, actions to looking for reading sources ; pleasure in reading; interest in reading; the desire to always read; and follow-up (following up on what they read).

Abstract

After conducting research with an in-depth study of the moral aspects with the perspective of Islamic Archipelago in the children’s story on Bobo Magazine and Kompas Daily, two substantial findings were found. First, the dominant moral values in children’s stories are the value of hard work, honesty, obedience, responsibility, friendship, courage, discipline, compassion, sharing, and creativity, all of which are related to children’s moral formation. Second, values moral stories of children in the context of the Islamic archipelago related to the relationship of children’s moral values in children’s stories in the context of Islam based on the Al-Qur’an and Al-Hadits and the context of children’s moral values in children in the context of Indonesian humanity and Archipelago culture.

Abstract

Humans are gifted by God with eight intelligences (multiple intelligences). They are: linguistic, logical-mathematical, spatial, bodykinesthetic, musical, interpersonal, intrapersonal, and naturalist. Adult (parent) and children also need to understand the concept of the holistic intelligence. People can start early.Children are like stories (homo fabulans) and beings who loved playing (homo ludens). So, to be able to develop intelligence to the fullest, parents need a tool that can trigger such creativity. Fairy tale is one tool that can be used to reach it. In fairy tales summarized the various aspects required in the process of development of intelligence such as language (linguistic), the storyline (the logic of the story), social sensitivity (interpersonal and intrapersonal), conflict (the rhythm), imagination (visual/spatial), movement (kinesthetic), and nature (naturalist) that can be used as a real picture or the prototype for the child to get to know aspects of intelligence they have.

Abstract

Abstract: Creative learning is learning that rets on four basic foundation, namely learning fun designed for children; learn to be more than getting a good value, but is to produce work in the form of learning: intellectual, creative permormance, and educational activities; learning always appreciated the value and the publication of the teacher; and learning that can improve the child's motivation to learn. With four foundsations above, this creative learning will be formed: children are intellegent, that who could resolve the issue of learning in accordance with the breadth of his knowledge; a creative child, the child can actualize ideas and feelings in concrete; and children who have personality and good character in their daily lives. Abstract

Abstract : Learning the story means the active action, interactive, and creative among (young) students with teachers in understanding the story as a medium of instruction, the stories read or recited by teachers for students. This confirms that the story as a learning medium used to establish interaction between pupils with the teacher, so that the active and creative interaction, children can gain knowledge related to an increase in intellectual, affective, psychomotor, linguistic, and social sensitivity of students. Creating the next generation of human beings who have a love for nature and the environment are important, one way that can be taken is to create contextual language learning that prioritizes the side effect of implanting the love to the environment since childhood. Abstract

In this era, writing is not enough to be done expressively with personal spontaneity, but need to be done collaboratively by considering aspects of the desired goals. One of them is when writing is done to fulfill economic goals as the basis of a community's creative industry. With the concept of a creative industry whose estuary is to produce economic products made from creative ideas, writing in Wadas Kelir Literacy Community (KLWK) is carried out with a planned collaborative working model based on creative industry standards of economic value. From here, the results of research that have been carried out are: first, writing collaboration begins with market research studies in an effort to identify trends in children's books that are being sought after by publishers and readers. Second, the formulation of themes and ideas is carried out in the discussion space and cooperation for the division of writing work. Third, the results of the writing work are reviewed and edited by a team of experts to identify the strengths and the weaknesses. Fourth, the book manuscript offer to publishers is carried out by a separate management who continues to carry out partnership work with publishers. Fifth, involved in efforts to distribute book sales through online stores that create trust in the community of publishers.
