Abstract

The hospital is a health care institution that conducts complete individual health services and provide inpatient, outpatient and emergency services. Medical record is a unit to collect data patient, processing data, and reporting data that will be an information about performance of the hospital for making decision. In the implementation, competent officers are needed, so that patient services can be carried out optimally and not be hampered by workload in the medical record unit (Zebua, 2016). Pertamina Central Hospital is a plenary hospital managed by PT Pertamina Bina Medika. In 2014-2016 the visitor of outpatient and inpatient was 1157662 visite. In 2017 to 2018, patient visits increased by 21109 visits or 9%. Based on a preliminary survey, the schedule of filling officer is divided into 3 shifts (morning, afternoon, and night). The filling officer has overload working time of 1.5-2.5 hours. The overload working time can be a cause of work accidents. According to Eko (2016), service activities in medical record unit will run smoothly if human resources are sufficient by human needs, workload determined, and based on the standard. In this research is using WISN (Workload Indicator Staffing Needs) method. WISN is a method used to calculate labor needs based on workload. In this method there are 5 stages, selecting the HR category, calculating available work time, calculating workload standards, calculating leeway factors, and calculating labor requirements.

Abstract

One indicator of the fulfillment of health services to the community, especially in hospitals, was the calculation of the Bed Occupancy Rate (BOR). Factors that caused inefficient BOR need to be analyzed. The impact of a inefficient BOR returning decrease of income economic to the hospital. The purpose of this research is to analyze causative factors of decrease BOR in Regional Public Hospital of Sleman. This research is a qualitative research, with the intention of digging deeper causative factors of decrease BOR. research conducted at Regional Public Hospital of Sleman in February-March 2020 with analyze unit is inpatient care unit at Regional Public Hospital of Sleman. Data collection methods by interview and observation as well data analyze by fishbone diagrams. The results showed that the cause of the decrease in BOR was due to uncertain doctor visit schedules, lack of availability of beds, system errors, lack of reward and punishment, number of competing hospitals and less optimal reporting census monitoring. Suggestions to fix the problems that occur in the process of inpatient services due to a decrease in BOR can be improved Evaluate the SPO of inpatient services, Hold outreach to officers, fix SIMRS, improve the performance of marketing parties to attract patients and provide patients with a complete service and policy development (SK) related to reward and punishment.

Abstract

The process of borrowing and returning medical record at Ambulu Public Health Center still use manual book and make return of medical record documents that are not in accordance with SOP (Standard Operational Procedure). The purpose of this research is to design and create an information system of borrowing and returning the medical records based SMS gateway at Ambulu Public Health Center. The method used in this research is waterfall. The stages of the waterfall is requirements analysis, system design, create system, and system testing. The data collected by interview, observation, and FGD (Focus Group Discussion). The results of this research is the information systems of borrowing and returning the medical records based SMS gateway as message or automatic reminder. The suggestion for Ambulu Public Health Center is use information system of borrowing and returning the medical record based SMS gateway can make simple the reports.

Abstract

Regional general hospital of sleman (RSUD) is a hospital located in Sleman Regency with type B education. As a teaching education, Sleman District Hospital often borrows medical records for research (Education) related to the release of medical record information with third parties necessary. Because the character of medical record documents and their contents are confidential, the hospital must guarantee the confidentiality of the contents of the medical record file and legal protection for patients and hospitals. This research was aimed to find out how the release of medical record information based on legal aspects of the security and confidentiality of medical records for education in Sleman Hospital. This type of research is qualitative by collecting data using interviews, observation and documentation. The results of research at the Sleman Hospital in the application of releasing medical record information for educational purposes in the Sleman Hospital are still not by existing regulations related to the unavailability of expedition books or medical record file borrowing books where the book is very important in knowing the existence of medical record files that come out. And a special place for researchers when researching medical record files is not yet available due to limited space in the medical record unit of Sleman District Hospital.

Abstract

The release of medical information is a notification/provision of information about the health condition/illnes suffered by the patient and the risks that occur to the patient. There are several problems that exist in the process of releasing medical information to Jasa Raharja insurance in Sleman District Hospital including the absence of a power of attorney written by the patient or the patient’s guardian (if the patient is unable to make a power of attorney) and incomplete information contained in the Standard Operating Procedures (SOP). The purpose of this research is to evaluate the release of medical information for Jasa Raharja insurance based on legal aspects of the security and confidentiality of medical records in PERMENKES No. 269 of 2008. This research is descriptive qualitative, with data collection techniques carried out by observing the implementation of the release of medical information for jasa raharja’s insurance claims, interviews with officers responsible for releasing medical information in RSUD Sleman, and documentation of SOP information release. The result of this research are new Standard Operating Procedures (SOP) recommendations related to the process of releasing medical information for Jasa Raharja insurance and new Standard Operating Procedures (SOP) socialization related to releasing medical information for Jasa Raharja insurance. SOP update is very important because it can maintain the security and confidentiality  of medical record files so that it cannot be misused by irresponsible parties.

Abstract

Bhayangkara Hospital Lumajang is one of SNARS accredited health agencies and SISMADAK is used in the reporting process to KARS. The problems is still found in its application, which resulted in staff being less satisfied with SISMADAK. The aims of this study was to evaluate SISMADAK in terms of user satisfaction using the PIECES method. The study was quantitative descriptive study, which is described as a descriptive evaluation by comparing user satisfaction with information systems. The result of description evaluate of SISMADAK using PIECES method in Bhayangkara Hospital Lumajang about satisfaction percentage found 81% of performance variable was showed very good, 81% of information variable was stated very good, 69% of economic variable was good, 76% control variable was stated very good, 70% of efficiency variable was stated very good, and 80% of service variable was responded very good. For further advices were the planned of SISMADAK users need related information that is not yet available, beside the staff should increase the confidentiality of data on SISMADAK. The conclusion of this study is SISMADAK able to facilitate users in the hospital accreditation process of KARS.

Abstract

Dr. Kariadi Public Hospital has 44 inpatient Wards. Kutilang Ward has quite high number of BedOccupancy Rate (BOR) during 2017-2019. The purpose of this study was to determine bed requirementsin Kutilang Ward dr.Kariadi Public Hospital Semarang for 3 years (2020-2022) by using analysis of trendsregression linear (least square) method. This research was a descriptive study, with methods ofobservation and interviews. The object of research was inpatients recapitulation data on Kutilang dasarward and Kutilang 1 ward. The research subjects were Analising and Reporting officers in Dr KariadiPublic Hospital. The result showed that the needs of bed in Kutilang dasar ward in the year 2017 - 2019was 1 unit, Kutilang 1 ward need 31-63 units. The solution is the hospital management should consider toadd more the patient’s bed needs.

Abstract

The observation results of the medical record filing unit in Kademangan Public Health Service Bondowosofound that, in a week, 51 of 366 outpatient medical records were on the wrong location or lost (misfile) Theimpact of misfiling is the delay of service patients in search of medical records, duplication of numbersrecord the medical, and the contents of medical records document become not sustainable. This studyaimed to analyze the factors that cause medical records misfile in Kademangan Public Health ServiceBondowoso. This study used qualitative methods and data obtained through an interview, observation, andUSG (Urgency, Seriousness, Growth) method. The results of the research showed that the priority problemof misfile’s causes was the less of leadership motivation such as reward and punishment. The alternativesolutions were giving punishment or reward to medical record officers, especially to the filling officer tocreate their discipline in doing their work. The discipline of the filing officers in conducting work makes themanagement of filing medical records run optimally

Abstract

Management of medical records consists of the process of identification patients, patient identity record, assembling, coding, indexing and storage the medical records.Storage of medical records is an activity to protect the medical records from physical damage and the content of the medical records. The process of storing the medical records has risks that can threatening the occupational safety and health of medical records staff. At filing RSUD Sleman found some risk that at risk of respiratory diseases because not used masks, scraped paper or sharp medical record mapbecause not used handscoon, and and strangulated the roll o’pack. The purposed of the risearch was to examine occupational health and safety in the filing unit at Sleman District Hospital. This typed of research is a qualitative study with data collection used interview and observation techniques. The results of this research there are 14,3% low risk, 71,4 % moderat risk, and 14,3% in high risk. So manage or control the risk is 1) substitutions, this control aims to replace materials, processes, operations or equipment from dangerous to more harmless. 2) Administrative, danger control by making modifications to the workers' interactions with the work environment, such as the preparation of Standart Procedure Operasional. 3) self-protection, danger control, using self-protective devices designed to protect staff from dangers in the workplace.

Abstract

Regulation of health ministry 2018 number 269 on Medical Record stated that the records of medical patient care inpatient in hospital must be kept at least five (5) years on a period since the date of the last patient treated or discharged. Since the Mitra Medika Bondowoso Hospital was found in 2011 the implementation of retention and destruction of medical record documents has never been carried out. This research was aimed to analyze and know the priority cause unimplementation retention and destruction of documents recording the medical -patient hospitalization by USG (Urgency, Seriousness, and Growth) and brainstorming in Mitra Medika Bondowoso Hospital. Type of this research is that qualitative and the technique collection of data by interviews, observation, documentation, and brainstorming. The results were obtained that the priority of the cause unimplementation of retention and destruction of documents medical record that is the double job of the medical record employees, Retention archive schedule in SOPs, and a lack of understanding of the medical record employees with SOP retention and extermination. The improvement efforts for the problems are to add more medical record employees and to build more teamwork between the employees. Another solutions are to make new SOP recently with the addition of the retention archive schedule as well as involving the employees in the preparation of SOPs.
