Abstract

Latar belakang: Rumah sakit merupakan suatu lembaga kesehatan yang berwewenang menyediakan pelayanan kesehatan kepada seluruh masyarakat. Covid-19 merupakan salah satu masalah kesehatan di dunia sejak tahun 2020. Rumah sakit keluarga sehat pati memiliki komitmen untuk memberikan pelayanan terhadap pasien terutama saat pandemi covid-19. Namun saat terjadi pandemi Covid-19 sebagian pasien RS keluarga sehat pati masih mempunyai keluhaan terhadap mutu pelayanan pasien terkait administrasi, fasilitas, caring petugas, dan prosedur klinis dan pelayanan. Tujuan: Analisis pengaruh mutu pelayanan kesehatan pada pasien Covid – 19 yang terdiri dari Sumber Daya Manusia, Standar Operasional Prosedur, Alat Kesehatan, Penatalaksanaan Medis dan Penatalaksanaan Non Medis terhadap kepuasan pasien Covid– 19 selama di rawat inap. Metode: Penelitian ini merupakan penelitian kualitatif, dimana metode pengumpulan data dilakukan dengan cara observasi, telaah dokumen, dan wawancara mendalam terhadap informan utama dan informan triangulasi. Dimana analisis data dimulai dari pengumpulan data, reduksi data, menyajikan data sampai penarikan kesimpulan. Hasil: Berdasarkan hasil penelitian diperoleh kesimpulan bahwa dari beberapa parameter mutu pelayanan kesehatan, pada saat terjadi lonjakan pasien covid-19 dapat berpengaruh terhadap mutu pelayanan di Rumah Sakit Keluarga Sehat Pati. Dimana masih kurangnya sumber daya manusia baik medis dan paramedis, dengan adanya jam kerja yang berlebih untuk dokter spesialis paru dan perawat, standar operasional prosedur dalam pendokumentasian obat yang masih harus di perbaiki, pemeliharaan alat kesehatan yang tidak terjadwal dengan rutin oleh sarana medis, belum dilakukannya evaluasi terhadap penatalaksanaan medis, dan penatalaksanaan non medis yang sudah sesuai dengan kebutuhan pasien. Kesimpulan: Analisis mutu pelayanan pada pasien covid-19 di rawat inap Rumah Sakit Keluarga Sehat Pati pada saat terjadi pandemi covid-19 dimana dilihat dari parameter sumber daya manusia, standar operasional prosedur, alat kesehatan, penatalaksanaan medis dan penatalaksanaan non medis masih perlu dilakukan perbaikan, sehingga mutu yang diberikan dapat menimbulkan kepuasan pada pasien covid– 19 yang di rawat.

Abstract

Waiting is an activity which frequently causes uncomfortable feelings, particularly in health services. Patients who attend to health care facilities such as hospitals, are eager to receive good and quality health care system without having to wait for a long time. The quality of pharmaceutical services in hospitals influences the level of patient satisfaction with the services provided by pharmacy staff in drug services to patients. There are still numerous waiting times for outpatient prescription drug services in Indonesian hospitals which do not fulfill minimum service standards. Thus, the objective of this review articles is to determine the factors affecting the waiting time for outpatient prescription services at the hospital. This review was conducted by employing the literature review method with a sample size of 12 articles. The results acquired are several factors influencing the waiting time for outpatient drugs in hospitals encompassing: receiving prescriptions, providing etiquette, working on prescription drug concoctions, submitting drugs, facilities and infrastructure, human resources, hospital management information systems and standard operating procedures. Based on the complexity of the issue which was found in this review, the most dominant and influencing factors on the waiting time of outpatient drugs at the hospital are prescription reception, providing etiquette, preparation of prescription drugs, and submitting drugs.

Abstract

Tingginya kasus stunting di Kabupaten Brebes membutuhkan upaya penanganan stunting melalui program Bina Keluarga Balita (BKB). Namun cakupan BKB di Kabupaten Brebes belum mencapai target sasaran dan saat pandemi COVID-19 baru tercapai 5,7% pada tahun 2020. Penelitian ini bertujuan untuk menganalisis pelaksanaan program BKB  pada era pandemi COVID-19 di lokus desa stunting Kecamatan Kersana Kabupaten Brebes. Penelitian ini adalah penelitian kualitatif. Data dikumpulkan dengan wawancara mendalam kepada informan utama dan informan triangulasi. Informan utama adalah pelaksana program. Informan triangulasi terdiri dari peserta program dan pengelola program. Pengolahan dan analisa data menggunakan metode analisis isi (content analysis). Hasil penelitian menunjukkan bahwa fungsi perencanaan program BKB sudah dilakukan yakni sudah menetapkan tujuan dan target kinerja akan tetapi terdapat keterbatasan antara lain belum semua kelompok BKB memiliki buku pedoman atau petunjuk teknis, belum ada Standar Operasional Prosedur (SOP) pelaksanaan program BKB era pandemi COVID-19 , belum dibuat peta kerja, keterbatasan perencanaan sumber daya meliputi Sumber Daya manusia (SDM), sarana dan prasarana, anggaran operasional, serta belum disusunnya program kerja BKB. Fungsi pengorganisasian program BKB sudah berjalan ini dapat dilihat dari sudah adanya pembagian tugas akan tetapi belum mempertimbangkan kompetensi dan belum ada pembuatan jadwal piket. Fungsi penggerakkan program BKB sudah dijalankan antara lain pengelola BKB sudah memberikan arahan atau bimbingan pada pelaksana BKB, sudah ada kerjasama lintas sektor dan mitra terkait, akan tetapi pada masa pandemi COVID belum semua kelompok melakukan inovasi. Fungsi pengawasan program BKB sudah berjalan dengan baik dimana sudah ada monitoring dan evaluasi capaian kinerja. Upaya perbaikan kinerja dilakukan dengan cara meningkatkan promosi program BKB melalui media dan advokasi dalam rangka mendapatkan dukungan.

Abstract

Abstrak Demam Berdarah Dengue (DBD), disebabkan oleh virus Dengue dan ditularkan melalui vektor nyamuk. Penelitian ini bertujuan untuk menganalisis perilaku stakeholder dalam pelaksanaan pengendalian Demam Berdarah Dengue (DBD) di Kota Semarang. Jenis penelitian yang digunakan peneliti dalam penelitian ini adalah menggunakan penelitian kualitatif. Program pengendalian DBD yang dijalankan oleh stakeholder adalah penggunaan celana panjang, Semar Greget (surveilas berbasis sistem tunggal dara, SRSJ,  Sicentik, PJN PSN serentak) Pokjanal DBD , Gasurkes, Sistem tunggal dara, dan Sosialisasi, sebagian besar informan di wilayah kasus tinggi hanya menyebutkan PSN dan Jum’at bersih. Koordinasi di Dinas Kesehatan maupun di wilayah tinggi sudah dijalankan koordinasi secara langung maupun online dan secara terjadwal maupun kondisional. Sebagian besar dari informan sudah memiliki pengalaman sebelum menjalankan program pengendalian DBD. Sarana prasarana yang digunakan untuk pengendalian DBD, form pemantauan jentik, kartu jentik, senter, sirine, sistem pelaporan tunggal dara, larvasida, alat test DBD, layanan rujukan RS. Di wilayah tinggi kasus semua informan belum bisa menyebutkan regulasi yang dipakai untuk pengendalian DBD. Tokoh masyarakat yang ikut mendukung yaitu Kader, RT, RW, PKK, FKK. muspika, tokoh agama dan kader. Petugas Kesehatan yang ikut mendukung yaitu petugas Dinas Kesehatan Kota Semarang, Puskesmas, Gasurkes, dan petugas kesehatan yang tinggal di wilayah. Pihak lain yang ikut mendukung yaitu Rumah Sakit, BMKG, Kecamatan, Kelurahan, Swasta, dan akademisi. Pelaksanaan pengendalian DBD yang dilihat dari perilaku stkeholder sudah berjalan meskipun masih perlu ditingkatkan.

Abstract

ABSTRACT Cancer is a non-communicable disease whose prevalence is increasing every year. Comprehensive therapy must be carried out to handle it, one of the things that patients often forget is adherence to taking medication. Even the adherence rate of cancer patients to take medication, in an oncology center at a hospital in Semarang is quite low. If the patient is not regular in taking medication, it will make the disease more progressive and result in an even greater burden on the health budget. This must find a solution to solve it. One of them is by making a medication reminder application using technology that can be installed on the patient's smartphone. Objective to find out the relationship between giving a reminder application to adherence to taking medication in cancer patients. The study uses quantitative research with a Quasi Experiment Control Group Design, and a pre-test and post-test group design with a control group without randomization. The application of cancer telenutrition in the form of diet reminders has a positive relationship in increasing the weight of cancer patients (p<0.001) with a strong relationship of 0.711 (R squared >0.67). The use of reminder applications is associated with increasing adherence to taking medication in cancer patients. It is hoped that this research can be used as a basis for implementing information technology innovations in improving cancer service management practices in hospitals. Keywords: Cancer, Reminder Application, Medication Adherence  ABSTRAK Kanker adalah penyakit tidak menular yang saat ini prevalensinya meningkat setiap tahunnya. Terapi komperhensif harus dilakukan untuk menanganinya, salah satu hal yang sering dilupakan pasien adalah kepatuhan minum obat. Bahkan angka kepatuhan pasien kanker untuk meminum obat, di salah satu pusat onkologi di Rumah Sakit di Semarang cukup rendah. Dengan tidak teraturnya pasien dalam minum obat, akan membuat panyakit bertambah progresif dan mengakibatkan beban anggaran kesehatan yang semakin besar.  Hal ini harus dicarikan suatu solusi untuk memecahkannya. Salah satunya dengan membuat aplikasi pengingat minum obat dengan memanfaatkan teknologi yang dapat diinstal ke ponsel pintar pasien. Tujuan untuk mengetahui hubungan pemberian aplikasi pengingat terhadap kepatuhan minum obat pasien kanker. Studi menggunakan penelitian kuantitatif dengan Quasi Eksperiment Control Group Design,  dan rancangan pre test and post test group design dengan kelompok kontrol tanpa randomisasi. Aplikasi telenutrisi kanker dalam bentuk pengingat diit mempunyai hubungan positif dalam meningkatkan berat badan pasien kanker (p<0,001)  dengan hubungan kuat yakni 0,711(R squared >0.67). Penggunaan aplikasi pengingat berhubungan dengan peningkatan kepatuhan minum obat pasien kanker. Diharapkan penelitian ini dapat digunakan sebagai dasar penerapan inovasi teknologi informasi dalam meningkatkan praktek menajemen pelayanan kanker di Rumah Sakit. Kata Kunci: Kanker, Aplikasi Pengingat, Kepatuhan Minum Obat

Abstract

Latar Belakang: Kawasan tanpa rokok adalah ruangan atau area yang di nyatakan di larang untuk kegiatan merokok atau kegiatan memproduksi, menjual, mengiklankan atau mempromosikan produk-produk tembakau. Di Indonesia rokok masih menjadi permasalahan penting yang harus segera diatasi. Mengingat dampak dari rokok tidak hanya memberikan dampak buruk bagi kesehatan tetapi juga berdampak pada beberapa aspek penting lainya seperti ekonomi dan sosial. Dari beberapa permasalahan yang terjadi karena dampak dari rokok, membuat pemerintah mengeluarkan UU no 36 tentang instruksi memberlakukan KTR di wilayah kerja masing masing pemerindah daerah. Dari diberlakukanya kebijakan tersebut adalah untuk melindungi masyarakat Indonesia dari paparan asap rokok dan memberikan rasa nyaman dalam menghirup udara segar. Tujuan: Tinjauan pustaka ini adalah untuk mengetahui faktor-faktor yang mempengaruhi penerapan kebijakan kawasan tanpa rokok. Metode: Metode yang digunakan dalam Menyusun artikel ini adalah literature review, pencarian artikel di ini menggunakan google scolar, dengan kata kunci “kawasan tanpa rokok, kebijakan kawasan tanpa rokok dan perilaku merokok”. Artikel yang ditemukan dalam pencarian sebanyak 2050 artikel, kemudian dievaluasi kembali sesuai dengan kriteria inklusi dan eksklusi yang telah ditetapkan sehingga akhirnya tersedia 5 artikel yang dapat direview. Hasil: Penelaah ditemukan bahwa implementasi kebijakan kawasan tanpa rokok banyak dipengaruhi oleh beberapa faktor yaitu peyampaian informasi, kejelasan informasi, sumber daya, komunikasi, disposisi, struktur birokrasi, perilaku merokok dan pengetahuan tentang peraturan daerah kawasan tanpa rokok. Saran: Disarankan untuk membuat standar operasional prosedur (SOP) dalam menerapkan kebijakan kawasan tanpa rokok serta membentuk komite atau tim khusus agar sebuah kebijakan dapat berjalan efektif dan efisien.

Abstract

Abstract The hospital pharmacy installation plays an essential role in the hospital, where the hospital pharmacy installation manages the management and distribution of drugs. The achievement of quality indicators related to the waiting time for concoction and non-concoction drug services at the Outpatient Pharmacy Installation of X Kendal Hospital still needs to be consistent. Patients also complain about long waiting times. This study aims to determine whether the implementation of patient drug prescription services in the Hospital Outpatient Pharmacy Installation has been appropriately managed or needs improvement. This research is qualitative research with a phenomenological approach. Purposive sampling researchers selected the research subjects. The subjects in this study consisted of four primary informants and two triangulation informants. The results of the in-depth interviews found that during no initial check was made on the identity of the patient or the prescription received because due to the habit of piling up prescriptions, the dose given by the doctor was not correct. Hence, it had to be recalculated, the amount of drug stock in the system was not correct always the same as the available physical stock, lack of pharmaceutical staff during the drug preparation process, and there were medication errors in the drugs that had been prepared so that the staff had to re-prepare them, and errors in the drug delivery process due to staff negligence, causing longer waiting times for prescription services. Therefore, it was concluded that the implementation of prescription services at hospital X in Kendal district had not been managed optimally or appropriately. Hospitals still need to evaluate and improve the process of implementing prescription services to reduce waiting times. Keyword: Drug Prescription, Pharmacy Installation, Service

Abstract

The development and progress of information technology are advancing very rapidly, electronic medical records are one form of these developments and advances in the health sector. A hospital is a place of service in the health sector that performs services related to medical records. Medical records can be made in full and clearly in electronic (digital) form. The implementation of medical record technology can solve many problems in the economic field, clinical, and information of clinical in the hospital system. The high cost of operational and complicated procedures is a crucial problem in the hospital system. So, the advance of medical technology can press high costs, cutting off the manual process to faster and more effective system and efficiency costs of operational. The study uses Preferred Reporting Items for Systematic Review and Meta-Analyses (PRISMA), with many articles on the same issue. The same issues that can be found at the same topics of articles are probably more than fifty topics of articles. Articles were taken from various sources and database references, including Scopus 2 journals, Science Direct 16 journals, ProQuest 10 journals, Springer Link 3 journals, Google Scholar 36 journals, Research Gate 21 journals, PubMed 31 journals, and Emerald Insight 31 journals. So the total journal database is 133 journals. Then filtering was carried out by identifying the suitability of the journal until it was narrow and 27 articles were selected to be used as a reference in this research.. The purpose of this study is to determine the benefits of applying Electronic Medical Records in terms of economic, clinical, and access to clinical information. Some of the economic benefits include cost savings, effectiveness, and cost efficiency. Clinical benefits include reducing medical errors, improving data readability, improving the quality of care services, and increasing the productivity of medical personnel. And the benefits of accessing clinical information include improving the accessibility of patient history information, improving patient confidentiality, and assisting the decision-making process.

Abstract

A hospital is a place that provides health services which include observation, diagnostic, therapeutic, and rehabilitation activities for patients. Development of information systems using Prototype. To support the development of the information system, the PIECES framework is used, which is seen in terms of performance, information, economics, control, efficiency, and service. One part of the service at RSUD Banyumas that has used a management information system is an outpatient installation. However, the clinical pathway audit process has not been carried out in the outpatient department, so it cannot measure the quality of services provided by doctors and cannot be done in decision and policy making. This type of research is qualitative. The management of clinical pathway data audits still uses a manual system and does not optimize computerized data processing. Based on the results of the research that has been carried out, the following conclusions were obtained Outpatient information system at RSUD Banyumas so far, there are problems with the process carried out manually related to the management of Clinical Pathway audits. There needs to be an integrated information system to the hospital management to facilitate access to the information needed.

Abstract

Non-communicable diseases (NCDs) are the leading cause of death worldwide. In Indonesia, NCDs has increased and shifted patterns. Changes in behavior and lifestyle increase the risk of NCDs. Difficulties in monitoring treatment and patient visits are a challenge for health workers. Currently, there is still no integrated information system for patient monitoring. The general objective of this research is to develop an integrated Non-Communicable Disease Information System (NCDSI) in Semarang City. The current study uses the Rapid Application Development (RAD) method with a qualitative approach. RAD is a complete approach model for information system development and covers the entire system life cycle. This research was conducted in three stages: planning and needs analysis, designing, developing, and collecting feedback. During the development and collection of feedback, functionality testing activities are carried out to obtain feedback and evaluation as the system is improved. The research was carried out by the Semarang City Office. The functional test subjects consisted of three agents and five patients with primary hypertension and diabetes mellitus. The results: the planning and needs analysis phases were carried out through thorough interviews with end-users. The system design phase was described through contextual diagrams and database tables, and the development and feedback phase was conducted through the NCDSI functionality test. Conclusion: The developed NCDSI has been integrated and has four user levels (users of the City Health Office, Head of Puskesmas, Cadres, and patients).
