Abstract

Abstract Background: Access to condoms is an important intervention in preventing HIV transmission among key populations, particularly Men who have Sex with Men (MSM), as they represent the highest group living with HIV in Indonesia. Objective: To investigate the determinants of access to condoms among MSM. Method: This study employed a cross-sectional design and used data from the 2018-2019 Integrated Biological and Behavioral Survey (IBBS) conducted in 24 provinces in Indonesia. The study population consisted of 4,290 individuals selected using Respondent Driven Sampling (RDS) technique. After data collection, multivariate predictive modeling analysis was conducted using STATA 14 software. Results: The study found that exposure to HIV information (OR: 1.64 with 95% CI: 1.18-2.28), perceived risk of HIV transmission (OR: 1.69 with 95% CI: 1.29-2.20), and consistent condom use (OR: 4.06 with 95% CI: 3.03-5.45) were significant determinants of access to condoms among MSM. The most dominant variable was consistent condom use during sex. Conclusion: MSM who have a good understanding of HIV, perceive their risk of HIV transmission, and consistently use condoms during sexual intercourse are more likely to have access to condoms. The study recommends interventions to increase MSM's knowledge about HIV and the need for monitoring and evaluation by various agencies to ensure the availability and ease of access to condoms for key populations. Keywords: Determinant; Access to condoms; Men who have Sex with Men (MSM)   Abstrak Latar belakang: Akses dalam memperoleh kondom merupakan salah satu bentuk intervensi dalam pencegahan penularan HIV pada kelompok populasi kunci, terutama Lelaki Seks Lelaki (LSL) mengingat LSL merupakan kelompok tertinggi yang hidup dengan HIV di Indonesia Tujuan: melihat determinan akses memperoleh kondom pada kalangan LSL Metode: Desain penelitian adalah desain cross sectional  dengan menggunakan data STBP 2018-2019 dimana penelitian ini dilakukan di 24 provinsi yang ada di Indonesia dengan sampel penelitian adalah total populasi berjumlah 4.290 orang. Teknik pengambilan sampel adalah Respondent Driven Sampling (RDS). Setelah data diperoleh, maka dilakukan analisis sampai dengan analisis multivariat model prediksi dengan menggunakan software STATA 14 Hasil: Hasil penelitian ini menemukan jika keterpaparan informasi tentang HIV (OR: 1,64 dengan nilai 95% CI: 1,18 – 2,28), risiko tertular HIV (OR: 1,69 dengan nilai 95% CI: 1,29 – 2,20) dan konsistensi penggunaan kondom (OR: 4,06 dengan 95% CI: 3,03 – 5,45). Variabel dominan adalah konsistensi penggunaan kondom. Kesimpulan: LSL yang memahami tentang konsep HIV dan memiliki persepsi risiko berisiko tertular HIV serta konsisten menggunakan kondom saat melakukan hubungan seks akan lebih mudah dalam mengakses kondom. penelitian ini merekomendasikan perlunya upaya intervensi dari segi peningkatan pengetahuan LSL tentang HIV serta perlunya monitoring dan evaluasi dari berbagai instansi untuk menjamin ketersediaan dan kemudahan akses kondom pada kelompok kunci. Abstract

Abstract Background: Making decisions to marry among teenagers will lead to high rates of child marriage before the age of 18, which will later have long-term risks for the health, psychological, and social problems of teenagers. Objective: Analyzing the determinants that influence the decision to marry among teenagers in Indonesia. Method: This study was cross-sectional by analyzing IDHS data in 2018, where the study sample was 10,619 women aged between 20-24 years who had been married before turning 18 years old. Results: about 72.2% of Indonesian adolescents participated in the decision to get married, another result of the study found that. The factors of sexual experience that were related to education (p-value: 0.001 and AOR:1.94 with 95% CI: 1.52-2.48), information exposure (p-value: 0.002 and AOR: 1.37 with 95% CI: 1.12-1.66) and sexual experiences (p-value=0.001 and AOR=0.64 with 95% CI=0.52-0.79). Education was The dominant variable that influenced the decision-making of marriage in adolescents was education. Conclusion: The decision-making of marriage in adolescents is influenced by education variables, information exposure, and sexual experiences Recommendation: enhancing adolescent participation in PIKRs: promoting healthy marriage and sexuality education through social media. Keywords: determinants, marriage decision-making, Indonesian teenagers.   Abstrak Latar belakang: Memutuskan untuk menikah pada usia remaja dapat berdampak jangka panjang pada pada masalah kesehatan, psikologis dan sosial remaja. Tujuan: Mengetahui deteminan keputusan menikah pada remaja Indonesia. Metode: Penelitian ini berdesain cross sectional dengan menganalisis data SDKI tahun 2018, dimana sampel penelitian sejumlah 10.619 orang wanita berusia antara 20–24 tahun  yang pernah menikah sebelum menginjak 18 tahun. Hasil: Sekitar 72,2% remaja Indonesia berpartisipasi dalam pengambilan keputusan untuk menikah, variabel pendidikan dengan nilai p-value=0,001 serta AOR=1,94 (95% CI=1,52-2,48), keterpaparan media dengan nilai p-value:=0,002 dan AOR=1,37 (95% CI=1,12-1,66) dan pengalaman seks dengan nilai p-value=0,01 dan AOR=0,64 (95% CI:0,52-0,79) berhubungan dengan pengambilan keputusan sendiri untuk menikah pada remaja, dan variabel yang dominan adalah variabel pendidikan. Kesimpulan: Pendidikan, keterpaparan informasi serta pengalaman seks remaja memiliki peran dalam  keputusan menikah. Rekomendasi: Menggalakkan kampanye tentang pernikahan yang sehat serta edukasi seksualitas melalui media sosial serta meningkatkan peran remaja pada Pusat Konseling dan Informasi Remaja (PIKR) yang ada di sekolah.     Kata kunci: determinan, pengambilan keputusan menikah, remaja Indonesia.

Abstract

Background: The monogamous sexual behavior of married men who have sex with men (MSM) increases their risk of acquiring sexually transmitted infections (STIs). This study aims to examine the influence of ABCD behaviors on the incidence of STIs among married MSM in Indonesia. Method: A cross-sectional study was conducted in 24 provinces in Indonesia. The study population comprised MSM who had ever engaged in sexual intercourse, either occasionally, once, or frequently, totaling 6,000 individuals. A sample of 775 participants who were married MSM was selected using the Respondent Driven Sampling (RDS) method. Data from the Integrated Biological and Behavioral Survey (STBP) 2018-2019 was utilized. The data was analyzed using multivariate analysis with STATA 14 software. Results: The variables of no vaginal sex, no anal sex, condom use, and needle drug use behavior are associated with the incidence of STIs when controlled by the variable of alcohol consumption behavior. Conclusion: Practicing ABCD behaviors among married MSM can help reduce and prevent STIs.

Abstract

Background: In Indonesia, water-based lubricant use among men who have sex with men (MSM) is lower than that reported in transgender populations (84.4%). This study examined the association between water or gel based lubricant use and condom failure, defined as leakage or breakage, among young MSM. Methods: Data were drawn from the 2018–2019 Integrated Biological and Behavioral Survey (IBBS), a cross-sectional survey in 24 provinces. Of 6,000 MSM, 2,213 respondents aged 15–25 years were recruited using Respondent Driven Sampling (RDS). Logistic regression in STATA 16 assessed associations, adjusting for circumcision status and number of male sexual partners. Results: Condom failure in the past month was reported by 8.8% of respondents, and 66.3% used water-based lubricants. Non-use of water- or gel-based lubricants was associated with higher adjusted odds of condom failure, but not statistically significant (AOR 1.79; 95% CI 0.79–4.05). Condom failure was more common among those reporting more male clients. Circumcision status was also related, with lower odds of failure among uncircumcised participants. Conclusions: Water-based lubricant use was not significantly linked to reduced condom failure. Improving condom-use skills, promoting compatible lubricants, and targeting MSM with multiple partners remain important for reducing condom failure and HIV/STI risks.
