Abstract

Type 2 diabetes mellitus is a growing global health problem that requires effective early screening strategies. This study aimed to develop an e-Health-based Clinical Decision Support System (CDSS) for assessing type 2 diabetes risk using a rule-based scoring method. The system was developed using a prototype-based system development approach, involving requirement analysis, system design, prototype development, and system evaluation. Risk determination was performed using six major factors including age, sex, body mass index, family history of diabetes, hypertension, and history of gestational diabetes based on ADA and PERKENI guidelines. System evaluation was conducted using black box testing to assess functional performance and usability evaluation using System Usability Scale (SUS) involving end users. The developed system successfully generated risk scores, categorized risk levels, and provided follow-up recommendations aligned with clinical guidelines. Evaluation results indicated that the system functions properly and demonstrates good usability performance. Therefore, the proposed CDSS shows potential as a practical e-Health screening tool to support early diabetes detection in primary healthcare settings.

Abstract

Kendit Public Health Center currently uses paper-based medical records and has only recently begun implementing electronic medical records (EMR) in several service areas. However, this implementation faces numerous challenges, including data duplication, file damage, and the accumulation of 9,424 physical records. This situation highlights the importance of accelerating the comprehensive implementation of Electronic Medical Records (EMR) to enhance the efficiency of patient data management. This study aims to analyze the readiness for EMR implementation using the DOQ-IT method. This descriptive research involved 53 respondents, with data collected through questionnaires. The results showed that organizational alignment readiness was in the “very ready” category with a score of 31.72 out of a maximum score range of 45. The subvariables included culture (mean: 3.56), leadership (mean: 3.53), and strategy (mean: 3.47) on a 1 – 4 scale. Meanwhile, organizational capacity readiness was in the “moderately ready” category with a score of 59.77 out of a maximum score range of 100. The subvariables included information management (mean: 3.11), clinical and administrative staff (mean: 3.07), training (mean: 3.42), workflow process (mean: 2.92), accountability (mean: 2.75), budgeting (mean: 3.12), patient involvement (mean: 2.58), IT management (mean: 3.06), and IT infrastructure (mean: 2.80) on a 1 – 4 scale. Improvements are needed in organizational capacity, particularly in patient involvement, accountability, and IT infrastructure.

Abstract

V-Kes serves as a Teaching Factory (TeFa) for the Health Information Management Study Program within the Department of Health. Based on a preliminary study, it was found that V-Kes does not yet have a web portal specifically designed for early detection and integrated information services related to Diabetes Mellitus (DM). This study aims to develop a novel web-based portal that integrates early DM detection features with an educational and service information platform for the V-Kes TeFa, distinguishing it from existing general health web portals. The study focuses on the system outcomes and its impact on users rather than detailed development processes. The system was implemented as a web portal, with the frontend developed using React and Vite, and the backend utilizing Laravel. The study involved five V-Kes TeFa managers as informants for user identification and system evaluation. User satisfaction evaluation was conducted using qualitative methods supported by structured feedback indicators, including usability, information quality, and system responsiveness. Data were analyzed through data reduction, data presentation, and conclusion drawing. The study resulted in the development of a web-based portal at the V-Kes TeFa that enables early DM risk screening through user input data and provides real-time health information access. Evaluation results indicate that most users found the system easy to use, informative, and relevant to their needs, although specific issues were identified in system response time. However, improvements are still required in the timeliness dimension to provide faster response times according to user needs, as well as the addition of a user manual. Therefore, future development will focus on enhancing system access speed and incorporating a manual book.

Abstract

Claims are requests for payment of healthcare service costs by healthcare facilities to BPJS Kesehatan. Hospitals must complete the required documents according to BPJS verification procedures to avoid pending claims. The issue of pending claims still occurs at RS Tk.III Baladhika Husada Jember, with a pending rate of 11.4% from July to December 2023. This study aims to analyze the factors causing pending BPJS Kesehatan inpatient claims at RS Tk.III Baladhika Husada Jember using Robbins’ Performance Theory. This qualitative study collected data through interviews, observations, documentation, and brainstorming. The study found that pending BPJS Kesehatan inpatient claims are caused by motivation factors (lack of a reward system), opportunity factors (absence of job descriptions, insufficient staff for casemix and TPPRI), and ability factors (staff with less than 5 years of experience, lack of knowledge about BPJS inpatient claims). The study concludes that the causes of pending BPJS Kesehatan inpatient claims are motivation, ability, and opportunity factors. Proposed improvements include creating job descriptions, regularly maintaining internet networks, recruiting new staff with medical record qualifications, and providing training to enhance staff knowledge.

Abstract

The standard time for provided outpatient medical records was ≤ 10 minutes. Based on the results of preliminary study, based on 99 files, 90 medical records (90.9%) experienced delays in provision. The purpose of this study was to analyze the factors caused the delay in provided outpatient medical records at Doctor Mohamad Saleh Hospital, Probolinggo City used the performance theory from Robbins. Type of research used qualitative, data collection techniques were interviews, observations, documentation and brainstorming. The research subjects consisted of 1 head of medical records, 7 outpatient medical record storage officers, 1 medical record distribution officer, 6 outpatient registration officers and 3 nurses. The results of the study were based on ability factors were the lack of knowledge of officers about the procedure for providing outpatient medical records, officers didn’t have medical record education qualifications, officers never participated in medical record management training, and work experience less than 3 years. Based on the opportunity factor were errors in SIMRS, insufficient shelves and storage space for medical records and there was no SOP for provision of outpatient medical records. Meanwhile, the opportunity factor is in the sub-variables of SIMRS, shelves and storage rooms for medical records and SOP. The improvement plan are apply for training, replace officers according qualifications, switch to using RME, improve facilities and infrastructure, and make SOP.

Abstract

Incomplete medical resume documentation remains a persistent issue, including at Bahagia General Hospital Makassar. In the Cempaka inpatient ward, the percentage of incomplete medical resumes in November, December, and January reached an average of 8.00 and showed an increasing trend. This study aims to analyze the factors contributing to the incompleteness of medical resume documentation based on Armstrong and Baron’s (1998) performance theory. The study employed a qualitative approach through interviews, observations, documentation review, and brainstorming. Research subjects consisted of physicians, nurses, and medical record staff. The findings indicate several contributing factors. Individual factors include the suitability of staff educational backgrounds, the absence of training related to medical resume completion, and limited knowledge regarding completeness standards (SPM). Leadership factors involve the lack of motivational strategies such as rewards for staff who consistently complete medical resumes, as well as insufficient involvement of all staff in completeness evaluations. System factors include suboptimal use of checklist forms, the absence of standardized operating procedures (SOP) for completing and assessing medical resumes, and the lack of SOP socialization. Situational factors are related to workload issues, particularly staff performing multiple roles (double jobs). Meanwhile, group factors were found to function well through effective teamwork among staff. In conclusion, incompleteness in medical resume documentation is influenced by individual, leadership, system, and situational factors. It is recommended to provide training for involved staff, develop and implement SOPs, and conduct SOP socialization to improve medical resume completeness.

Abstract

Household organic waste such as fruit and vegetable scraps can be utilized as the main ingredient for eco enzyme, which serves as a natural and environmentally friendly base for liquid detergent production. This community service activity aims to empower local residents to transform kitchen waste into useful products that contribute to pollution reduction. The methods included education sessions, eco enzyme production training, liquid detergent formulation, and hands-on practice. The target group was the PKK women’s organization of Puger Kulon Village, Puger District, Jember Regency, who play an essential role in household waste management. The results showed an increase in participants’ knowledge and skills in independently producing liquid detergent. The product proved effective for household cleaning and has the potential to develop into a small-scale business opportunity. This program is expected to encourage the adoption of a green lifestyle and raise community awareness of sustainable waste management practices.
