Abstract

Based on preliminary studies conducted by the author at the Bringin Community Health Center, 35 duplicate outpatient medical records were found in 2020.  The purpose of this study was to identify the factors causing the duplication of outpatient medical record numbers at the Bringin Community Health Center. This study used a qualitative method with a descriptive approach. The sample in this study consisted of 4 medical record officers. The results of the study in February-March 2021 showed that there were two duplicate medical record numbers. The causes of duplicate medical record numbering were based on the 5M aspects (Man, Material, Method, Machine, Money). The man factor was related to the limited number of staff. The material factor was related to patients who did not bring their medical treatment identity card when visiting. The method factor indicates the lack of specific standard operating procedures regarding medical record numbering. The machine factor indicates that the system is still manual. The money factor indicates a lack of funds needed to add medical record staff. The recommendation to the Bringin Community Health Center is to create standard operating procedures for Medical Record Numbering and provide training for staff who are not medical record graduates.

Abstract

The purpose of this study is to identify the Standard Operating Procedures (SOP) for outpatient registration, the menu items involved, the features of the Sikda Optima system, and the obstacles encountered in outpatient registration at the Kebon Jeruk District Health Center. This research uses a descriptive quantitative method, which involves the collection, processing, and interpretation of data in numerical form. The results indicate that the SOP for outpatient registration has been implemented since January 3, 2023, and that the features of Sikda Optima are complete, meet user needs, and are easy to use; however, several obstacles remain, particularly system failures or errors during operation. The study concludes that the SOP is designed to facilitate administrative processes and ensure effective healthcare services, while the registration items play an important role in ensuring a smooth registration process. Despite the use of the Sikda Optima application, challenges still occur, such as system failures lasting more than 10 minutes that cause the website to become inaccessible, and the lack of integration with the P-Care system, requiring staff to perform double data entry. Therefore, it is recommended that a responsive technician be available to monitor system connectivity during service hours and that system features be improved to enhance efficiency and effectiveness, as the SOP ultimately helps improve service quality, reduce queues, and ensure more accurate patient data recording.

Abstract

The implementation of Electronic Medical Records (EMR) in all health service facilities is outlined in Minister of Health Regulation Number 24 of 2022, with a deadline of 31 December 2023. Ensuring the security of EMR is vital to protect patient data privacy, prevent unauthorized data access, and avoid breaches. This study evaluates the security practices of EMR systems at the Islamic Hospital Jakarta Sukapura using a descriptive qualitative research method with seven informants. The study focuses on six key aspects of EMR security: privacy, integrity, authentication, availability, access control, and non-repudiation. The results show that the hospital has established security procedures, such as using usernames and passwords, but there are significant areas for improvement. The study notes the need for automatic logout features to prevent unauthorized access when computer screens are left unattended and emphasizes the importance of regularly updating passwords to improve security, as well as hospitals using encryption and firewall technologies to protect data during transmission and storage. Apart from this, research shows that some staff members still use default passwords, posing a security risk. Overall, this study provides recommendations for strengthening RME security frameworks in hospitals.
