Abstract

Aims: Good self-efficacy will encourage patients to behave assertively and adhere to medication, diet, and fluid restrictions in patients with chronic kidney disease undergoing hemodialysis. However, nowadays, patient non-compliance with fluid restrictions is still high, which results in increased complications experienced by patients. The study aimed to determine the relationship between self-efficacy and adherence to fluid intake restrictions in chronic kidney patients undergoing hemodialysis at RSUP Dr. M. Djamil Padang. Methods: This study used a correlation study design and a cross-sectional method. The total sample was 81 respondents. Using purposive sampling with inclusion criteria, patients routinely undergo HD > 3 months, are over 18 years old, and can communicate well. The instruments used were the Self-Efficacy for Managing Chronic Disease (SEMCD) questionnaire and the Fluid Intake Restriction Adherence questionnaire—data analysis using the Spearman correlation test. Result: The results revealed a significant relationship between self-efficacy and adherence to fluid intake restrictions (p = <0.001) with a value (r = 0.703) which means the correlation for both variables is firmly in positive relation. Conclusion: Nurses in the HD room are expected to be able to provide self-efficacy training to patients to increase patient compliance, the higher the self-efficacy, the more compliant the patient with fluid intake restrictions. Keywords: Adherence, Fluid,  Hemodialysis, Self-Efficacy.

Abstract

The Penta-helix collaboration on disaster preparedness requires a comprehensive and collaborative role between various fields in society as an initiative from the National Agency for Disaster Countermeasure by strengthening the role of academics, government, the private sector, organizations and the community itself. This activity aims to evaluate the disaster preparedness necessity of the community after the 7.3 magnitude earthquake through a focus group discussion of the disaster preparedness penta-helix collaboration team in a district of West Sumatra. This activity was carried out on May 24. This activity invited disaster nursing experts from the Polytechnic University of Hong Kong. The activity was carried out with focus group discussions inviting various essential community disaster preparedness stakeholders, namely the Faculty of Nursing Universitas Andalas, BNPB, BASARNAS, Ministry of Health, Indonesia Red Cross (PMI), and community leaders. On this occasion, thematic results were obtained from focus group discussions on the importance of community empowerment, strengthening family values and vulnerable groups’ advocacy. Through evaluating disaster preparedness planning, it is crucial to know the need to be met after the community responds to an actual disaster event. The collaborative role of disaster penta-helix stakeholders can provide recommendations for strengthening disaster preparedness planning.

Abstract

Adolescents are exposed to a variety of stressors during the challenging adolescent transition phase. Nowadays, adolescents are more susceptible to mental health issues, which are reportedly becoming more prevalent globally. Involve schools in initiatives to address adolescent mental health to raise teenage understanding and awareness. Adolescents' mental health status can be assessed by mental health screening and education, and mental health education can help them know more about their mental health. This activity aims to expand students' awareness of mental health issues at public junior high schools in Padang. The IDASS-Y adolescent health self-reporting instrument was used to implement the intervention. The dissemination of information about adolescents' mental health issues and strategies for preventing them served as the basis for educational initiatives. According to the activity's findings, 25 students in Junior High School had mild depression (92.6%), one who had moderate depression (3.7%), and one who had severe depression (3.7%). The prevalence of anxiety among SMPN 23 students was 25 mildly anxious individuals (92.6%) and two moderately anxious individuals (7.4%). Twenty participants (74.1%) reported light stress, six (22.2%) reported moderate stress, and one (3.7%) reported severe stress. The knowledge score has improved before and during mental health education—the importance of involving schools in mental health management, including education and screening.

Abstract

Background: Managing dietary and fluid intake for hemodialysis patients is crucial, but it is also challenging. Non-compliance to diet and fluid intake restriction may increase a patient’s morbidity and mortality. Exploring the experiences of hemodialysis patients who successfully manage those regimens may provide new insight to develop more effective strategies. Purpose: The study aimed to explore the experiences of hemodialysis patients who successfully manage the dietary and fluid intake restrictions.  Methods: A descriptive qualitative study was applied. Semi–structured interviews were conducted with 15 hemodialysis patients purposively recruited from a hemodialysis unit of a public hospital in West Sumatera, Indonesia. Interviews were transcribed verbatim and thematically analyzed to describe the phenomenon of hemodialysis patients. The study was reported following the COREQ guidelines. Results: Most   participants in this study are female, aged between  38 – 63 years and length of hemodialysis 4 months – 6 years. The finding of this study was that the essential themes that explained how hemodialysis patients adhere to fluid and dietary restrictions   are 1) strategies for restricting fluid and dietary intake, 2) motivation to restrict fluid and dietary intake, 3) fear of the effects of non-adherence to fluid and dietary restrictions and 4) social support.

Abstract

Background: The progress of the fight against HIV is highlighted by significant change. HIV of the past is different from HIV of the present. Healthcare services have played an essential role in achieving the optimal goals needed to end the HIV epidemic. However, people living with HIV and those at risk of catching it (PLWH) often misunderstand the rapid growth of HIV healthcare service options. Objective: This study aimed to explore the experiences of PLWH in the healthcare services featured in this study. Methods: A qualitative phenomenological approach was used. Semi-structured interviews were conducted in 2017 with 12 PLWH who engaged with healthcare services in Jakarta, Indonesia, by using a purposive sampling technique. Semi-structured questions were asked which related to their experiences of using the services. Stevick Colaizzi Keen method was used to extract the thematic analysis of the study. Results: The study developed four essential themes of PLWH healthcare use. They were accessibility, availability at all healthcare levels, comprehensiveness of service, and affordability. Conclusion: Providing accessible healthcare services is considered essential by PLWH. It is also pivotal to helping people feel positive about the community-related healthcare services on offer. Nurse-led HIV services must maintain this progress by continuously evaluating the quality-of-service outcomes and promoting the accessibility of the services to the broader population.

Abstract

The flash floods and landslides that occurred in Surantih, Pesisir Selatan Regency caused various health problems. Post-disaster emergency response has been well coordinated through government hospitals and public health centers. However, the analysis of the Crisis Center of the Indonesian Ministry of Health and the Public Health Office of Pesisir Selatan District indicates a potential decline in health conditions, both physically and psychologically. The risk of post-disaster illness and psychological trauma needs to be prevented through the provision of preventive-oriented health services. Barriers to access to health services, especially for residents still living in disaster sites and refugees who are scattered due to infrastructure damage, exacerbate the situation. Therefore, it is necessary to provide a mobile clinic to overcome these obstacles and prevent a decline in physical and psychological health conditions. This activity aims to make a real contribution to post-disaster health management, and actively involve stakeholders in the establishment of mobile clinics. The implementation of the mobile clinic was carried out at four points that reached victims affected by the cold lava disaster. The number of health workers deployed was 8 specialist doctors 6 nurses 2 pharmacists and 4 psychologists. Mobile clinics provide services for post-disaster health problems and post-disaster chronic diseases. From the results of screening and health history taking, there were 90 disaster victims who had health problems such as skin diseases, chronic diseases, and post-disaster trauma problems. The implementation of mobile clinics provides solutions to obstructed access during disaster events, immediate handling of post-disaster physical problems and provides continuity of care for chronic diseases when post-disaster health services stop.
