Abstract

BACKGROUNDS: Nurses are at risk for mental health problems while caring for Corona Virus Disease 2019 (COVID-19) patients. The COVID-19 pandemic had impacted not only on emotions but also nurses' coping strategies. The difference between this study and previous research in this study adds up the total DASS 42 (Depression Anxiety Stress Scale) scores as parameter to assess mental health problems of the subjects. OBJECTIVE: To determine the association between coping strategies and the DASS score among nurses on duty in the COVID-19 isolation room of Dr Kariadi Hospital. METHODS: This cross-sectional study involved nurses who treated patients in the COVID-19 isolation ward, the COVID-19 intensive room, and the COVID-19 emergency room. All participants were involved by the consecutive sampling method. The research instrument used a sociodemographic questionnaire, the Brief COPE, and the DASS 42. Inclusion criteria included nurses who served in the COVID-19 isolation room and aged 22-60 years. Higher DASS score indicates that the subject is experiencing general psychological distress compared to subjects with a lower score. RESULTS: Most of the respondents in this study (n=112 subjects) had problem-focused coping strategies. The mean DASS score on the subjects is 14.29 ± 13.25. There was an association between coping strategies (p=0.048), sex (p < 0.001), place of work (p = 0.041), and DASS score. CONCLUSION: There was a relationship between coping strategies and the DASS score of nurses on duty in the COVID-19-19 isolation room. Further research needs to explore environmental factors and social support, also examine physical illnesses in more detail.

Abstract

This study aims to evaluate the use of the Orlando Model in emergency psychiatric nursing through a narrative review. The Orlando model is a conceptual framework used in nursing practice to understand patient experience and provide more holistic care. In this study, we conducted a literature search across various databases and evaluated literature studies relevant to the use of the Orlando Model in emergency psychiatric nursing. The results of this narrative review show that the use of the Orlando Model in emergency psychiatric nursing can help nurses understand the patient experience holistically and provide more effective care. The Orlando model can assist nurses in identifying psychological and social problems underlying the patient's condition, as well as providing more individualized care and focusing on the patient's needs. In addition, the use of the Orlando Model can also improve communication between nurses and patients, thereby helping to build a better relationship between the two. However, there are some limitations in this literature study, such as the lack of a number of relevant studies and the limited variation in population and research settings. Therefore, further research is needed to confirm the findings from this literature study and evaluate the effectiveness of using the Orlando Model in emergency psychiatric nursing in a more comprehensive manner.

Abstract

Introduction: Nowadays, mental health literacy through internet videos, such as YouTube®, plays an important role for individuals experiencing depression. However, the majority of YouTube® videos related to depression are generally in English. This study aims to provide an overview of Indonesian-language YouTube® videos related to depression. Methods: The keyword “depression” is entered into the YouTube® search engine, and based on the highest number of views, the top 100 videos are selected for analysis regarding the characteristics of the videos and various information related to depression. This includes causes of depression, symptoms of depression, prevention/therapy of depression, recovery from depression, and sources of information about depression. Each characteristic will be compared based on the number of views, likes or dislikes, and number of comments. Results: Only 10% of Indonesian-language videos with the keyword “depression” are uploaded by professional channels, and 6.57% of the videos are sourced from professionals. Most videos do not provide information about prevention or therapy (57%) and the recovery from depression (78%). The majority of the videos mention that depression is caused by life stress (49%) with symptoms of suicidal thoughts (39%) and sadness (25%). However, only 16% of the videos state that depression can be treated or prevented with professional consultation. Conclusion: Depression literacy through Indonesian-language YouTube® videos is still lacking. There is a need for YouTube® videos that provide information about depression, come from professional sources, and include encouragement to seek professional help.

Abstract

Schizophrenia is a serious mental disorder that affects around 1% of the world's population. Some patients with schizophrenia experience psychiatric emergencies that require prompt and appropriate intervention to reduce the risk of complications and promote a good outcome. This systematic literature review aims to describe of emergency psychiatric interventions in schizophrenia. The results of the review show that appropriate and prompt intervention are very important in reducing the risk of complications and improving good outcomes in schizophrenic patients who experience psychiatric emergencies. Some effective interventions include administering antipsychotic drugs, inhaled loxapine and providing non-pharmacological interventions, namely verbal de-escalation. However, more research is needed to clarify the benefits and effectiveness of each of these interventions. Suggestions that can be given are increasing public awareness about the symptoms of schizophrenia and what to do in an emergency situation, special training for medical personnel and emergency workers, as well as further research on emergency psychiatric interventions in schizophrenia.

Abstract

Background: Cushing’s syndrome is condition caused by excessive glucocorticoid with insomnia as one of its neuropsychiatric manifestation. Cushing’s syndrome may be caused by excessive adrenocorticotropin hormone (ACTH-dependent), for example from ACTH producing pituitary tumors, or by overproduction of cortisol by adrenocortical tumors. In this report, we presented a case with Cushing’s syndrome manifesting as chronic insomnia with adrenal cortical adenoma and pituitary microadenoma.Case presentation: A 30-year-old woman was consulted from the Neurologic Department to the Internal Medicine Department with the chief complaint of insomnia and worsening headache since 6 months prior to the admission. She had undergone head MRI and abdominal CT scan previously and was found to have both pituitary microadenoma and left adrenal mass. From the physical examination she had clinical signs of Cushing’s syndrome like Cushingoid face and purplish striae on her stomach. Midnight cortisol serum examination was done initially and showed high level of cortisol. High dose dexamethasone suppression test or DST (8 mg overnight) was later performed to help determine the main cause of Cushing’s syndrome. The result failed to reach 50% suppression of cortisol serum, suggestive that the Cushing’s syndrome was not ACTH-dependent from the pituitary but potentially from overproduction of cortisol by the left adrenal mass. Therefore, left adrenalectomy was performed and the histopathological study supported the diagnosis of adrenal cortical adenoma. Conclusion: Chronic insomnia is a very important symptoms of Cushing’s syndrome that should not be neglected. The patient had both microadenoma pituitary and left adrenal mass thus high dose DST test (8 mg overnight) needed to be performed to differentiate the source of Cushing’s syndrome. The result showed only little suppression therefore the pituitary microadenoma was not the source of Cushing’s syndrome and more suggestive from the adrenal etiology.
