Abstract

ABSTRACT Self-foot care is an essential measure to prevent foot ulcers caused by complications of Diabetes Mellitus (DM). Routine foot inspection and care are components of self-care behavior that should be practiced by individuals with DM. This study aimed to determine the effect of health education on foot care behavior among DM patients at Tanjung Priok Public Health Center, North Jakarta. The study employed a quasi-experimental design with a post-test only non-equivalent control group approach. A total of 60 DM patients were selected using non-probability sampling with a quota sampling method, consisting of 30 participants in both the experimental and control groups. Data collection was conducted in June 2024. The intervention consisted of a one-time health education session delivered using PowerPoint and educational videos. The instrument used to assess foot care behavior was the Foot Care Behavior Scale (FCBS) questionnaire. Univariate analysis showed that the majority of respondents in the experimental group were aged 56–65 years (50.0%), while those in the control group were aged 46–55 years (46.7%). Most respondents were female (63.3% experimental; 73.3% control), had a senior high school education (50.0%) in the experimental group and elementary/junior high school education (53.3%) in the control group, were housewives (40.0% and 33.3%), and had been living with DM for more than five years (56.7% and 53.3%). The Independent Sample T-Test showed a significant difference in foot care behavior between the intervention and control groups (p = 0.000; p 0.05). The findings indicate that health education has a positive effect on improving foot care behavior in DM patients. It is recommended that foot care education be provided routinely by healthcare professionals and that families actively support patients in performing daily foot care practices. Keywords: Diabetes Mellitus, Health Education, Foot Care Behavior  ABSTRAK Perawatan kaki secara mandiri merupakan langkah penting dalam mencegah terjadinya luka kaki akibat komplikasi Diabetes Mellitus (DM). Pemeriksaan dan perawatan kaki secara rutin termasuk dalam perilaku perawatan kaki mandiri yang perlu dilakukan oleh penderita DM. Penelitian ini bertujuan untuk mengetahui pengaruh edukasi kesehatan terhadap perilaku perawatan kaki pada penderita DM di Puskesmas Tanjung Priok, Jakarta Utara. Desain penelitian ini adalah kuasi-eksperimental dengan pendekatan post-test only non-equivalent control group. Sampel terdiri dari 60 pasien DM yang dipilih menggunakan teknik non-probability sampling dengan metode quota sampling, masing-masing 30 responden pada kelompok eksperimen dan kontrol. Pengumpulan data dilakukan pada Juni 2024. Intervensi berupa edukasi kesehatan diberikan satu kali menggunakan media presentasi (PowerPoint) dan video edukatif. Instrumen yang digunakan adalah kuesioner Foot Care Behavior Scale (FCBS) untuk mengukur perilaku perawatan kaki. Hasil univariat menunjukkan bahwa mayoritas responden pada kelompok eksperimen berusia 56–65 tahun (50,0%) dan kelompok kontrol 46–55 tahun (46,7%), sebagian besar berjenis kelamin perempuan (63,3% dan 73,3%), memiliki pendidikan terakhir SMA/SMK (50,0%) dan SD/SMP (53,3%), bekerja sebagai ibu rumah tangga (40,0% dan 33,3%), serta telah menderita DM lebih dari lima tahun (56,7% dan 53,3%). Hasil uji Independent Sample T-Test menunjukkan terdapat perbedaan yang signifikan pada perilaku perawatan kaki antara kelompok eksperimen dan kontrol (p = 0,000; p 0,05). Dapat disimpulkan bahwa edukasi kesehatan berpengaruh terhadap peningkatan perilaku perawatan kaki pada penderita DM. Diharapkan edukasi tersebut dapat diberikan secara rutin oleh tenaga kesehatan, serta didukung oleh keluarga untuk meningkatkan kepatuhan pasien dalam melakukan perawatan kaki sehari-hari. Kata Kunci: Diabetes Mellitus, Edukasi Kesehatan, Perilaku Perawatan Kaki

Abstract

Background: The modern era that occurs couses lifestyle changes such as hing consumption patterns of fast food and lock of physical activity couses an increase in cronic diseases, one pf them Diabetes Mellitus (DM). The purpose of this study was to analyze the effectiveness of community-based healthy lifestyle intervention program (Co-HELP) Modification in DM patients at the Waai Health Center, Ambon. Methods: Quasy experiments were utilize. There were 76 respondents which chosen using purposive sampling techniques and divided into 2 groups where 51 respondents into intervention group and 25 respondents as control group. Quality of life was measured by DQOL questionnaire. Analysis of research results with the Wilcoxon and Mann-Whitney test Results: It was revealed that majority of respondents on the elderly category, women (77.6%), suffer from DM >5 years (67.1%), light work (82.9%). After 6 weeks of Co-HELP Modification, statistical test declared that there were significant differences in quality of life (DQOL) (p = 0,000), fasting blood sugar (FBS) levels (p=0,000) pre-post intervention, however there were no significant difference for systole and diastole blood pressure. The logistical regression test revealed that Co-HELP Modification intervention, age, gender, long suffered from DM and occupation were significant impact to quality of life simultaneously (p=1,000),blood glucose (p=0,975), SBP (p=1,000) and DBP (p=0.315). Conclusion: It is recommended to implement Co-HELP Modification as a preventive aspect to prevent complication so that quality of life improves

Abstract

Diabetes mellitus (DM) is as a chronic metabolic illness with a high prevalence that may leads to various complications. HbA1C level is used primary indicator to evaluate whether diabetes management is well controlled. This study aimed to analyze the relationship between duration of illness and self-management with Glycemic control among patients with diabetes mellitus at Hospital X Jakarta. This study employed a quantitative study with descriptive correlation and cross-sectional approach. A total of 62 respondents were selected using purposive sampling technique based on inclusive and exclusive criteria. Data were collected using instrument of DSMI (Diabetes Self-Management Instrument) questionnaire and HbA1C from medical record. Data were analyzed with Kendall’s tau C statistical test. The result showed significant relationship between self-management with HbA1C level with p-value of 0.022 (p<0.05). Meanwhile, there was no significant association between duration of illness with HbA1C level with p-value of 0.143 (p>0.05). The findings indicated that patients with lower self-management tended to have uncontrolled HbA1C levels, whereas patients with a longer duration of DM tended to demonstrate poorer glycemic control, although the association was not statistically significant.  This study suggests that nurses should provide education and health intervention to improve patient’s self-management in order to prevent complications and enhance quality of life.

Abstract

Background: Diabetes mellitus (DM) is a chronic metabolic disease characterized by hyperglycemia and glucose intolerance, which may lead to various complications, including diabetic foot ulcers. Inadequate foot care practices among patients with DM remain a significant concern and may increase the risk of foot injuries and infections. Objective: This study aimed to identify factors associated with foot care behavior among patients with DM at the Bakauheni Health Center, South Lampung. Methods: This study employed a quantitative, descriptive observational analytic design with a cross-sectional approach. The sample consisted of 91 respondents selected through a purposive sampling technique. Data collection was conducted at the Bakauheni Health Center. The research instrument used was the HDFSS & NAFF questionnaire to measure family support and foot care behavior. Results: The results showed that the majority of respondents were female (60.4%), in the late elderly group (41.8%), and had a basic level of education (84.6%). Most respondents reported high family support (51.6%) and demonstrated good foot care behavior (79.1%). Bivariate analysis using Kendall’s Tau-b test revealed a significant correlation between duration of illness and family support with foot care behavior in DM patients (p < 0.05). Furthermore, ordinal logistic regression analysis indicated that family support was the most influential factor affecting foot care behavior (p = 0.037; OR = 4.039). Conclusion: In conclusion, family support plays a crucial role in improving foot care behavior among patients with DM. It is recommended that healthcare teams, particularly community nurses, involve family members to enhance adherence to foot care practices and prevent diabetic foot complications.

Abstract

Peripheral Arterial Disease (PAD) merupakan penyempitan pembuluh darah arteri perifer yang disebabkan karena aterosklerosis akibat komplikasi dari Diabetes Mellitus (DM). Akibatnya terjadi penurunan aliran darah ke ekstremitas yang bila  tidak ditangani akan menyebabkan klaudisio intermiten, luka gangrene hingga berisiko amputasi. Penelitian ini bertujuan untuk mengetahui Efektivitas Buerger Allen exercise (BAE) dibandingkan dengan Rendam Kaki Air Hangat Terhadap Nilai Ankle Brachial Index (ABI) dan Gula Darah Sewaktu(GDS) pada pasien DM tipe 2. Jenis penelitian ini adalah Quassy Eksperimen prepost test and between two groups. Total sampel 54 penderita diabetes melitus di Rumah Sakit Sumber Waras yang diperoleh secara random dengan blok permutasi. Sampel dibagi menjadi 2 kelompok perlakuan yaitu BAEdan Rendam kaki air hangat. Perlakuan diberikan selama 5 hari berturut-turut dengan pelaksanaan 2 kali sehari yaitu pagi dan sore selama 15-20 menit. Alat yang digunakan untuk mengukur nilai ABIadalah Vascular Dopler. Berdasarkan uji paired T test di temukan ada perbedaan yang bermakna nilai ABI sebelum dan sesudah intervensi pada kelompok BAE (p=0.00), dan pada kelompok Rendam kaki air hangat (p=0.00); namun tidak ada perbedaan bermakna pada nilai GDS sebelum dan sesudah intervensi pada kedua kelompok perlakuan (p>0.05).Uji statistic Ancova di temukan ada pengaruh BAE terhadap nilai ABI dan Gula Darah Sewaktu yang dikontrol oleh variabel confounding dengan Corrected Model 0,000; namun tidak ada pengaruh rendaman kaki air hangat terhadap nilai ABIdan GDS yang dikontrol oleh variabel confounding dengan Corrected Model 0,065. BAE merupakan intervensi yang efektif dalam meningkatkan nilai ABI dibandingkan rendam kaki air hangat, namun Rendam kaki air hangat  lebih efektif dalam menurunkan nilai GDS.

Abstract

Diabetes Mellitus (DM) merupakan penyakit yang membutuhkan pengontrolan status metabolik untuk meminimalkan terjadinya komplikasi multi organ. Diperlukan edukasi yang terarah untuk meningkatkan perawatan diri dan indeks glikemik yang terkontrol. Tujuan penelitian ini untuk mengetahui efektivitas edukasi Self-care terhadap perilaku manajemen diri dan Gula Darah Sewaktu (GDS) pada pasien DM. Penelitian ini dilakukan bulan Juni-Juli 2020 diwilayah kerja Puskesmas Kelurahan Sukapura Jakarta. Desain penelitian adalah quasy eksperimental dengan pre-test dan post-test one group. Sampel dalam penelitian sebanyak 25 responden dipilih secara purposive sampling. Variabel GDS dan perilaku manajemen diri dinilai pada minggu 1 dan 4 dengan menggunakan kuesioner Diabetes Self-Management Instrument (DSMI). Berdasarkan uji Paired T-test ditemukan ada perbedaan bermakna pada perilaku manajemen diri sebelum dan sesudah diberikan edukasi Self-care (p 0,000 < 0,05 ), namun, tidak ditemukan adanya perbedaan bermakna pada GDS sebelum dan sesudah diberikan edukasi Self-care (p 0,956 > 0,05). Berdasarkan uji multivariate ditemukan bahwa lamanya menderita DM dan kadar GDS sebelum intervensi berpengaruh secara parsial terhadap variabel GDS setelah intervensi (p<0.05), sedangkan variabel usia dan lamanya menderita DM berpengaruh secara parsial terhadap perilaku manajemen diri (p< 0.05). Edukasi Self-care merupakan program efektif untuk memperbaiki perilaku perawatan diri dan GDS sebagai kontrol glikemik. Penelitian ini merekomendasikan perlunya menerapkan edukasi Self-care secara berkelanjutan untuk mencegah terjadinya komplikasi pada pasien DM.

Abstract

Latar belakang: Tingginya kejadian obesitas menjadi salah satu pemicu terjadinya prediabetes, dan banyak masyarakat yang tidak menyadari bahwa mereka menderita prediabetes. Penelitian bertujuan untuk mengetahui hubungan antara self-awareness pola makan terhadap kejadian Prediabetes di wilayah kerja Puskesmas Johar Baru. Tujuan: Penelitian yang bertujuan untuk menganalisa hubungan Self-awareness pola makan terhadap kejadian Prediabetes pada responden yang berada di wilayah kerja Puskesmas Johar Baru. Metode: Penelitian menggunakan Kuantitatif-non eksperimental dengan pendekatan cross sectional. Jumlah responden sebanyak 64orang yang dipilih menggunakan teknik purposive sampling. Penelitian ini menggunakan kuesioner Self Awareness Pola Makan dan Finnish Diabetes Risk Score (FINDRISC) untuk mengukur prediebetes. Analisa data menggunakan uji statistic Kendall Tau-C. Hasil: Analisis univariat menunjukkan mayoritas responden memiliki self-awareness rendah (62.5%) dan responden memiliki risiko prediabetes sedikit meningkat (34.4%) diikuti dengan risiko prediabetes tinggi (26.6%). Analisis bivarat menunjukkan ada hubungan yang bermakna antara self-awareness pola makan terhadap kejadian prediabetes (p <0.05). Kesimpulan: Hasil penelitian ini menunjukkan terdapat bahwa self-awareness pola makan berhubungan dengan kejadian prediabetes di wilayah kerja Puskesmas Johar Baru, Jakarta.
