Abstract

Conjoined twins are a rare resulting from late and incomplete division of monozygotic embryonic disk generally after 13th day of fertilization. Most cases of separation are extremely risky and life threatening. We present two successful cases of conjoined twins separations, who were admitted to Cipto Mangunkusumo Hospital, both in early 2010. Lulu-Lala, the abdominophagus conjoined twins and NaylaNabila the Abdominothorakophagus conjoined twins. The successions of this conjoined twins separation depends on the techniques, precautions and the team that works together to treat the patients. One of the separation techniques is the utilization of tissue expander to loosen the skin and so can be close primarily by primary closure. The foremost precaution is the infection control, including preparation and sterilization. Conjoined twins have a particular structural defect. Forty percent of them are stillborn and 35% survive only for one day. The mortality is increased by many causes and the most common cause is infection after surgery. All aspects in the team have to give priority to the sterility to avoid infection of those babies. The efforts to avoid the infections must have been done start from pre-surgery managements until the post-surgery.

Abstract

Hemiglossectomy defects need reconstructions to restore normal functions and aesthetic outcomes. It is therefore prudent that well-planned reconstructive technique is needed for a good functional substitution for the defect. This case report describes a 30 years old male who developed T2N1M0 squamous cell carcinoma of the left tongue. The patient underwent a tongue reconstruction following his hemiglossectomy involving floor of the mouth without mandibular resection. The optimum method of reconstructing a major defect such as hemiglossectomy with floor of the mouth resection is best carried out by using microvascular free tissue transfer.  We conclude that the radial forearm free flap is good method for reconstructing major defects of the tongue. The radial forearm free flap is thin, can be raised without patient position change, has constant vascular anatomy, and provides an advantage in the aesthetic aspect at the donor site.

Abstract

Neurofibromatosis type 1 is a rare disease which can manifest itself by the development of plexiform neurofibromatosis, with craniofacial deformities. In this paper, we propose special consideration in excision nasolabial fold mass to help lifting procedures in neurofibromatosis patients. We are reporting two case, both presented with von Recklinghausen’s disease, a 21 years old male patient with neurofibromatosis in his right hemifacial for which he had been operated on five times previously at other center, and a 24 years old male with neurofibromatosis in his left hemifacial. After several stages of reconstruction which started with nasolabial fold mass excision, the result was satisfactory, by using the multiple stage repair, it was done until the end result is similar to anatomical form. The mass debulking procedures for these patients, which started with Nasolabial Fold mass excisison are continued with durable lifting procedures. This method gives results a satisfactory lifting procedure.

Abstract

School management strategy in an effort to increase the absorption capacity of graduates is due to the low absorptive capacity of vocational school graduates in the business and industrial world. One of the indications is influenced by the lack of communication between the school and the business and industrial world. This study aims to describe and analyze the collaboration management of SMK Negeri 3 Semarang with the business and industrial world, starting from planning, implementation, and evaluation. This research is a qualitative research. Sources of data obtained from (1) informants from SMK N 3 Semarang and industry; (2) the process of planning, organizing, implementing, supervising collaboration between SMK N 3 Semarang and the business and industrial world. While data collection techniques are observation, interview and documentation. Data analysis techniques used include the following procedures: (1) data collection in the field; (2) data reduction; (3) data display and (4) verification and conclusion drawing. The results of the study: (1) Planning was completed through: the formulation of the plan by presenting an industry delegate, setting the plan with a plenary meeting attended by the teacher council, parents of students and school committees, and evaluation of the plan by Evabang; (2) Organizing, coordinating and collaborating between vocational high schools and industry to carry out learning in the form of theory by inserting character education, school practice, and practice, (3) Implementation is adjusted to the establishment of plenary meeting plans (4) Evaluation from industry, BAN-S/M and ISO.

Abstract

Background: The change in !ap blood flow patency can be monitored by measures. Subjective clinical observation through flap examinations is greatly biased depending on examiner’s experience. Standardised equipments of assessment are more reliable. This study propose the use of a device to provide an objective, safe, reproducible, simple, portable, and cost-effective method of monitoring flaps vitality by measuring serum concentration of lactic acid. Methods: An experimental lab-based study was conducted utilising Sprague-Dawley rats. Twenty rats were randomly assigned into two groups. In all subjects, bilateral groin-flaps were elevated. In rats of Group A, the vein pedicle of one-side of the flap was occluded while in Group B both the artery and vein on one-side of the flap were occluded. The other side of flaps in each rat were left unoccluded to serve as controls. Baseline serum lactate was measured in all flaps, then remeasured 60 and 120 minutes in all flaps after pedicle manipulations. Results: The mean lactate concentration of Group A rats with vein-occluded !aps was 2.5 ± 0.17 mmol/L at 0-min initially, and increased to 7.9 ± 0.16 mmol/L 120-min after occlusion (p<0.0005). The mean lactate concentration of Group B rats with arterial and venous flap occlusion was 2.55 ± 0.21 at baseline, which increased four-fold at 120-min to 9.86 ± 0.28 mmol/L (p<0.0005). Among the two groups, the lactate difference was also found to be significant. Conclusion: This study demonstrates that the proposed method detects serum lactate changes in flaps with vein and arteriovenous occlusions. This thus can be used as an objective parameter to evaluate compromised blood flow on cutaneous flaps.

Abstract

Fingerroot (Kaempferia pandurata Roxb.) is one other from varient traditional medicine plant was many grow in Indonesia. This plant had many compounds of secondary metabolite that used for antibacterial. The other one bacterial causing tooth plaque was tooth and mouth diseases which many alert by Indonesians. The most roling bacterial in causing it is Streptococcus mutans. The study aims to determine isolation flavonoid compound and the activity to inhibit bacterial growth. The compound obtained with isolates the fingerroot using coloumn chromatography was extracted with maseration method. Coloumn chromatography result analized with UV-Vis and FTIR spectrofotometre. Antibacterial activity method is agar difusion using paper disk. The results showed that isolation compound is flavanone, that is 6.7.3’.4’-tetrahidroxy flavanone. Activity showed that flavanone is resistent against Streptococcus mutans and have lower activity than ethanol extract samples amount 1.85 mm.

Abstract: Celery, Hypercholesterolemia. Dyslipidemia is a lipid metabolic disorder characterized by elevated total cholesterol, LDL cholesterol, triglycerides above normal values and a decrease in HDL cholesterol. In traditional medicine, herb celery has been used as a medicine to lower cholesterol levels because of the content of unsaturated fatty acids in celery plants. The purpose of this study was to determine the effect of celery extract on the decrease of hypercholesterolemia in Elderly. Type The design of this study is quasi experimental with one group pre test post test design. The population in this study were all elderly in Panti Wreda Darma Bhakti Kasih Surakarta in 2016 with total sampling that is all respondents of the research after examination total cholesterol, triglycerides and LDL at Parahita Diagnostic Center Surakarta Laboratory with high cholesterol result of 29 people. Result of Analyst Paired t-test Total Cholesteol levels before and after giving Celery extract in Elderly showed P value 0.029 which means there is influence between before and after giving Celery extract. Result of Willcoxon test analysis of Triglyceride level before and after giving Celery extract in Elderly obtained P value: 0,414 which means no influence between before and after giving Celery extract. The results of Willcoxon test analysis of LDL levels before and after giving Celery extract in Elderly obtained P value: 0,102 which means no influence between before and after giving extract Celery
