Abstract

Background: One of the most prevalent knee injuries among athletes is a torn anterior cruciate ligament (ACL). ACL injuries may lead to various unpleasant symptoms, including joint instability, muscular atrophy, and soreness. ACL reconstruction is usually recommended to restore peace and function to the knee joint. After ACL reconstruction, knee muscle strength can remain compromised, leading to deficits in stability and functional limitations. ACL reconstruction also has psychological consequences, such as fear of moving due to injury or kinesiophobia. Purpose: To determine the relationship between lower leg muscle strength and kinesiophobia with functional stability and knee function in patients with ACL reconstruction. Method: The research method used is a literature review using data from scientific articles in scientific journal databases related to muscle strength, kinesiophobia, functional stability, and knee function after ACL reconstruction. The literature search used the keywords "ACL," "Strength," "Function," and "Kinesiophobia" by combining the Boolean Operators" OR "and "AND." Results: Based on the results of the journal review, it was found that after ACL reconstruction, muscle strength and kinesiophobia will affect the functional stability and function of the knee. Decreased muscle strength will lead to lower knee stability and function. Kinesiophobia level is associated with poorer clinical outcomes after ACL reconstruction. Conclusion: Based on the review results, it can be concluded that there may be problems arising from ACL reconstruction. Decreased muscle strength and psychological consequences such as fear of moving due to injury or kinesiophobia can affect the knee's functional stability and function.

Abstract

Background: Anterior Cruciate Ligament Reconstruction (ACLR) is an orthopedic surgical procedure used to restore a functional knee. Surgical treatment in the form of ACL reconstruction can be performed to optimize quality of life after ACL injury. Decreased knee function after ACLR is closely related to a person's quality of life in the long term considering that the knee and other lower limbs have a crucial role in mobilizing and controlling the human body. Objective: To determine if there is a relationship between lower limb strength after ACL reconstruction and knee function also quality of life. Methods: The research method used is a literature review method using secondary data sources through a database of various scientific treatises accessible from internet sites. Scientific papers were searched using keywords such as ACL reconstruction, lower limb strength, knee function, and quality of life to facilitate proper literature retrieval. Result: Based on the results of the journal study, it was found that there was a decrease in lower extremity muscle strength and a decrease in knee function compared to participants who did not experience ACL injury-ACLR which caused a decrease in the quality of life of a person so that the preference of physical activity carried out in daily activities also changed with a low level of satisfaction from the expectations and expectations of individuals with ACLR. Conclusion: Based on some literature that has been discussed, there may be a correlation between the strength of the lower extremities and knee function with the quality of life of a person after anterior cruciate ligament reconstruction with the main predictor of decreased performance in daily physical activity which is closely related to the main function of the lower extremities, especially the knee.

Abstract

Introduction: Assessing the importance of muscle strength and functional stability in post-ACL reconstruction (ACLR) patients demands an in-depth understanding of how physical factors correlate with knee functional stability. Therefore, this study aims to determine the relationship between quadriceps muscle strength and functional stability in ACLR patients in the Indonesian ACL Community. Methods: This research design is observational analytic with a cross-sectional approach. The research subjects were 26 ACLR patients from the Indonesian ACL Community in Bali. This study used a hand-held dynamometer to measure quadriceps muscle strength and a single hop test to measure functional stability. Results: In the results of the Spearman Rho analysis, it was found that there was a significant relationship between quadriceps muscle strength and functional stability, as evidenced by the p-value = 0.000 (p <0.05) and the correlation coefficient value of 0.644 and positive. In addition, the multivariate results showed that subjects with poor quadriceps muscle strength were consistently associated with poor functional stability scores with odds 13.5 times higher than subjects with good quadriceps muscle strength p=0.013 (p<0.05). Discussion: It can be concluded that there is a significant relationship between quadriceps muscle strength and functional stability in ACLR patients at ACL Community Indonesia.

Abstract

Background: ACLR is an operative procedure to restore knee stability and reduce the risk of subsequent knee injury so that patients can return to sports and daily activities. Although this procedure has been successful in restoring knee stability, it is inevitable that some patients will experience the risk of postoperative subsequent knee injury. Currently, there is limited information regarding the causes of subsequent knee injury after surgery, including graft type, time of surgery, BMI, gender, age, and concomitant injuries. Therefore, the aim of this study was to identify the risk factors associated with subsequent knee injury after ACLR. Methods: This study used a prospective cohort design consisting of patients who underwent ACL reconstruction in the ACL Indonesia Community (n=116). Inclusion criteria included participants who joined the Indonesian ACL Community WhatsApp group, aged between 17 and 60 years, experienced unilateral or bilateral ACLR, and were willing to participate in this study. Exclusion criteria included the presence of fractures, infections, incomplete questionnaire, or MRI forms. In this study, secondary data were obtained through an online survey either by selecting available options or typing in additional information according to the patient's condition. It was then validated through a telephone interview to confirm the veracity of the patient's subsequent knee injury and the timing of its occurrence. Follow-up was conducted over a two-year period after the ACLR procedure to monitor the occurrence of subsequent knee injury. Results: The results of univariate and multivariate analysis using the Cox Proportional Hazard model showed that several patient characteristic variables had a significant association with the risk of subsequent knee injury after ACL reconstruction (ACLR). Some variables showed an increased risk of 1.98-3.86 times, while one variable showed a decreased risk of 0.96 times compared to the reference group. Multivariate analysis also found that individuals who sustained ACL injuries along with injuries to the meniscus and other ligaments had a risk of subsequent knee injury 3.73 times higher than those with ACL injuries alone. Conclusion: Individuals who sustained ACL injuries in addition to injuries to the meniscus and other ligaments had a higher propensity to reinjure their knee after undergoing ACL reconstruction surgery compared to individuals who only sustained ACL injuries with meniscus involvement alone.

Abstract

The growing number of elderly individuals in Indonesia has led to an increased prevalence of physical function impairments, including reduced lower limb muscle strength and postural changes such as kyphosis, both of which are fall risk factors in elderly women. An analytical observational study with a cross-sectional design was conducted on 60 elderly women (≥60 years) selected by purposive sampling from a population of 423 individuals. Lower limb muscle strength was measured using the Back and Leg Dynamometer, kyphosis degree using a flexicurve ruler, and fall risk using the Modified Indonesian Fall Risk Assessment Tool (M-IFRAT). Analysis was performed using Spearman correlation test. The mean respondent age was 67.63 years. The majority had very poor muscle strength (90.0%), 76.7% had normal posture, and 56.7% were classified as high fall risk. There was a significant relationship between lower limb muscle strength and fall risk (r = −0.618; p < 0.001) and between kyphosis degree and fall risk (r = 0.553; p < 0.001). Regression analysis showed that both variables together explained 53.4% of fall risk variation (R² = 0.534). Higher lower limb muscle strength is associated with lower fall risk, while higher kyphosis degree is associated with greater fall risk. Physiotherapy interventions targeting lower limb muscle strengthening and postural correction are recommended as fall prevention strategies for elderly women.

Abstract

The background of this community service is the shift in physical activity patterns among children in the digital era. Children now spend more time in static positions, negatively impacting their gross motor development. In Indonesia, prevalence data regarding the impact of this sedentary lifestyle show concerning figures. According to a recent study by Ma’arif (2022), it was found that approximately 25.98% of students fall into the low physical activity category, and a similar percentage (25.98%) was identified in the overweight/obese category. In response to these findings, the subsequent impact of inactivity and increased Body Mass Index (BMI) leads to a decline in vital biomotor components, specifically postural balance. Paradoxically, balance function has been proven to correlate with spatial ability and proportional reasoning, which support a child's academic performance. To address this, the offered solutions include: (1) early detection of balance disorders using the One-Legged Stance Test (OLST), (2) health education on the importance of physical activity, and (3) Home-Based Exercise training. The implementation was conducted through direct visits to SDN 1, SDN 2, and SDN 3 in Lebih Village, Gianyar, between July and August 2024. The participants consisted of 170 students (aged 9–12 years). The examination results showed that the majority of students fell into the "Excellent" category, totaling 164 students (96.5%), followed by the "Above Average" category (1.2%) and the "Average" category (2.4%). This program successfully enhanced the students' understanding of the importance of maintaining physical balance through independent exercise. Peserta kegiatan berjumlah 170 siswa (usia 9–12 tahun). Hasil pemeriksaan menunjukkan mayoritas siswa memiliki keseimbangan kategori "Sangat Baik" sebanyak 164 siswa (96,5%), diikuti kategori "Di atas Rata-rata" (1,2%) dan "Rata-rata" (2,4%). Kegiatan ini berhasil meningkatkan pemahaman siswa tentang pentingnya menjaga keseimbangan fisik melalui latihan mandiri.
