Abstract

Latar Belakang: Eritroderma merupakan suatu reaksi pada kulit ditandai eritema dan skuama generalisata. Penyakit yang mendasari eritrodema dapat ditelusuri melalui pemeriksaan histopatologis bila dilakukan biopsi multipel. Dermoskopi dilaporkan berguna sebagai alat diagnostik karena dapat menunjukkan pola reaksi yang spesifik. Tujuan: Melaporkan pemeriksaan dermoskopi sebagai alat bantu diagnostik pada kasus eritroderma psoriasis. Kasus: Kasus 1. Laki-laki, 38 tahun, suku Bali dengan eritroderma dan infeksi Human Immunodeficiency Virus (HIV). Pemeriksaan dermoskopi didapatkan pembuluh darah kapiler berbentuk titik dan glomerular dengan dasar eritema dan skuama putih. Pasien mendapat metil prednisolon 3x8 mg, sefoperason sulbaktam 2x1 gram, kotrimoksasol 2x960 mg, azitromisin 1x500 mg, flukonasol 150 mg, nistatin 4x100.000 IU, klortrimeton 3x4 mg, oleum olivarum pada kulit yang bersisik, dan krim kloramfenikol 2% pada ulkus di genetalia. Kasus 2. Perempuan, 46 tahun, suku Sumba dengan eritroderma. Pemeriksaan dermoskopi didapatkan pembuluh darah kapiler berbentuk titik dengan dasar eritema dan skuama putih. Hasil pemeriksaan histopatologis kedua kasus sesuai psoriasis vulgaris. Pasien mendapat metil prednisolon 2x8 mg, cetirizin 1x10 mg, oleum olivarum pada kulit yang bersisik, dan krim natrium fusidat 2% pada ulkus di ujung jari. Simpulan: Pola monomorf dengan skuama putih dan pembuluh darah berbentuk dotted/glomerular dengan latar belakang merah homogen merupakan pola khas dermoskopi untuk eritroderma psoriasis. Hasil dermoskopi pada kedua kasus didukung oleh pemeriksaan histopatologis. Pemeriksaan dermoskopi dapat digunakan sebagai alat bantu diagnostik kasus eritroderma psoriasis.

Abstract

Background: Orbital cellulitis is an active infection of the orbital soft tissue posterior to the orbital septum, which can cause Tolosa-Hunt Syndrome (THS) complication. Purpose: We reported a child with orbital cellulitis with THS complication caused by methicillin-resistant Staphylococcus aureus (MRSA). Case: A two-year-old girl complained of swelling and pain in the left eye accompanied by fever. In nasal region and left eye showed multiple erythema patches with geographic shape, size 1x2cm - 4x5cm, unclear border, erosion with sizes 0.5x1cm - 1x1.5 cm size covered with blackish crusts. After several days of hospitalization, she complained of proptosis and pain in moving her eye. We did the magnetic resonance imaging (MRI) scan examination, which showed a mass size of 2.2 x1.1 x 0.9 cm in a left intraconal orbital and dilation of the left-sided cavernous sinus. The blood culture showed MRSA bacteria. She was diagnosed with orbital cellulitis with THS complication and showed a good response with Meropenem, Methylprednisolone, cendo lyteers eye drops, levofloxacin eye drops, and gentamicin eye ointment. Discussion: Orbital cellulitis presents as ill-defined erythema, edema, warmth, and pain around the nasal and the orbital region and is more often found in children. The THS complication is characterized by ophthalmoplegia, unilateral orbital or periorbital pain, unilateral headache, and leukocytosis. Orbital cellulitis management includes antibiotic intravenous, corticosteroid, and eye care. Conclusion: THS is a rare complication of orbital cellulitis. Early diagnosis and management of orbital cellulitis are essential to prevent THS complication.

Abstract

Background: Purple sweet potato has been shown to contain anthocyanins, polyphenols, flavonoids, and tannins that act as antioxidants and have the potential to correct signs of aging in the skin, such as wrinkles, lack of hydration, and pigmentation. Purpose: to known the potential of 8% purple sweet potato extract for wrinkles, moisture, and elasticity. Methods: A one-group experimental study with pre and post-test assessment. The subject is women with photoaging aged 30-45 years. A pre-test was conducted by examining wrinkles using the Glogau scale and a simple A-one scanner, treatment with 8% purple sweet potato extract cream for 6 weeks, and then a post-test at the end of the 6th week to check wrinkles, moisture, and elasticity using a simple A-one scan. Statistical tests using the SPSS program, version 25.0. The analysis performed was descriptive statistical and comparative analysis using a paired T test.  Result: A total of 25 respondents with a mean age ± SD of 38.22 ± 4.23 years, the most Glogau scale in type 2 is 22 respondents (88%), the most sun exposure for 30 minutes per day is 14 respondents (56%). The degree of wrinkles decreased after treatment -4.960±1.353 [95% CI -7.681-(-2.239); P=0.001]. The degree of moisture increased after treatment 8.840±2.990% (95% CI 2.829-14.851; P=0.005. The degree of elasticity increased after treatment 14.840±4.623 (95% CI 5.544-24.136; P=0.002). Conclusion: 8% purple sweet potato extract can reduce the degree of wrinkles, improve the degree of skin moisture, and improve the degree of skin elasticity.
