Abstract

Background The pathophysiology of laryngopharyngeal reflux (LPR) is yet to be completely understood, but autonomic dysregulation may have a role in the opening of esophageal sphincters controlled by the parasympathetic nervous system, especially the vagal nerve. Autonomic dysregulation can be detected through heart rate variability (HRV). Currently, studies that identify autonomic nerve dysfunction through HRV in LPR patients are few, especially in the pediatric population. Objective To identify autonomic nerve dysfunction  in pediatric LPR patients with HRV components. Method This study involved 74 pediatric patients, 35 with LPR and 39 without LPR, from  Dr. Cipto Mangunkusumo Hospital, Jakarta, from September 2023 to April 2024. Diagnostics for LPR included endoscopy and Reflux Symptom and Sign Instrument (RSSI) scoring, while measurement of autonomic nerve dysfunction was determined by HRV. The HRV was evaluated by standard deviation normal to normal (SDNN) assessment of the heart’s interbeat interval (IBI) and low frequency/high frequency ratio (LF/HF). Results  Neither HRV measurement, SDNN or LF/HF, were significantly associated with autonomic nerve dysfunction in pediatric patients with LPR. Conclusion Autonomic nerve dysfunction measured by HRV was not associated with LPR in pediatric patients.

Abstract

Healthcare facilities generate medical waste. If not properly managed, medical waste may damage the environment and spread diseases. Many hospitals in Indonesia do not adhere to medical waste management regulations. This study aimed to evaluate the management of solid medical waste at four public hospitals in Bogor District, Indonesia. A case study design was used to obtain a comprehensive description of the solid medical waste management activities inthe hospitals. Data were collected through direct observations, interviews, and document reviews. This study revealed that the medical waste generated in allhospitals was infectious, pathological, pharmaceutical, chemical, and cytotoxic, totaling approximately 4,000-12,000 kg. Hospitals A, B, C, and D fulfilled the Regulation of the Indonesian Ministry of Environment and Forestry No. P56 of 2015 criteria, achieving compliance rates of 77%, 83.78%, 83.3%, and 86.48%, respectively. However, several activities of the hospitals did not meet the standards for waste reduction, segregation, on-site transport, temporary storage, and human resource quality. It is important that hospitals establish comprehensive and compliant medical waste management systems.

Abstract

Urinary glutathione levels are known to be an early indicator of oxidative stress in travelers. This study analyzed the association between particulate matter (PM2.5) exposure on the road and urine glutathione levels in Jakarta’s microbus drivers. This cross-sectional study involved 96 microbuses (one of Jakarta’s public transportations) drivers of nine routes in Kampung Melayu Bus Station, Jakarta, Indonesia. An anthropometric assessment and a structured questionnaire were employed. Along with the participants driving on the road, real-time personal equipment measuring PM2.5 exposure concentrations was used. Total glutathione levels were measured using a colorimetric method. A correlation test and linear regression analysis were used to examine the effect of PM2.5 exposure on total glutathione levels. The average PM2.5 exposure concentration was 90.9±1.8 μg/m3, with a maximum concentration of 114.7 μg/m3. The average urinary glutathione level was 1.3±0.5 μM. The regression analysis showed that PM2.5 was associated with urinary glutathione levels after controlling for body mass index and smoking status. To conclude, the drivers experience exposure to an extremely high level of PM2.5 that could influence the glutathione levels.

Abstract

Peluncuran Kartu Jakarta Sehat (KJS) pada bulan November 2012 mengakibatkan jumlah kunjungan pasien yang melonjak drastis di fasilitas kesehatan, khususnya rumah sakit, baik itu rumah sakit pemerintah pusat, rumah sakit pemerintahdaerah maupun rumah sakit swasta yang berkomitmen dengan Pemerintah Daerah Jakarta. KJS memberikan kemudahan akses layanan kesehatan agar pasien dapat berobat secara gratis. Namun persoalan yang muncul kemudian adalah pasien harus mengantre lama di loket pendaftaran, ruang pemeriksaan, maupun ruang pengambilan obat. Penelitian ini bertujuan untuk mengetahui hubungan faktor sosial budaya, pribadi, dan psikologis dengan kepuasan pasien pengguna KJS pada 110 responden di Unit Rawat Jalan RSU UKI, dan telah dilaksanakan pada bulan Maret 2013. Rancangan penelitian yang digunakan adalah deskriptif kuantitatif dengan pendekatan potong lintang. Hasil penelitian menunjukkan kepuasan pasien KJS sebesar 40,9%. Akses ke rumah sakit, motivasi, informasi baik dan informasi tidak baik berhubungan signifikan dengan kepuasan pasien KJS, tetapi motivasi adalah hal yang paling dominan berhubungan. Selain itu, kepuasan pasien KJS juga berhubungan bermakna dengan pemanfaatan kembali layanan dan minat merekomendasikan layanan pada orang lain. Jakarta Health Card (JHC) was launched on November 2012, followed by sudden increase number of patient in any hospital accordingly. Jakarta Health Card ease the patient to afford health care without payment. We conducted study on the relation among socio-culture, demographic characteristics, and psychology factors toward patient satisfaction in using JHC. The study subjects were 110 outpatient of UKI Hospital, with study design descriptive quantitative with cross sectional approach. The result showed 40,9% of the patients were satisfied with the hospital service. Factors such as hospital access, motivation, good information and bad information were associated with patient satisfaction significantly, and motivation was the dominant factor. Besides, patient satisfaction also has significant correlation with revisiting/reusing and recommendation of the hospital to other patients.

Abstract

Reproductive health for adolescents with disabilities is often disregarded, and education on personal hygiene is insufficient because some people perceive menstruation as a sensitive topic and have a false belief that adolescents with disabilities have no sexual desire. This study aimed to determine the overview of personal hygiene behavior among adolescents with physical and intellectual disabilities when facing menstruation. The method used was qualitative, with case studies, in-depth interviews, and observations for data collection. The informants comprised four adolescents aged 19-21 years, two mothers, and two therapists for special needs children. This study was conducted at two growth and development clinics in South Jakarta, Indonesia, from July to September 2021. This study found a lack of apprehension about menstrual personal hygiene among mothers and adolescents with disabilities, a lack of understanding about menstrual personal hygiene in general among adolescents with disabilities, and inappropriate menstrual personal hygiene behavior among adolescents with disabilities. To conclude, this study underlines the importance of improving the ability of adolescents with disabilities in terms of menstrual hygiene behavior.

Abstract

Loyalitas pasien merupakan bagian penting rumah sakit untuk bertahan dalam persaingan industri pelayanan kesehatan. Tingkat loyalitas pasien rendah dapat menyebabkan penurunan daya saing rumah sakit. Terdapat beberapa faktor berpengaruh pada loyalitas pasien di rumah sakit. Tujuan dari penelitian ini adalah untuk menjelaskan faktor-faktor yang mempengaruhi loyalitas pasien rawat jalan di rumah sakit. Metode yang digunakan adalah systematic literature review. Pencarian dengan kata kunci “Loyalitas Pasien” AND “Rawat Jalan” AND “Rumah Sakit”. Pencarian melalui Google Scholar dan Researchgate. Kriteria inklusinya yaitu artikel dipublikasi kurun waktu tahun 2023 dan dibatasi pada 3 halaman pertama. Identifikasi awal ditemukan 5 artikel di google scholar, 3 di researchgate. Kemudian dilakukan pemilahan relevansinya dan diskrining sesuai karakter inklusi yang masuk hanya 8. Dari 8 artikel tersebut terdapat 5 artikel yang sesuai dengan pembahasan faktor yang mempengaruhi loyalitas pasien rawat jalan di rumah sakit. Dari hasil penelitian menunjukan faktor yang paling dominan dalam mempengaruhi loyalitas pasien rawat jalan di rumah sakit adalah faktor kepuasan dan kualitas pelayanan. Terdapat faktor lain seperti bauran pemasaran yaitu product, price, promotion, place, process, people dan phsyical evidence. Kesimpulannya, diperlukan strategi peningkatan kepuasan dan kualitas pelayanan karena beberapa hasil penelitian menunjukkan pengaruh kedua faktor tersebut sangat dominan.

Abstract

Meskipun prevalensi stunting nasional menunjukkan tren penurunan, NTT tetap menjadi salah satu provinsi dengan angka tertinggi di Indonesia. Studi ini bertujuan untuk mengidentifikasi faktor-faktor yang berhubungan dengan kejadian stunting pada balita di Provinsi NTT berdasarkan data Survei Kesehatan Indonesia (SKI) 2023. Penelitian ini menggunakan desain cross-sectional berbasis data sekunder SKI 2023 dengan total sampel 3.449 anak usia 0-59 bulan. Analisis dilakukan menggunakan pendekatan complex samples. Uji bivariat dengan chi-square (p < 0,05) diikuti analisis regresi logistik multivariat untuk variabel dengan p < 0,25. Hasil disajikan dalam Adjusted Odds Ratio (AOR) dan 95% Confidence Interval (CI). Prevalensi stunting di NTT sebesar 32,5%. Faktor signifikan yang berhubungan dengan stunting meliputi usia anak, dengan risiko tertinggi pada usia 36–47 bulan (AOR = 3,014; p < 0,001), jenis kelamin laki-laki (AOR = 1,294; 95% CI: 1,095–1,528; p = 0,003), berat lahir <2.500 gram (AOR = 1,762; p < 0,001), pendidikan ibu rendah (SD: AOR = 1,601; SMP: AOR = 1,888; p < 0,001), status gizi ibu kurang (LILA <23 cm) (AOR = 1,309; p < 0,001), tinggi badan ibu <150 cm (AOR = 1,964; p < 0,001), dan tempat tinggal di pedesaan (AOR = 1,905; p < 0,001). Kesimpulannya, stunting di NTT dipengaruhi oleh faktor biologis dan sosial. Intervensi multisektoral yang berfokus pada peningkatan gizi ibu dan anak serta prioritas wilayah pedesaan diperlukan untuk percepatan penurunan prevalensi stunting.
