Abstract

As the population of elderly increases, the number of andropause also increases. Andropause is characterized by the decrease of testosteron level which is accompanied by hypogonadism symptoms. Many studies mentioned that decreased testosterone level will contribute to inflammation state, one of the cytokine is TNF-α, which then lead to many chronic inflammation disorders and influence the quality of life. Understanding the significant correlation between androgen and inflammation allows accurate consideration making in using androgen replacement therapy, therefore the quality of life will be improved significantly.This research was an analytical cross-sectional study. Subjects were recruited through random sampling at Denpasar in 2019. TNF-α serum level was measured by human immunoassay method. Andropause was measured by ADAM questionnaire and confirmed by total decreased testosterone serum level. Quality of life was measured by WHOQOL-BREF questionnaire which was then continued by bivariate and multivariate analysis to control the confounding variables. It was considered statistically significant if P value is more than 0.05. There were 60 elderly involved as research subjects consisting of 43 andropause cases and 17 non andropause cases. There was significant correlation between TNF-α serum and quality of life. This study also found that there was significant difference in quality of life between andropause and non andropause group. This study shows that there was significant correlation between TNF-α serum level and andropause with quality of life in elderly.

Abstract

Introduction. Sarcopenia is a condition characterized by a decrease in muscle mass accompanied by a decrease in strength and/or function. Sarcopenia is closely related to the aging process, but the mechanism of its occurrence is still not fully understood. Skeletal muscle is the largest organ in the human body, so the myokine produced through muscle contraction opens a new paradigm regarding the central role of muscles in regulating metabolism and other organ functions. One of the myokines that are currently found to affect physical activity is Irisin. This study aims to determine the role of low Irisin levels in serum as a risk factor for sarcopenia in the elderly. Methods. This case-control study was conducted in Denpasar City from January 2020 to October 2020. The sample was selected using purposive consecutive sampling with matching age and gender, with 54 subjects, 27 case groups, and 27 control groups aged ≥60 years. Sarcopenia status is enforced by the criteria of the Asian Working Group for Sarcopenia (AWGS). Irisin serum levels were measured by enzyme-linked immunosorbent assay (ELISA). Results. Based on 54 subjects, the mean age in cases was 69.79 (SD 5.00) years and controls 67.89 (SD 4.50) years (p = 0.152), the proportion of male:female both in cases and controls group was 9:18. The cut-off point value wass based on the irisin ROC curve 4.51 ng/L. Nutritional status (OR = 3.40 (95% CI = 1.11 - 10.40); p = 0.029) and irisin (OR = 25.30 (95% CI = 6,00 - 106.66); p = <0.001) had an effect on sarcopenia. Factors that purely influence sarcopenia were found in low irisin (OR = 103.41 (95% CI = 7.83 – 1,364.99; p = <0.001). Conclusions. Low Irisin level is a risk factor for sarcopenia in the elderly. Irisin could be considered for further investigation in an effort to become a target for both preventive and curative therapy for sarcopenia.

Abstract

Electronic medical records must adhere to the principles of data and information security, which include availability, integrity, and confidentiality. Medical records serve as legitimate legal evidence, thereby satisfying these principles both individually and institutionally. Consequently, the aim of this research is to explore the roles of law on medical records for data and information security. To achieve this aim, a systematic literature review (SLR) was employed. The Scopus and PubMed databases were chosen for their global recognition. Full-text articles from these databases were subjected to PRISMA analysis using Mendeley Reference Manager. The researchers collected 32 research articles by the categories of data protection law, medical records' law enforcement, and data protection law recommendation. The research findings indicate that the roles of law deal with the role of law in compliance and data protection, law enforcement and sanctions, a recommendation for improving compliance and data protection. In addition, the law is crucial for ensuring compliance and data protection in electronic medical records implementation in hospitals. However, compliance levels have not reached optimal levels. To improve security, awareness, and enforcement, collaboration with legal bodies, and educational initiatives are needed. Legislation like HIPAA and GDPR influence data protection measures, but penalties are insufficient. To enhance compliance, medical personnel should undergo rigorous training and improve oversight of health data management procedures

Abstract

Background: One in five Indonesian elderly was frail. Multicomponent exercise is one of the recommended strategies for frailty management in the elderly. However, it would be difficult for the elders to practice the exercise without proper exercise guidelines. In this study, a Multicomponent Exercise Program (MEP) module has been developed and implemented as exercise guidelines, but its effectiveness to improve physical performance of frail elderly has not been tested.Objective: The study aimed to determine the effectiveness of the MEP in the module to improve frailty, hemodynamic parameters, and balance in pre-frail and frail elderly.Methods: Quasi-experimental method was used in the study involving 17 participants (73.47 + 8.43 years). The MEP was conducted 3 times per week for 12 weeks, consisted of flexibility, balance, aerobic, and strength training. Categorical data were analyzed using McNemar's test while analysis of continuous data using paired t-test or Wilcoxon Signed Rank, α = 0.05.Results: The MEP decreased frailty scores, systolic, diastolic blood pressure, and mean arterial pressure (p < 0.000), and improved balance (p = 0.023) in the elderly. Furthermore, the program reduced the frailty status, as evidenced by 41.18% of frail participants becoming pre-frail and 5.88% participants shifting from pre-frail to robustness.Conclusion: The MEP in the module was effective to improve frailty, hemodynamic parameters, and balance in the elderly. However, this needs to be confirmed with larger samples and better research designs.

Abstract

Kanker adalah salah satu penyakit dengan angka kematian tertinggi di dunia, dan radioterapi merupakan salah satu metode pengobatan yang umum digunakan. Namun, pengobatan ini juga membawa risiko, baik bagi pasien maupun tenaga medis. perlindungan hukum menjadi sangat penting untuk menjamin keselamatan dan hak-hak pasien serta memberikan kepastian hukum bagi tenaga medis dalam menjalankan praktik mereka.perlindungan hukum bagi pasien kanker yang menjalani radioterapi harus mencakup hak untuk mendapatkan informasi yang jelas bagi pasien , tanggung jawab tenaga medis dalam memberikan perawatan yang berkualitas, serta perhatian terhadap aspek psikologis dan sosial pasien. Tujuan penelitian untuk menganalisis dan mengidentifikasi perlindungan hukum bagi pasien kanker yang menjalani radioterapi dalam perspektif undang-undang. Penelitian ini menggunakan tipe penelitian hukum yuridis normatif, yaitu penelitian hukum kepustakaan, dengan pendekatan perundang-undangan, dan pendekatan konseptual. Hasil analisis menunjukkan bahwa perlindungan hukum bagi pasien kanker yang menjalani radioterapi dalam perspektif Undang-Undang Nomor 17 Tahun 2023 adalah meningkatkan pemahaman mengenai hak-hak pasien kanker dalam konteks radioterapi dan perbaikan kebijakan untuk meningkatkan perlindungan hukum bagi pasien kanker.

Abstract

The advancement of information and communication technology, particularly telemedicine, has transformed the healthcare sector, including its application in traditional medicine in Indonesia. Traditional medicine is recognized under Law No. 17 of 2023 concerning Health; however, the implementation of telemedicine in this field has not yet been specifically regulated. This study aims to analyze the legal aspects of using telemedicine in traditional medicine based on the aforementioned Health Law. The research method employed is normative legal research using statutory and conceptual approaches. The findings reveal several challenges, including regulatory gaps, standardization issues, patient data protection, and practitioner competence. Although the Health Law provides a general legal framework, specific regulations regarding the use of telemedicine in traditional medicine are still lacking. The study concludes that additional regulations are needed to govern the use of telemedicine, ensure quality control, protect patient rights, and certify practitioners. The recommendations include drafting specific regulations, enhancing practitioner competence, and increasing public education to ensure the safe and effective implementation of telemedicine in traditional medicine.

Abstract

A 16-year-old male patient came to a private dental practice presenting chief complaints about frequent gum bleeding when brushing his teeth. The patient came without his parents. Anamnesis and clinical examination result shows that the patient was suspected of having gingivitis associated with sexually transmitted disease. No medical treatment was performed on the patient because the patient was not accompanied by parents while the patient was not legally able to sign informed consent. The patient was educated about sexually transmitted diseases and asked to come back after taking blood and urine tests, with his parents to accompany him during the examination because the patient. The patient then came back with the results of the blood and urine tests, but without his parents. The dentist cannot provide dental treatment because underage patients must be accompanied by their parents in performing a non-emergency medical treatment. Dentists have the right to postpone the non-emergency medical procedures in private practices for these patients. Dentists are obliged to provide education and refer patients to the hospital for further examination and treatment in accordance with government programs and applicable legal provisions in Indonesia.
