Abstract

Background:Effective prioritization is crucial in delivering high-quality emergency medical services, although it is often lacking. Objective:This study aims to evaluate the application of triage based on the Emergency Severity Index (ESI) in improving the quality of emergency departments in hospitals in Indonesia. Methods:An accidental sample of 100 emergency room patients was selected. Patient satisfaction was measured using questionnaires, while infrastructure, support facilities, and the triage process were assessed through direct observation. Results:The evaluation found that nurse staffing levels did not meet standards and were inadequate to meet patient needs. Triage facilities and infrastructure at both hospitals met established standards. The triage process, conducted on 100 respondents by 30 nurses, revealed that the majority of patients were categorized as ESI level 3, comprising 82 (82%) respondents. There was an excessive use of triage resources beyond projections, and patient placement did not always adhere to standards, although the average response time was within the accepted range. Patient satisfaction was evaluated across five dimensions; the most satisfying dimensions were assurance and empathy, with 90% of respondents rating thempositively, while the tangibles dimension received the lowest satisfaction rating at 84%. The correlation between the application of ESI triage and patient satisfaction was not statistically significant (p-value = 0.101, higher than 0.05). Conclusion:It is recommended that hospitals increase the number and competence of nursing staff in emergency departments to ensure response times and patient placement align with ESI triage standards, thereby enhancing the effectiveness of triage implementation. Abstract

Reopening schools during the COVID-19 pandemic requires the full consideration of parents’ decisions due to the worry of exposure for their children. This study evaluated parents’ preparedness for offline learning decisions among elementary school children during the COVID-19 pandemic. This is a cross-sectional study with 134 parents as respondents. The consecutive sampling technique was used. The online questionnaire was distributed on social media to collect data from various regions in Indonesia. The finding indicated that 99 (73%) parents sent their children for offline learning from July to November 2021 for one to three times a week (44.8%) and one to three hours a day (46.8%). The significant factor for parents’ decisions was school preparedness for COVID-19 prevention (p < 0.05). However, parents' preparedness for self-protection and the child's mental condition had no significant relationship because almost all parents were already prepared for these factors (> 95%). Therefore, the government and primary health care have a vital role in supervising and providing a safe environment for children during the COVID-19 pandemic.

Abstract

Background: The clinical nurse career path system significantly impacts nurses' professional growth, job satisfaction, and service quality. Evaluating its effectiveness is vital for enhancing human resource management. Purpose: This study aimed to evaluate the nursing career path in Indonesia, identifying areas for improvement and development. Method: This study used a mixed-methods approach with nine participants and 339 samples. Surveys targeted clinical nurses, while interviews involved nursing management. Data was analyzed using descriptive statistics and thematic analysis. Results: Managerial experiences revealed three themes: (1) implementation of the nurse career path as a mandatory accreditation process, (2) challenges in the implementation of the nurse career path, and (3) expectations for career path implementation. The process evaluation pinpointed areas for improvement, including organizing, internship programs, Continuous Professional Development (CPD), and career advancement. In product evaluation, 80.8% of nurses had good perceptions, with a 77% job satisfaction rate. Analysis showed a positive correlation between perception and job satisfaction (P < 0.001). Conclusion: The clinical nurse career path in hospitals requires more attention, especially in areas such as human resources, which are not yet full-time, and leadership policies regarding rewards, regulations, and nurse placement in the implementation of career paths related to organizing, internship programs, CPD, and career advancement. Enhancing these aspects is vital for boosting nurses' job satisfaction and improving service quality and patient care.

Abstract

The higher prevalence of stunting in South Sulawesi compared to the national average poses a significant health concern for children, thus necessitating accelerated efforts to reduce stunting. One of the updated strategies for accelerating stunting reduction is the family-centered approach through mentoring families at risk of stunting via the formation of Family Mentor Teams (TPK). Therefore, the objective of this community service is to enhance the role of TPKs by providing comprehensive training on the handling and prevention of stunting from pre-conception to children aged 0-59 months. The implementation method utilizes community-based participatory research (CBPR). The activity was attended by 139 TPKs from 12 districts. The results of the activity demonstrate the effectiveness of the training, as evidenced by an increase in participants' knowledge scores in the post-test (12.96 ± 1.44) compared to the pre-test scores (11.12 ± 1.73).

Abstract

Early detection of visual impairment in elementary school students is crucial for supporting academic achievement and child development. This community service activity aimed to increase the knowledge of teachers at SD Inpres Jongayya and Andi Tonro Makassar regarding the early detection of visual impairment. The method involved training and simulation on the use of the Snellen chart. Participant knowledge was evaluated using a pre-test and post-test design. The results showed an increase in teacher knowledge scores after the training, from an average pre-test score of 7.41 ± 1.570 to 7.79 ± 1.612 on the post-test. However, this increase was not statistically significant. This training provided an initial positive impact, but to ensure program sustainability, supporting efforts such as periodic supervision, provision of standard guidelines, and incentives for implementing teachers are necessary.

Abstract

Introduction: This study aimed to identify family, caregiver, patient, nurse, and healthcare service factors associated with caregivers’ ability to support medication adherence among relatives with mental disorders. Methods: This cross-sectional study was conducted between February and June 2024 at seven community health centers (Puskesmas) in Makassar, Indonesia. Purposive sampling was used to recruit 155 caregivers. Questionnaires were validated prior to data collection. Bivariate associations were examined using the chi-square test, and factors associated with caregivers’ ability to support medication adherence were analyzed using multivariate logistic regression. Results: The multivariate model was statistically significant (model χ²  =  42.87, p < 0.001; Nagelkerke R² = 0.32). In bivariate analyses, the caregiver’s ability to support patients’ medication adherence was associated with family functioning, caregiver factors (gender, stress, motivation, knowledge), patient factors (relapse frequency, severity level), and nurse psychological factors. In the multivariate analysis, family economic status (AOR = 0.22, 95% CI = 0.07–0.69, p = 0.010), family functioning (AOR  =  2.58, 95% CI = 1.19–5.58, p = 0.016), caregiver motivation (AOR = 0.29, 95% CI = 0.12–0.73, p = 0.008), and caregiver knowledge (AOR = 0.40, 95% CI = 0.16–0.96, p = 0.041) remained significantly associated with caregiver ability to support medication adherence. Conclusions: These results suggest that family factors and caregiver psychosocial preparedness play critical roles in facilitating adherence-related caregiving practices. Strengthening family functioning and caregiver motivation and knowledge may improve support for medication adherence.

Abstract

Kaluku Bodoa Community Health Center in Tallo District is among the top three areas with the highest incidence of low birth weight (LBW) cases in Makassar, recording 48 cases in 2022. A 2024 study revealed that many LBW infants had not received neonatal home visits and did not possess the Small Baby MCH Book, further exacerbated by the lack of socialization and training among health workers. Through the integration of the Healthy Indonesia Program with Family Approach (PIS-PK), empowering community health cadres in small baby care is essential to prevent stunting. This community service program aimed to enhance cadre capacity through managerial training, utilization of the Small Baby MCH Book, and the establishment of an online support forum for mothers with small infants. The activities were implemented through technical training on the integration of the Small Baby MCH Book and health education using a health coaching approach for health workers and cadres, followed by mentoring sessions at pilot Posyandu through book distribution and educational sessions using visual and guideline-based media The results of the activity demonstrated an increase in the average knowledge and skills of community health cadres and maternal and child health (MCH) healthcare providers in delivering services for mothers and infants with a history of low birth weight, with scores improving from 46.7 on the pre-test to 60.4 on the post-test., and successfully initiated the implementation of a dedicated Posyandu for LBW infants. Targeted care for small babies is a crucial effort to prevent future growth and developmental disorders, emphasizing the need for special attention and differentiated care from health professionals and cadres.
