Abstract

Pharmaceutical care by pharmacists in the management have an important role of treatment monitoring. Optimal asthma treatment for patients requires cooperation between patients, healthcares, involving doctors, nurses, and pharmacists. Therefore the purpose of this study is to know the perception of healthcares and patients about asthma treatment and pharmaceutical cares on asthma mangement. The method of this study is based on phenomenological perspective with interpretative phenomenological analysis (IPA) on pulmonologist, outpatient asthma, pharmacist and nurse who directly correlated with asthma treatment at a hospital in Surabaya conducted for two years using purposive sampling. The results of this study indicate that asthma patients know how to manage asthma and use asthma drugs well. In addition it is known that asthma patients feel the role of doctors in the treatment of asthma is very dominating; pulmonologists and nurses perceive that the role of pharmacists is limited to drug delivery; and pharmacists assume that their role has been well done although not all patients get the same asthma treatment education. It is therefore necessary to evaluate the role of pharmacist in performing its role in the treatment of this asthma in order for a more harmonious collaboration.

Abstract

Dengue Hemorrhagic Fever (DHF) patients in Indonesia reached 129,650 cases with a death toll of 1,071 people in 2015. DHF puts an economic burden on the community and government. This study aims to determine the difference in real cost tariffs and INA CBG's for DHF in a hospital in Sidoarjo, East Java. This cross-sectional study applied a descriptive design, conducted from January to December 2019. The data in this study consisted of characteristic data and differences in real hospital tariffs and INA CBG tariffs for DHF in the hospital treatment room. 625 hospitalized cases were included in this study. This hospital underwent a deficit (loss) of 30% from the difference between the total INA CBG's package tariffs and the total real hospital tariffs in 2019. There is a negative difference between the real hospital tariffs and the tariffs according to INA CBG's in the treatment of dengue fever in a hospital in Sidoarjo which produces losses for the hospital.

Abstract

COPD is the main cause of death, and is currently the fifth cause of death in the world. COPD patients experience decreased capacity for quality of life, physical disability and increased costs of living. In COPD patients, the use of more than one drug therapy and long-term use can carry the risk of drug-related problems. The aim is to analyze the costs and rationality of treating COPD exacerbations. The research design was retrospective to determine the costs and description of therapy for COPD patients at RSU Anwar Medika Sidoarjo. The variables are real costs, INA-CBGs rates, severity and treatment therapy. The research sample was inpatients with BPJS insurance who were diagnosed with COPD attacks at RSU Anwar Medika Sidoarjo for January 2019-December 2021, using the total sampling method. The data collection by observing research materials including medical records. Data analysis is descriptive for data analysis and the incidence of drug-related problems (MTO), and the Mann-Whitney test to determine significant differences between real hospital costs and INA CBG'S inpatient rates. The number of respondents involved was 81 people. The highest components of direct costs incurred by COPD patients during hospitalization are hotel room costs and the cost of consumable medical materials. There was no difference in real costs compared to the rates INA-CBG's (p=0.158). The total incidence of MTO that occurred was 83 cases consisting of inappropriate drug selection (19.27%), drugs without indications (66.26%), inappropriate drug combinations (4.81%), indications without drugs (2 .40%), too many drugs for  same indication (7.22%).

Abstract

Cigarettes are one of the leading causes of death in the world and  Indonesia. Cigarettes emit cigarette smoke, which contains toxins and has pro-inflammatory properties that interfere  the metabolism of vitamin D in the lungs. The study aimed to determine the risk of vitamin D deficiency in active smokers in Rungkut Village, Surabaya City. This research method is a mix-method with a  Convergent Parallel Design type strategy, which is a way of collecting quantitative (cross-sectional) and qualitative data (interpretative phenomenological analysis) simultaneously, with purposive sampling technique. The variable was the risk of vitamin D deficiency and vitamin D level. Subjects were active smokers aged ≥19 years in the Rungkut Subdistrict. The number of samples involved in this research was 125 samples in the quantitative phase, and 15 people in the qualitative phase.  The highest level of smoking severity was light smokers (45.60%). Most of respondents had normal vitamin D levels (≥20 mg/mL) of 73.33%, averaging of 37.66 ± 5.89 mg/mL. Most active smokers are at risk of vitamin D deficiency. Risk factors for deficiency in heavy smokers are not consuming foods containing vitamin D (such as fish, eggs, and milk), not consuming fish oil supplements and vitamins, and not sunbathing. However, the level of physical activity tended to be moderate-heavy. The conclusion is that smokers must pay attention to their intake of foods containing vitamin D and exposure to sunlight to achieve sufficient vitamin D levels.

Abstract

This research discusses the use of matrices in sustained-release tablet formulations to optimize the active substance release profile. Extended-release tablets are a type of tablet formulation designed to release the active substance gradually in the body over a certain period of time. The main purpose of sustained-release tablets is to maintain drug levels in the blood or target area over a longer period of time, compared to conventional tablets which release the active substance quickly after ingestion. The method used in this research is a qualitative approach which aims to deepen an in-depth understanding of the phenomenon being studied. As a result, selecting the right type of matrix proves to be crucial in achieving the expected therapeutic effectiveness. Polymer matrices, both natural and synthetic, play an important role in controlling the stability and efficiency of tablets and their ability to gradually release active substances.
