Abstract

Angka penemuan kasus dan angka keberhasilan pengobatan hingga akhir 2017 di Puskesmas Kandangan masih rendah yaitu 35,8% dan 65,5%. Angka ini belum memenuhi target yang ditetapkan yaitu 70% dan 85%. Penelitian ini bertujuan mengevaluasi dari upaya penanggulangan Tb Paru dengan strategi DOTS. Jenis penelitian ini merupakan penelitian kualitatif dengan metode deskriptif, sumber data penelitian dengan sumber data primer dan sekunder, data primer melalui wawancara. Wawancara dilakukan pada 3 informan utama dan 7 informan triangulasi. Instrumen yang digunakan yaitu pedoman wawancara. Hasil penelitian ini menunjukkan bahwa dari segi input, SDM, sarana prasarana dan SOP sudah mencukupi akan tetapi dana belum cukup. Dari segi proses, komitmen politis sudah saling mendukung akan tetapi dalam hal pendanaan belum, penemuan kasus, distribusi obat sudah sesuai dengan permenkes No. 67 tahun 2016, pmo belum bekerja sesuai fungsinya, pencatatan dan pelaporan secara online dengan SITT belum terlaksana dengan baik. Dari segi output capaian angka penemun kasus hingga april 2018 sebesar 40% dan capaian angka keberhasilan pengobatan 72%. Saran penelitian ini agar meningkatkan kualitas dari segi proses dan melakukan pengawasan terhadap pasien.

Abstract

ABSTRACT The number of public health center in Semarang City is accredited as of February 2018, namely 31 health centers. Moreover, it was unknown about the description of patient satisfaction level, both before and after being accredited. The purpose of this study was to determine the relationship between the accreditation status and the satisfaction level of outpatient (case study 5 public health centers in Semarang City). The type of this research is observational analytic with design cross sectional. The sample was set at 101 respondents using the proportionate simple random sampling technique. The instruments used were structured questionnaires and observation sheets. Data were analyzed using chi-square test and logistic regression test. The results of this study were accreditation status category non-accredited (p=0,000), basic accredited (p=0,000), middle accredited (p=0,024), main accredited (p=0,217), and plenary accredited as reference. After being controlled by the variables of education, income, employment status, type of financing (r2=42.0%; AUC=82.7%) only accreditation status with non-accredited and basic accredited category is related to patient satisfaction. The conclusion, accreditation status of public health center (non-accredited, basic accredited, and middle accredited category) is related to the level of patient satisfaction. Abstract

ABSTRAK Berdasarkan data dari STBM Kementerian Kesehatan Indonesia, pada tahun 2017 akses sanitasi di Kabupaten Demak paling rendah berada di wilayah Puskesmas Wedung II yaitu sebesar 60,9 %. Berdasarkan data STBM di Kecamatan Wedung dari 20 desa baru ada satu desa yang sudah verified Open Defecation Free (ODF). Upaya untuk menanggulangi permasalahan yang ada, pemerintah Kabupaten Demak menetapkan Peraturan Bupati Demak nomor 50 tahun 2017. Tujuan Penelitian ini adalah untuk mengetahui bagaimana implementasi Peraturan Bupati Demak nomor 50 tahun 2017 di wilayah kerja Puskesmas Wedung II. Jenis penelitian ini adalah kualitatif dengan pendekatan deskriptif. Instrumen yang digunakan adalah pedoman wawancara. Adapun teknik pengambilan sampel dengan menggunakan teknik purposive sampling. Hasil penelitian menunjukan bahwa masih ada beberapa aspek yang belum optimal dalam implementasi peraturan Bupati Demak nomor 50 tahun 2017 anatara lain sumber daya, karakteristik agen pelaksana,kondisi lingkungan dan sikap pelaksana. Saran penelitian ini yaitu Pemerintah desa harus memiliki standar operasional khusus untuk menjalankan program desa ODF agar pelaksanaan lebih terstruktur dan terarah dan Puskesmas perlu melakukan sosialisasi atau pelatihan untuk meningkatkan pengetahuan kader kesehatan ABSTRACK Based on data from STBM of Indonesia, in 2017 the lowest access to sanitation in Demak Regency is in the Puskesmas Wedung II, which is 60.9%. The purpose of this study was to find out how the implementation of Demak Regent Regulation number 50 of 2017 in the working area of ​​Puskesmas Wedung II. This type of research is qualitative with a descriptive approach. The instrument used was an interview guide. The sampling technique using purposive sampling technique. The results showed that there were still some aspects that were not optimal in the implementation of Demak Regent regulation number 50 of 2017 among other resources, characteristics of the implementing agent, environmental conditions and the attitude of the implementer. The suggestion of this research is that the village government must have special operational standards to run the ODF village program so that the implementation is more structured and directed and the Puskesmas need to conduct socialization or training to improve the knowledge of health cadres.

Abstract

Abstrak Bauran pemasaran merupakan seperangkat alat pemasaran yang digunakan rumah sakit dalam mempengaruhi dan mengetahui tanggapan pembeli jasa pelayanan kesehatan. Jumlah kunjungan pasien umum di Instalasi Rawat Jalan RSUD Tugurejo Semarang pada tahun 2016 sebanyak 17.981 orang, tahun 2017 sebanyak 14.348 orang, dan tahun 2018 jumlah sebanyak 9.801 orang. Tujuan penelitian ini adalah untuk mengetahui pengaruh bauran pemasaran terhadap proses keputusan pasien memilih layanan kesehatan. Jenis penelitian ini adalah analitik observasional dengan rancangan cross sectional. Sampel penelitian yang ditetapkan adalah 87 responden. Teknik pengambilan sampel secara consecutive sampling. Instrumen yang digunakan adalah kuesioner terstruktur. Data dianalisis dengan menggunakan uji chi square dengan perangkat SPSS. Hasil penelitian ini adalah bauran pemasaran yang berpengaruh yaitu bauran produk (p value=0,000), bauran tempat (p value=0,000), bauran promosi (p value=0,000), dan bauran proses (p value=0,000). Sedangkan yang tidak berpengaruh yaitu bauran harga (p value=0,248), bauran petugas (p value=0,165), dan bauran bukti fisik (p value=0,248). Saran penelitian ini adalah sebaiknya RSUD Tugurejo meningkatkan upaya promosi melalui media promosi berupa brosur, leaflet, dan website resmi rumah sakit. Abstract Marketing mix is a marketing tool used by hospitals in influencing and getting responses from health service buyers. The number of general patient visits at the Outpatient Installation of Tugurejo Regional Hospital Semarang in 2016 was 17,981 people, in 2017 there were 14,348 people, and in 2018 there were 9,801 people. The purpose of this study was to study the effect of the marketing mix on the decision process of patients choosing health services. This type of research is observational analytic with cross sectional design. The research sample specified was 87 respondents. The sampling technique was consecutive sampling. The instrument used was a structured questionnaire. Data were analyzed using chi square test with SPSS devices. The results of this study are influential marketing mix namely product mix (p value = 0,000), place mix (p value = 0,000), promotion mix (p value = 0,000), and process mix (p value = 0,000). While those that did not affect were price mix (p value = 0.248), officer mix (p value = 0.165), and physical evidence mix (p value = 0.248). The suggestion of this research is that Tugurejo District Hospital should increase its promotional efforts through promotional media in the form of brochures, leaflets, and the official website of the hospital. Abstract

AbstrakJantung koroner adalah penyakit akibat gangguan fungsi jantung dimana otot jantung kekurangan suplai darah disebabkan adanya penyempitan pembuluh darah koroner. Kasus jantung di RSUD Tugurejo tahun 2019 sebanyak 339 pasien dan menghabiskan biaya INA CBGs sebanyak Rp. 2.386.990.000. Tujuan dari penelitian ini untuk mengetahui perbedaan biaya riil dengan tarif INA CBGs, dan faktor-faktor yang mempengaruhi biaya riil. Penelitian ini dilaksanakan bulan Januari sampai Februari 2020. Penelitian ini merupakan penelitian mixed method, dengan pendekatan cross sectional. Sampel sebanyak 85 pasien. Instrumen yang digunakan adalah lembar pengumpulan data dan pedoman wawancara. Data dianalisis menggunakan analisis deskriptif, analisis one sample t-test, dan uji anova. Hasil uji statistik (p=0,00) terdapat ketidaksesuaian antara total biaya riil dan tarif INA CBGs Faktor-faktor yang berpengaruh adalah usia, tingkat keparahan, kelas perawatan, lama hari rawat, dan jumlah prosedur. AbstractCoronary heart disease is a disease due to impaired heart function where the heart muscle lacks blood supply due to the narrowing of the coronary arteries. Heart cases in Tugurejo District Hospital in 2019 were 339 patients and cost the INA CBGs Rp. 2,386,990,000. The purpose of this study is to determine the difference in real costs with INA CBG rates, and the factors that affect real costs. This research was conducted from January to February 2020. This study was an mixed method, with a cross sectional approach. A sample of 85 patients. The instrument used was the data collection sheet and interview guidelines. Data were analyzed using descriptive analysis, one sample t-test analysis, and ANOVA test. Factors influencing were age, severity, care class, length of stay, and number of procedures. Statistical test results (p = 0.00) there is a mismatch between total real costs and INA CBG tariffs.

Abstract

Abstrak Cakupan kepesertaan program Jaminan Kesehatan Nasional di Kelurahan Susukan pada awal tahun 2020 sebanyak 68,13%. Dibandingkan dengan desa lain diwilayah kerja Puskesmas Kalongan, Kelurahan Susukan memiliki cakupan kepesertaan JKN terkecil. Perlu adanya peningkatan cakupan pada jenis peserta JKN non PBI mandiri. Tujuan penelitian ini adalah mengetahui faktor-faktor yang berhubungan dengan keikutsertaan masyarakat dalam JKN mandiri. Jenis penelitian analitik observasional dengan desain cross sectional. Jumlah sampel 104 responden. Pengambilan sampel menggunakan teknik purposive sampling. Instrumen yang digunakan adalah kuesioner. Analisis data dilakukan menggunakan uji chi square dan uji fisher (α=0,05) dengan bantuan SPSS. Hasil menunjukkan bahwa tingkat pendidikan (RP=4,8; 95% CI=2,1-10,7), tingkat pengetahuan (RP=1,6; 95% CI=1,1-2,2), pekerjaan (RP=1,8; 95% CI=1,2-2,7), tingkat pendapatan (RP=2,2; 95% CI=1,6-2,9), informasi (RP=2,6; 95% CI=1,5-4,5), dan dukungan sosial (RP=1,9; 95% CI=1,5-2,7) berhubungan dengan keikutsertaan masyarakat dalam JKN mandiri. Saran penelitian ini adalah untuk memperluas penyebaran informasi tentang program JKN. Melalui sosialisasi maupun media informasi lain yang tepat dan rutin. Abstract ` Coverage of membership in The National Health Insurance program in the Susukan Village in early January 2020 was 68,13%. Compared to other villages in the working area of Kalongan primary healthcare, The Susukan Village has the lowest coverage. There needs to be an increase in the coverage of independent JKN participants. The purpose of this study was to determine the factors associated with community participation in independent JKN. The research was unmatched cross sectional study. Samples were 104 peoples using purposive sampling technique. The instrument used was structured questionnaire. Data were analyzed with chi square test and fisher test using SPSS. Results showed that level of education (RP=4,8; 95% CI=2,1-10,7), knowledge level (RP=1,6; 95% CI=1,1-2,2), profession (RP=1,8; 95% CI=1,2-2,7), income level (RP=2,2; 95% CI=1,6-2,9), information (RP=2,6; 95% CI=1,5-4,5) and social support (RP=1,9; 95% CI=1,5-2,7) related to community participation in independent JKN. This research suggestion is to expand information dissemination. Trough socialization and other information precisely and regularly.

Abstract

Latar Belakang: Penyakit DBD telah menjadi masalah kesehatan masyarakat selama 41 tahun terakhir di Indonesia. Case Fatality Rate DBD di Jawa Tengah selama lima tahun terakhir selalu mengalami peningkatan. Kelurahan Mangunjiwan merupakan salah satu wilayah endemis dengan incidence rate DBD tertinggi di Kecamatan Demak Kota. Angka Bebas Jentik (ABJ) di Kelurahan Mangunjiwan adalah 31% masih sangat jauh dari target nasional yaitu ≥ 95%. Tujuan penelitian ini adalah menganalisis perbedaan angka bebas jentik (ABJ) antara sebelum dan sesudah penerapan model jumantik bergilir berbasis dasawisma.Metode: Jenis penelitian ini adalah eskperimen semu dengan pendekatan Research and Development, dengan 80 responden. Teknik pengambilan sampel dalam penelitian ini adalah proportional sampling. Teknik analisis data dalam penelitian ini adalah analisis analisis bivariat dengan uji McNemar.Hasil: Berdasarkan hasil penelitian diperoleh nilai p < 0,01. Hal ini menunjukan adanya perbedaan ABJ sebelum dan sesudah penerapan model jumantik bergilir berbasis dasawisma.Simpulan: Penerapan model juamantik bergilir berbasis dasawisma dapat meningkatkan Angka Bebas Jentik (ABJ) di Kelurahan Mangunjiwan Demak.

Abstract

Exclusive breastfeeding is giving only breast milk without giving other food and drinks to babies from birth to 6 months of age and can be given until the age of 2 years. Breastfeeding coverage in Central Java tends to be low, namely in 2018 (45.21%). Dai Pemalang in 2015 (56.68%), 2016 (31.42%), 2017 (37.61%), 2018 (23.32%), 2019 (31.6%). The research objective was to analyze the factors that influence exclusive breastfeeding in Pemalang. This study used a cross-sectional design. The research sample of 110 respondents was obtained by using the cluster random sampling technique. Data collection techniques are interviews, observation, questionnaires. Data analysis in the study used the Chi-Square test and multivariate analysis. The results showed that the experience of breastfeeding and verbal persuasion had a direct or indirect effect on exclusive breastfeeding. The factor of observation of other people and verbal persuasion does not affect exclusive breastfeeding. Breastfeeding self-efficacy has a direct effect on exclusive breastfeeding with values (b = 3.82; 95% CI = 1.56 to 6.07; p = 0.001). The public health center in Pemalang needs to improve education for mothers and the community, including giving to husbands of breastfeeding mothers and the community.

Abstract

Efforts to improve public health are based on community participation in overcoming the environment because the environment is one of the factors that play a role in the emergence of a disease. In 2015, the Central Java government determined the most located area in Pemalang Regency which was later determined by a Regent's Decree. There are four sub-districts in the Pemalang Regency area which are included in the category that does not have latrines, namely Pemalang, Taman, Petarukan, and Comal sub-districts.The purpose of the study was to analyze the factors that influence community participation in the construction of family latrines in Petarukan District, Pemalang Regency. This study used a cross sectional design. The sampling technique is proportional stratified simple random sampling. The population is 3,882 households in 3 urban village/village, while the sample is 100. The instrument uses a questionnaire sheet. Data were analyzed by univariate, bivariate with Chi Square and multivariate with logistic regression. The results of the Chi square test show that knowledge factors that influence participation in latrine construction are age (p = 0.02), education (p = 0.01), income (p = 0.00), and (p = 0.00) . The results of the logistic regression showed that the factors that had the most influence on community participation in latrine construction were the level of knowledge.

Abstract

Dangerous and toxic waste from the Covid-19 pandemic increased by 30 percent. Dangerous waste management is one of the most serious problems in health facilities because a medical waste, especially infectious waste, is very potential in transmitting infectious diseases either through direct or indirect contact through environmental media. The purpose of this study was to analyze dangerous and toxic waste management based on Permenkes No. 7/2019 and Ministry of Environment and Forestry at the West Nusa Tenggara Hospital during the Covid-19 pandemic in 2020. The research design was descriptive qualitative. The data was taken by using the purposive sampling technique, the informants were selected by snowball sampling. The container stage uses yellow plastic, the transportation of covid and infectious B3 waste uses a yellow trolley and the waste transport officer is a cleaning service. Storage of dangerous and toxic waste and covid waste in temporary shelters every other day, if the weather does not support the transportation process for a maximum of one week, treatment of infectious waste and covid waste is carried out by PT PRIA as a third party. The hospital periodically controls related to the implementation of the container, transportation, and storage so that it is always following the applicable of Standard Operation Procedure.
