Abstract

Central Java Province is one of the regions in Indonesia with a high disaster risk. This situation directly affects the disruption of health services, including reproductive health, which is crucial for vulnerable groups such as pregnant women, infants, and adolescent girls. Reproductive health services in crisis situations still face various challenges, such as limited interagency coordination, human resource constraints, and lack of service system integration. This study aims to explore the opportunities and challenges of forming a reproductive health sub-cluster as part of a health crisis preparedness strategy in Central Java. The method used is a qualitative descriptive approach involving cross-sector stakeholders such as the Health Office, BKKBN, BPBD, Community Health Centers (Puskesmas), community organizations, and universities. Activities were carried out through several stages: regional identification, coordination team formation, Minimum Initial Service Package (MISP) training, and sub-cluster implementation evaluation. The results showed the establishment of a sub-cluster structure that was able to strengthen coordination systems and improve stakeholder capacity in crisis response. The MISP workshop significantly increased participants’ understanding of reproductive health services in emergency situations. Focus group discussions also indicated increased cross-sector collaboration and community engagement. Therefore, the formation of the reproductive health sub-cluster is an effective strategy in building a responsive and inclusive preparedness system in disaster-prone areas. This model can be replicated in other regions to strengthen the resilience of the national health system.

Abstract

Abstract

Central Java Province targets a 16% reduction in stunting by 2023, with the prevalence having been reduced from 31.2% in 2018(Riskesdas, 2018) to 20.8% in 2022 (SSGI). The causes of stunting in Central Java include a lack of knowledge about balanced nutrition, proper parenting, and lack of access to proper drinking water and sanitation. This study aims to determine the implementation of the Central Java Provincial Health Office strategy in reducing stunting rates. The study used a qualitative design with descriptive methods and in-depth interviews, involving samples from the Health Office, TP-PKK, and Head of Puskesmas through snowball sampling techniques. The results showed that implementation at the Puskesmas level has not been optimal, with low community participation. Semarang City has many stunting reduction programs, in contrast to Brebes Regency. The overall provincial target has been achieved, but operational funding constraints hinder optimal program implementation.

Abstract

Maternal and child mortality remains a major public health challenge in Indonesia, with Brebes Regency identified as one of the regions with a relatively high burden. This study aims to provide a narrative review of strategies implemented to reduce maternal and child mortality in Brebes, while highlighting their relevance within the broader Indonesian health policy framework. Literature from peer-reviewed journals indexed in Scopus and Web of Science, published between 2019 and 2024, was reviewed. The findings indicate that strategic planning at the district level emphasizes strengthening the role of midwives, improving access to primary health services, and integrating local programs with national policies. Behavioral nutrition interventions, such as the Baduta Program, and community-based innovations, such as “Buaian Services,” have shown potential in improving maternal and child health outcomes. However, challenges persist in the form of limited resources, disparities in service quality, and cultural barriers to health-seeking behavior. This review underscores the importance of combining top-down policy approaches with bottom-up community-driven initiatives. Future research is recommended to adopt mixed-method designs for a more comprehensive evaluation of program effectiveness.

Abstract

The phenomenon of adolescents involved in the prostitution industry was widespread globally, including in Indonesia. This study focused on the changing patterns of prostitution, motivations, perceptions, and risks faced by adolescent sex workers. The research used a mixed-method approach, employing both quantitative and qualitative designs. Data were collected through surveys involving adolescent sex workers and general adolescents, as well as in-depth interviews with adolescent sex workers. The quantitative study included 360 respondents, comprising 180 general adolescents and 180 adolescent sex workers, while the qualitative study involved 30 adolescent sex workers as informants. The research was conducted in Bali, South Kalimantan, West Java, North Sulawesi, North Sumatra, and Central Java provinces. The findings revealed a shift in prostitution patterns among adolescents from conventional (street-based) prostitution to online prostitution via digital platforms. Adolescents had greater opportunities to become online sex workers due to the diversity of social media platforms and service types available. Economic factors accounted for 81.7% of adolescents' reasons for becoming sex workers. There was a disparity in perceptions between general adolescents and the actual challenges faced by adolescent sex workers. Among adolescent sex workers, 80% had low reproductive health knowledge, 23% had experienced sexually transmitted infections (STIs), 16% had faced unwanted pregnancies, and 7% had undergone abortions. It was concluded that there were significant changes in the patterns, motivations, perceptions, and risks associated with adolescent sex work.

Abstract

Exclusive breastfeeding is an important policy to ensure the health of infants and mothers. Semarang, as one of Indonesia's metropolitan cities, faces challenges in implementing this policy. Evaluation of the exclusive breastfeeding program is necessary to understand its implementation success and influencing factors. This study aims to evaluate the implementation of the exclusive breastfeeding policy in Semarang. The method used is qualitative research with a case study approach. Data were collected through in-depth interviews with health officials, lactating mothers, and relevant stakeholders in Semarang's exclusive breastfeeding policy implementation. The research findings indicate several challenges in the implementation, such as low public awareness of the importance of exclusive breastfeeding and inadequate government supervision. Based on the research results, evaluation of input from various research informants indicates that the planned breastfeeding program has been well communicated despite some obstacles. Program evaluation is conducted monthly by health centers through recording and reporting. Evaluation of the implementation of the exclusive breastfeeding policy in Semarang is carried out using an output evaluation model, assessing the coverage rate of exclusive breastfeeding in Semarang, which shows an increase in the coverage rate of exclusive breastfeeding.
