Abstract

Introduction:  Oral health is essential for children's mental and physical well-being. Caries is one of the most common oral diseases worldwide, especially in children. One of the contributing factors to this disease is parental engagement. Parents play a crucial role in children’s oral health, especially their Knowledge, Attitude, and Practice (KAP). Teledentistry is one of the innovative ways to improve parental KAP due to technological advancements in leveraging the number of smartphones and the internet. Teledentistry could help parents/caregivers obtain education, consultation, diagnosis, and treatment planning about oral health via remote connection without going to dental offices. This engagement of parents/caregivers through teledentistry could build good oral health in children. This review aims to assess the effectiveness of smartphone applications for teledentistry in parental KAP related to children's oral health. Methods: This review followed PRISMA guidelines. We searched the articles in five databases (PubMed, EBSCOhost via Medline, Embase, ProQuest, and Scopus) using specific keywords and a PICO framework from January 2018 to December 2023. Two reviewers independently selected the articles based on inclusion and exclusion criteria. The selected articles were extracted and assessed for risk of bias using MMAT tool. Results: Of the 1529 articles initially identified, 14 met the inclusion criteria. All smartphone application reviewed were designed for parents/caregivers and integrated with teledentistry; however, only two applications incorporated with artificial intelligence. All application showed good assessment through application tests using SUS (3 articles), JN’s principles (1 article), Net Promotor Score (1 article), MARS (1 article), TAM (1 articles), and sensitivity and specificity tests (2 articles). Parental/caregiver engagement was assessed through their Knowledge, Attitude, and Practice (KAP). This has impacted the children’s oral health by improving both the plaque and gingival index. Conclusion: Smartphone applications for teledentistry effectively increase the engagement and KAP of the parents/caregivers, significantly improving the oral health status of the children.

Abstract

Introduction: According to the World Health Organization (WHO), over 2.2 billion people globally are affected by visual impairment, which significantly affects their dental and oral hygiene behaviors. Consequently, the prevalence of dental caries among visually impaired children remains high, primarily due to challenges in maintaining proper hygiene. This study aims to analyze differences in dental caries status among visually impaired children based on risk factors. Method: The study procedures were carried out using a cross-sectional design. The sample population comprised 94 students at the National Special School (SLB) A Pembina, who were selected using a purposive sampling method. Data collection was conducted in 2 stages. The first stage involved gathering information on risk factors through interviews regarding toothache experience, visits to the dentist, tooth brushing frequency, and fluoride toothpaste use. The second stage consisted of dental and oral examinations conducted by five trained investigators. Results: The overall prevalence of dental caries among visually impaired children was 42.6%, with a deft/DMFT index of 1.05/0.8. In addition, there were significant differences in dental caries status based on risk factors, such as toothache experience (p=0.001, ≤0.05, Pearson’s Chi-square, 95% CI), visits to the dentist (p=0.000, ≤0.05, Pearson’s Chi-square, 95% CI), tooth brushing frequency (p=0.000, ≤0.05, Pearson’s Chi-square, 95% CI), and fluoride toothpaste use (p=0.000, ≤0.05, Pearson’s Chi-square, 95% CI). Conclusion: Based on these results, the prevalence of dental caries among visually impaired children is relatively high. Caries experience is significantly associated with toothache experience, visits to the dentist, tooth brushing frequency, and fluoride toothpaste use.

Abstract

Introduction: This study aimed to evaluate the effectiveness of a video-based educational intervention for improving knowledge of dental malocclusion among students at SMAN 39, Jakarta, and to examine the associations between knowledge levels and sociodemographic factors. Materials and Methods: A quasi-experimental design involving 159 students was employed. Data were collected via structured pre-test      and post-test      questionnaires assessing students’ knowledge of dental malocclusion. Statistical analyses included the Wilcoxon signed-rank test, Mann–Whitney U test, and Spearman correlation coefficient to examine associations between variables. Results and Discussion: All 159 participants completed both the pre-test and post-test. A statistically significant improvement in knowledge was observed following the intervention (p = 0.001). While sex, age, and socioeconomic status were not significantly associated with baseline knowledge, the post-intervention results indicated that female students, older age groups, and students from higher-income households demonstrated significantly greater knowledge retention (p-value < 0.05). Conclusion: Video-based interventions effectively enhanced student’s knowledge of dental malocclusion; however, the extent of improvement varied across sociodemographic groups. These findings suggest that sex, age, and socioeconomic status may influence educational outcomes. Future oral health education initiatives should consider these factors to increase the effectiveness and inclusivity of digital learning interventions.

Abstract

Introduction: The life course approach highlights how early life, beginning with fetal growth, influences future disease risk. Oral health studies in Indonesia using this approach are still limited. This study examined the relationship between ECC status and retrospective data on stunted children under three and their mothers’ pregnancy history at Juwiring District Community Health Center. Objective: This study aims to examine the influence of pre, peri-, and postnatal factors on ECC occurrence in stunted children in Juwiring Regency, Central Java. Methods: Secondary data of pre, peri-, and postnatal information from 265 stunted children were obtained from the medical records. Oral examination was assisted by local dentists from the health center. Multiple logistic regression analysis was performed to construct a prediction model and determine the factors with the greatest influence on dental caries in stunted children in Juwiring District. Results: The prevalence of dental caries was 69.1% in 246 subjects. The variables in the final prediction model for factors related to ECC were the mother's perception of her child's dental problems, the mother's education level, the mother’s knowledge of dental caries, and the child’s stunting status. The most influential factor was the mother's perception of her child's dental problem, with the highest odds ratio (OR) of 5.1 (1.83–14.23). Conclusion: This study revealed that prenatal factors related mothers’ education level and postnatal factors related to mothers’ perceived dental problems, poor knowledge of dental caries and children’s stunting status were included in the ECC prediction model for the stunted children of Juwiring District.

Abstract

Latar Belakang: Stunting adalah malnutrisi kronis pada anak yang masih menjadi tantangan kesehatan masyarakat. Stunting tidak hanya dapat menyebabkan gangguan pertumbuhan fisik dan kognitif, namun juga lebih rentan mengalami karies gigi. Penelitian ini bertujuan mengetahui hubungan antara stunting dan kejadian karies gigi pada anak stunting usia 0-3 tahun di kabupaten Klaten, Jawa Tengah. Bahan dan Metode: Penelitian ini menggunakan desain potong lintang dengan data primer dari program JUWITA 1000 HARTA oleh Puskesmas Juwiring. Sebanyak 264 anak yang mengalami stunting menjadi subjek dengan teknik purposive sampling. Status karies gigi diperiksa menggunakan kamera intraoral. Analisis bivariat dilakukan dengan menggunakan uji chi-square. Hasil dan Pembahasan: Dari 248 anak yang memenuhi kriteria, proporsi karies gigi 75% pada anak dengan kondisi sangat pendek (severely stunted), dan 53,9% pada pendek (stunted). Hubungan antara status stunting dan kejadian karies gigi adalah signifikan (p 0,01) dengan OR= 1,39 (95% CI; 1,11–1,74). Selain itu, faktor-faktor status ekonomi keluarga, tingkat pendidikan orang tua, dan pola makan anak juga menunjukkan hubungan yang signifikan terhadap kejadian karies gigi (p 0,05). Kesimpulan: Terdapat hubungan antara stunting dengan kejadian karies pada anak penderita stunting usia 0-3 tahun di Kabupaten Klaten, Jawa Tengah. Diperlukan program pencegahan stunting untuk mencegah terjadinya kejadian karies gigi pada anak secara tidak langsung.

Abstract

Oral health promotion among school age children requires innovative and engaging strategies to address persistent challenges such as dental caries and poor hygiene practices. Co-design methodology emphasizes the active involvement of end user chil-dren, parents, teachers, and health professionals in developing interventions tailored to their needs and context. This study re-views the use of a co-design approach in developing a school-based digital health application to improve oral health behaviors in children. The review examined stakeholder involvement, including children, parents, teachers, and health professionals in sha-ping content, usability, and engagement features of oral health applications. Evidence indicates that co-design enhances the re-levance, usability, and acceptability of digital oral health interventions by integrating the lived experiences and preferences of tar-get users. Applications developed through co-design showed higher levels of engagement, improved oral health knowledge, and better adherence to preventive behaviors among school children. It is concluded that co-design enhances the relevance, usabi-bility, and acceptability of digital oral health interventions by integrating the lived experiences and preferences of target users.

Abstract

Promosi kesehatan mulut pada anak usia sekolah memerlukan strategi inovatif dan menarik untuk mengatasi tantangan yang persisten seperti karies gigi dan kebiasaan higiene yang buruk. Metode co-design menekankan keterlibatan aktif pengguna akhir, yaitu anak-anak, orang tua, guru, dan tenaga kesehatan, dalam mengembangkan intervensi yang disesuaikan dengan kebutuhan dan konteks mereka. Studi ini mengeksplorasi penggunaan pendekatan co-design dalam mengembangkan aplikasi kesehatan digital berbasis sekolah untuk meningkatkan perilaku kesehatan mulut pada anak. Dilakukan tinjauan naratif untuk menganalisis pustaka tentang praktik co-design dalam kesehatan gigi digital, dengan fokus pada aplikasi untuk populasi usia sekolah. Bukti menunjukkan bahwa co-design meningkatkan relevansi, kegunaan, dan penerimaan intervensi kesehatan mulut digital dengan mengintegrasikan pengalaman dan preferensi pengguna target. Aplikasi yang dikembangkan melalui co-design menunjukkan tingkat keterlibatan yang lebih tinggi, peningkatan pengetahuan kesehatan mulut, dan kepatuhan yang lebih baik terhadap perilaku pencegahan di kalangan anak sekolah. Disimpulkan bahwa co-design meningkatkan relevansi, kegunaan, dan penerimaan intervensi kesehatan mulut digital dengan mengintegrasikan pengalaman dan preferensi pengguna target.

Abstract

Introduction: Untreated caries is one of the oral diseases that poses a significant burden in Indonesia. The perceived need for people to receive dental treatment is influenced by sociodemographic, economic, environmental, and healthcare service factors. The purpose of this study was to determine sociodemographic factors towards untreated caries and dental fillings in Indonesia, and then visualize them in geospatial mapping. Material and Methods: A cross-sectional study used secondary data from Basic Health Research 2018. A sample was 14.031 subjects, then classified based on WHO age group and sociodemographic factors. As gender, residence, educational level, employment status, and frequency of dental visits as independent variables. Then, the number of untreated caries and fillings is the dependent variable. The bivariate analysis was used to determine different factors. QGIS was used to visualize dependent variables in 26 provinces in Indonesia. Results and Discussions: There was a statistically significant difference (p<0.05) in the 35-44 years age group between females, rural residents, those with no schooling, and informal workers concerning untreated caries. Meanwhile, females, urban residents, college diploma holders, formal workers, and those with dental visits showed a tendency toward dental fillings. This result closely aligns with overlay mapping geospatial analysis. Conclusion: Sociodemographic characteristics had an impact on untreated caries and dental fillings, and should be considered to find out comprehensive strategies to improve oral health care.

Abstract

Introduction: The high burden of childhood dental caries highlights the need for innovative approaches. Artificial Intelligence (AI) offers a breakthrough in enabling more efficient, measurable efforts to prevent and detect dental caries. Therefore, this scoping review aims to map the existing evidence on the use of AI interventions in the prevention of childhood dental caries based on Leavell and Clark’s prevention model. Review: This scoping review analyzed 49 studies from various countries that examined the application of AI in childhood caries prevention, using the Leavell and Clark framework. At the health promotion level, behavior-based chatbot applications were shown to significantly improve caregivers’ knowledge and toothbrushing habits. In the specific protection setting, predictive models based on tabular data and the salivary microbiome identified high-risk children before lesions formed, with AUC values ranging from 0.77 to 0.94. For early detection, deep learning algorithms trained on radiographic images and intraoral photographs demonstrated high accuracy in detecting caries lesions, including incipient lesions, with a sensitivity of up to 94.7%. For disability prevention, AI models successfully predicted the success of caries arrest therapy with silver diamine fluoride. The main limitations identified were a small sample size, a lack of cross-population external validation, and gaps in model interpretability at the clinical level. Conclusion: The use of AI interventions for childhood dental caries is unevenly distributed across Leavell and Clark's five preventive levels, especially for specific protection.
