Abstract

Medical records according to Permenkes No. 269 of 2008 is a file containing notes and documents about patient identity. The problem that often arises in Klaten General Hospital is the length of the file search process due to the absence of applications that control the entry and exit of files, when officers borrow and return only to write in the control book. Supporting all the problems and objectives needed by the data, so the data collection methods used are observation and interviews. Analysis of the data obtained from the interview and observation process is to draw conclusions and identify the most urgent problems. The goal to solve this problem is to make an application that controls the borrowing and return of the RM file so that the officer knows quickly where the file will be sought. The application is made using the waterfall method in the workflow process and uses several programming languages such as PHP, HTML and CSS. The results of the application have been tested by several users using blackbok testing, but still do not know how effective and efficient when the application is tested in actual service.

Abstract

One of the hospital's obligations is to organize a medical record. Good or bad medical record-up is a description of the quality of the hospital. The minimum service standard for the patient's return of the patient's medical record is completed by obtaining medical services until the medical record file returns to the medical record room within 2x24 hours. The return of an inpatient medical record in Universitas Airlangga Hospital takes more than 2 X 24 hours, even up to 1 week is calculated since the patient returned home and the file has not been completed 100%. Such problems will result in delayed reporting, provision of old patient medical record files should still be sought in the last location of the borrower's medical record file if not found in the filing rack. There is still an unrecorded return time in the expedition book. There is a problem with the delay in the return of an inpatient medical record so that it is necessary to identify the problem that aims to correct or minimize potential problems. The identification methods used are observation and interview methods. Observations are performed on data sources collected either primary data or secondary data related to the cause of the return of the medical record file. The interview is conducted by the researcher to the officer related to the late return of the inpatient medical record file. The study uses the 5M method (Man, Machine, Method, Material, and Money) by Harrington Emerson.

Abstract

The  Filing medical records information system is a computerized information that can also be used to help medical records officers in the filing unit when borrowing and returning medical record files and to help them when doing a retention. Filing information system at Citra Husada Hospital, of Jember Regency is still using a manual system for recording the loan file of medical record and also there is no recording date system for returning medical record file. The purpose of this study was to design and create a medical record information system for the filing section at Citra Husada Hospital, Jember Regency using the waterfall method. Data collection uses interviews, observation and FGD (Focus Group Discussion). In the process of designing this system using a Flowchart System, Context Diagrams, Data Flow Diagrams, Entity Relationship. Digram and in implementing programs using Microsoft Visual Basic 2010. The results of this study are the making of a medical record information system for filing to facilitate officers in controlling medical record documents in filing through borrowing, repayment and retention. The next researcher is expected to be able to integrate with the existing SIMRS so that filling in the medical record data can automatically appear when in the filing section

Abstract

The existing problem that related to workload of medical records officer in Primary Health Care of Ambulu there are never done the analysis and evaluation, more over since 2018 medical record unit and registration unit splinted into two unit to be on one’s own. This research that intend to do the analysis workload of medical record officers therfore be discovered the level of workload of officer with WISN (workload indicator staff need) methode. If ther is already known the workload of medical record officers the next is an analysis of causative factors of workload was carried out with  fishbone methode with approach of  5M (man, money, methode, material, mechine) and then the next with FGD (focus group discusion) to determine the main priority that caused workload so that improvements of workload can be made. The type of this research is a qualitative with collecting data tecnic in the form of observation, interview, documentation, and FGD. The subject of this research are 3 medical record officer, there are 1 medical recorder and 2 medical recorder helper. The based on this research workload analysis the job description is well done because in the job description there is no division of tasks according to posisition so completion of the task have done together. The result of the calculation of workload is knowing the amount of officer needs the results obtined ideal number of officers there are 4,85 there for be rounded to 5 so the conclusion is the workload in the unit medical record is high with rhe total 0,6. The efforts to improve the workload including addition of human resources and the held of training thus increasing the knowledge and skills of officers.

Abstract

The medical record file control activity at Wijaya Kusuma Lumajang Hospital has not been well implemented.This is evidenced by the absence of the use of tracers as a substitute for files that leave the medical recordrack and the use of expedition books only on borrowing inpatient medical record files. This study aims todesign and create a medical record lending and return information system with a barcode and web-basedWhatsApp notification at Wijaya Kusuma Lumajang Hospital that produces patient labels, tracers, reports onborrowing and return of medical records. This type of research is the development of a prototype method withqualitative data collection techniques in the form of interviews, observation, documentation, and brainstorming.The stages of the prototype method include analyzing user needs, making prototypes, adjusting prototypes tothe user's wishes, creating new systems, testing systems, adjusting the system to the user's wishes, andusing the system. The results of this study are information systems for borrowing and returning medicalrecords with a web-based barcode and WhatsApp notification system that facilitates the process of trackingfiles and overcoming problems in the process of borrowing and returning medical records

Abstract

The information system is made based on problems in recording the implementation of loans, returningmedical records. This computerized information system where in the borrowing and returning of medicalrecords is no longer manual. The purpose of this information system is to facilitate officers in recording,borrowing, returning, controlling and controlling medical records. The borrowing and return of medical recordsmust be controlled properly to reduce duplication of medical record numbers and delays in medical recorddocuments. Qualitative research using documentation data collection methods and interviews at theBanjarsengon Health Center in Jember. The method used in this research is the 2011 somerviel waterfallmethod. The steps taken are requirements analysis, system design, program code writing, and programtesting. The design of this information system uses flowchart design, context diagrams, Data Flow Diagrams(DFD), and Entity Relationship Diagrams (ERD) then the design results are implemented into a visual basicprogramming language using Miscrosoft Visual Studio 2010. The results of this study are lending informationsystems. and return of medical record documents. This information system is equipped with a warning alert asa notification that medical record documents borrowed by POLI to be returned immediately, can help ease theworkload of filing officers by minimizing the number of delays in medical record documents and duplication ofmedical record documents.

Abstract

Pelayanan gawat darurat bertujuan menyelamatkan penderita gawat darurat dan menanggulangi korban bencana pada masyarakat dengan penanganan yang cepat dan tepat, semua itu dapat dicapai dengan meningkatkan sarana, prasarana, sumberdaya manusia dan manajemen UGD puskesmas sesuai dengan standart. Pelayanan Kesehatan puskesmas mencakup perencanaan, pelaksanaan, evaluasi, pencatatan, pelaporan, dan dituangkan dalam suatu sistem. Petugas Unit Gawat Darurat Puskesmas Puger dalam melakukan  pencatatan  masih menggunakan buku ekspedisi yang mengakibatkan petugas kesulitan dalam pencarian data saat diperlukan dan pencatatan masih menggunakan formulir UGD yang dicatat secara manual sehingga pendokumentasian  data korban bencana yang dihasilkan kurang lengkap sehingga mengakibatkan kualitas informasi kurang akurat. Penelitian ini bertujuan merancang Rekam Medis Korban Bencana Berbasis web di Unit Gawat Darurat Puskesmas Puger Kabupaten Jember. Jenis Penelitian adalah kualitatif. Metode pengembangan sistem yang digunakan yaitu metode waterfall. Subyek penelitian berjumlah 4 orang meliputi 2 petugas UGD, petugas pelaporan bencana dan kepala puskesmas. Pengumpulan data menggunakan observasi, wawancara dan dokumentasi. Pembuatan rekam medis elektronik korban bencana di unit gawat darurat dimulai dari analisis kebutuhan perangkat lunak, desain sistem, pembuatan kode program, dan pengujian black box testing. Hasil penelitian ini adalah sebuah website pencatatan rekam medis korban bencana di Unit Gawat Darurat Puskesmas Puger, website ini diharapkan dapat mempermudah petugas dalam pencatatan dan penyimpanan data korban bencana di unit gawat darurat

Abstract

Banjarsengon public health center is one of the health agencies that deal with patient care and treatment. The pharmacy section of the Banjarsengon Health Center in carrying out registration, recording medical records, drug data collection and managing reports is still done manually. In addition, data collection errors are often encountered which can lead to inaccurate reporting data. The purpose of this research is to solve problems in drug data collection by designing a pharmacy information system at the Banjarsengon health center. The making of pharmacy information system application was using Microsoft Visual Studio 2019. System development was using waterfall method. Data collections consist of observation, interviews and documentation. The result of black box testing showed that this pharmacy information system can operate properly. The advantages are the pharmacy information system been integrated with the operation of pharmacists and management that can reduce the possibility of errors that affect the service to patients, improve the efficiency of the drug recording system quickly and accurately, facilitate pharmacists in recording, reduce and add drug stock automatically and also print reports in pdf form.

Abstract

X Hospital in Jember regency applies a hospital management system or hospital information system (SIMRS) since 2012. The results of a pilot study conducted by the researcher showed that there are still several issues in the implementation of hospital management information systems (SIMRS) including data mismatches and incomplete features. The objective of this study was to evaluate the success of the SIMRS implementation at X Hospital in Jember regency using the Task Technology Fit (TTF) theory. This study was quantitative research. Data collection techniques used questionnaires with a total of 54 respondents as the sample. The results showed that the identification of the TTF variable at the X Hospital in Jember regency showed that the Task Characteristic (TAC) had a total score of 80.1%, Technology Characteristic (TEC) had a total score of 77.5%, Task Technology Fit (TTF) had a total score. 78.17%, Performance Impact (PI) has a total score of 78.19%, and Utilization (U) has a total score of 80.25%. The suggestion proposed by the researcher regarding the above problems is that the hospital needs to make improvements and development of systems related to data consistency, feature completeness, and feature optimization in SIMRS according to user needs to increase the success of its implementation.

Abstract

Negara Indonesia salah satu negara tropis. Curah hujan yang tinggi dengan kepadatan penduduk di wilayah Indonesia menghasilkan berbagai banyak penyakit. Salah satu penyakit yang sering muncul adalah penyakit yang diakibatkan oleh infeksi dan parasit tropis. Penyakit yang tergolong dengan infeksi dan parasit yang sering di negara Indonesia yaitu Thypoid fever dan Dengue fever. Sistem pakar adalah inovasi terbaru dalam mendeteksi kondisi awal. Sistem pakar dapat diimplementasikan di sektor kesehatan, salah satunya adalah sistem diagnosis ahli yang berguna untuk mendiagnosis penyakit dengan gejala demam pada manusia dengan melihat karakteristik dan gejala yang dialami pasien. Penelitian ini bertujuan untuk mendiagnosis demam tifoid dan Dengue Fever yang disebabkan oleh gejala demam pada pasien. Metode penelitian ini adalah metode penelitian pengembangan dengan menggunakan metode sanders yang meliputi definisi masalah, analisis sistem, perancangan sistem, dan implementasi sistem. Hasil penelitian ini, menggunakan metode dempster shafer untuk menentukan penilaian terhadapat penyakit Typoid fever dan dengue Fever sehingga dari hasil tersebut mendapatkan nilai kekauratan dengan persentase penyakit typoid fever mencapai 93%  dan dengue fever mencapai 94% dapat digunakan sebagai referensi untuk keakuratan diagnosis untuk mencegah penyakit Typoid Fever dan Dengue Fever.
