Abstract

Background and purpose: Average Bed Occupation Rate (BOR) of Mental Hospital in Bali Province in 2014 amounted to 85.3% with an average length of stay about 50 days. Preliminary study indicated inadequate performance of nurses at Mental Hospital in Bali. The purpose of this study was to determine the association of competence, motivation and nurses workload with the performance of nurses in wards of Mental Hospital in Bali Province. Methods: This research was a cross sectional survey. The subjects of the study consisted of all nurses in the inpatient unit, amounting to 111 people. Data were collected using self-administered questionnaires and observation sheet filled out by the head of the room. Data were analyzed using bivariate with chi-square test, and multivariate with logistic regression method. Results: Bivariate analysis showed significant association between the performance of nurses with competence and motivation (p<0.001), however no association with workload (p=0.94). Multivariate analysis showed that the performance of nurses was associated with competence with AOR=65.38 (95%CI: 6.88-621.52) and with motivation (AOR=61.71; 95%CI: 7.15-532.59), however was not significantly associated with workload (AOR=1.012; 95%CI: 0.32-3.17). Conclusion: The performance of nurses was significantly associated with competence and motivation, however not associated with workload.

Abstract

Background and purpose: Quality of health services in the public health centers in Karangasem remain a public health concern. Preliminary findings indicated there was a problem regarding implementation of health center management and working commitment of staff. This study aims to determine the association between quality of medical services, the implementation of health centre management and working commitment of staff. Methods: Study was cross-sectional among 61 doctors and nurses at 12 public health centres who were selected purposively. Data were collected from February to March 2015 using questionnaires. Data analysis using logistic regression to examine the relationship between the quality of medical services with the implementation of health center management and working commitment of staff. Results: Findings indicated that the quality of medical services, the implementation of health centre management and working commitment of staff were relatively low. The quality of medical services provided was found associated with working commitment of staff (AOR=11.3; 95%C: 1.75-73.06) and not associated with implementation of health center management (AOR=0.9; 95%CI: 0.18-5.24). Conclusion: The quality of medical services provided was found associated with working commitment of staff and not associated with implementation of health center management.

Abstract

Background and objectives: The Emergency Department (ER) at Udayana University Hospital is required to provide quality health services in accordance with the Minimum Service Standards (SPM) set by the government. However, there are several SPM indicators that have not been achieved. Therefore, this research aims to develop a strategy to increase SPM achievement in the emergency room at Udayana University Hospital using the two-stage Delphi method. Method: A qualitative approach with a two-stage Delphi method was used to achieve this objective. Data was collected through in-depth interviews with 16 informants, FGDs and questionnaires. Results: The research found that internal factors categorized as strengths and weaknesses influenced the achievement of SPM in the IGD. External factors are classified into opportunities and threats. SWOT analysis shows that the IE matrix describes a grown and built organization. In the 4-Quadrant SWOT Matrix, it is in quadrant I which describes the opportunities and strengths of the organization and strategies that can be carried out with aggressive strategies. The TOWS matrix produces 11 alternative strategies which are ranked using the QSPM method. Alternative strategies generated using the Delphi method resulted in 6 priority strategies. Conclusion: From this research, 7 strength factors, 7 weakness factors, 4 opportunity factors and 3 threat factors were found at UNUD Hospital and there are 6 priority strategies that can be used to improve SPM IGD achievements.

Abstract

Background and purpose: The pandemic of COVID-19 is an unprecedented medical and economic challenge for healthcare system. The number of patients during pandemic of COVID-19 had a rapid growth which led to changes in services aspects that threaten financial viability of hospitals, including private hospitals. This study aims to determine the efforts of private hospitals in maintaining financial stability during the pandemic of COVID-19. Methods:  A qualitative study with a multi-case design carried out in 7 (seven) private hospitals in Denpasar. Informants were obtained based on a purposive sampling approach. This study recruited 19 informants from 7 private hospitals in Denpasar. Data was collected through in-depth interviews conducted face-to-face, using the Zoom and Whatsapp applications. The data were analysed using a content analysis approach. Results: Almost all hospitals stated that their finances disrupted. Financial mitigation efforts were difficult to do, hospital preparation and responses were dependent on the hospital's financial condition and regulations that changing, and recovery efforts were not easy either because in general the community's economy has not recovered. Conclusion: Private hospital finances were experiencing disruption. Mitigation efforts were difficult to do due to the unpredictable changing regulations and policies during the pandemic. Based on the study results, it can be recommended that the hospital management needs to develop a service strategy that is able to overcome the problem of the surge in the number of patients.

Abstract

Background and purpose: Leadership style, work environment and motivation of nurses are related to the implementation of nursing care standards. This study aims to determine factors associated with the implementation of nursing care standards in the hospital’s inpatient services.  Methods: A cross-sectional study was conducted with all of the total 65 nurses working in the inpatient wards of the Udayana University Hospital, Bali in December 2022. Data were collected with a structured questionnaire covering nurses’ characteristics, leadership style, work environment, motivation and implementation of nursing care standards. Data were analyzed descriptively and analytically through path analysis assisted by a statistical software. Results: Of the 65 nurses, 69.2% were aged 26-30 years, 64.5% were female, 75.5% have worked under 3 years, and 75.4% have a bachelor's degree in nursing. There is a relationship between motivation (with a path coefficient of 0.603, p<0.001) and supportive leadership style (with a path coefficient of 0.615, p=0.028) with the implementation of standard nursing care. In addition, supportive leadership style (path coefficient 0.554, p=0.002), participative leadership style (path coefficient -0.353, p=0.061) and achievement-oriented leadership style (path coefficient 0.426, p=0.002) were associated with work motivation. Conclusion: Nurse motivation is directly associated, while supportive leadership style is indirectly related to the implementation of nursing care standards through increasing nurse motivation. Supportive, participatory leadership styles and achievement orientation are also directly related to nursing motivation. The hospital leaders and room manager must maintan and strengthen the participatory leadership style which will improve motivation and nursing care standard.

Abstract

Background and purpose: National Health Insurance Provider (BPJS Kesehatan) facilitates the distribution of National Health Insurance (NHI) participants. High number of 1,947 NHI participants were switched primary health providers in Klungkung, Bali between July to December 2021.This study aims to explore the reasons for the switching of providers among NHI participants and the health providers’ efforts to minimize it. Methods: A qualitative descriptive study guided by an access theory was conducted in Klungkung District between December 2022 to March 2023. A total of 18 informants were purposively recruited, consisting of NHI participants (12 people), health service providers (4 people), BPJS Kesehatan (1 person) and staff of the district health office (1 person). Data was collected through in-depth interviews guided by interview guidelines. Data were analyzed using a thematic approach and presented in a descriptive narrative. Results: NHI participants in Klungkung District switched provider due to spatial and non-spatial reasons. Spatial reason was the distance and time to the health provider. Non-spatial reasons included (1) conflict in social relations due to service and negative attitudes of health workers, (2) economic considerations in the health system which are difficult to reach, and (3) external encouragement. Strategies that have been or can be implemented to minimize the switching of PHCs were adjusting operational hours, improving the infrastructures, administration through fulfilling accreditation, service quality and training for human resources. Conclusion: The dominant reason for switching of PHCs is associated with negative social interaction experiences related to human resources and service facilities at PHCs. PHCs are expected to be able to improve the quality of services through improving human resources and service delivery facilities.

Abstract

Pelayanan kesehatan gigi dan mulut merupakan komponen penting dalam menjaga kualitas hidup masyarakat, terutama peserta Jaminan Kesehatan Nasional (JKN). Klinik Kimia Farma Nusa Dua sebagai fasilitas kesehatan tingkat pertama (FKTP) berperan menyediakan layanan gigi dan mulut bagi peserta JKN. Namun, tantangan dalam memenuhi harapan pasien terhadap aksesibilitas, biaya, dan kualitas layanan masih sering terjadi. Penelitian ini bertujuan untuk menggambarkan karakteristik pasien JKN dan tingkat kepuasan mereka terhadap pelayanan kesehatan gigi dan mulut di Klinik Kimia Farma Nusa Dua. Penelitian ini menggunakan desain kuantitatif cross-sectional deskriptif. Data dikumpulkan dari 130 pasien JKN yang memenuhi kriteria inklusi, seperti usia 17–65 tahun dan minimal satu kali kunjungan ke klinik selama periode Mei 2024. Pengambilan sampel dilakukan secara simple random sampling menggunakan aplikasi random number generator. Data dikumpulkan melalui kuesioner terstruktur berdasarkan Dental Satisfaction Questionnaire (DSQ) yang mencakup tujuh dimensi kepuasan. Analisis data dilakukan secara deskriptif menggunakan perangkat lunak SPSS dan Excel. Hasil penelitian menunjukkan bahwa mayoritas pasien adalah perempuan (53,8%), berusia 17–25 tahun (49,2%), berpendidikan SMA (43,8%), dan bekerja sebagai pegawai swasta atau tidak bekerja (28,5%). Sebanyak 70% pasien melaporkan kepuasan yang tinggi terhadap layanan klinik. Dimensi dengan nilai kepuasan tertinggi adalah kualitas layanan (mean 3,92), sedangkan aspek kenyamanan pemeriksaan menunjukkan nilai terendah (mean 3,65). Penelitian ini menyimpulkan bahwa sebagian besar pasien merasa puas dengan layanan di klinik, meskipun beberapa dimensi, seperti kenyamanan dan konsistensi pelayanan oleh dokter yang sama, memerlukan peningkatan.

Abstract

Implementasi RME tidak berjalan optimal karena masih terjadi ketidaklengkapan pengisiannya dari Juli-September 2022 sebanyak 349 berkas. Penelitian ini bertujuan untuk mengetahui kelengkapan RME dan faktor yang berhubungan dengan kelengkapan pengisian RME rawat jalan pada nakes di rumah sakit. Penelitian ini merupakan penelitian obervasional analitik kuantitatif dengan desain cross sectional. Populasi penelitian ini berjumlah 113 nakes. Sampel yang digunakan 100 orang nakes yang diambil dengan Teknik stratified random sampling. Variabel yang dianalisis yaitu kelengkapan pengisian RME, minat menggunakan, ekspektasi kinerja, ekspektasi usaha, pengaruh sosial, kondisi yang memfasilitasi, motivasi hedonis, kebiasaan, umur dan jenis kelamin pengguna. Analisis data dilakukan secara deskriptif dilanjutkan dengan menggunakan path analysis. Komponen dengan kelengkapan tertinggi adalah No. RME. Terdapat hubungan antara ekspektasi kinerja terhadap minat menggunakan RME. Kebiasaan pengguna berhubungan dengan minat menggunakan RME. Ada hubungan ekspektasi kinerja dan ekspektasi usaha dengan kelengkapan RME. Motivasi hedonis memiliki hubungan dengan kelengkapan RME. Ekepektasi kinerja dan kebiasaan pengguna memiliki hubungan dengan minat menggunakan RME. Ekspektasi kinerja, dan ekspektasi usaha memiliki hubungan langsung dengan kelengkapan pengisian RME dan motivasi hedonis memiliki hubungan dengan kelengkapan pengisian RME. Manajemen Rumah Sakit diharapkan memberikan pelatihan lebih lanjut, menyediakan modul penggunaan RME dan memberikan edukasi mengenai kelemahan dan kelebihan RME.    Kata Kunci: Rumah Sakit, Rekam Medis, Rekam Medis Elektronik

Abstract

Diabetes Mellitus and Hypertension is a common condition that often arises together, both diseases are considered as the risk of cardiovascular disease and microvascular complications of many factors that influence it like bad lifestyle changes. This study aims to determine the relationship between hypertension, physical activity, and body mass index with blood sugar levels in Diabetes Mellitus patients at Public Health Center in Buleleng. This research uses a quantitative research design cross-sectional method with a simple random sampling technique with 150 samples, taken by observation, and observation is guided by the Baecke questionnaire. Data analysis with Spearman Rank correlation test shows no relationship between hypertension with blood sugar levels with value ρ = (0.938)> α (0.05), no relationship between physical activity with blood sugar level body mass index with blood sugar level with value ρ = (0.914)> α (0.05). There is no relationship between hypertension, physical activity, and body mass index with blood sugar levels in Diabetes Mellitus patients

Abstract

Pekerja seks perempuan (PSP) merupakan kelompok yang termarginalkan secara sosial dan memiliki kerentanan yang tinggi terhadap masalah kesehatan. Upaya perluasan Jaminan Kesehatan Nasional (JKN) pada PSP masih terbatas sehingga penting dilakukan untuk mendukung pencapaian universal health coverage. Penelitian ini bertujuan untuk memperoleh gambaran mengenai akses JKN pada PSP di Denpasar. Penelitian ini merupakan studi kualitatif. Data dikumpulkan melalui wawancara mendalam terhadap 15 orang PSP dan empat orang mucikari di Denpasar pada Agustus hingga Oktober 2014. Hasil wawancara diolah dengan analisis tematik. Kerangka analisis yang digunakan adalah The Health Access Livelihood Framework. Kepemilikan JKN pada PSP di Denpasar masih rendah, meskipun sebagian PSP memiliki kemauan untuk menjadi peserta JKN dan memiliki kemampuan membayar iuran JKN. Faktor penghambat akses JKN pada PSP adalah rendahnya pengetahuan mengenai prosedur pendaftaran dan portabilitas JKN, kekhawatiran keberlanjutan pembayaran iuran, persepsi buruk mengenai kualitas layanan yang akan diterima jika menggunakan JKN, ketidaklengkapan administrasi kependudukan serta kebijakan yang mengharuskan peserta bukan penerima bantuan iuran (Non-PBI) Mandiri untuk mendaftarkan seluruh anggota keluarga. Akses JKN pada PSP terhambat oleh faktor-faktor individual, layanan dan kebijakan yang perlu diatasi untuk meningkatkan cakupan JKN pada PSP. Female sex workers (FSW) is marginalized social group having a high vulnerability of health problems. Effort to expand national health insurance on FSW is still limited, so it is necessarily performed in order to support the achievement of universal health coverage. This study aimed to obtain the depiction of the insurance access among FSW in Denpasar. This study was qualitative. Data was collected through in-depth interview of 15 FSW and four pimps in Denpasar from August to October 2014. The interview result was analyzed using thematic analysis. The analysis framework used was The Health Access Livelihood Framework. The insurance ownership among FSW in Denpasar was low, even though some FSW were willing to be participants and afford to pay the premium. Factors inhibiting the insurance access were the lack of knowledge regarding registration procedures and portability, fear of premium payment sustainability, negative perceptions of quality of services that would be received if using the insurance, incomplete population administration and policy requiring independent non-premium support receiver participants to register all of their family members. The insurance access among FSW was hindered by individual, service and policy factors that need to be conquered to increase the insurance coverage among FSW.
