Abstract

ABSTRAKRumah sakit Pelayanan Obstetri Neonatal Emergency Komprehensif(PONEK) 24 Jam merupakan bagian dari sistem rujukan pelayanan kedaruratan maternal dan neonatal yang sangat berperan dalam menurunkan angka kematian ibu dan bayi baru lahir. Rumah Sakit Umum Swasta di Kota Denpasar telah terlatih PONEK sejak tahun 2015, namun belum pernah dilakukan evaluasi sehingga belum diketahui ketersediaan input untuk penyelenggaraan PONEK di RSU Swasta di Kota Denpasar. Penelitian ini bertujuan untuk mengetahui ketersediaan input PONEK di RSU Swasta di Kota Denpasar. Penelitan ini merupakan studi evaluatif terhadap Rumah Sakit Swasta terlatih PONEK di Kota Denpasar. Pengumpulan data dilakukan dengan metode observasi, studi dokumentasi dan wawancara mendalam di RSU Puri Raharja dan RSU Prima Medika. Teknik pengambilan sampel pada penelitian ini dengan menggunakan metode purposive sampling.Observasi dilakukan di area cuci tangan dan pemrosesan alat di unit maternal dan neonatal, area resusitasi dan stabilisasi di ruang obstetri dan neonatus di UGD, ruang bersalin, kamar operasi, unit perawatan intensif, area laktasi dan area pencucian inkubator. Wawancara mendalam dilakukan kepada 4 orang informan kedua rumah sakit yang terdiri dari 1 orang kepala bidang pelayanan medik dan 1 orang ketua tim PONEK. Hasil penelitian menunjukkan ketersediaan SDM di RSU Swasta di Kota Denpasar masih kurang. Di rumah sakit baru tersedia Tim PONEK sesuai standar Tim PONEK esensial. Dana penyelenggaraan PONEK telah tersedia yang digunakan untuk penyediaan sarana, prasarana dan peningkatan kapasitas SDM. Untuk ketersediaan sarana dan prasarana masih kurang. Terdapat 2 kendala penyediaan sarana dan prasarana yakni terkait keterbatasan tempat dan dana. Pedoman penyelenggaraan PONEK 24 jam di rumah sakit dan SPO terkait pelayanan kegawatdaruratan maternal dan neonatal terdapat di rumah sakit dan telah disosialisasikan. Kebijakan pendukung pelayanan PONEK di rumah sakit telah ada dalam bentuk SK Direktur RS tentang Pembentukan Tim PONEK RS dan pemberlakukan Pedoman Pelayanan PONEK di Rumah Sakit. Dapat disimpulkan bahwa ketersediaan input dalam penyelenggaraan PONEK di RSU Swasta di Kota Denpasar belum optimal. Diperlukan adanya peningkatanSDM, sarana dan prasarana serta peninjauan kembali standar prosesdur operasional.Kata kunci: Ketersediaan Input, Penyelenggaraan PONEK, RSU Swasta

Abstract

ABSTRAK Layanan rujukan gawat darurat Ambulans Desa merupakan salah satu layanan yang cukup banyak dimanfaatkan oleh masyarakat di wilayah kerja Puskesmas Mengwi 1 Kabupaten Badung. Namun, hingga saat ini belum pernah dilakukan evaluasi terhadap kepuasan pasien layanan rujukan gawat darurat Ambulans Desa. Penelitian ini bertujuan untuk menilai kepuasan pasien terhadap rujukan gawat darurat Penelitian ini merupakan penelitian cross-sectional deskriptif kuantitatif dengan pengumpulan datanya menggunakan survei kuesioner yang diisi melalui wawancara responden. Sampel dipilih secara consecutive sampling dengan jumlah sampel 75 orang dari 6 desa di wilayah kerja Puskesmas Mengwi 1 Kabupaten Badung. Analisis data yang dilakukan adalah analisis univariat. Penghitungan indeks kepuasan yang dimodifikasi dari Peraturan Menteri Pendayagunaan Aparatur Negara Reformasi Birokrasi nomor 14 tahun 2017. Hasil penelitian ini menunjukkan bahwa secara keseluruhan layanan rujukan gawat darurat Ambulans Desa sudah memuaskan dengan memperoleh nilai Indeks Kepuasan Masyarakat mencapai 3,64 (konversi 91). Kepuasan dari dimensi bukti fisik 3,58 (konversi 89,50), dimensi kehandalan 3,65 (konversi 91,25), dimensi daya tanggap 3,70 (konversi 92,50), dimensi jaminan 3,67 (konversi 91,75), dan dimensi empati 3,64 (konversi 91). Secara umum layanan rujukan gawat Ambulans Desa dinilai sudah memuaskan. Namun, tetap perlu dilakukan monitoring evaluasi kualitas layanan secara berkesinambungan dan peningkatan pada komponen-komponen yang masih dinilai kurang memuaskan.. Kata kunci: Kepuasan pasien, rujukan gawat darurat, ambulans desa ABSTRACT The Village Ambulance emergency referral service is one of the services that is quite widely utilized by the community in the working area of the Mengwi 1 Badung public health center. There has never been an evaluation of the patient satisfaction of the Village Ambulance emergency referral service. This study aims to assess patient satisfaction emergency referral services. This research is a quantitative descriptive cross-sectional study with data collection using a questionnaire survey filled with interview to respondents. A total of 75 service users was selected with consecutive sampling from 6 villages in the working area of the Mengwi 1 Badung public health center. Data analysis was performed by univariate analysis. The calculation of the satisfaction index refers to and is modified from the Regulation of the Minister of Administrative Reforms Bureaucracy Reform number 14 of 2017. The results of this study indicate that overall the emergency ambulance referral service for the Village has been satisfactory by obtaining a Community Satisfaction Index score 3,64 (convertion 91). Satisfaction from tangible dimension 3,58 (convertion 89.50), reliability dimension 3,65 (convertion 91.25), responsiveness dimension 3,70 (convertion 92.50), assurance dimension 3,67 (convertion 91.75), and empathy dimensions 3,64 (convertion 91). However, monitoring of service quality evaluations still needs to be carried out continuously and improvements to components that are still considered unsatisfactory should be conducted. Abstract

The target to decrease Infant Mortality Rate (IMR) is one of the goals of the Millennium Development Goals (MDG`s) in 2015. Muntigunung`s village has a relatively high infant mortality rate (29.4 / 1,000 live births) compared by the infant mortality rate of Bali Province (8.19/1,000 live births). The area also have geographical conditions and different socio-cultural community with the surrounding area in Bali, so a more specific approach is required in handling the problems in the area. A qualitative study is needed to explore the factors underlying these conditions. The purpose of this study is to examine the behavior of mothers in Cangkeng Hamlet, Muntigunung in prenatal care practice, childbirth and puerperium. This study used the Rapid Assessment Procedure approach. The qualitative method was used in this study by interviewing 24 women participants and 10 man pasticipants. Participants were selected using purposive technique. The information gathered was analyzed using thematic analysis, and used triangulation of resources and method. The results of this study indicate that prenatal care, childbirth process and post-partum care in Cangkeng is still far from clean and safe criteria. Low education levels, lack of knowledge, negative attitudes towards health care, supernatural perception in the healthy concept of illness, difficult access, mother in-laws influence and lack of husband's role to be underlying conditions of infant morbidity and mortality in this region. This study suggests the existence of a partnership between goverment, NGOs or academic institutions in an effort to closer access to the health services such as forming “Posyandu” in this group;  increase knowledge about reproductive health (prenatal care, early sign of pregnancy, clean and save delivery, and infant care)  for  mother and husband.

Abstract

Village AIDS Cadres (KDPA) Program has been conducted in Denpasar district since 2010. Despite the importance of community participation for a successful community based program, no research has been conducted to explore community perception regarding KDPA program.This research is aimed to explore community perceptions regarding acceptability, utilization, and adoption of the KDPA program from community perspective.The design used in this research is descriptive with qualitative data collection method through six focus group discussions in six villages in Denpasar. Data was analysed using thematic analysis.KDPA is well-accepted as it is perceived as an important issue to be tackled in the community. However, many opinions arise concerning who should be given the role "cadre". Lack of program introduction to the community, also high stigma and discrimination resulted in low utilization of the program. Villages with active KDPA have conducted a variety of HIV-related activities rarely carried out by the less active villages. There is Lack of community participation on HIV-related activities. Inequity of access to HIV information exists between adults and teenagers, between men and women, and between the member of traditional and non-traditional hamlets. Program adoption is hampered by the lack of commitment of village authorities and limited resources of KDPA program.The utilization and adoption of KDPA program are sub-optimal. Introduction of KDPA program to the community and provision of program resources should be improved.

Abstract

Sebuah upaya deteksi dini kanker payudara secara bergerak bernama ‘Mangupura Woman Services’ (MAWAS) telah dilaksanakan oleh Dinas Kesehatan Badung, Provinsi Bali. Penelitian tentang tingkat kepuasan masyarakat terhadap layanan MAWAS belum pernah dilaksanakan, untuk itu penelitian ini bertujuan untuk mengukur tingkat kepuasan masyarakat pengguna layanan MAWAS. Penelitian cross-sectional dilakukan di Kabupaten Badung terhadap 90 orang responden yang dipilih secara purposive dengan kriteria wanita usia subur yang pernah mengakses layanan MAWAS. Tingkat kepuasan masyarakat dilihat dari dimensi tangible, reliability, responsiveness, assurance dan empathy. Data dianalisis menggunakan teknik Importance Performance Analysis dan diagram Kartesius. Skor tingkat kepentingan tertinggi adalah dimensi assurance (333,67) dan yang terendah adalah dimensi responsiveness (323). Skor tingkat kinerja tertinggi adalah dimensi empathy (310,25) dan terendah adalah dimensi reliability (293,6). Penilaian tingkat kesesuaian/kepuasan tertinggi adalah dimensi empathy (94,66%) dan terendah reliability (87,90%). Item-item yang menjadi prioritas utama pengembangan pelayanan adalah kenyamanan ruang pelayanan, keterampilan dan kemampuan petugas, pemberian informasi tentang prosedur pelayanan dan hasil pemeriksaan. Hasil penelitian menunjukkan bahwa dimensi tangible dan reliability belum memuaskan sedangkan dimensi responsiveness, assurance dan empathy sudah memuaskan pengguna layanan. Oleh karena itu, pihak pengelola program perlu melakukan pelatihan untuk meningkatkan keterampilan petugas dalam hal pemberian layanan kepada masyarakat. Abstract

ABSTRAK Layanan Kesehatan Perempuan (MAWAS) merupakan layanan deteksi dini kanker payudara di Kabupaten Badung sejak tahun 2015. Pemanfaatan Layanan tersebut oleh karyawati Dinas Kesehatan Kabupaten Badung masih rendah. Tujuan penelitian ini untuk mengetahui hubungan karakteristik sosio demografi, pengetahuan tentang layanan dan persepsi risiko kanker payudara dengan pemanfaatan Layanan Kesehatan Perempuan (MAWAS) oleh karyawati Dinas Kesehatan Kabupaten Badung. Rancangan penelitian ini adalah cross sectional analitik. Responden penelitian ini terdiri dari seluruh karyawati Dinas Kesehatan Kabupaten Badung yang berjumlah 111 orang yang dipilih melalui total sampling. Metode pengumpulan data dengan menggunakan survey kuesioner yang diisi sendiri oleh responden. Analisis data dilakukan dengan menggunakan uji chi square. Hasil penelitian menunjukkan sebanyak 58,56% responden pernah memanfaatkan layanan MAWAS. Sebagian besar responden memiliki pengetahuan kurang tentang layanan (54,05%) dan sebagian besar responden memiliki persepsi kurang berisiko terhadap kanker payudara (71,17%). Terdapat hubungan antara karakteristik umur (p = 0,034), status kepegawaian (p = 0,005) dan pengetahuan tentang layanan (p < 0,001) dengan pemanfaatan layanan. Peningkatan sosialisasi tentang Layanan Kesehatan Perempuan (MAWAS) berupa media cetak seperti brosur perlu dilakukan kepada karyawati untuk meningkatkan pemanfaatan layanan.

Abstract

One of the reasons for establishing Public Service Agency (BLU) policy at Community Health Care Centers is the number of complaints for poor quality services. This policy was implemented in January 2010 by the Gianyar Government in every Community Health Care Center in Gianyar.  This is the realization of the Gianyar Government's attention in improving the quality of the Community Health Care Center Services.  This study aims to determine the public perception of the quality of services provided by the Community Health Care Center in Gianyar District. This study was a cross-sectional analytic study and conducted at four Community Health Care Centers with BLUD’s status in Gianyar District with 105 respondents selected by multistage random sampling method. The data collection included survey questionnaires and analyzed descriptively by chi square test. The results showed that overall the respondents had a "good" perception towards the elements of Community Health Care Center services with BLU status in Gianyar District. However, there are three elements that were "perceived as poor" comprising responsiveness (14.3%), accuracy (11.4%) and environmental comfort (12.4%). The results of the chi square test showed that there were significant differences between the age factors with the respondent perceptions of service quality in Community Health Care Center with BLU status (p = 0.004), but gender, education level and occupation did not have a meaningful relationship. This research suggests that the Health Office should increase the intensity of coaching and facilitation of procedures related to the Community Health Care Center so that the expected performance of attendant care becomes more patterned.

Abstract

Pengabdian masyarakat berupa pelatihan ini bertujuan untuk meningkatkan pengetahuan danketerampilan staf Dinkes dan Puskesmas di Gianyar dalam merencanakan kebutuhan SDM denganmetode WISN. Pelatihan WISN diselenggarakan pada tanggal 12 Juni 2015 di Gedung PSKM FKUnud. Sebelum pelatihan dimulai, dilakukan instalasi software WISN pada seluruh laptop peserta.Kegiatan pelatihan terdiri dari pemberian materi, diskusi, serta praktik penggunaan software WISN.Untuk evaluasi, seluruh peserta diminta untuk mengisi kuesioner pre dan post test. Nilai pre dan posttest dianalisis secara statistik dengan uji t dua sampel berpasangan untuk melihat ada tidaknyaperbedaan pengetahuan. Pelatihan dihadiri oleh 25 orang peserta (89,3%) dari Dinkes Gianyar dan13 Puskesmas di Kabupaten Gianyar. Saat pelatihan, seluruh peserta telah mendapatkan danmengetahui cara instalasi software WISN. Terdapat peningkatan pengetahuan peserta pelatihansebelum dan sesudah mengikuti pelatihan WISN sebesar 5,08 dengan nilai p <0.001. Seluruh pesertadapat melakukan analisis dengan software WISN dengan panduan tim pelaksana dan menyatakanakan menerapkan metode WISN di institusi mereka. Pelatihan telah berjalan dengan baik danberhasil meningkatkan pengetahuan dan keterampilan petugas Dinkes dan Puskesmas di Gianyardalam menggunakan software WISN. Bimbingan teknis dan follow up perlu dilakukan untukmendorong implementasi metode WISN untuk perencanaan kebutuhan SDM di Gianyar.

Abstract

Bali as the world tourism destination will face huge impacts (economic, health and social) if there was avian influence outbreak, because tourism is a sensitive sector to the public health problems. It is therefore, an action that could increase the knowledge of community in order to prevent from the disease especially avian influences is badly in need. Taro village, Tegallalang is one of tourism destinations in Bali. In order to maintain other countries feel comfortable and secure for their citizens to visit Bali, Taro’s community in preventing diseases especially at avian influences is very crucial. Cooperation with other sectors such as education, health and others to conduct such health education is important to be able to produce good quality of program. The health education was conducted for students (SMPKerta Wisata) on 18 September 2010. The responds toward the health education program was very good, while many students ask questions about avian influences, they also could answer the entire questions already asked to the students after the programs. The questions were not asked only from the students but also from the teachers. It showed that the program was interesting for the audience. This program would improve the knowledge of community and in the future, it could be conducted in other cities in Bali.

Abstract

ABSTRACT The increasing numbers of smokers among adolescent is very worrying. According to the obtained data around 13,2 % from adolescent aged 15-19 years already smoke in this time, if it is not prevented, the number will increase and become a habit, especially for students of SMU. Smoking behaviour of adolescent may result 25 disease types, among otehrs lung cancer, hypertension and heart. It is required, however that campaign of the smoke danger for health, started since early age child, such as Elementary school, Yunior and Senior High Schools. This can be done through the through introducing free smoking area so that students, teachers, the employees are prohibited to smoke no smoking. This activity aim to see the image of knowledge and attitude of SMU concerning with Cigarette. Counseling and adjacent in installation of poster and sticker anti cigarette was the forms of the activty and the results are the knowledge of student of SMU concerning smoking danger still very unadequate after being given counseling, only the knolwledge equal to 6,7% and change of attitude equal to 4% to be able to desist to smoke from active smokers amount 1.3% in SMU N 8 Denpasar.
