Abstract

Children are one of the most vulnerable populations to the spread of tuberculosis (TB) ger, however, unfortunately the success of TB treatment in DKI Jakarta is still quite low. This study aimed to determine the factors that most influence TB child treatment adherence. The method used was descriptive analysis with cross sectional approach. The sample was determined using techniques Consecutive Sampling: Non Probability Sampling which numbered 168 people. This study illustrated that belief variable was the most influencing factors of TB treatment adherence in children (p value 0,001; α=0.05; OR 8,02 after controlled by long-term treatment of TB, PMO, cognitive developments stages, coping behaviour, memory, treatment regimens, communication with health care providers, social support and access to health facilities. This study expected to be source of knowledge for nurses to provide client’s belief and improve adherence to TB treatment in children.

Abstract

It is a challenge for mothers to meet the need of children on nutrition. Failure in meeting this need causes malnutrition in children. This research is aimed to explore a mother’s experience in caring for children with severe malnutrition. A qualitative design was applied using a descriptive phenomenology approach. The total of participants involved were eight persons selected purposively. Data were collected through in-depth interview. This research found that mothers did not realize that their children experience malnutrition, causing malnutrition condition is not their main priority. Aside from that, mothers got bad treatment from the health workers during the consultation. Mothers need to be supported and empowered, handling and preventing their children from malnutrition. Health workers need to change their attitude towards patients and develop their communication and counseling skills.Pengalaman Ibu Merawat Anak dengan Gizi BurukAbstrakMerupakan tantangan bagi para ibu untuk memenuhi kebutuhan anak-anak akan nutrisi. Kegagalan dalam memenuhi kebutuhan ini menyebabkan kekurangan gizi pada anak-anak. Penelitian ini bertujuan untuk mengeksplorasi pengalaman seorang ibu dalam merawat anak-anak dengan gizi buruk. Desain kualitatif diterapkan menggunakan pendekatan fenomenologi deskriptif. Total peserta yang terlibat adalah delapan orang yang dipilih secara purposive. Data dikumpulkan melalui wawancara mendalam. Penelitian ini menemukan bahwa para ibu tidak menyadari bahwa anak-anak mereka mengalami kekurangan gizi, menyebabkan kondisi gizi buruk bukan prioritas utama mereka. Selain itu, para ibu mendapat perlakuan buruk dari petugas kesehatan selama konsultasi. Para ibu perlu didukung dan diberdayakan, menangani dan mencegah anak-anak mereka kekurangan gizi. Petugas kesehatan perlu mengubah sikap mereka terhadap pasien dan mengembangkan keterampilan komunikasi dan konseling mereka.

Abstract

Complexity of intensive care of premature babies causes some routine nursing cares are overlooked which may lead to extension of length of stay, risk of rehospitalization, additional disease’ complications, and decrease of parents’ satisfaction. This cross-sectional study aimed to identify factors related to the implementation of preterm infant care. the consecutive sampling technique was used to select 59 nurses caring for premature babies in the Perinatology Room at the Jakarta Hospital. The analysis was carried out univariately, and bivariately. The statistical test used is the Pearson test and linear regression. The results of the analysis found that there is a significant relationship between the source of nurses, communication, age, length of service of nurses, and training with the implementation of preterm baby care with p Value 0.014, 0.004, <0.001, <0.001, <0.001. Nursing care should be evaluated to improve the performance of nurses and the quality of care of premature babies

Abstract

Background: Acute pain during suction in pediatric patients in the intensive care unit is unavoidable. However, it is necessary to provide non-pharmacological interventions that nurses can guide. One of the interventions that can applied in that situation is maternal voice. That intervention giving the opportunity to the children hearing the voice of their mother via voice recorder.Objective: This study was conducted to determine the effect of maternal voice on acute pain experienced by critically ill children during suctioning procedure.Methods: The research method used was quasi experiment pretest posttest without control group design. The study was conducted in May-June 2022 in the PICU in one of the hospitals in Jakarta. The research sample was selected by purposive sampling, 21 respondents assessed at the time of pretest and post-test (5 minutes before suctioning procedure, during procedure, and 5 minutes after procedure). The pain of the critically ill children was observed by FLACC (Face, Leg, Activity, Cry, Consolability) Scale.Results: The Saphiro Wilk test of normality data was carried out for all the data, either pretest or post-test data. Bivariate tests were analysed using Wilcoxon Signed Rank Test (for the abnormal data) and Paired T-Test (for the normal data) with the results of pain scores before and after being given maternal voice (when starting suction, p value = 0.001; after starting suction, p value = 0.001; 5 minutes after suction, p value = 0.002).Conclusions: PICU nurses should practice maternal voice along with suction to reduce the pain sensation received by children.

Abstract

Abstrak Perawat anak memiliki peran penting untuk mencegah terjadinya dampak negatif hospitalisasi melalui aplikasi konsep perawatan atraumatik dalam memberikan asuhan keperawatan pada anak. Tujuan: Tujuan penelitian adalah mengeksplorasi secara mendalam tentang pengalaman yang dialami perawat dalam memberikan perawatan atraumatik pada anak yang mengalami hospitalisasi. Metode: Penelitian ini menggunakan metode kualitatif deskriptif fenomenologi di RSAB Harapan Kita Jakarta pada bulan Mei 2016. Pengambilan data melalui wawancara semi terstruktur terhadap tujuh perawat yang dipilih dengan purposive sampling. Data transkrip dianalisis secara manual dengan metode Yin (2011) dan Graneheim & Lundman (2004). Hasil penelitian: dihasilkan empat tema yaitu ada dualisme peran orang tua dalam perawatan atraumatik, dilema dukungan rumah sakit dalam perawatan atraumatik, intervensi yang digunakan perawat dalam perawatan atraumatik, dan refleksi perawatan atraumatik untuk perawat. Kesimpulan: Gambaran eksplorasi pengalaman perawat dalam memberikan perawatan atraumatik meliputi dualisme peran (positif dan negatif) orang tua, beragam intervensi perawatan atraumatik yang telah diaplikasikan, dilema dukungan rumah sakit dalam perawatan atraumatik berhubungan dengan infeksi nosokomial maupun hambatan internal, serta perawat melakukan refleksi perawatan atraumatik yang telah dilakukan.Kata Kunci: anak, pengalaman, perawat, perawatan atraumatikThe Experiences of Nurses in Providing Atraumatic Care for Children during Hospitalization AbstractA nurse has an important role to prevent the negative impact of hospitalization through the application of concepts atraumatic care in providing nursing care in children. Objective: The research aim is to explore the experiences of nurses in providing atraumatic care for children during hospitalization. Method: This research method using qualitative phenomenology descriptive study at RSAB Harapan Kita Jakarta in May 2016. Collecting data through interviews with seven nurses selected with using purposive sampling. Transcript data was analyzed manually with Yin (2011) and Graneheim & Lundman (2004) method. Results: The results of the study describes the four themes, namely a duality of the role of parents in the atraumatic care, support hospitals dilemma in atraumatic care, nursing interventions used in atraumatic care, and the reflection of atraumatic care for nurses. Conclusion: An illustration of nurses’ exploratory experiences in providing atraumatic care includes dualism of the roles (positive and negative) of parents, various atraumatic care interventions that have been applied, hospital support dilemmas in atraumatic care related to nosocomial infections and internal support, and nurses doing reflecting on atraumatic care.Keywords: atraumatic care, experience, nurse, pediatric

Abstract

Background: The practice of infant and young children still does not meet the WHO guidelines, although different types of education have been carried out to increase mothers' knowledge of infant and young children's feeding practices.Objective: To identify the effectiveness of digital education and direct education in increasing the knowledge and practice of infants and children aged 6-23 months.Method: The research design is a systematic review using PRISMA flow. The databases used to search for articles in English published in the period 2013 to 2023 are Pubmed, ScienceDirect, Scopus, and MEDLINE using the search keywords ‘Mother OR maternal’, ‘Children OR infants’, ‘Digital based AND health education method’, and ‘Complementary feeding’. A total of 461 articles were obtained using RCT, quasi-experiment, and combined RCT-cohort research designs. Researchers use the Covidence tool for the data selection process through the extraction. Risk of bias assessment uses the JBI instrument to evaluate articles.Results: Four articles detail the efficacy of digital-based education (utilizing smartphone applications, WeChat, WhatsApp, and digital job aid education), while five articles delve into the effectiveness of direct education approaches (peer counseling, personalized counseling, and home visits). Another article examines the effectiveness of both home visits and interactive voice calls. The research spanned multiple countries, including Indonesia, Bangladesh, India, China, Cambodia, Iran, and Brazil, encompassing a participant pool ranging from 110 to 1885 research subjects. Findings indicate that digital education enhances mothers’ understanding of nutrition, essential knowledge, practices in providing complementary feeding in line with IYCF guidelines, and mother’s confidence in preparing complementary foods. Moreover, direct education increased compliance with suggested feeding practices, encompassing dietary variety, intake of animal protein, and the consumption of iron-rich foods.Conclusion: Nurses can enhance knowledge and practice of complementary feeding by embracing digital education as a potential solution and concurrently promoting cultural sensitivity and tailored education.

Abstract

Pemenuhan kebutuhan dasar yang penting untuk pertumbuhan dan perkembangan anak, salah satunya adalah pemenuhan kebutuhan nutrisi, yang sangat tergantung pada orangtua terutama ibu. Apabila kebutuhan nutrisi anak tidak sesuai dengan gizi seimbang dan kebutuhan anak, maka akan terjadi gizi kurang, sehingga diperlukan kepercayaan diri ibu yang tinggi dalam praktik pemenuhan kebutuhan makan anak. Tujuan penelitian ini untuk mengidentifikasi hubungan kepercayaan diri menjadi ibu dengan status gizi anak usia 0-12 bulan. Metode penelitian yang digunakan deskriptif analisis dengan pendekatan cross sectional. Jumlah sampel 101 ibu dengan anak usia 0-12 bulan yang diambil secara stratified random sampling. Analisis menggunakan Kolmogorov-Smirnov. Terdapat hubungan kepercayaan diri menjadi ibu dengan status gizi anak usia 0-12 bulan (p=0,013). Peningkatan status gizi anak memerlukan kepercayaan diri yang tinggi menjadi seorang ibu.

Abstract

Invasive treatments to the children during the period of hospitalization cause pain reactions. The nurses need to know the factors related to pain reactions. The study aims to identify the factors related with pain reactions related to invasive treatments. The design of this study is observational research with cross sectional approach by using non probability sampling method through consecutive sampling with the amount of 90 children respondents. Multivariate data analysis use multinomial regression statistic test. The result of the analysis indicated that there was a significant relationship between fear and pain reaction (p value = 0,018). The emergence of pain reaction “avoidance” from the children who are in fear is as much 5 times greater than children who are not in fear. Nurses need to conduct nursing interventions that can reduce fear in the children behavior in order to minimize pain reaction in form of avoidance.

Abstract

Background: Discharge planning is an important process to ensure continuity of care after discharge from the hospital. This process helps reduce readmission rates, especially in children with chronic illnesses. Discharge planning can be a strategic approach to preventing readmissions by ensuring a smooth transition of care from the hospital to home. Purpose: To determine the effect of discharge planning in reducing readmissions in children with chronic diseases. Method: This study uses a systematic review approach with reference to the PRISMA guidelines. The articles analyzed came from the PubMed, SAGE, and ScienceDirect databases with a Randomized Controlled Trial (RCT) research design. A total of 4 (four) articles were selected based on inclusion criteria, discharge planning topics, focus on children with chronic diseases, and publications in the last 5 (five) years (2020-2024). Results: Comprehensive discharge planning significantly reduces readmission rates in children with chronic illness. Findings include that structured family education improves parental readiness to care for children and that social factors, such as parental literacy and household resource availability, influence discharge planning success. Case management provides superior outcomes in reducing readmission rates and improving patient quality of life. An as-needed follow-up approach is highly efficacious without compromising intervention effectiveness. Conclusion: Discharge planning is an effective intervention in reducing readmission rates and improving the quality of home care for children with chronic illness. Successful implementation requires integrated family education, social support, and case management. Adapting the intervention to account for social and cultural factors is also important to improve success across contexts.   Keyword: Children With Chronic Illness; Discharge Planning; Readmission.   Pendahuluan: Discharge planning adalah proses penting untuk memastikan kelanjutan perawatan pasien setelah keluar dari rumah sakit. Proses ini membantu mengurangi angka readmisi, terutama pada anak dengan penyakit kronis. Discharge planning dapat menjadi pendekatan strategis untuk mencegah readmisi dengan memastikan transisi perawatan yang lancar dari rumah sakit ke rumah. Tujuan: Untuk mengetahui efektivitas discharge planning dalam mengurangi readmisi pada anak dengan penyakit kronis. Metode: Penelitian systematic review mengacu pada panduan PRISMA. Artikel yang dianalisis berasal dari database PubMed, SAGE, dan ScienceDirect dengan desain penelitian Randomized Controlled Trial (RCT). Sebanyak 4 artikel dipilih berdasarkan kriteria inklusi, topik discharge planning, fokus pada anak dengan penyakit kronis, dan publikasi dalam 5 tahun terakhir (2020-2024). Hasil: Discharge planning yang komprehensif secara signifikan menurunkan angka readmisi pada anak dengan penyakit kronis. Temuan mencakup edukasi keluarga terstruktur meningkatkan kesiapan orang tua dalam merawat anak dan faktor sosial, seperti literasi orang tua dan ketersediaan sumber daya rumah tangga memengaruhi keberhasilan discharge planning. Manajemen kasus memberikan hasil superior dalam menurunkan angka rawat inap ulang dan meningkatkan kualitas hidup pasien. Pendekatan follow-up berbasis kebutuhan (as-needed) memiliki efisiensi tinggi tanpa mengurangi efektivitas intervensi. Simpulan: Discharge planning efektif dalam mengurangi angka readmisi dan meningkatkan kualitas perawatan di rumah bagi anak dengan penyakit kronis. Keberhasilan intervensi ini membutuhkan edukasi keluarga, dukungan sosial, dan manajemen kasus yang terintegrasi. Penyesuaian intervensi dengan mempertimbangkan faktor sosial dan kultural juga penting untuk meningkatkan keberhasilan di berbagai konteks.   Kata Kunci: Anak-Anak dengan Penyakit Kronis; Readmisi; Rencana Pemulangan Pasien.
