Abstract

Koperasi Serba Usaha (KSU) Mitra is a cooperative that located at Genteng, Banyuwangi. KSU Mitra have a loan services products to their member. KSU Mitra using the principles of 5C assessment to assess worthiness of the loan application. The existing conditions, the principle of 5C assessment not fully performed. This led to the credit risk on loans granted. This occurs because the assessment process is not recorded. And the recording of loan payments still recorded in simple manner. To resolve the issue then assessment worthiness of loan giving and loan payments recording is made. Applications are made to assess the worthiness of loan giving based on the principles of 5C and can handle the recording of loan payments. Based on trial results that have been done, can be concluded that this application complies with the expected goals. This application has been able to assess the worthiness of loan giving based on the principles of 5C assessment and recorded the loan payments on KSU Mitra.

Abstract

Clinic Paradise Surabaya is a medical services have full facilities and experts in the form of general practitioner, beauty, dentist, pharmacy, laboratory, and beauty care services. Obstacles faced Clinic Paradise Surabaya is in the administration of patient services ranging from patient registration, queues, record-keeping, drug and redemption payments are still written or recorded on a sheet of paper. To overcome the above problems, it is made Design of Information System Administration Patient Services Web Based in Clinic Paradise Surabaya processing registration examination, the queue of patients, recording medical records, prescription drugs redemption, payment and reporting required owner. Web-based applications that can run on any operating system and can be accessed through many media such as computers, tablets, and smartphones. Conclusion of Information System Administration Patient Services Web Based is able to help the registration process checks for patients who are already registered, new patient registration, automatic numbering for each poly queues, record-keeping, drug redemption, and payment. Generate reports of patient visit, new patient registration statements, reports of medical records of patients, most diseases reports, income statements, and reports of drug expenditures.Keywords : Information System, Administration Patient Service, Medical Record, Clinic.

Abstract

PMI Surabaya dengan jumlah 192 karyawan yang dibagi tiap levelnya yaitu 10 orang level manajer dan 182 orang level operator. Bagian seksi kepegawaian kesulitan dalam melakukan penilaian kinerja karyawan, seperti kesulitan perhitungan nilai dari data yang didapatkan karena masih menggunakan perhitungan manual, keterlambatan dalam pembuatan laporan penilaian kinerja karyawan yang sering menghambat dalam pemberian reward/punishment, belum adanya rekap data dari keseluruhan penilaian kinerja setiap karyawan/setiap bulan/setiap tahun, dan karyawan belum mengetahui detail dari hasil penilaian kinerja, sehingga beberapa karyawan merasa tidak puas dengan hasil akhir penilaian.Berdasarkan uraian permasalahan di atas, dibangunlah aplikasi penilaian kinerja karyawan berbasis web, yang dapat membantu kepala divisi untuk memudahkan dalam menginputkan nilai dari kinerja karyawannya berdasarkan dokumen DP3. Aplikasi ini dapat membantu bagian seksi kepegawaian dalam menghitung nilai karyawan yang sudah diinputkan oleh kepala divisi dan membuat laporan, serta membantu keseluruhan karyawan untuk menampilkan informasi hasil perhitungan nilai secara detail.Dengan adanya aplikasi penilaian kinerja karyawan berbasis web dapat membantu dalam proses perhitungan penilaian kinerja. Adanya pembuatan laporan penilaian kinerja, mempermudah karyawan melihat secara detail rekap penilaiannya.

Abstract

The company's market Area (PDPS) is a company engaged in community service in place of fulfillment order, the means, and the infrastructure of traditional market in the city of Surabaya and the duties and responsibilities of a sub section of the administrative regulations based on SOP/TTU/01. Based on the four functions of SOP/TTU/01, in it there are processes that are currently experiencing difficulty in running it. This is due to the absence of the system (applications) that can help the performance of administrative sub division officer. The difficulty lies in the process of storage (tape/record keeping and archiving), search, retraction, distribution and provision of the code number. Abstract

Abstract: STIKOM surabaya are the institutions that moves that puts forward in the fields of information and technology education. This is proven by the facilities and infrastructure that education be able to support the entire process. One of them is computer laboratories (labkom) that are located on the floor blue of stikom six buildings of surabaya. Labkom ever happened incident power lines fell, the incident had an impact on server down, and all the activity of going on labkom come to rest and wait for the recovery of server. The handling of the incident is now only management knowledge based server.  Server added management if there is an incident recording and the provisin of this was not conducted the status of resolution, so if the incident happen then have to wait staff who know the improvement of the incident. The incident happened chronicled in detail should be complete by the time the events of server management. Abstract

One academic activities carried out by the Institute of Business and Information Stikom Surabaya is a lectures scheduling activities. Facts that occurred in the preparation of the current class schedule and the process requires considerable time. This occurs because the need to classify and sort the data written using paper media and draw up a schedule with the help of Microsoft Excel. The difficulties encountered when preparing the class schedule is the schedule should be adjusted to the terms and policies that have been determined by matching them one by one. Based on these problems, made a lecture scheduling application using Genetic Algorithms. Lecture scheduling application with regard to the data compiled estimates of the number of participants of each course, the data subject to be held, the data capacity of each course, data on the number of lecturers, the data competence of lecturers, lecturers teaching load data, and data classrooms. The results showed that the application of this lectures scheduling has can produce in one semester class schedules in accordance with the existing terms and policies. Results of testing for lectures scheduling as many as 75 classes obtained the best parameter to the value of chromosome 30, the amount of generation of 30, the probability of crossover and mutation probabilities 70 30. By using the best parameters of the test results produced by the class schedule with an average success rate of 86% to 30 trials.
