Abstract

Abstract

Introduction: Chronic kidney disease (CKD) markedly reduces quality of life and increases mortality risk, predominantly due to cardiovascular complications. Indoxyl sulfate, a protein-bound uremic toxin, is increasingly recognized for its role in accelerating cardiovascular disease in CKD patients. This study aims to evaluate the association between indoxyl sulfate levels and major cardiovascular events (MCE) in patients with stage 5 CKD undergoing hemodialysis. Methods: This observational case-control study included 50 patients with stage 5 CKD on hemodialysis. Inclusion criteria were informed consent, willingness to undergo laboratory assessments, and a confirmed CKD diagnosis. Patients with a history of acute myocardial infarction, percutaneous coronary intervention, coronary artery bypass grafting, cardiac arrest, arrhythmias, or incomplete laboratory data were excluded. Serum indoxyl sulfate, creatinine, urea, and estimated glomerular filtration rate (eGFR) were measured. Statistical analyses, including t-tests and logistic regression, were used to compare clinical parameters between patients with and without MCE. Results: Of the 50 patients (mean age: 55.4 ± 8.2 years for MCE group, 47.8 ± 7.9 years for non-MCE group; p=0.015), 29 experienced MCE, and 21 did not. Hemodialysis duration averaged 15.9 ± 4.3 years (MCE) versus 25.9 ± 5.1 years (non-MCE; p=0.005). Significant differences were found in creatinine (p=0.014), creatinine-urea ratio (p=0.007), and eGFR (p<0.001). Elevated indoxyl sulfate levels were strongly associated with MCE (p=0.001). Conclusion: Higher indoxyl sulfate levels are significantly correlated with major cardiovascular events in hemodialysis-dependent CKD patients, underscoring its potential as a predictive biomarker for cardiovascular risk.

Abstract

This study is about City Branding. This research is motivated by the potential of City Branding creation as a way to improve regional competitiveness. The purpose of this study is to describe the various efforts made by the local government of DIY to improve the competitiveness of the region with City Branding. This research uses the qualitative method. Data collection method is done by triangulation technique of data source. While data processing researcher does data reduction. The results obtained in this study are First, City Branding DIY has been running well. Rebranding DIY is likely to increase the competitiveness of regions, including local revenue. This is evidenced by the increase in investment achieved by the Government of Yogyakarta. It’s just that City Branding is still focused on tourism, but it can be used for other sectors. Second, the DIY rebranding (2014) provides many improvements, especially in terms of concept, logo, and tagline. In addition, the Yogyakarta provincial government has established city branding councils, city branding strategy roadmap and governor regulation of city branding in DIY. It’s just that there has been no evaluation since the rebranding was done by the provincial government of DIY, how this rebranding can be implemented by the local government and city branding council and there is no extent to which the strategy in the roadmap implemented.

Abstract

Ease of investing supports the competitiveness of the region. Currently the licensing process in Boyolali Distric has a number of problems, among others, not all head of regions/head of Departments agree to delegate their authority to the head of Department of the One Stop Integrated Service (DPM-PTSP), insufficient human resources, and conflict between the licensing agency in the region with the technical unit (formed by the decision of the District Head that consists of elements of the related Regional Work Unit in charge of carrying out the field examination, technical discussion and provide recommendations/considerations regarding a license). The purpose of this study is to describe the efforts of the regional government in solving problems in the delegation of authority to the DPM-PTSP, overcoming the limitation of human resources and the conflict between the technical unit of the licensing services, transparency, accountability and certainty for the applicant. This study is using the qualitative case study approach. Data collection methods were conducted through in-depth interviews, secondary data and field observations. For the validity of the data, the researchers perform data triangulation. The sources of this study are the Head of Boyolali District, the Head of DPMPTSP and its officers. For data processing, the researchers perform open coding, axial coding and selective coding. The findings of this research are that some of the problem are solved by issuing the Boyolali Head of District Regulation Number 10 of 2013 Regarding Implementation Guideline of the Head of District Regulation Number 4 of 2012 (on the Integrated Licensing and Non-Licensing of Boyolali District) to delegate the District Head authority and synchronize the OPD. To overcome the limitations of their human resources, an online licensing application was created to provide an effective and efficient licensing process in Boyolali District.

Abstract

This study aims to analyze the design of extreme poverty reduction policies in West Lombok District. The design of the policy refers to the substance/content of the policies developed for the reduction/prevention of extreme poverty used by the regions. A case study research approach is used to explore the design of extreme poverty alleviation policies. The West Lombok Regency government's policy on poverty alleviation contains various local government programs synergized in accelerating the acceleration of extreme poverty reduction. In addition, it is in the form of efforts to assist the poor for a movement that is actualized in bridging aid recipients to get out of poverty immediately. The construction of extreme poverty reduction policies in West Lombok Regency adopts the concept of an innovation hub to accelerate cross-government programs that target basic services (health and education) in interventions for individual/family change as well as increasing the business capacity of community groups.

Abstract

This article aims to develop video learning media specifically crafted to assist educators in presenting learning materials more tangibly and understandably, particularly within early childhood education. Guided by the Research and Development (R&D) process, the study focuses on two primary objectives: (1) elucidating the intricacies of developing STEAM-based video learning media for Early Childhood Education, incorporating Religious and Moral Values, and (2) evaluating the appropriateness and effectiveness of such media in the educational context. Adopting the ADDIE development model, the research encompasses five pivotal stages: Analysis, Design, Development, Implementation, and Evaluation. The validation process for the developed product entails collaboration with media, material, and design experts, complemented by feedback from colleagues. One-to-one tests, Small Group Tests, and Field Test methods are employed to assess the effectiveness of the media. Validation results from these experts affirm that the developed media aligns seamlessly with the criteria for successful application in educational settings. During the product implementation phase involving 15 students, it became evident that this development was an effective learning tool. The post-test scores surpassing the pre-test scores indicate the product's efficacy in enhancing the understanding of morning phenomena, instilling religious and moral values, and fostering STEAM creativity in early childhood education.

Abstract

Derivational morphology plays a crucial role in expanding English vocabulary and contributes significantly to lexical development in authentic written discourse. However, studies examining derivational morphemes in business news articles and their implications for English as a Foreign Language (EFL) vocabulary learning remain limited. This study aims to identify the types of derivational morphemes that occur in BBC Business News articles, analyse their morphological functions, and explore their implications for EFL vocabulary learning. Employing a descriptive qualitative approach, the study analyzed five BBC Business News articles selected through purposive sampling. The data consisted of lexical items containing derivational morphemes, which were identified, classified, and analyzed based on Carstairs-McCarthy's (2002) theory of English morphology and Bauer's (2003) concept of morphological productivity. The findings reveal that suffixation is considerably more productive than prefixation, with noun-forming suffixes such as -tion, -ation, -ment, and -ity, as well as adjective-forming suffixes including -al, -ic, and -ial, occurring most frequently. Furthermore, class-changing derivation predominates over class-maintaining derivation, indicating that business journalism relies heavily on lexical expansion through nominalization and adjective formation to communicate complex economic concepts effectively. These findings highlight the pedagogical value of authentic BBC Business News articles in fostering EFL learners' morphological awareness, vocabulary development, and reading comprehension. The study contributes to English Applied Linguistics by providing empirical evidence of the productive use of derivational morphology in contemporary business news discourse and supporting the integration of authentic news texts into morphology-based vocabulary instruction.
