Abstract

Background: Women diagnosed with breast cancer often experience emotional distress, leading to a decrease in psychological well-being. Online self-management training during the COVID-19 pandemic is crucial for maintaining psychological well-being, as it is closely related to stress hormone cortisol levels, which play a vital role in boosting the body’s immune system. However, research on self-management strategies for breast cancer patients, particularly regarding psychological aspect and its relationship to cortisol levels during the COVID-19 pandemic, remains limited.Purpose: The purpose of this study was to measure the effectiveness of self-management training on psychological well-being and salivary cortisol levels in breast cancer patients.Methods: This study used a pre-post-test quasi-experimental design with a control group. The participants included 70 breast cancer patients undergoing chemotherapy at a hospital in Malang City, East Java, Indonesia, recruited using simple random sampling. Self-management training using the EDUDARA (Edukasi Kanker Payudara) was administered to the participants in the intervention group for six weeks. Data on wellbeing were collected using the Ryff Psychological Wellbeing Questionnaire, while salivary cortisol levels were measured with the DBD cortisol kit using the competitive ELISA method. SPSS with t-tests was used to analyze the data.Results: The average post-test psychological well-being score in the intervention group was 90.3, while in the control group, it was 82.69. The results of statistical tests showed a significant difference in psychological well-being between the two groups, with the intervention group showing greater improvement after the training. Additionally, the mean salivary cortisol level post-training was 4.531 in the intervention group and 6.169 in the control group (p=0.001). This indicates a greater reduction in cortisol levels in the intervention group after training, showing that participants’ psychological condition following the “EDUDARA” training was better than that of the control group.Conclusion: Self-management training had a positive effect on increasing psychological well-being and decreasing salivary cortisol levels among breast cancer patients. Therefore, structured self-management training for these patients can be used for nursing intervention

Abstract

Sampah menjadi salah satu sumber permasalahan yang memiliki dampak yang signifikan pada lingkungan sehingga diperlukan upaya pengelolaan yang baik. Bank sampah Desa Nglambangan mampu meningkatkan kesejahteraan hidup masyarakat sejak didirikan. Kondisi pandemi covid-19 menyebabkan kegiatan bank sampah berhenti beroperasi dan hal ini berkelanjutan hingga saat ini pasca covid-19 karena kegiatan ini tidak dijadikan prioritas pemerintah desa. Desa Nglambangan merupakan salah satu desa yang berada di Kabupaten Madiun yang memiliki tingkat penderita penyakit kronik cukup tinggi dibandingkan dengan desa lain. Selama ini, penderita penyakit kronik seperti diabetes melitus menggunakan asuransi BPJS untuk mendapatkan layanan kesehatan yang dibutuhkan. Asuransi bank sampah merupakan salah satu pilihan yang dapat dikembangkan dalam upaya mendukung program penanganan penyakit kronik. Edukasi bank sampah meliputi manajemen keuangan bank sampah dan konsep asuransi bank sampah menjadi alternatif atas permasalahan tersebut. Tujuan kegiatan pengabdian masyarakat ini adalah untuk meningkatkan pemahaman dan pengetahuan masyarakat akan pentingnya manajemen dalam bank sampah dan mengenalkan konsep asuransi bank sampah. Hasil dari kegiatan pengabdian masyarakat ini adalah edukasi manajemen bank sampah sebagai sarana asuransi kesehatan di Desa Ngalambangan Kecamatan Wungu Kabupaten Madiun telah berhasil meningkatkan pemahaman dan pengetahuan masyarakat. Hasil pre test dan post test menunjukkan pengetahuan dan pemahaman masyarakat akan pentingnya pengelolaan bank sampah dan asuransi meningkat. Kegiatan edukasi mengenai bank sampah tersebut juga dapat dilaksanakan secara berkelanjutan dengan melibatkan pemangku kepentingan untuk menjamin keberlanjutan bank sampah di masa yang akan datang. Abstract

The meaning of life is considered to affect breast cancer patients positively. Finding meaning in life is key to improving well-being during and after experiencing a traumatic event such as cancer. Previous research was still rare to explore the factors affecting the meaning of life in breast cancer patients. The study aimed to analyze the factors influencing the meaning of life of breast cancer patients.  This study was an analytical observational method with a cross-sectional design. The inclusion criteria of the respondents were breast cancer patients who had obtained at least one cycle of chemotherapy. We used purposive sampling, and the total sample in this study was 135 respondents. Questionnaires were used to collect the data. Data were analysed by multivariate test, namely a linear regression test with a significance level of p ≤ 0,05. The result showed only two variables affect the meaning of life, communication of health workers and spirituality, respectively. The strongest variable was the communication of health workers.  The equation from the analysis showed that 11 % explained the phenomenon. The spirituality and communication of health workers positively affected the meaning of life. Application of therapeutic communication as well as spiritual support are needed in nurses to patients while undergoing therapy. Therefore, nurses must pay attention to spirituality and good therapeutic communication when providing patient services.
