Abstract

Anaemia is a deficiency in red blood cells due to low haemoglobin levels. Red blood cells serve as a means of transporting nutrients and oxygen needed for physiological and biochemical processes in every tissue of the body. The normal haemoglobin level for a pregnant woman is 11 gr%. This scientific paper aims to carry out postpartum maternal care for Mrs. 'N' with moderate anaemia at Sitti Khadijah I Women and Children’s Hospital of Makassar in 2023 using a midwifery management approach within the midwife's scope of practice. Based on the research results, the diagnosis of the patient moderate anaemia was conducted by assessing the patient's history and carrying out the physical examinations and supporting tests (Hb ≤7 gr/dL). The management of moderate anaemia can be done through the administration of additional blood supplements (Iron Tablets), Vitamin C, and educating the patient about iron-rich foods. In conclusion, the appropriate and effective management of moderate anaemia, based on the evidence-based practices, involves the importance of nutritional education for pregnant woman, and the active management implementation of stage III should be conducted properly and correctly.

Abstract

Pendahuluan Keluarga berencana merupakan tindakan membantu individu atau pasangan suami istri untuk menunda kehamilan dan menjarangkangkan kelahiran serta menghindari kelahiran yang tidak diinginkan. Dari hasil pencatatan dipuskesmas Jumpandang Baru Makassar tahun 2021 sejumlah 1875 aksektor, pengguna akseptor implant sebanyak 79 akseptor (4,06%). Pada tahun 2022 sejumlah 1760 akseptor dengan pengguna akseptor implant sebanyak 158 akseptor (8,44%), sedangkan pada tahun 2023 bulan Januari-Mei sejumlah 807 akseptor, penggunaan kontrasepsi implant sebanyak 76 akseptor (10,93%). Implant adalah kontrasepsi yang mengandung Levonogestrel (LNG) yang dibungkus dalam kapsul silastic silicon dan dipasang dibawah kulit. Cara kerja Implant sangat efektif dengan kegagalan 0,2-1 kehamilan per 100 perempuan dengan lama efektifitas 3 tahun. Metode pendekatan Manajemen Asuhan Kebidanan Keluarga Berencana pada Ny “D” Calon Akseptor Baru KB Implant di Puskesmas Jumpandang Baru sesuai dengan 7 langkah varney dan SOAP. Hasil dari studi kasus ditegakkan diagnosa Ny “D” P1A0 dengan calon akseptor baru KB implant indikasi pemesangan implan berdasarkan anamnesis, pemeriksaan fisik dan pemeriksaan penunjang sehingga tidak ditemukan hambatan pada saat pemasangan implan. Pemantauan pasca pemasangan implan dilakukan sebanyak 6 kali selama kurang lebih 2 bulan termasuk melakukan pemeriksaan fisik, TTV dan penimbangan berat badan. Pada tanggal 02 Oktober 2023 sampai 04 Oktober 2023 pasca pemasangan implant, lengan berwarna merah, nyeri dan bengkak. Tanggal 8 Oktober 2023 seminggu pasca pemasangan implant luka insisi mulai sembuh. Tanggal 15 Oktober 2023 dua minggu pasca pemasangan implant merasa nyeri pada lengan yang terpasang implant ketika mengangkat barang yang berat. Pemantauan selanjutnya pasca pemasangan implant tanggal 29 Oktober 2023 sampai tanggal 26 November 2023 ibu mengalami kenaikan berat badan. Kesimpulan dari studi kasus yaitu Ny “D” telah menjadi akseptor KB implant dan mulai ibu tidak mengalami komplikasi setelah pemasangan implant. Ibu mengalami efek samping implant yaitu kenaikan berat badan penyebabnya adalah meningkatnya pola makan.

Abstract

Introduction The high number of maternal deaths during pregnancy or within 42 days after pregnancy termination, regardless of the duration and place of delivery, is caused by pregnancy or its management, leading to maternal mortality (MMR). In 2020, a total of 133 maternal deaths were reported, equivalent to 85.95 per 100,000 live births. These included 29 cases (22%) of maternal deaths during pregnancy, 36 cases (23%) during childbirth, and 77 cases (55%) during the postpartum period (Dinas Kesehatan, 2021). Method This study uses a case study approach, focusing on the Management of Continuous Postpartum Midwifery Care for Mrs. “S” at RSIA Sitti Khadijah 1 on March 31, 2024, in accordance with the midwife’s authority and needs, following the 7-step Varney management method and the SOAP approach. Results The case study involved an assessment through anamnesis and monitoring conducted four times within 42 days, followed by an additional visit after 42 days. During the midwifery care, early mobilization, uterine fundus massage, health education, breast care, breastfeeding techniques, on-demand breastfeeding, newborn care, and family planning counseling were provided. Conclusion The case study on Mrs. “S” with an episiotomy wound showed significant improvements. Postpartum monitoring at RSIA Sitti Khadijah 1 from March 31, 2024, to May 25, 2024, indicated that the postpartum mother had recovered from perineal sutures pain, experienced milk production, maintained normal vital signs, and showed no abnormalities during physical examinations

Abstract

Introduction Perimenopause is a transition period between pre- menopause and post-menopause characterized by natural complaints such as irregular menstruation, hot flushes, and disturbances. Women with irregular menstruation require midwifery care to address the impact of irregular menstrual cycles, such as feelings of anxiety, worry, and varying levels of fear could disturbing activity day living. Therefore, the researcher aims to implement perimenopausal midwifery care management on Mrs “K” at palllangga Health Center, Gowa Regency, following the 7 steps of varney and SOAP. Result of the case study conducted on Mrs. “K” with perimenopause did not encounter any obstacles during giving the treatment on her. Monitoring was conducted six times over a period of approximately body heat during the night, and had difficulty in sleeping. However, after receiving care and understand about perimenopause, she no longer complained and became more accepting of her condition. Conclusion Based on that perimenopausal case study, it is concluded that the 7 steps og varney and SOAP used for the problem- solving process in midwifery care, have successfully provided care, and Mrs. “K” can accept her condition. The role of midwives in this care is essential in providing understanding and information about the preparation and symptoms that will arise during the perimenopausal period.
