Abstract

Depression is psychological distress often occurred on cancer patient. Depression can increase perception about pain, reducing drug efficacy and longer length of stay at hospital. The objective of this study was to describe the condition of cancer patients depression who undergoing treatment at RSUP Dr. Sardjito Yogyakarta and RSUD Prof. Dr. Margono Soekarjo Purwokerto. This was a descriptive study using a cross sectional design and a pilot study. Subject were taken using non probability with purposive sampling technique. Beck Depression Inventory (BDI) was employed for selection purposes. From this study, 25,71% are experienced with mild depression, 45,71% had moderate depression and 28,58% suffered severe depression. We propose that medicine has been rated helpful by cancer patient. Psycotherapy like music therapy is part of Complementery and Alternative Medicine (CAM) can also be considered useful in lowering negative emotions beside of routine medicine like psychopharmaceutical. Doctor and Nurse can safely recommend any of these interventions for depression.ABSTRAK Depresi merupakan masalah psikologi yang sering terjadi pada pasien kanker. Depresi dapat meningkatkan persepsi pasien akan rasa sakit, menurunkan sensitivitas khasiat pengobatan, dan memperpanjang waktu rawat di rumah sakit. Tujuan dari penelitian ini adalah untuk mengetahui gambaran kondisi depresi pada pasien kanker yang dirawat di RSUP Dr. Sardjito, Yogyakarta, dan RSUD Prof. Dr. Margono Soekarjo, Purwokerto. Ini adalah penelitian deskriptif dengan desain cross sectional dan merupakan pilot study . Total jumlah sampel adalah 70 responden. Pengambilan sampel dilakukan dengan cara non probability sampling dengan teknik purposive sampling . Alat ukur yang digunakan adalah Beck Depression Inventory (BDI). Hasil penelitian menunjukkan sebanyak 25,71% pasien kanker mengalami depresi ringan; 45,71% mengalami depresi sedang; dan 28,58% mengalami depresi berat. Saran dalam penelitian ini adalah bahwa diperlukan intervensi yang tepat untuk mengatasi gejala depresi. Selain pemberian psikofarmaka, tenaga kesehatan, khususnya dokter dan perawat, dapat memberikan psikoterapi lain, misalnya terapi musik yang merupakan bagian dari terapi komplementer

Abstract

ABSTRACTPalliative cancer patients undergo severe pain, and pharmacological therapy in some cases cannot fully relieve pain. Self-selected Individual Music Therapy (SeLIMuT) is non-pharmacological relaxation stimulating complementary therapy which is safe, accessible, inexpensive and effective. The study aimed to identify effect of SeLIMuT to pain in palliative cancer patients. The study was a quasi experiment- pre-test and post-test design using comparison group with purposive and consecutive sampling carried out at inpatient ward I of Dr Sardjito Hospital Yogyakarta. Respondentswere divided into intervention group (n=23) with SeLIMuT therapy four times each within 15-20 minutes and control group (n=23) without therapy. Pain was assessed in both groups using Visual Analog Scale (VAS). The result of the study showed that there was significant difference in average pre-post in both groups with score of p=0.001 (p<0.05). Pain decrease occurred in SeLIMuT group after intervention with score of mean 2.144 (0.91). Pain decrease in SeLIMuT group was also clinically significant (mean?1.0). Increase in pain level occurred in the control group with score of mean-0.03 (0.15). SeLIMuT intervention both statistically and clinically affected pain level in palliative cancer patients. SeLIMuT was effective in reducing painABSTRAKPasien kanker paliatif melaporkan nyeri yang lebih berat. Pada beberapa kasus, terapi farmakologi pada tidak sepenuhnya dapat mengurangi nyeri. Self-selected Individual Music Therapy (SeLIMuT) merupakan terapi komplementer perangsang relaksasi nonfarmakologis yang aman, mudah, murah, dan efektif. Penelitian ini bertujuan untuk mengetahui pengaruh SeLIMuT terhadap tingkat nyeri pasien kanker paliatif. Penelitian intervensi Quasi Experiment- pre-test and post-test design with Comparison Group dengan purposive and consecutive sampling ini dilakukan di IRNA I RSUP Dr. Sardjito, Yogyakarta. Responden dibagi dalam kelompok intervensi (n=23) yang menerima terapi SeLIMuT sebanyak empat kali masing-masing selama 15?20 menit dan kelompok kontrol (n=23) yang tidak diberikan terapi. Kedua kelompok dilakukan pengukuran nyeri pre- dan post- dengan Visual Analog Scale (VAS). Hasil penelitian menunjukkan bahwa terdapat perbedaan yang signifikan rerata selisih nyeri pre-post pada kedua kelompok dengan nilai p=0,001 (p<0,05). Penurunan nyeri terjadi pada kelompok SeLIMuT setelah mendapatkan intervensi dengan nilai mean (SD) 2,144 (0,91). Penurunan nyeri pada kelompok SeLIMuT juga bermakna secara klinis (mean ? 1,0). Peningkatan skor nyeri terdapat pada kelompok kontrol dengan nilai mean (SD) -0,03 (0,15). Dapat disimpulkan bahwa secara statistik dan klinis, intervensi SeLIMuT berpengaruh terhadap tingkat nyeri pasien kanker paliatif.Pengaruh tersebut berupa efektivitas SeLIMuT dalam menurunkan nyeri.

Abstract

Background: Patient satisfaction in palliative care is an important dimension that can describe a patient’s experience acceptable of care. Patient satisfaction is used to evaluate performance and improve the quality of palliative care. Objective: This study aimed to know the description of patient satisfaction of palliative cancer against palliative care which has been given in IRNA I RSUP Dr.Sardjito Yogyakarta based on a ranking that includes 5 subscales of satisfaction in palliative care. Method: Type of research was mixed methods with sequential explanatory design, collecting data began from collecting quantitative data and followed by collecting qualitative data. Subject of study involved cancer patient in Central Hospital of Dr. Sardjito Yogyakarta during September and October 2016 number of respondent was 48 patients and 8 participants. Sampling technique in quantitative research used purposive sampling, while the qualitative research used the techniques sampling criterion. Instrument used FAMCARE-P to measure satisfaction and the interview based on interview guideline. Quantitative data analysis used descriptive analysis and analysis of qualitative data used the content analysis. Integration of the results of quantitative research and qualitative used the joint displays. Results: Cancer patients have a high satisfaction on the subscale of support in decision-making with a mean (4,60±0,49), followed by a subscale of symptom management (3,96±0,42), communication and information (3,93±0,35), coordination and consistency (3,8±0,38) and accessibility (3,7±0,44). Qualitative research gained eight categories. Abstract

Covid-19 menyebabkan kematian yang tinggi terutama pada pasien-pasien dengan komorbid. Diabetes mellitus (DM) merupakan salah satu komorbid yang menjadi faktor resiko kematian pada pasien Covid-19. Scoping review ini bertujuan untuk mengetahui apakah sampai dengan saat ini DM menjadi faktor resiko kematian pada pasien Covid-19 dewasa. Pencarian literatur dilakukan di database Pubmed dan Science Direct dengan menggunakan kata kunci “Covid-19 AND risk factors diabetes mellitus AND mortality AND adult”. Sebanyak 2.233 artikel didapatkan dari dua database tersebut dan 1 artikel hand searching yang berupa artikel review dan artikel riset original yang kemudian diskrining duplikasi, tahun terbit Desember 2019-Maret 2022, open access/free fulltext, dan berbahasa Inggris, serta subyek merupakan pasien dewasa (>18 tahun) sehingga didapatkan 20 artikel yang masuk dalam review ini. Enam belas artikel menunjukkan bahwa DM adalah factor resiko kematian pada pasien Covid-19 dewasa. Empat artikel menunjukkan bahwa secara statistik, DM bukan merupakan faktor resiko kematian pada pasien Covid-19 dewasa akan tetapi berpengaruh terhadap beratnya gejala yang dialami. DM merupakan faktor resiko terhadap kematian pasien Covid-19 dewasa dan memperparah gejala. Apabila DM pada pasien Covid-19 dikelola dengan baik tingkat kematian dan keparahan gejala dapat menurun. Abstract

The mortality rate among adult hospital patients is a significant indicator in assessing the quality of care and health services. High mortality rates can affect a hospital's reputation, reduce the credibility of medical personnel, and trigger public anxiety. Failure to recognize the worsening of a patient's condition can delay appropriate life support management. This scoping review aims to identify and analyze the Early Warning Score (EWS) application for adults with risk factors for death in regional general hospitals (RSUD). The scoping review was conducted by searching literature using electronic databases from January 2018-May 2023, such as PubMed, ScienceDirect, LibMed UGM, and hand searching. Relevant articles were selected based on inclusion criteria. The results contained two main themes: factors related to mortality in adult patients and the benefits of implementing EWS on the clinical outcomes of adult patients. Implementing EWS in adult patients with high-risk factors for mortality at Regional General Hospitals can help identify the worsening of the patient's condition early, reducing the risk of death. Increasing awareness of the importance of EWS, EWS training, and consistent use of EWS according to hospital protocols can help reduce mortality and improve overall patient clinical outcomes.

Abstract

Early Warning Score (EWS) Implementation in Inpatient Ward: Scooping Review.The implementation of EWS is known to reduce the worsening rate of patients in the inpatient ward, accuracy in implementing can affect decision making in treating patients. Research shows that there is still little literature review that discusses the application of EWS. Therefore, it is necessary to conduct a scooping review related to the application of EWS. The purpose of this study was to identify how to apply EWS in inpatient wards in hospitals. The research method follows the framework of Arksey and O'Malley. The scooping review was conducted by 3 databases of PubMed, ScienceDirect, Scopus published from 2014-2023 in March, using English and Indonesian which discussed the implementation of EWS in hospitals using  the keywords  'Nurse', 'Early Warning Score', 'Early Warning System', detect deterioration patient'. The results obtained  as many as 179 potentially relevant articles, 7 articles identified. The findings revealed that the application of EWS carried out in the hospital consisted of recording observation sheets, frequency of patient monitoring according to the EWS score, actions according to the EWS protocol applied and the time of measurement of patient vital signs

Abstract

ABSTRAKIntensive care unit (ICU) jantung merupakan unit dengan pasien yang rentan terhadap penularan infeksi. Pengunjung yang tidak menerapkan kebersihan tangan menunjukkan pertumbuhan bakteri penyebab Hospital associated Infections (HAIs). Kurangnya pengetahuan dan persepsi pengunjung tentang kebersihan tangan menyebabkan tingkat kepatuhan kebersihan tangan yang rendah. Tujuan Penelitian: Mengetahui gambaran pengetahuan dan persepsi kebersihan tangan pengunjung serta hubungannya dengan karakteristik responden di ICU jantung RSUP Dr. Sardjito. Metode: Menggunakan desain analitik observasional dengan rancangan cross sectional. Jumlah sampel sebanyak 101 responden yang dikumpulkan dengan teknik consecutive sampling. Instrumen pada penelitian ini dibuat oleh peneliti berdasarkan panduan kebersihan tangan dari World Health Organization (WHO).  Instrumen yang digunakan telah valid dan reliable. Data dianalisis secara univariat dan bivariat dengan uji chi-square.  Hasil: Rata-rata skor pengetahuan kebersihan tangan 6,1 dan persepsi kebersihan tangan 31,07. Enam puluh enam responden memiliki pengetahuan yang baik dan lima puluh delapan dari seratus satu responden memiliki persepsi yang rendah. Ada hubungan antara pengetahuan dan pendidikan (p=0,0280), dan dengan riwayat penyakit (p=0,002). Ada hubungan antara persepsi dan usia (p=0,028), dan dengan riwayat berkunjung (p=0,023). Diskusi: Mayoritas pengetahuan kebersihan tangan pengunjung yang tinggi sedangkan persepsi rendah dipengaruhi oleh efektivitas pemberian informasi dan peran petugas kesehatan untuk terus mengingatkan pengunjung terkait kebersihan tangan. Kesimpulan: Perawat atau peneliti selanjutnya perlu mengembangkan strategi yang efektif guna memberikan informasi terkait kebersihan tangan kepada pengunjung.Kata Kunci: intensive care units, kebersihan tangan, pengetahuan, pengunjung pasien, persepsi Knowledge and Perceptions of Hand Hygiene in Cardiac Intensive Care Unit Visitors ABSTRACTThe cardiac intensive care unit (ICU) is a unit in which patients who are susceptible to infection transmission. Visitors who do not apply hand hygiene show the growth of bacteria that causes Hospital associated Infections (HAIs). Inadequate knowledge and perceptions of visitors about hand hygiene causes a low level of hand hygiene compliance. Objective: To obtain an overview of knowledge and perceptions of visitor hand hygiene and their correlation with the characteristics of respondents at the Cardiac ICU of Dr. Sardjito Hospital. Methods: This research employed an observational analytic design with a cross sectional design. The sample size was 101 respondents taken using consecutive sampling technique. The instrument in this research was made by the researchers based on hand hygiene guidelines from the World Health Organization (WHO). The instrument used was valid and reliable. Data were analyzed by using univariate and bivariate with the chi-square test. Results: The average score of hand hygiene knowledge was 6.1 and that of perception of hand hygiene was 31.07. Sixty-six respondents had good knowledge and fifty-eight out of one hundred and one respondents had low perceptions. There was a correlation between knowledge and education (p=0.0280), and with a history of disease (p=0.002). There was a correlation between perception and age (p=0.028), and with a history of visiting (p=0.023). Discussion: The majority of visitors' high knowledge of hand hygiene and low perception was affected by the effectiveness of providing information and the role of health workers to keep reminding visitors about hand hygiene. Conclusion: Nurses or future researchers need to develop effective strategies to provide information about hand hygiene to visitors.Keywords: intensive care units, hand hygiene, knowledge, patient visitors, perception

Abstract

Disasters preparedness behavior is influenced by self-efficacy. Self-efficacy affects the resilience and ability of communities in adapting to changes in disaster conditions. In Indonesia there are no instruments for measuring self-efficacy in the context of disasters. Objective: This study aimed to adapt the Indonesian version of the General Self-Efficacy Scale into the context of disasters and test its validity and reliability. The Indonesian version of General Self-Efficacy Scale was adapted culturally then modified into the disaster context and adapted to the research target culture and also tests of the content validity and the construct validity were conducted on 100 residents. The analysis used Pearson product-moment, exploratory factor analysis and confirmatory factor analysis. Reliability testing used internal consistency analysis (Cronbach’s alpha). The Indonesian version of the General Self-Efficacy Scale was in accordance with the context of the disaster and the target culture of the study. Pearson correlation values ​​range from 0.528 to 0.707. The EFA results were formed of 2 factors. Confirmatory factor analysis with 2 factors showed a fit model (p-value = 0.032, SRMR = 0.078, CFI = 0.91). The CFA results with 1 factor showed a fit and very good model (p-value = 0.135, RMSEA = 0.058, SRMR = 0.060, CFI = 0.977). The Cronbach’s alpha coefficient has a value of 0.847. The Indonesian version of the General Self-Efficacy Scale in the context of disaster with a 2-factor model and a 1-factor model shows acceptable validity and reliability for measuring self-efficacy in the face of disasters.
