Abstract

XYZ is a company operating in the textile industry and producing rayon fabric. In the production process, defects were still found in the period October 2023 – March 2024. Rayon fabric production was 286973m, consisting of defects in final weft 11643m, wavy 8391m, unwoven 7569m, double weft 6173m, stretch weft 8126m. Based on the problem, it can be solved using the six sigma method and FMEA with the DMAIC stage used to control quality from the initial process until a proposal for improvement is found. Obtained from the calculation results, the average sigma is 3.39 with an average DPMO of 29046.87 in production approval. The Pareto diagram of the most dominant defect percentages is final weft defects 27.8%, wavy 20.0%, loose weft 19.4%, non-woven 18.1%, double weft 14.7%. The highest RPN value was 280 for final feed defects and waves caused by workers' lack of accuracy regarding machine settings. Abstract

PT Sinar Semesta is an industrial company engaged in metal casting, one of which is Base Plate which is located on Jl. Raya Solo-Yogya Km. 26 Klepu, Ceper, Klaten. The problem that arises at PT Sinar Semesta is product quality that needs to be improved so that it can compete with other products on the market. Therefore to improve product quality, PT Sinar Semesta needs to be directly supported by all parties in the company and all operators, especially the production operators. This study used the Seven Tools Quality with the results of the research that in January - December 2021 PT Sinar Semesta produced 2091 Base Plate products, with a total of 301 defects. There were three types of defects studied in January - September 2021, namely perforated with 95 defects in Base Plate products, porous defects with 139 defects, and rough surface defects with 67 defects with a defect percentage of 31.56%, 46, 17%, and 22.26%.

Abstract

PT Sinar Semesta is an industrial company engaged in metal casting. The problems that arise at PT Sinar Semesta are what type of disability is the most dominant and what factors influence the type of disability. The purpose of this research is to find out the types of product defects, and the solutions that can be taken to overcome these problems. This study uses the Statistical Quality Control (SQC) method used to control quality from the initial process to the finished product and Fault Tree Analysis (FTA) is used to determine what factors cause defects in the product. Based on the results of a pareto chart analysis from November 2021 to October 2022, the total production was 3008 products, there were 838 defective products. There are two types of defects in the Eq Spacing product, including defects in the tail of rats obtained at a percentage of 91.53% with a cumulative percentage of 91.53% and water defects obtained at a percentage of 8.47% with a cumulative percentage of 100%. Factors causing defects with the greatest weight are careless operators and fatigue for the proposal obtained, namely supervising workers so that accuracy can be maintained.

Abstract

Abstract. UD Cantenan is an industrial company engaged in metal casting, especially made from Aluminum. This company is known to have quality control problems in the manufacture of lip handle products and canal parts. The result of this problem is defective products in the lip handle and canal parts. In this case, of course, the company will spend more to repair the product so that the company experiences losses. Companies need to find ways to reduce the number of defective products. In this study, the Six Sigma method was used with DMAIC (Define, Measure, Analyze, Improve Control) and TRIZ stages to analyze and provide solutions to improve problems that occurred in the company. Based on the research conducted, at the define stage there are 3 types of defects in the lip handle product and canal parts, namely small scratches, small pinholes, and asymmetrical patterns. Then the measure stage obtained the percentage of small scratch defects of 54.47%, small pinholes of 17.3% and asymmetrical patterns of 28.2%. The average DPMO score obtained is 25,720 with a sigma level of 3.45 which shows that UD Cantenan can be categorized as good for industry in Indonesia. Furthermore, at the analysis stage Based on the identification of causal factors using FMEA analysis, it is known that the biggest contributing factor to failure is operator error in the operation of the milling machine. Then at the improve stage, make improvement proposals based on obtaining the highest RPN value from the factors causing disability using the TRIZ method. And the last stage, namely control, is the result of proposed improvements that can be made by the company to solve problems to reduce the number of product defects that occur during the production process.

Abstract

Faktor yang menjadi potensi terjadinya kecelakaan kerja di PT Madu Baru PG. Madukismo ialah bahwa pada area penggiling produksi belum menerapkan 5S secara keseluruhan seperti banyaknya barang yang belum tersusun dengan rapi, area kerja yang kotor dan kurangnya perhatian terhadap alat pelindung diri (APD). Usulan penerapan program 5S yang diterapkan pada setiap area penggilingan produksi yang berada di PT. Madu Baru dengan penerapan yang pertama yaitu dengan melakukan seiri atau pemilahan item pada setiap area kerja sehingga barang-barang yang tidak diperlukan disingkirkan dari area kerja. Penerapan yang kedua yaitu seiton atau penataan dilakukan agar peralatan yang disimpan terlihat lebih rapi agar menghindari potensi kecelakaan kerja terpleset benda yang brantakan. Usulan penerapan yang ketiga ialah seiso atau pembersihan, pembersihan yang diusulkan dengan memberikan tanggung jawab pada setiap operator memiliki tanggung jawab pada setiap area kerjanya sehingga area kerja terlihat bersih dan nyaman ketika melakukan pekerjaan. Penerapan selanjutnya yaitu seiketsu dengan pemberian label warna, batas pada benda, pembuatan Standard Operational Procedure (SOP) dan memberikan pemeriksaan secara harian bagi owner.

Abstract

Dalam era persaingan bisnis yang semakin meningkat, Usaha Mikro, Kecil, dan Menengah (UMKM) dihadapkan pada tantangan untuk bersaing secara kompetitif. Menghadapi hal ini, strategi peningkatan efisiensi dan efektivitas menjadi krusial, terutama dalam aspek produksi. Studi ini dilakukan untuk mengevaluasi indeks produktivitas dan penjualan produk tempe di UMKM Tahu Tempe Pak Tugiyo, dengan fokus khusus pada peningkatan produksi. Menggunakan metode Objective Matrix (OMAX), yang mencakup langkah-langkah Defining, Quantifying (dengan penilaian pada level 0, level 3, dan level 10), dan Monitoring (melalui Score, Weight, dan Performance Indicator), penelitian ini berhasil mengungkap dinamika produktivitas. Hasilnya menunjukkan bahwa puncak produktivitas terjadi pada November 2021 dengan indeks 185%, sedangkan titik terendah dicatat pada Mei 2022 dengan indeks -53,33%. Peningkatan indeks produktivitas dikaitkan dengan efisiensi dalam produksi, termasuk peran operator, penggunaan tenaga kerja, bahan baku, dan energi listrik. Analisis sebab-akibat mengidentifikasi bahwa kinerja mesin penggilingan kedelai yang suboptimal berdampak signifikan terhadap output produksi. Lebih lanjut, masalah pada mesin penggilingan tidak hanya mengurangi jumlah produksi tetapi juga berpotensi mengganggu jam kerja reguler, merugikan efektivitas produksi secara umum.

Abstract

CV Harico, perusahaan manufaktur briket berbasis make-to-order, mengalami kendala dalam efisiensi produksi terutama terkait dengan waktu tunggu yang panjang. Penelitian ini bertujuan untuk mengidentifikasi dan menganalisis pemborosan dalam proses produksi briket di CV Harico, menentukan faktor penyebab pemborosan, serta menyusun rekomendasi perbaikan untuk mengurangi atau mengeliminasi pemborosan tersebut. Menggunakan prinsip Lean Manufacturing dengan alat utama Value Stream Mapping, penelitian ini menerapkan metode hitung Borda untuk evaluasi pemborosan. Pemborosan terbesar yang teridentifikasi adalah waktu menunggu, dengan bobot prioritas sebesar 0,24. Process Activity Mapping (PAM) dipilih sebagai alat yang paling efektif untuk mengurangi pemborosan ini, dengan skor 6,42. Penyebab pemborosan dianalisis menggunakan diagram fishbone, dan solusi yang diusulkan adalah pemasangan kipas blower di setiap oven untuk mempercepat proses pendinginan briket. Pemetaan keadaan masa depan menunjukkan penurunan waktu lead time dari 276.068 detik menjadi 264.425 detik.

Abstract

Dari total 12.180 unit yang diproduksi oleh UMKM Tahu Pak Sugino, terdapat 1.512 unit produk cacat. Masalah pengendalian kualitas ini menyebabkan penurunan tingkat kepuasan pelanggan dan kerugian bagi perusahaan. Penelitian ini bertujuan untuk mengurangi jumlah produk cacat dengan menerapkan strategi Six Sigma dan Analisis Pohon Kesalahan (Fault Tree Analysis/FTA). Tiga kategori cacat produk yang teridentifikasi adalah cacat ukuran unit (36,6%), cacat unit kotor (32,3%), dan cacat tekstur unit (31,0%). Tingkat sigma yang diperoleh berdasarkan perhitungan Six Sigma adalah 2,65 dengan DPMO (Defects Per Million Opportunities) sebesar 124.138 peluang per juta. Dengan memanfaatkan metode 5W+1H dan FTA pada ketiga jenis cacat tersebut, diusulkan rencana peningkatan kualitas produk.

Abstract

Minomartani Sleman village is the location of Bakpia X MSMEs, which make dry and wet bakpia of various flavors. However, until now, bakpia products produced are still many defects stuffing out, burnt, and bulging. The purpose of this study was to determine the factors that cause disability to occur and efforts to improve it. This study uses Six Sigma method with DMAIC (Define, Measure, Analyze, Improve, and Control) and proposed improvements with Kaizen. The results of DMAIC analysis showed that the three product defects were caused by human factors, materials, methods, and machines. Sand defects amounted to 276 items, or 49.8%, 110 items, or 19.9%, and defects in bakpia filling out amounted to 168 items, or 30.3%. On the human factor, for example, forget turning bakpia. The factor of the machine or tool is uneven heat in the pan. The mixing process is uneven, and the baking sheet is sticky. Method factor because there is no SOP in MSMEs. Material factors the problem of green beans are not good.

Abstract

CV Harico is a manufacturing company that produces briquetted charcoal final products. In this research, the company had problems with the quality of charcoal briquettes which did not meet standards, amounting to 12% of the production process, including slanted shapes, holes and cracks. In this research, problem solving is done by combining the Six Sigma and TRIZ methods to get suggestions for improvement. The Six Sigma method is an organized and systematic method, Six Sgma has 5 stages, which can be known as the DMAIC phase. Based on the calculation of the DPMO value and the calculation of the sigma value at the measurement stage, the average DPMO value for October 2023-November 2023 was 27219 with a sigma value of 3.43. Based on the identification results using the Fishbone Diagram method and the FMEA method, the largest defects were found in the slanted shape of the charcoal briquettes, these results were obtained from the factor with the highest RPN value that caused defects. Proposed improvements based on the TRIZ method analysis include providing training, carrying out periodic actions, making maintenance schedules, evaluating work processes, changing tools to reduce the occurrence of defects. The aim of this research is to analyze the problems that occur as well as efforts to control quality and reduce product defects in briquetted charcoal products.
