Abstract

Rational drug use ensures medication is prescribed based on clinical needs in appropriate quantities. However, irrational drug use remains common, including incorrect indications, improper dosages, and administration errors. This study aims to evaluate the rationality of antibiotic use for non-specific diarrhea, non-pneumonia ARI, and myalgia in North Gorontalo Regency, Gorontalo Province, in 2023. Using a descriptive observational cross-sectional design, data were collected retrospectively from patient prescriptions and medical records. Findings indicate that the average number of drugs per prescription sheet at North Gorontalo District Health Centers exceeds WHO’s recommended standard of 1.8–2.2 items. While antibiotic and injection drug use generally aligns with WHO indicators, some centers exceeded recommended levels in certain months. Excessive drug prescriptions pose polypharmacy risks, while overuse of antibiotics may contribute to resistance. Overall, antibiotic use for non-specific diarrhea and non-pneumonia ARI, as well as injection drug use for myalgia, is largely rational, though occasional deviations from WHO guidelines were observed.

Abstract

Fameflower Leaves (Talinum paniculatum (Jacq) Gaertn) are medicinal plants that communities have traditionally used due to their active compounds, such as saponins, flavonoids, tannins, triterpenes, sterols, and polyphenols found in both the leaves and roots. This study aimed to determine the dose of ethanol extract of fameflower leaves affecting the histology of the spleen in male rats (Rattus norvegicus) following ovalbumin antigen stimulation, as well as to observe changes in the spleen tissue structure after the extract treatment. The study used 20 Wistar rats divided into four groups. The rats were first adapted for 2 weeks and then fasted for 8 hours before the treatment—a group I served as the standard control. Group II was induced with ovalbumin antigen. Groups III and IV were induced with the antigen, and the ethanol extract of fameflower leaves was administered at doses of 0.4g/kgBW and 0.8g/kgBW, respectively. The extract was administered orally for 7 days to the test animals. On day 8, the test animals were sacrificed, and their spleens were harvested for histological preparation using the Hematoxylin-Eosin staining method. Microscopic observation showed no histological damage in the spleen, such as inflammation, degeneration, congestion, hemorrhage, or necrosis. Therefore, it can be concluded that the ethanol extract of Bugis Ginseng leaves did not affect the spleen of rats at doses of 0.4g/kgBW and 0.8g/kgBW

Abstract

Hypertension, or high blood pressure, is a condition characterized by a sustained increase in blood pressure, exceeding the normal value of ≥140/90 mmHg. Hypertension therapy aims to lower both systolic and diastolic blood pressure to restore normal levels and minimize the risk of complications. The use of herbal plants such as bidara leaves (Ziziphus mauritiana L.) and Javanese ginseng (Talinum paniculatum (Jacq.) Gaertn.) holds potential as an alternative in the management of hypertension. Medicinal plants and plant-derived compounds can exhibit various toxicological effects, including renal, cardiac, and hepatic toxicity. This study aims to investigate the effects of Bidara leaf extract and Java ginseng leaf extract on the macroscopic appearance of the kidneys, heart, and liver of prednisone-induced hypertensive rats. The study used 20 rats divided into five groups: negative control (Na-CMC 1%), positive control (Captopril 25 mg), and three groups receiving combined doses of bidara leaf and Javanese ginseng leaf ethanol extracts (EEBG 50+50 mg/kgBW, EEBG 100+100 mg/kgBW, EEBG 200+200 mg/kgBW). The results showed that the combination extract of Bidara leaves (Ziziphus mauritiana L.) and Ginseng leaves (Talinum paniculatum (Jacq.) Geartn.) at doses of 50+50, 100+100, and 200+200 mg/kgBW did not cause any macroscopic changes. Macroscopic observations showed that the kidneys, hearts, and livers of mice administered EEBG maintained a reddish-brown color with a firm consistency and smooth surface

Abstract

Gude Leaves (Cajanus cajan) are a traditional medicinal plant for treating diarrhea. The ingredients contained in Gude leaves are flavonoids, such as kajanol, quarcetin and luteolin and also contain other secondary metabolites, tannins, saponins, alkaloids, terpenoids and phenols. Scientifically, Gude leaves have antimalarial, antidiabetic, anthelmintic, antioxidant, antibacterial and anticancer activity. This research aims to determine the toxicity of gude leaves (C. cajan) using the Brine Shrimp Lethality Test (BSLT) method. This research began by carrying out two extraction methods, namely maceration and UAE (Ultrasonic Assisted Extraction) on samples of gude leaves using 70% ethanol solvent. Then a phytochemical screening was carried out which showed that samples of gude leaves contained alkaloids, flavonoids, steroids, saponins and tannins. Experimentation the toxicity of gude leaf extract using maceration and UAE methods on Artemia Salina Leach larvae, each divided into 5 test groups, namely 4 treatment groups (concentrations of (250 ppm,  LC=23,33% & 33,33%) ; (500 ppm, LC= 56,67% & 60%); (750 ppm, LC= 70% & 73%); and (1000 ppm, LC= 80%) and 1 positive control group (sea water ). The research results can be seen through probit analysis by calculating the LC50 value. The LC50 value of gude leaf extract using the maceration method was 242.52 and the result of gude leaf extract using the UAE method was 394.16 (Toxic).  This research provides information regarding the toxic effects of gude leaf extract.
