Abstract

Kemampuan hidup Acinetobacter baumannii pada berbagai keadaan dikombinasikan dengan resistensi berkaitan erat dengan Healthcare Associated Infections (HAIs). Kemunculan strain resisten telah banyak dilaporkan. Penelitian ini bersifat observasional deskriptif. Pemeriksaan spesimen klinik dilakukan di Laboratorium Mikrobiologi Klinik RSUD dr. Zainoel Abidin Banda Aceh. Isolat Acinetobacter baumannii dikumpulkan,  dilakukan uji kepekaan serta dinilai tingkat resistensinya. Angka insidensi Acinetobacter baumannii adalah 4,6% yang dominan terisolasi dari spesimen sputum. Strain resisten sangat umum dijumpai pada ruang rawat intensif, ruangan non intensif memiliki lebih banyak strain susceptible. Strain MDR menunjukkan kepekaan terhadap amikacin, trimethoprim-sulfamethoxazole, tobramycin, ampicillin-sulbactam, dan meropenem. Acinetobacter baumannii strain XDR hanya menunjukkan kepekaan yang masih baik terhadap amikacin. Data epidemiologi resistensi antibiotik adalah komponen utama Program Pengendalian Resistensi Antibiotik di rumah sakit

Abstract

Candidiasis has become increasingly widespread in the community alongside with the developing resistance of Candida sp. to some antifungals. A prevalence study in the present research is required to surveil the distribution of Candida-related infections to administrate the appropriate antifungal treatments. The objective of this research was to determine the species distribution of candidiasis with their antifungal susceptibility isolated from clinical specimens at the Zainoel Abidin Hospital (ZAH) Banda Aceh, Indonesia. The clinical specimens were from inpatients and outpatients in the hospital during January to June 2019. The identification of Candida species and antifungal sensitivity assay were conducted by using VITEK® 2 Compact. Of a total of 68 isolates, there were six species of Candida with the highest species prevalence was Candida tropicalis (52.94%). Additionally, the highest prevalence of candidiasis came from urine specimen (54.41%) and mostly from inpatients in the internal medicine unit (54.41%). Candidiasis occurred predominantly in men (58.82%) and during adulthood (55.88%). The antibiogram of Candida sp. shows a high percentage of sensitivity towards some antifungals including fluconazole (100%), voriconazole (100%), caspofungin (100%), micafungin (100%), amphotericin B (99.5%), and flucytosine (100%). From this result, it can be concluded that candidiasis cases in the ZAH Banda Aceh, Indonesia has prevalently occurred with the few antifungal therapies for candidiasis of were still effective empirically and definitively. This is an initial study of Candida prevalence within different clinical samples in Banda Aceh and the study is expected to be a basis for prevention and control of Candida-related infections in the area.

Abstract

Invasive dermatophytosis is a dermatophyte infection characterized by fungal invasion into deeper skin tissue including the dermis and subcutaneous areas. Herein, a man who was an HIV/AIDS sufferer was reported to develop an unusually invasive dermatophytosis in the form of nodules and ulceration. His CD4+ cells count was 88 cells/µl with a percentage of 1.87%. Self-medication with topical antibiotics did not respond. Microbiological examination was carried out including direct potassium hydroxide preparation and fungal culture. The etiologic diagnosis of invasive dermatophytosis by Trichophyton verrucosum  was established. This is one of the zoophilic fungus that causes ringworm in cattle. It was suspected that the patient was inoculated due to contact with fomites containing fungi while working as a construction worker. Dermatophytes can cause atypical infections with a completely unexpected appearance in immunocompromised patients. The diagnosis is largely determined by a high level of clinical suspicion followed by microbiological examination.

Abstract

Purple Urine Bag Syndrome (PUBS) is a rare phenomenon that is commonly overlooked. PUBS occurs in patients who use indwelling urinary catheters. This condition is characterized by a change in the color of urine including the bag and catheter tube turning purple. The purple color comes from the pigment mixture from the end result of the tryptophan metabolic chain reaction with the help of bacteria that produce sulfatase or phosphatase enzymes in the urine. The occurrence of PUBS requires the interaction of various risk factors simultaneously. PUBS can cause concern for patients, families and medical personnel. Lack of understanding and awareness can lead to misdiagnosis and treatment. For this reason, clinicians and medical personnel must be able to identify PUBS and understand its prevention and treatment. PUBS usually has a good prognosis. Correction of the underlying disease and treatment of symptomatic UTIs can reduce the chances of developing this syndrome.

Abstract

Stroke merupakan suatu penyakit serebrovaskular akibat penurunan atau terhentinya aliran darah ke otak. Penderita stroke biasanya akan mengalami kerusakan fungsi kognitif tergantung dengan derajat stroke yang dideritanya. Tujuan penelitian ini adalah untuk mengetahui hubungan derajat stroke terhadap status kognitif pada pasien stroke iskemik di Poliklinik Saraf RSUD dr. Zainoel Abidin Banda Aceh. Jenis penelitian ini adalah analitik observasional dengan desain cross sectional yang dilakukan pada bulan Oktober sampai Desember 2016.Penelitian ini menggunakan instrumen National Institutes of Health Stroke Scale (NIHSS) dan Mini Mental State Examination (MMSE).Data dianalisis dengan menggunakan uji korelasi Spearman. Sampel berjumlah 40 orang yang terdiri dari 24 orang (60,0%) pria dan 16 orang (40,0%) wanita dengan karakteristik responden usia diantara 56-65 tahun sebanyak 21 orang (52,5%), lulusan SMA sebanyak 20 orang (50,0%), memiliki riwayat hipertensi sebanyak 21 orang (52,5%) dan riwayat merokok sebanyak 22 orang (55,0%). Hasil penelitian didapatkan 22 orang (55,0%) mempunyai derajat stroke sedang dan 19 orang (47,5%) mengalami gangguan kognitif ringan. Terdapat hubungan yang kuat antara derajat stroke dengan status kognitif (p = 0,000, r = 0,603). Kesimpulannya terdapat hubungan yang kuat antara derajat stroke terhadap status kognitif pada pasien stroke iskemik di Poliklinik Saraf RSUD dr. Zainoel Abidin Banda Aceh.

Abstract

Respon glikemik merupakan kondisi fisiologis pada kadar glukosa selama periode tertentu setelah seseorang mengonsumsi suatu pangan. Setiap pangan yang mengandung karbohidrat mempunyai respon glikemik yang berbeda-beda. Pangan yang dapat menaikkan kadar glukosa darah dengan cepat memiliki indeks glikemik yang tinggi, salah satunya adalah nasi putih. Overweight adalah hasil dari ketidakseimbangan energi dimana terlalu banyak kalori yang dimakan dibandingkan energi yang digunakan oleh tubuh untuk melakukan aktivitas fisik. Orang overweight cenderung mudah terkena penyakit-penyakit metabolisme seperti diabetes mellitus. Penelitian ini bertujuan untuk melihat perbandingan respon glikemik akibat pemberian nasi putih organik dan nasi putih nonorganik pada mahasiswa yang overweight. Penelitian ini merupakan penelitian eksperimental dengan desain cross-over acak pada mahasiswa Fakultas Kedokteran Universitas Syiah Kuala. Sampel berjumlah 30 orang yang masing-masing mendapatkan 2 perlakuan yang sama yaitu mendapatkan nasi putih organik dan nasi putih nonorganik. Data dikumpulkan dengan menggunakan wawancara, pengukuran antropometri dan respon glikemik yang diambil dari sampel darah kapiler. Data yang diperoleh dianalisis menggunakan uji t dependent. Hasil penelitian ini,  respon glikemik pada pemberian nasi putih organik lebih rendah secara bermakna (p = 0,013) dibandingkan dengan respon glikemik pada pemberian nasi putih nonorganik pada mahasiswa yang overweight. Kesimpulan mengkonsumsi nasi putih organik memperlihatkan respon glikemik lebih rendah, menunda rasa lapar dan mempertahankan rasa kenyang lebih lama dibandingkan nasi putih nonorganik.Glycemic response is a physiological condition in glucose levels during a certain period after a person consumes food. Every food which contains carbohydrates has a different glycemic response. The food that can raise blood glucose levels rapidly have a high glycemic index, one of them is white rice. Overweight is a result of inequity energy which caused by calories that have been consumed is larger than the used of energy by the body to do the physical activity. Overweight people are prone to metabolic diseases such as diabetes mellitus. This study is aimed to compare the glycemic response to the consequence of consuming organic white rice and non-organic white rice on overweight students. This study is an experimental study with a randomized cross-over design to the students of Faculty Medicine, University of Syiah Kuala. The samples were 30 people and each of them gets two of equal treatment to consume organic white rice and non-organic white rice. The data was collected using interviews, anthropometric measurements, and glycemic response drawn from capillary blood sample. The data were analyzed using t-test dependent. The results of this study is the glycemic response to the consumption of organic white rice was significantly lower (p = 0.013) compared to the glycemic response of  the consumption non-organic white rice on overweight students. Conclution in this study is the consume of organic white rice showed the lower glycemic response, postponted hunger feeling and maintained the satiety longer than non-organic white rice

Abstract

Introduction: Hand hygiene is a non-pharmacological intervention recommended worldwide to prevent and control infectious diseases. Hand sanitizer is an antiseptic that contains active agents and could eradicate pathogenic microorganisms caused by infectious diseases. The addition of patchouli oil as an active agent in hand sanitizers has been proven to have a bactericidal and bacteriostatic effect against bacteria and viruses. This study aims to compare Hand sanitizer A's effectiveness (which does not contain patchouli oil) with Hand sanitizer B (containing patchouli oil) by comparing the number of normal hand flora colonies before and after using both hand sanitizers. Methods: This study is a pre-experimental design with a static group pretest and posttest design. There were 16 Medical Laboratory Technologists (MLT) enrolled in the study. Each MLT received two interventions, using Hand sanitizer A and B. The hand swabs were collected before and after using both hand sanitizers. The swabs inoculated on the media, incubated, and colony-forming units were counted. Result: This study showed a significant difference between hand sanitizers containing and not containing patchouli oil in reducing the number of normal hand flora colonies with a p-value = 0.033 (α < 0.05). The median value of total colonies decreased in Hand sanitizer B is 15, lower than the median value of Hand sanitizer A, which is 36. Conclusion: Hand sanitizer B containing patchouli oil possessed preferable effects to Hand sanitizer A, which does not contain patchouli oil in reducing the number of normal hand flora colonies.

Abstract

Highlights The use of eucalyptus oil is natural and risk-free. It has a lengthy history of use as a traditional medicine in Asia, including Indonesia. It has been demonstrated that eucalyptus oil possesses antibacterial activity against MRSA, and this activity was controlled by the oil's concentration. Abstract Background: Eucalyptus (Melaleuca leucadendra) oil is used by Indonesians as an herbal medicine. Eucalyptus containing 1,8-cineol at 72.30% is expected to be used as an antibacterial.  Objective: The study aimed to assess the antibacterial activity of eucalyptus oil against Methicillin-resistant Staphylococcus aureus (MRSA) bacteria in vitro. Material and Method: The materials used were eucalyptus oil (M. leucadendra) in various concentrations with ethyl acetate as solvent. The research method used was the agar-well diffusion assay. The MRSA was suspended to 0.5 McFarland turbidity. The MRSA suspension was thoroughly swabbed onto the surface of the Mueller-Hinton agar plate. The wells were made with a diameter of 0.6 mm on Muller-Hinton agar aseptically and 100 µl of eucalyptus oil was put into the well using a micropipette and incubated for 24 hours at 37°C. The diameter of the inhibition zone was measured with a caliper. Statistical analysis using the SPSS software edition 23. Result: Eucalyptus oil solution starting from a concentration of 10% to 100% had an antibacterial response, which could be seen by the formation of a bacterial inhibition zone around the eucalyptus oil (M. leucadendra) wells. The inhibition ability of eucalyptus oil against MRSA bacteria was greatest at a concentration of 90% with an inhibition zone of 31.26 mm. It was found that the concentration of eucalyptus oil affected the diameter of the bacterial inhibition area. Conclusion: Eucalyptus oil has been shown to have antibacterial activity against MRSA and was influenced by the concentration of the oil.

Abstract

Fungal infection can occur in almost all parts of the human body, including the respiratory system. One group of fungi causing opportunistic infections in the lungs also known as pulmonary candidiasis is Candida spp. This study aimed to determine species diversity and antifungal sensitivity of Candida sp. causing pulmonary candidiasis from sputum specimen isolates in the Zainoel Abidin Hospital (ZAH). The sputum specimens were from inpatients and outpatients during a period of January 2019 to January 2021. Identification of Candida spp. and the antifungal sensitivity were carried out using culture and Gram Staining as well as the VITEK® 2 Compact. The results of this study indicated that there were six species of Candida sp. obtained from the study period. Of total 73 isolates, the highest percentage of species was Candida albicans (56.16%). The percentage of pulmonary candidiasis based on age was dominated by the early-late age category 46-65 years (50.68%) and based on sex was dominated by men (71.23%). Antifungal susceptibility assays revealed that the Candida species remained highly susceptible to the antifungals (amphotericin B, caspofungin, flucytosine, fluconazole, micafungin, and voriconazole), i.e., greater than 91 percent. Overall, the results of this study indicated that pulmonary candidiasis were predominantly related to C. albicans and C. tropicalis infections in ZAH and the sensitivity of antifungal drugs remained empirically and definitively effective. This research might be important as a part of infection prevention and control strategies, as well as the administration of empirical antifungals to combat Candida-mediated lung infections.
