Abstract

Head injuries are one of the leading causes of morbidity and mortality in the world, with Subarachnoid Hemorrhage (SAH) being one of the most frequent complications that can increase the risk of death. The World Federation of Neurosurgical Societies (WFNS) Subarachnoid Hemorrhage (SAH) Grading Scale is used to assess the severity of SAH patients and predict their prognosis. This study aims to determine the relationship between WFNS SAH Grading Scale and mortality in head injury patients with SAH at Royal Prima Hospital Medan. The research method used an analytical observational approach with a cross-sectional design. The research sample amounted to 47 patients who met the inclusion criteria, using medical record data for the period January 2023 to August 2025. Data analysis was carried out using the Chi-Square test. The results showed that the majority of patients were in Grade I (38.3%) and Grade IV (27.7%), with a mortality rate of 23.4%. Statistical tests showed a value of p = 0.001 which means that there is a significant relationship between the WFNS SAH Grading Scale and the mortality of SAH patients. The lower the patient awareness level (GCS), the higher the risk of death. This study emphasizes the importance of using the WFNS SAH Grading Scale in assessing prognosis and determining the priority management of SAH patients in hospitals.

Abstract

Budget efficiency is a crucial aspect in road infrastructure project management. This study analyzes budget efficiency in the provincial road improvement project on the Parsoburan-Borbor-Pangururan-Janji Maria route in Toba Regency, North Sumatra, with a contract value of IDR 5,784,624,000. The research aims to evaluate project budget efficiency and identify factors that influence it quantitatively. The research method uses a quantitative approach with a cross-sectional design involving 45 respondents. The results show that the budget efficiency level reaches 99.5% (fairly efficient category). Correlation tests show significant relationships between planning quality (r=0.762; p<0.001), procurement management (r=0.684; p<0.001), and HR competence (r=0.591; p<0.01) with budget efficiency. Regression analysis shows the model can explain 73.4% of budget efficiency variation (R²=0.734). Planning quality factor provides the largest contribution (β=0.451; p<0.001).

Abstract

This study analyzes the budget efficiency of the Mamminasata Bypass Road and Bridge Construction Project Segment I and II using the Cost Performance Index (CPI) from the Earned Value Management (EVM) method. This project has a contract value of IDR 139.90 billion and an implementation period from November 2022 to March 2024. The data used is secondary data in the form of monthly progress reports, financial statements, and contract documents. The parameters analyzed included Planned Value (PV), Earned Value (EV), Actual Cost (AC), CPI, Cost Variance (CV), and Schedule Performance Index (SPI). The results showed an average CPI value of 0.99, which indicates excellent budget efficiency with a cost overrun of only 0.99%. CPI fluctuations over the course of the project are relatively minimal, indicating the stability of cost performance. Multiple regression analysis identified four significant factors that affect budget efficiency, namely soil conditions (β = -0.412, p = 0.005), design changes (β = -0.289, p = 0.010), volume changes (β = -0.198, p = 0.043), and price escalation (β = -0.367, p = 0.001), with the regression model having R² = 0.847. A comparison of budget efficiency between Segments I and II showed no significant difference (p-value = 0.682), with Segment I CPI 0.990 and Segment II 0.989. This study contributes to the implementation of EVM in road projects in Indonesia and recommends the use of CPI as an effective monitoring tool for controlling the cost of infrastructure projects.

Abstract

Musculoskeletal Disorders (MSDs) referred to complaints affecting the muscles and bones, ranging from sprains to chronic disorders that carried a risk of disability. Proper ergonomic implementation supported workers' health, comfort, and productivity, while poor ergonomics could trigger complaints. Identifying work positions, processes, and layouts was necessary to prevent MSDs. This study analyzed the effect of Workplace Stretching Exercise (WSE) on MSDs complaints among building construction project workers in Malang Regency. This study employed a quantitative approach with a Quasi-Experimental design and the Pretest-Posttest Control Group method. The study results indicated differences in MSDs complaints before and after the WSE intervention, demonstrating the effectiveness of the intervention. Ordinal regression analysis showed that several factors had a significant effect on MSDs complaints, including age 17-25 years (ρ=0.016), age 26-35 years (ρ=0.004), very low work posture (ρ=0.006), low work posture (ρ=0.008), work duration (ρ=0.007), low workload (ρ=0.023), moderate workload (ρ=0.015), and the WSE intervention (ρ=0.020). These findings indicated that, in addition to individual and work-related factors, the implementation of WSE played a role in reducing MSDs complaints among building construction project workers in Malang Regency. The researchers recommended that workers maintain optimal body posture, apply safe lifting techniques, avoid prolonged static positions, and perform regular stretching exercises as a preventive measure against musculoskeletal injuries

Abstract

Tonsillitis is one of the most common ENT disorders in children and may affect growth and overall quality of life. This study aimed to describe the clinical profile of pediatric tonsillitis patients at Royal Prima Hospital during 2023–2025. A retrospective descriptive observational design was used, involving 60 medical records that met the inclusion criteria. The results showed that the highest proportion of patients was aged 13–18 years (63.3%), with males being slightly more dominant (51.7%). Most patients had a normal nutritional status (70%), and the most frequent chief complaint was painful swallowing (58.3%). Physical examination revealed tonsillar enlargement predominantly at grade T3-T3 (55%). Chronic tonsillitis was more prevalent (66.7%) than acute tonsillitis (33.3%). Operative treatment was more commonly performed (55%) compared to medical therapy (45%). In conclusion, chronic tonsillitis remains a significant health issue in children, emphasizing the need for appropriate management to prevent complications and improve patient well-being.

Abstract

This study aims to systematically review the role of hospital image in building patient loyalty through a Systematic Literature Review (SLR) and bibliometric analysis approach. Articles were searched using a combination of Boolean keywords for the period 2020–2025. The screening process followed the PRISMA protocol, resulting in 30 articles that met the final criteria for thematic and bibliometric analysis using VOSviewer. The results showed four main clusters: (1) the relationship between hospital image, service quality, and satisfaction; (2) the reputation of doctors and institutions as determinants of trust; (3) brand image and patient experience as predictors of revisit intention; and (4) trust and service quality as the main mediating mechanisms. Publication trends increased rapidly after 2023, with the focus shifting from classic satisfaction-based models to a new paradigm emphasizing patient experience and trust. This study provides conceptual and practical contributions for hospitals in managing their image, strengthening their digital reputation, and building patient loyalty in a sustainable manner through services oriented towards patient experience and emotional value.

Abstract

This study aims to analyze the effectiveness of the digitalization policy of the Land and Building Acquisition Duty (BPHTB) in the Special Capital Region of Jakarta in promoting transparent, efficient, and accountable local tax governance. The research employs a descriptive qualitative approach, collecting data through interviews, observations, and document analysis involving the Regional Revenue Agency (Bapenda), Land Deed Officials (PPAT), and taxpayers using the e-BPHTB system. The findings indicate that the digital tax system has improved administrative efficiency and service transparency. However, its overall effectiveness remains limited due to technological infrastructure issues, uneven human resource capacity, weak inter-agency coordination, and low digital literacy among taxpayers. From the perspective of good governance, the e-BPHTB system fulfills the principles of transparency and accountability, but responsiveness and public participation require further enhancement. The study recommends strengthening system integration across institutions, providing digital training for tax officers and PPAT, improving taxpayers’ digital literacy, and establishing a regular policy evaluation mechanism to ensure the sustainability of digital tax governance effectiveness.

Abstract

Penelitian ini bertujuan untuk mengetahui pengaruh pemberian ekstrak Channa striata terhadap kadar Malondialdehida (MDA) dan gambaran histopatologi jantung pada tikus model sirosis hati. Penelitian ini merupakan penelitian eksperimental dengan rancangan randomized post-test only control group design menggunakan tikus putih jantan galur Wistar (Rattus norvegicus) sebagai hewan coba. Induksi sirosis hati dilakukan dengan pemberian karbon tetraklorida (CCl₄) selama enam minggu. Tikus dibagi ke dalam kelompok kontrol dan kelompok perlakuan dengan berbagai dosis ekstrak Channa striata. Parameter yang diamati meliputi kadar MDA jantung serta perubahan histopatologi jantung yang diamati melalui pewarnaan hematoksilin-eosin (HE). Hasil penelitian menunjukkan bahwa pemberian ekstrak Channa striata dapat menurunkan kadar MDA secara signifikan dibandingkan kelompok kontrol, serta memperlihatkan perbaikan struktur histologis jantung pada tikus model sirosis hati. Hal ini menunjukkan bahwa Channa striata memiliki potensi sebagai agen hepatokardioprotektif melalui mekanisme antioksidan yang dapat mengurangi kerusakan jaringan.

Abstract

This study aims to determine the effect of Channa striata extract on Malondialdehyde (MDA) levels and the histopathological features of the heart in liver cirrhosis model rats. This experimental research used a randomized post-test only control group design with male Wistar rats (Rattus norvegicus) as test subjects. Liver cirrhosis was induced using carbon tetrachloride (CCl₄) for six weeks. The rats were divided into control and treatment groups, each receiving different doses of Channa striata extract. The observed parameters included cardiac MDA levels as an indicator of oxidative stress and histopathological changes in heart tissue examined using hematoxylin-eosin (HE) staining. The results showed that Channa striata extract significantly reduced MDA levels compared to the control group and improved the histological structure of the heart in cirrhosis model rats. These findings indicate that Channa striata possesses hepatocardioprotective potential through antioxidant mechanisms that can reduce tissue damage caused by oxidative stress.

Abstract

Wound healing is a complex biological process involving cellular and tissue responses, including fibroblast formation and wound diameter closure. Berastagi orange peel extract (Citrus sinensis L.) contains flavonoids, tannins, and vitamin C, which play roles in anti-inflammatory, antioxidant, and collagen-stimulating processes, thus potentially accelerating wound healing. This study aimed to determine the effect of topical administration of Berastagi orange peel extract (Citrus sinensis L.) on the healing of incised wounds in rats based on fibroblast count and wound diameter. This research employed a quantitative analytic true experimental design with a post-test group design. Rats were divided into several treatment groups, namely gel base, 10% povidone iodine, and Berastagi orange peel extract gel (EEKJ) at concentrations of 40%, 60%, and 80%. The observed parameters were fibroblast count and wound diameter on day 0 and day 14. Data were analyzed using ANOVA followed by Post Hoc tests. The results showed an increase in fibroblast count and a greater reduction in wound diameter in the groups treated with EEKJ gel compared to the gel base control group. Compared to 10% povidone iodine, EEKJ gel also showed relatively comparable effects in accelerating wound healing. The 60% concentration demonstrated more optimal results in stimulating fibroblast proliferation and reducing wound diameter, although statistically, the differences between concentrations were not significant (p > 0.05). Topical administration of Berastagi orange peel extract proved effective in accelerating the healing process of incised wounds by increasing fibroblast count and reducing wound diameter. The 60% concentration was the most optimal compared to other concentrations, although the differences were not statistically significant.
