Abstract

Diabetes mellitus is a chronic disease that requires long-term treatment, which affects medication adherence. Adherence measurement can be done using a questionnaire like the Probabilistic Medication Adherence Scale (ProMAS). This study aims to determine the relationship between demographic factors and medication adherence scores in patients with diabetes mellitus. The research uses a quantitative research approach with a cross-sectional design. The convenience sampling method was used to select 112 respondents between January and April 2023 at various primary healthcare facilities in the Special Region of Yogyakarta. The ProMAS scoring results were based on the highest rankings, with 42.8% of respondents having a moderate-to-high adherence level, 41.1% of respondents having a high adherence level, 15.2% of respondents having a moderate-low adherence level, and 0.9% of respondents having a low adherence level. Diabetes mellitus patients in Yogyakarta show a high level of medication adherence. The most frequent non-adherent behavior was found in 4 questions: respondents forgot to take or inject medication (61.61%); respondents took or injected medication (one of them) slower than usual schedule (55.36%); respondents did not always take or inject medication at the same time every day (62.5%); and respondents forgot to take or inject medication at least once in the past month (58.04%). The statistical analysis indicates no relationship between demographic characteristics, the total number of medications, and the total frequency of medication with adherence levels. This indicates that diabetes mellitus patients with low or high adherence levels are unaffected by the above factors.

Abstract

Research on effective antivirals for COVID-19 is still ongoing. Favipiravir and remdesivir are antivirals that have been recommended in Indonesia. This study aimed to compare the clinical outcomes of favipiravir and remdesivir in moderate COVID-19 patients. This research is a retrospective cohort study conducted at the UGM Academic Hospital Yogyakarta using electronic medical records of moderate COVID-19 who were hospitalized from June 2021 to March 2022. Each therapy group consisted of 88 subjects. Clinical outcomes in the form of improved and not-improved conditions were assessed using a 7-point ordinal scale of progressivity and recovery of COVID-19 from WHO. Chi-square analysis and multiple logistic regression were performed to determine the relationship between the research variables. The highest percentage of research subjects were aged 18 – 59 years (70,5%), male (53,4%), and not obese (64,2%). The most common comorbidities were diabetes mellitus (33%) and hypertension (30%). The favipiravir group had a better clinical outcome than remdesivir. The proportion of patients improving in the favipiravir group was 50,0% and 35,3% in the remdesivir group (p-value=0,048). There was no significant difference in the number of adverse events between both study groups (p-value > 0,05). The most common adverse drug events were nausea (35,2%), vomiting (14,2%), and heartburn (11,4%).

Abstract

Community-acquired Pneumonia (CAP) is an acute infectious disease that affects the tissues (lungs) precisely in the alveoli acquired outside the hospital. The incidence of pneumonia increases with age due to changes in physiologic and immunological status associated with aging and the presence of comorbidities in old age. Pneumonia is the biggest infectious disease in Mardi Rahayu Hospital from 2020 to 2022. Inappropriate selection of empiric antibiotics causes antibiotic resistance and their prolonged use can increase the length of stay so that the cost of treatment increases. Rationale analysis of the use of specific empiric antibiotics in geriatric patients with CAP at Mardi Rahayu Hospital has never been carried out. This study aims to determine the rationality of empirical antibiotic therapy in geriatric patients with CAP. This study used a descriptive and analytic observational design with a cross-sectional method. Analysis of the rationality of empirical antibiotic therapy using the Gyssens method. The results showed that the number of geriatric patients diagnosed with CAP who were hospitalized at Mardi Rahayu Hospital from January to December 2022 who met the inclusion criteria was 139 patients with a total of 158 antibiotic regimens. The results of the rationality analysis showed that as many as 95 antibiotic regimens (60.1%) were included in the 0 (zero) category, which means rational use of antibiotics. Irrational use of antibiotics occurred in 63 regimens in category IV-A as 40 cases (25.3%), IV-B as 9 cases (5.7%), IV-C as 2 cases (1.2%), III- A as 9 cases (5.7%), III-B as 20 cases (12.6%) and II-A as 2 cases (1.2%).

Abstract

Background: The use of traditional medicines globally continues to increase and must be balanced with increased knowledge, attitudes, and practice in using traditional medicines through pharmacist education and mentoring.Objectives: This research aims to determine the description of the use of traditional medicine, the level of knowledge, attitudes, and practice of the community in using it, and the influence of pharmacist education and mentoring on it.Methods: This research design was quasi-experimental with a pre-test and post-test control group with 406 community research subjects. It involved 14 pharmacists in providing education and mentoring through WhatsApp group media by providing material according to the modules prepared. The instrument used a questionnaire developed from previous research and tested for validity and reliability. Data collection was carried out prospectively by the participant-self, and analyzed using the Wilcoxon Signed Ranks Test.Results: This study's total number of respondents was 406, divided into 203 as the intervention group and 203 as the control group. The description of the respondent's KAP level before the intervention is in the medium category and after the intervention is in the high category based on the Wilcoxon Signed Ranks Test results at a 95% confidence level (α=0.05) in the intervention group.Conclusion: The use of traditional medicines in the phytopharmaca class is 3%, standardized herbal medicines are 35%, and jamu is 62%. The level of knowledge, attitudes, and practice of the community in using traditional medicine in Mojokerto Regency is, on average, in the medium category. This research also shows the influence of pharmacist education and mentoring on increasing knowledge, attitudes, and practice scores from medium to high.

Abstract

Background: Ethylene glycol (EG) is an antifreeze substance commonly used as an additive for syrup preparations. One of the detrimental effects caused by EG toxicity is acute renal failure (ARF) and death, assuming therapy and management are delayed. Presently, there are limited studies on toxicity of EG and therapy. Therefore, this study aimed to provide an overview of EG toxicity levels, clinical manifestations, arising reactions, and therapy management methods.Objectives: A narrative review design was employed with data collected from PubMed and Google Scholar. The strategy used keywords such as “ethylene glycol toxicity” and "acute renal" with the assistance of Boolean operators including AND and OR.Methods: The result showed that the minimum EG level for toxicity was at 22 mg/dL, followed by metabolic acidosis with an increased anion gap (>10mmol/L). Based on case reports from one experimental study, the clinical manifestations of patients experiencing EG poisoning included decreased consciousness, tachycardia, and coma, with ARF occurring after 24-72 hours.Results: Write study findings in this section.Conclusion: EG poisoning could cause kidney damage in the form of ARF and lead to death, assuming therapy was delayed. Presently, the most effective therapy for EG poisoning was fomepizole.

Abstract

Background: Drug management information systems can serve as tools for decision-making. The effectiveness of such a system can be assessed through evaluation. One of the evaluation methods used is the PIECES method.Objectives: This research aims to evaluate and develop a drug management information system using the PIECES method.Methods: The research conducted was a qualitative descriptive study with inductive analysis. Sampling for this research was conducted purposive sampling. System development in this study utilized the prototype method, with interviews conducted for Management Information System (MIS) evaluation using the PIECES Framework. The design of the MIS system was created using Data Flow Diagrams and Entity Relationship Diagrams. The designed MIS system development was evaluated using the same method. Data processing in qualitative research consists of valid, clear, and accountable word descriptions, sentences, or narratives provided by the researchers, followed by data validation using triangulation techniques and Nvivo 12 Plus software.Results: Evaluation results using the PIECES Framework indicate that the implemented MIS system benefits and aids staff performance in daily drug dispensing. However, there are still manual tasks and lacking features such as distribution, drug expiration warnings, and minimum stock on the dashboard. The proposed design for MIS system development includes several features such as stock in, stock out, and distribution, complemented with drug expiration warnings, minimum stock alerts on the dashboard, and reports for each feature.Conclusion: Implementing the proposed MIS system design can assist and streamline work processes by providing comprehensive information, thus serving as a decision-making tool.

Abstract

Background: Pharmacies should improve people's quality of life by providing pharmaceutical services. In this case, the distribution and accessibility of pharmacy facilities play an essential role.Objectives: This research aims to provide an overview of the distribution of pharmacies in Bandar Lampung municipality and analyze their distribution and accessibility.Methods: Non-experimental research with descriptive methods was conducted using spatial analysis. All population pharmacies in Bandar Lampung were selected as a sample, and their location was pointed using the Garmin Global Positioning System (GPS). Arc-Gis software was used for data analysis. A nearest-neighbor analysis was conducted to get a distribution pattern, and the pharmacies' density was calculated to obtain accessibility data. The overlapping approach was carried out to determine factors related to pharmacy distribution patterns.Results: Bandar Lampung municipality has 268 pharmacies spread across 20 districts. This research shows that the average density of pharmacies per 10,000 residents is 2.26. The highest pharmacy accessibility is in Sukarame, with a ratio of 4.07, and the lowest is in Bumi Waras, with a ratio of 0.62. The distribution of pharmacies is a clustered pattern (T value 0.55) and centralizes on collector roads, residential urban land use, and health facilities.Conclusion: The distribution of pharmacies is odd. They are distributed in a cluster pattern.

Abstract

Background: Diabetes is one of the top 10 causes of death worldwide. Many factors increase the risk of mortality and morbidity in patients with diabetes such as age, sex, BMI, educational attainment, and marital status.Objectives: The study aimed to analyze clinical outcomes based on patient demographics.Methods: This method was cross-sectional and was conducted at Antapani Medika Clinic, Bandung. Inclusion criteria for this study were diabetic patients who had complete medical records, and who were receiving oral antidiabetic therapy for at least 6 months. Exclusion criteria for study were patients with diabetes who were on insulin therapy, patients with tuberculosis, and pregnant and lactating women. Data on demographic characteristics were analyzed descriptively. Chi-square and likelihood ratio tests were used to analyze differences in clinical outcomes based on therapy achievement on patient demographics.Results: The results showed that HbA1c target levels were achieved by patients who were 65 years of age or older (68%), had a BMI in non-obese category (80.8%), had diabetes for six years or more (80.8%), were married (62.9%), attended college (68.4%), exercised 3 to 6 times per week (64.9%), did not smoke (61.9%), and had no comorbidities (65.3%). This study concludes that there is an association between BMI and duration of diabetes and achievement HbA1c target (p<0.05).Conclusion: The clinical implications of this study are as a guide for health workers in Indonesia in management of diabetes so that they can provide services to patients according to the medical needs of patients.

Abstract

Diabetes mellitus (DM) is a metabolic disease characterized by increased blood sugar levels (hyperglycemia) and is associated with impaired function of β cells. The uncontrolled and prolonged hyperglycemia in DM can cause complications. One of the most common is diabetic non-healing wounds, the leading cause of disability in people with diabetes, therefore adjuvant therapy is needed. Compounds from natural ingredients are currently getting the spotlight in recent studies conducted to prove their effect on non-healing wounds, such as Resveratrol (RSV). This review aims to discuss the potential of RSV as a novel adjuvant therapy and its delivery system strategy in the case of diabetic non-healing wounds. We conducted an extensive search from three online scientific databases such as ScienceDirect, ResearchGate, and PubMed on February-March 2023. Keywords used are "resveratrol", "diabetic wound healing", and "diabetes mellitus". As a result, 86 studies were reviewed, and only 79 selected studies met all inclusion criteria. The formulation of RSV nanoparticles in oral drug delivery systems and topical administration has shown promise to overcome the limitations of RSV as adjuvant therapy in diabetic non-healing wounds. The best option that we found through this literature review is the nano-oral drug delivery system as it could minimize drug metabolism in the gastrointestinal tract, therefore the drug bioavailability and drug concentration on the target site could be maximized, even though the systemic side effects might occur.

Abstract

Inappropriate use of antibiotics due to lack of understanding and public knowledge is one of the causes of antibiotic resistance. Providing education via video to the public is a strategy that can be used to increase knowledge about the use of antibiotics. This study aims to assess the effect of providing local language educational videos about antibiotics and their use on increasing the knowledge and attitudes of women living in the Banjar Regency area, South Kalimantan. This research is Quasi-experimental research with a pretest posttest intervention with a control group research design using a purposive sampling technique and an intervention in the form of educational videos in the Banjar language. A valid and reliable questionnaire was used to assess knowledge and behavior in using antibiotics. Analysis of changes in knowledge and behavior scores between the control and intervention groups was carried out with the Mann-Whitney test. The number of respondents involved in the study was 80 women who were divided into a control group (40 people) and an intervention group (40 people). The results showed that the average pre-test and post-test knowledge scores in the control group (7.58 ± 1.81 and 7.85 ± 2.11) and the intervention group (7.33 ± 2.30 and 8.78 ± 2.93) with a significance value of p = 0.026. The mean scores of pre-test and post-test behavior in the control group (6.03 ± 2.60 and 6.15 ± 2.70) and the intervention group (6.33 ± 2.37 and 7.50 ± 2.05) with a significance value of p = 0.016. It can be concluded that providing educational videos in Banjar language can increase Females knowledge and behavior in using antibiotics in Banjar Regency.
