Abstract

Background: The low performance of nurses' services is a hotly discussed problem with several complaints from patients as health service users. Many factors affect the performance of excellent service both internally and externally. Purpose: to analyze the determinants of what factors support nurses in performing excellent service performance. Methods: quantitative design with cross-sectional approach. The sample size was 87 nurses from the entire population with D3 and NERS education, at least 6 months working in 6 surgical medical rooms with total sampling technique. The measurement instrument is a checklist and questionnaire consisting of statement items with 4 Likert scales. Validity and reliability tests on 30 respondents, the results r count> r table 0.361 with Cronbach alpha> 0.6. Descriptive, Pearson product-moment correlation and logistic regression were used to analyze the data with α=0.05. Results: The variables of perception of workload (p=0.004; Exp (B)=4.591), perception of leadership (p=0.012; Exp (B)=3.729), and motivation (p=0.018; Exp (B)=3.579) simultaneously affect the performance of excellent service by 33.5%. Conclusion: The determinant factor of nurses' excellent service performance is workload. Efforts are needed to increase work motivation, through suitability of salary/benefits received, positive policies from management, supervision, human relations, working conditions, and good organizational culture.

Abstract

Background: Non-hemorrhagic strokes result in neuromuscular disorders with reduced activity and weakness in joint movement and even disability. This condition receives treatment at the hospital in the form of educational therapy, diet, medication and ROM (active and passive). Purpose: The aim of the research was to determine the effect of Mirror Therapy on extremity muscle strength in non-hemorrhagic stroke patients based on self-care theory. Methods: Quasi experiment research design with a pretest posttest approach with control group design. The population of all non-hemorrhagic stroke patients with a sample size of 40 people, divided into 2 groups, namely the control group and the intervention group, was selected using a purposive sampling technique. The instruments used in this research include respondent characteristics, MMT (Manual Muscle Testing) observation sheets and Mirror Therapy SOP. Data analysis used the Mann Whitney U-test with a significance value of P value < 0.05. Results: The results of the study showed that the average muscle strength score in the Mirror Therapy intervention group was 1.55 before and after 2.65, while in the control group with standard hospital treatment before 1,40 and after 1.95. The Wilcoxon Test results show P value = 0.000 in the intervention group and P value = 0.000 in the control group, meaning there is an increase in extremity muscle strength in the intervention and control groups. Based on the Mann Whitney test, it shows P value = 0.013, meaning there is no differences in Mirror Therapy on extremity muscle strength in non-hemorrhagic stroke patients. Conclusion: The application of Mirror Therapy affects the level of extremity muscle strength in non-haemorrhagic stroke patients. Nurses are expected to be able to apply Mirror Therapy to apply it in nursing practice.

Abstract

Background: Traffic accidents remain a major public health concern globally, particularly in developing countries like Indonesia, which ranks among the highest in road traffic-related mortality. Indonesia, as one of the fourth most populous countries in the world, is at greater risk for increased mortality from traffic accidents. Lower limb fractures are frequently associated with considerable morbidity and lengthy hospital stays. Purpose: This study identifies the perceptions and experiences of continuity of care (COC) nurses in lower limb fracture patients. Methods: This research utilizes a descriptive qualitative model with a phenomenology approach. Researchers recruited respondents after obtaining ethical clearance and an approval letter from West Sulawesi Provincial Hospital. In this study, 5 participants were determined according to the inclusion criteria set by the researcher. The data collection process was carried out by conducting a Focus group Discussion (FGD) for 40–60 minutes. During the FGD, the researchers used FGD notes and field notes. The analysis in this study used content analysis. Results: This study explored nurses' perceptions and experiences of continuity of care (COC) fracture patients in the lower extremity. This study identified four themes, namely, continuity of care (COC) in fracture patients, perceived physical discomfort, support needs in ambulating, and support for improving nurse competency. Conclusion: The implementation of COC can be a solution to the physical discomfort experienced by patients. Nurse support, family support, interprofessional support, and hospital support are needed so that COC can be implemented properly to improve musculoskeletal recovery and sustainable independence in lower limb fracture patients. The hope of the participants, in addition to motivation and the availability of tools for ambulation exercises, should be provided in the surgical room, especially in the treatment room for patients with musculoskeletal system disorders.

Abstract

Background: Many healthcare facilities have implemented evidence-based guidelines and best practices for preventing Central line-associated bloodstream infections (CLABSI), and nurses often actively adhere to these protocols. They may collaborate with infection control teams, implement proper hand hygiene practices, use sterile techniques during catheter insertion and maintenance, and monitor patients closely for signs of infection. While most cardiovascular nurses are diligent in preventing CLABSIs, challenges may arise due to factors such as heavy workload, time constraints, staff shortages, and variations in adherence to infection control protocols among healthcare workers. Purpose: Therefore, our research study aims to explore the underlying factors contributing to the increase of CLABSI despite the availability of prevention guidelines and bundles of care in a government tertiary-military specialized cardiac center in Saudi Arabia. Accordingly, the study explored the perception and attitude of cardiovascular nurses toward preventing Central line-associated bloodstream infections (CLABSI).  Methods: This is an exploratory qualitative research study in which Prince Sultan Cardiac Center nurses participated.  An invitation link was distributed to the target participants for a focus group interview. Then, the data collected was transcribed verbatim for content analysis for repeated patterns of themes. Results: The study revealed three main themes, namely, handling (i.e., the importance of handling a central line properly for cardiac patients), competency (i.e., the need for reconsidering the evaluation frequency of management of central line competency), and actual practice (i.e., aspects related to the actual practice). Conclusion: Accordingly, several sub-themes emerged as a result. Implications of the study and recommendations were made.
