Abstract

Premature ventricular complexes (PVCs) are arrhythmias due to abnormal impulse formation in the ventricles. The incidence is detected as much as 1% on standard electrocardiographic (ECG) examination. The aim of this literature review is to explain the diagnosis, evaluation, and management of PVC. The diagnosis of PVC is determined from the symptoms and evidence from the ECG recording. Symptoms may include a faster heartbeat sensation, chest discomfort, skipped beat, and intermittenly strong heartbeat. Evidence of PVC is established by regular ECG recordings or Holter. A 12-ECG can help determine the location of source. PVC evaluation includes determine the dangerousity, the next examination step, the treatment options, and whether ablation is needed. Managements include medical therapy and catheter ablation. Catheter ablation is performed where treatment is unsatisfactory or if the PVC causing life threathening arrhythmia or cardiomyopathy. PVC is considered dangerous if the underlying disease is found in the form of cardiac structural abnormalities, ischemic heart disease or other arrhythmic disorders, more than 2000 PVC is found in 24 hours, complex morphology, different morphology, increased with exercise, PVC with non-outflow tract form, with short coupling interval, and with a wider QRS duration The incidence of VVC that is considered to be able to induce cardiomyopathy is from 10-26%. PVC requires a structured and systematic diagnosis and evaluation step to determine the appropriate treatment.

Abstract

Weight gain can put you at risk of developing non-communicable diseases. Even during this pandemic, obesity is one of the risk factors associated with an increase in COVID-19 infection. However, the implementation of Community Activity Restrictions (PPKM) caused a lack of activity during COVID-19 and irregular eating patterns caused them to gain weight. In addition, obesity has an impact on a decreased quality of life. The purpose of the study was to determine the average increase in body weight during the covid pandemic and the relationship between obesity and the quality of life for women. The design of this research is cross-sectional. The research was conducted at the Ingin Jaya Health Center in August. Sources of data used are primary data and secondary data. The sample in this study were women of childbearing age (WCBA) in the work area of the Ingin Jaya Health Center, Aceh Besar. The results of this study are the average weight value of WCBA before the covid pandemic is 56.48 kg and during the covid pandemic it is 59.46 kg. There is a relationship between education and work variables on the quality of life in women of childbearing age (WCBA) in the work area of the Ingin Jaya Health Center

Abstract

Leprosy and the disability it causes, have a negative stigma in community; however only a few reports have investigated the link between leprosy and psychiatric morbidity such as depression. This study aimed to asses correlation between leprosy disability grade and depression severity among people with leprosy in Sicanang, Belawan, North Sumatera. This study was unpaired categorical comparative analytic study and 54 participants were enrolled to this study. We found a high percentage of leprosys disability, 88,9% participants were having second grade leprosys disability that means the disability was obviously seen. We found that 29,63% participant suffered from a moderate depression, 14,82% with severe depression and 10,27% suffered from extreme depression. The correlation between disability grade and depression severity was analyzed with chi square test and p value was 0,182 which means that no significant relationship between disability grade and depression severity. Depression severity among people with leprosy in Sicanang was determined by patients self perception, spouse, onset of diagnosis and patients perception about their religion.

Abstract

In schizophrenic patients, symptoms of depression are often found which can lead to adverse effects on the treatment process, such as increasing the length of treatment, increasing the possibility of relapse, non-adherence in taking medication, and can also lead to suicide in schizophrenic patients. In schizophrenic patients, the activity of the central angiotensin converting enzyme (ACE) gene is altered and angiotensin II interacts with central dopamine, thereby contributing to the pathophysiology of schizophrenia. In addition, it is likely that the ACE gene variant is also involved in the development of depressive symptoms in schizophrenic patients. This research is a categorical analytic study with a cross sectional method, the sampling method is non-probability sampling with consecutive sampling type, with a total sample of 42 people. There was no significant difference between depression symptoms and ACE gene polymorphisms with P value = 0.746. There was no significant difference between depression symptoms and ACE gene polymorphisms.

Abstract

Gastrointestinal diseases are one of the health problems included in the global burden of disease. Examination to see the mucosa and lumen of the upper digestive tract directly ranging from the oropharynx, esophagus, stomach to proximal duodenum using an endoscope device called Gastroscopy (Gastrointestinal Endoscopy (GIE)/Esophagogastrictroduodenoscopy (EGD)). The purpose of this study was to find out the characteristics of patients who conducted gastroscopic examinations at RSUD Drs. H. Amri Tambunan period 2020-2021. This study is a descriptive study that used cross sectional design and total sampling method which is as many as 510 patients who meet the criteria for inclusion of gastroscopy examination. The results of this study showed that gastroscopic examinations were most commonly performed in male patients (52.4%), with the most age range of 41 - 60 years (49.6%), the most indications are epigastric pain (28%), and the most gastroscopic examination results are gastritis (22%). Patients with complaints of epigastric pain are often found at the diagnosis of gastritis. Epigastric pain is the chief complaint of patients most in gastrointestinal diseases, especially the upper gastrointestinal tract. This corresponds to the gastric organ which is the location of the main lesion on the examination of gastroscopy.

Abstract

Water is an essential nutrient needed by the body. Humans cannot produce water to meet their bodily fluid needs. Data in 2016 on fluid intake in Indonesia shows that 22% of children aged 4-9 have deficiency fluids, 21% in children aged 10-17 years, and 28% at 18-65. This study aimed to analyze the relationship between the level of knowledge about body fluids and hydration status in medical students of the Faculty of Medicine, University of North Sumatra. This type of research is analytic with a cross-sectional design and using a simple random sampling technique with 99 students as the research sample. The variables studied were the level of knowledge measured by a questionnaire and the hydration status obtained by comparing the urine color with the PURI card. The results showed that most of the samples had a moderate level of knowledge about fluids (51.5%) and were dehydrated (48.5%). From the results of statistical tests using Chi-squared with ? value = 0.05, it is known that there is a relationship between the level of knowledge and hydration status (p = 0.034).

Abstract

Sugar sweetened-beverages (SSB) is not only found in soft drinks, fruit juice drinks, or energy drinks, but also various drinks that are currently "trending" such as boba drinks with various flavors, coffee or milk coffee, even tea or milk tea with various flavors. The lifestyle of consuming drinks that are high in sugar will lead to a tendency to several diseases such as prediabetes, diabetes mellitus (DM), metabolic syndrome and cardiovascular disease. Prevention and screening of prediabetes and DM at a young age in Indonesia must be increased, especially in high-risk groups because most of the initial conditions of the disease are asymptomatic. The aim of this study is to analyze the association between frequency of sugar sweetened-beverages (SSB) consumption and prediabetes in medical students. This study was an observational analytic study with cross sectional design conducted at Medical Faculty Universitas Muhammadiyah Sumatera Utara. The subjects of this study was 70 subjects. Examination of frequency of SSB consumption was carried out with a validated questionnaire and food frequency questionnaire (FFQ) of nutrient intake for the past 24 hours. There is no significant relationship between gender, family history of DM, smoking history, physical activity and hypertension with prediabetes. However, there is a significant relationship between obesity and prediabetes with p value 0,010. Meanwhile, the relationship between frequency of SSB consumption and prediabetes showed with p 0.05. There is no significant relationship between frequency of SSB consumption and prediabetes, but there is significant relationship between obesity and prediabetes.

Abstract

Cervical cancer (CC) is still one of the most common malignant tumors in women worldwide. Malignant tumors are known for evading immune surveillance. In cervical lesions, the mechanism has received less attention. Evasion of immune surveillance through a variety of mechanisms; one of which is the immune checkpoint pathway has attracted interest because it can be therapeutically targeted, the programmed cell death 1 (PD-1) pathway that tumor cells use to block antitumoral immune responses. Increasing the prevalence of PD-L1 expression in up to 80% of cervical malignancies, there is a significant indication using immunotherapy to maintain the immune response against malignancies. The purpose of this literature review is to examine the potential role of immune checkpoint inhibitors in the treatment of cervical neoplastic lesions, by reviewing articles on cervical intraepithelial neoplasia (CIN) and cervical cancer that carried out histopathological and immunohistochemical examination PD-L1 as well as immunotherapy treatment with anti-PD-L1. There were 10 articles were found to be associated with anti-PD-1/PD-L1 immunotherapy. In conclusion, PD-L1 is a potential therapeutic target in cervical cancer

Abstract

Management therapy of autoimmune hemolytic anemia (AIHA) with severe clinical symptoms requires supportive therapy such as blood components transfusion to improve hemodynamic conditions. This literature study aims to explain the role of blood transfusion therapy in AIHA cases. The method used is through searching articles in national and international journals published in the last 10 years that discuss transfusion in AIHA cases using the Google Scholar application. From the search results, it can be concluded that blood transfusion is a supportive therapy in cases of AIHA with symptoms of severe hypoxemia that interfere with hemodynamics and work function of body organ systems and findings of Hb levels 7 mg/dL with the selection of blood components in the form of packed red cells (PRC), washed packed red cells (WPRC) and Leucoreduction (LR).

Abstract

Background: Lipids come in from many different sources in ones diet. In this experiment, we intend to find out the kind of lipids that causes the highest increase in rat body weight and the most influence in the lipid profiles. Method: We compared three kinds of lipid: beef fats, egg yolk and palm oil which were added to the rats standard diet. Twenty-eight 20-24 week-old male Sprague-Dawley rats weighing 250300 g were randomly divided into 4 treatment groups given a high beef fat diet (BF), a high egg yolk (EY), a high vegetable fat (VF), and a standard chow diet (SD) as control for 30 days. The body weight of each rat was monitored weekly and the lipid profiles were measured enzymatically at the end of the experiment. Result: The EY and VF body weights increased significantly compared to the SD (p0.05). The total cholesterol level of BF, EY and VF increased significantly compared to the SD groups (p0.001). The triglycerides level of BF, EY, and VF increased significantly compared to SD (p0.001). The HDL-cholesterol levels were significantly lower in BF, EY and VF compared to SD (p0.05). The LDL-cholesterol in BF, EY and VF were significantly increased compared to SD (p0.05). Conclusion: High egg yolk diet causes a bigger increase in rat body weight, LDL levels, cholesterol levels, and triglycerides than the high beef fat diet and high vegetable fat diet compared to control. It also causes lower HDL levels than the other diet groups compared to control.
