Abstract

Background: Stunting is a condition of impaired growth and development in children under five years of age, resulting from chronic inadequate nutrient intake and recurrent infections, and is characterized by height-for-age below the standard reference. Nutritional improvement efforts for stunted toddlers commonly include the provision of supplementary foods. One potential locally based food product is chicken nuggets enriched with dali horbo. Dali horbo is a traditional Batak fermented food made from buffalo milk, which is rich in fat, protein, and carbohydrates and contains lactic acid bacteria. Objectives: This study aimed to determine the optimal formulation of chicken nuggets substituted with dali horbo based on organoleptic evaluation and proximate nutrient composition. Methods: A Completely Randomized Design was applied using mixtures of chicken meat and dali horbo with three formulations: A (70% chicken meat: 30% dali horbo), B (60% chicken meat: 40% dali horbo), and C (50% chicken meat: 50% dali horbo). The best formulation from the organoleptic test was subsequently subjected to physical and chemical property analyses. Results: The most preferred formulation for supplementary food for stunted toddlers was formulation C (50% chicken meat: 50% dali horbo). Proximate analysis of the selected product showed moisture content of 47.03%, ash 1.88%, protein 10.60%, fat 22.99%, and carbohydrate 17.49%. The nutrient profile met the Indonesian National Standard (SNI) for nuggets, except for fat content, which exceeded the SNI limit. Conclusions: Dali horbo nuggets may be used as a supplementary food intervention for stunted toddlers.

Abstract

Background: Psychobiotics, including probiotics, prebiotics, and synbiotics, have been proposed to improve mental health through modulation of the gut–brain axis. However, findings from randomized controlled trials (RCTs) remain inconsistent, Objectives: To systematically review and synthesize evidence on the effects of probiotics, prebiotics, and synbiotics on depression, stress, and mood in adult populations. Methods: A comprehensive search was conducted in PubMed, yielding 531 articles, of which 14 RCTs met the inclusion criteria. Risk of bias was assessed using the Cochrane Risk of Bias Tool. Meta-analyses were performed using both fixed- and random-effects models in RevMan. The study protocol was registered in PROSPERO (CRD420251103338). Discussions: Ten RCTs (n = 881) assessing depression did not show consistent benefits (SMD = –0.72; 95% CI: –1.42 to –0.01; p-value = 0.05; I² = 96%). Six RCTs (n = 444) evaluating stress also showed non-significant effects (MD = –1.35; 95% CI: –3.70 to 1.01; p-value = 0.26; I² = 73%). Conversely, six RCTs (n = 376) demonstrated significant improvements in mood (MD = 0.73; 95% CI: 0.25 to 1.20; p-value = 0.003; I² = 75%). Meta-regression indicated that larger sample sizes were associated with more consistent improvements in mood (p-value = 0.04). Sensitivity analyses confirmed the stability of the findings, and no evidence of publication bias was detected. Conclusions: Probiotic, prebiotic, and synbiotic supplementation may provide beneficial effects on mood and could serve as a potential nutritional intervention to enhance psychological health. However, effects on depression and stress inconsistent.

Abstract

Background: Adequate intake of protein and calcium is essential for muscle contraction and optimal athletic performance, yet hypocalcaemia and protein deficiency remain prevalent among Indonesian athletes. Innovation in locally based sports foods is therefore required. The combination of protein-rich winged bean (Psophocarpus tetragonolobus L.) seed flour and easily digestible arrowroot starch offers potential for developing a nutritious and acceptable steamed sponge cake. Objectives: To evaluate the effects of winged bean seed flour and arrowroot starch in steamed sponge cake formulations on nutrient composition, calcium content, and organoleptic properties as a sports food product. Methods: A completely randomized design was applied with three formulations: F1 (25%:75%), F2 (50%:50%), and F3 (75%:25%) ratios of winged bean seed flour to arrowroot starch, with two replications. Nutrient composition was analyzed using ANOVA followed by Duncan’s multiple range test, while organoleptic properties were assessed using the Kruskal–Wallis and Mann–Whitney tests. The best formulation was determined using the De Garmo method. Results: Ash content, total fat, energy from fat, total energy, carbohydrate, protein, and calcium differed significantly among formulations (p-value <0.001), whereas moisture content showed no significant difference (p-value >0.001). Organoleptic testing indicated no significant difference in color (p-value >0.001), while texture, aroma, and taste differed significantly (p-value <0.001). The optimal formulation was F3, containing 75%-winged bean seed flour and 25% arrowroot starch. Conclusions: Winged bean seed flour and arrowroot starch significantly influenced the nutritional and organoleptic characteristics of steamed sponge cake, highlighting its potential as an innovative local food–based sports food.

Abstract

Background: The COVID-19 pandemic and the implementation of Large-Scale Social Restrictions (LSSR) in Indonesia have raised concerns regarding mental health and behavioral changes, including eating patterns. Psychological stress and uncertainty related to social and economic limitations may increase the risk of eating disorders and ultimately affect nutritional status. Objectives: This study aims to investigate the relationship between eating disorders and nutritional status among adults during the COVID-19 pandemic in Indonesia. It also tries to identify factors influencing both conditions. Methods: This study used a cross-sectional research design. Secondary data were obtained from the IndoNutriLife-COVID-19 online survey, involving 287 adults. Collected variables included sociodemographic characteristics, eating disorders, nutritional status, and LSSR status. Data were analyzed using THE Chi-square test and logistic regression analysis. Results: Nearly half of the participants (49.8%) lived in LSSR areas. A total of 16% of participants experienced eating disorders, while the others were underweight (8.4%), overweight (25.4%), and obese (9.1%). LSSR status was not significantly associated with eating disorders or nutritional status. Eating disorders and nutritional status did not show a significant relationship. Age and sex were significantly associated with nutritional status (p-value = 0.02; p-value < 0.001). Conclusions: LSSR status is not associated with eating disorders or nutritional status during the COVID-19 pandemic. Maintaining balanced dietary intake and overall well-being remains essential during periods of activity restriction.

Abstract

Background: Adolescence is a vulnerable period for nutritional problems due to changes in nutritional needs and eating behaviors. In Surabaya, the prevalence of overweight among adolescents has reached 14.46%. One contributing factor is frequent consumption of discretionary foods, which are energy-dense and low in essential nutrients. Objectives: This study aims to analyze the relationship between balanced nutrition behaviors and the frequency of discretionary food consumption with adolescent nutritional status at SMA Negeri 4 Surabaya. Methods: An analytic observational study with a cross-sectional design was conducted among 83 students in the first and second grades of a higher secondary school, who were selected using proportional random sampling. Data were collected using a balanced nutrition behavior questionnaire, a Semi-Quantitative Food Frequency Questionnaire (SQ-FFQ) to assess the Balanced Nutrition Index 3-60 (IGS3-60), and an FFQ to measure discretionary food consumption. Nutritional status was determined using BMI-for-age based on WHO-AnthroPlus. The obtained data were analyzed using Pearson and Spearman correlation tests. Results: Most respondents had poor balanced nutrition behavior (51.8%). Discretionary foods rich in added sugars, fats, and sugar–fat combinations were frequently consumed on a weekly basis. Significant associations were found between discretionary food consumption and nutritional status, including high added sugars (p-value=0.015), high fat (p-value=0.013), and both high fat and added sugars (p-value=0.017). Conclusions: Balanced nutrition behaviors are not significantly associated with nutritional status, while frequent consumption of discretionary foods is related to higher nutritional status. School-based nutrition interventions are needed to prevent overnutrition among adolescents.

Abstract

Background: Stunting remains a significant public health issue in Indonesia. Low knowledge and perception of mothers about nutrition, health, and the First 1000 Days of Life are determining factors. Objectives: To observe the impact of home visits on the knowledge and perception of maternal children about the Zero New Stunting (ZNS) program. Methods: This study employed a mixed-methods approach with a sequential explanatory design, conducted from June to August 2025 in Sumberadi Village, Sleman, Yogyakarta, Indonesia. The subjects were 35 stunted toddler pairs with mothers who met the inclusion criteria. Interventions are integrative nutrition education packages through home visits by trained Posyandu cadres, nutrition education (growth monitoring, developmental stimulation, and IYCF), and counseling. A structured questionnaire containing knowledge and perception questions of 25 questions each was used. The analysis was carried out using a t-test to see the differences before and after the intervention. Meanwhile, qualitative data were analyzed manually according to the theme of the interview. Results: The mean knowledge score increased from 61.2±9.8 (pre-test) to 86.4±7.1 (post-test 3), with a mean difference of 25.2 points (p-value<0.001). The perception score increased from 58.4±10.2 to 82.4±7.4, with an average difference of 24.0 points (p-value<0.001). Meanwhile, the results of qualitative findings showed high acceptance of the program, including increased self-efficacy, emotional support, and practical guidance during home visits. Conclusions: Home visits by cadres with integrated nutrition education effectively improve mothers’ knowledge and perceptions on stunting prevention, supporting the ZNS program.

Abstract

Background: Food safety in school environments is a key factor in supporting the continuity of educational activities. However, monitoring of hygiene and sanitation practices among school food vendors remains suboptimal. Unsafe or contaminated food can pose serious health risk to students. Poor hygiene and sanitation practices by food vendors are a key contributor to foodborne illnesses in schools. Objectives: This study aimed to evaluate the hygiene and sanitation practices of school food vendors in Yogyakarta and the factors influencing the implementation of these practices. Methods: A cross-sectional study employing a quantitative observational design was conducted in nine elementary schools in Sleman, involving 60 food vendors operating in the surrounding school areas. Associations between independent variables (age, gender, educational background, years of experience, training history, knowledge level, and attitude) and hygiene and sanitation practices were assessed using the chi-square test. Logistic regression analysis was performed to examine the model and identify factors influencing the implementation of hygiene and sanitation practices among school food vendors. Results: The study found that 55% of respondents did not adequately implement hygiene and sanitation practices, particularly in the area of personal hygiene. Slightly more than half of the food vendors (51.67%) had insufficient knowledge. Education level (AOR=1.58, 95% CI=1.05-3.616) and knowledge level (AOR=6.535, 95% CI=1.652-25.86) were significantly associated with the implementation of hygiene and sanitation practices. Conclusions: Education and knowledge levels are key factors influencing hygiene and sanitation practices among school food vendors. Targeted educational interventions for all stakeholders involved in program implementation are recommended to enhance knowledge and promote good hygiene and sanitation practices.

Abstract

Background: The prevalence of malnutrition among toddlers in Surabaya is 4.8% for stunting and 6.1% for wasting. Therefore, the City Government implemented a snack distribution program targeting toddlers with growth problems. Given the importance of this nutrition intervention, an evaluation is needed to assess its implementation and effectiveness. Objectives: This study aims to evaluate the implementation of the snack distribution program for toddlers with stunting, pre-stunting, and malnutrition in Surabaya City in 2023. Methods: This qualitative evaluation involved in-depth interviews with City Health Office staff, nutritionists at community health centers (puskesmas), and mothers of target toddlers, as well as document analysis conducted from June to July 2024 at the City Health Office and five puskesmas in Surabaya City. Data was collected using structured questionnaires and relevant documents. Qualitative data was analyzed using thematic analysis. Results: The evaluation revealed several problems, such as a shortage of nutritionists and incomplete program mechanisms. Snack consumption among toddlers was inconsistent because some of them did not receive snacks or received snacks that were unfit for consumption due to the negligence of Micro, Small, and Medium Enterprises (MSMEs). In addition, snack acceptance was low due to differences in toddlers’ preferences. Five out of 15 toddlers showed improved growth indicators and a downward trend in the prevalence of stunting, pre-stunting, and malnutrition; however, the overall program implementation remained suboptimal. Conclusions: The snack distribution program for toddlers with growth problems in Surabaya City in 2023 has not yet been implemented optimally. Systematic and strategic improvement is required to improve the program’s effectiveness and results.

Abstract

Background: Obesity in children under five years of age is an emerging public health concern in Indonesia, where birth history and family characteristics may play a significant role. Objectives: This study aimed to analyze factors associated with obesity among toddlers in Central Java, Indonesia, using data from the 2022 Indonesian Nutritional Status Survey (SSGI). Methods: A cross-sectional study was conducted with a weighted sample of 21,987 toddlers (aged 0–59 months) from six cities and 29 districts in Central Java. Independent variables included toddler characteristics (sex, stunting at birth), family factors (area of residence, socioeconomic status), and maternal factors (marital status, education, employment, weight monitoring). Complex samples analysis was performed using chi-square tests for bivariate associations and multiple logistic regression for multivariate analysis, with a significance level of α=0.05. Results: Obesity prevalence was low at 1.4% (n=304). Bivariate analysis identified significant associations with stunting at birth (p-value=0.024), socioeconomic status (p-value<0.001), maternal education (p-value<0.001), and maternal employment (p-value<0.001). In the multivariate model, higher socioeconomic status (aOR=1.94, 95% CI=1.34–2.83, p-value<0.001) and maternal employment (aOR=1.42, 95% CI =1.03–1.96, p-value=0.032) were significant predictors of obesity. Stunting at birth approached significance (aOR=0.44, 95% CI=0.19–1.02, p-value=0.056). Sex, area of residence, marital status, maternal education, and weight monitoring were not significant predictors of childhood obesity. Conclusions: Higher socioeconomic status and maternal employment are key risk factors for toddler obesity in Central Java.

Abstract

Background: The commercial development of tepache beverages requires systematic mapping of sensory attributes, such as the Check-All-That-Apply (CATA) method, to ensure alignment with consumer perceptions. However, integration of emotional responses during sensory evaluation remains relatively underexplored. Emotional profiling offers complementary insights alongside conventional sensory acceptance data, offering a more holistic understanding of consumer responses to the product. Objectives: This study aimed to identify the sensory profile of tepache beverages using the CATA method and to determine emotional profiling through the Emotional Sensory Mapping (ESM). Methods: This study was conducted in two stages: Sample preparation and sensory testing. The materials used included three formulations of tepache beverage (A, B, C), mineral water, and crackers. The tools utilized were stationery, questionnaires, cups, and the XLSTAT 2020 software. Results: Correspondence Analysis showed that product A is closely correlated with acidity, product code B with citrate, and product code C with sparkle. Emotional profiling in Cochran’s Q test revealed significant difference at 5% level, with relaxing, energy, plain, and satisfied influencing consumer perceptions. Conclusions The correlation between the mouthfeel sensory attributes and preference for the tepache beverage samples was found in sweetness and richness, while the sensory attributes at swallow were lightness and richness. Emotional profile mapping indicates that consumers prefer relaxing emotional response observed in product C.
