Abstract

Background: Adolescence is a vulnerable period for nutritional problems due to changes in nutritional needs and eating behaviors. In Surabaya, the prevalence of overweight among adolescents has reached 14.46%. One contributing factor is frequent consumption of discretionary foods, which are energy-dense and low in essential nutrients. Objectives: This study aims to analyze the relationship between balanced nutrition behaviors and the frequency of discretionary food consumption with adolescent nutritional status at SMA Negeri 4 Surabaya. Methods: An analytic observational study with a cross-sectional design was conducted among 83 students in the first and second grades of a higher secondary school, who were selected using proportional random sampling. Data were collected using a balanced nutrition behavior questionnaire, a Semi-Quantitative Food Frequency Questionnaire (SQ-FFQ) to assess the Balanced Nutrition Index 3-60 (IGS3-60), and an FFQ to measure discretionary food consumption. Nutritional status was determined using BMI-for-age based on WHO-AnthroPlus. The obtained data were analyzed using Pearson and Spearman correlation tests. Results: Most respondents had poor balanced nutrition behavior (51.8%). Discretionary foods rich in added sugars, fats, and sugar–fat combinations were frequently consumed on a weekly basis. Significant associations were found between discretionary food consumption and nutritional status, including high added sugars (p-value=0.015), high fat (p-value=0.013), and both high fat and added sugars (p-value=0.017). Conclusions: Balanced nutrition behaviors are not significantly associated with nutritional status, while frequent consumption of discretionary foods is related to higher nutritional status. School-based nutrition interventions are needed to prevent overnutrition among adolescents.

Abstract

Background: Stunting remains a significant public health issue in Indonesia. Low knowledge and perception of mothers about nutrition, health, and the First 1000 Days of Life are determining factors. Objectives: To observe the impact of home visits on the knowledge and perception of maternal children about the Zero New Stunting (ZNS) program. Methods: This study employed a mixed-methods approach with a sequential explanatory design, conducted from June to August 2025 in Sumberadi Village, Sleman, Yogyakarta, Indonesia. The subjects were 35 stunted toddler pairs with mothers who met the inclusion criteria. Interventions are integrative nutrition education packages through home visits by trained Posyandu cadres, nutrition education (growth monitoring, developmental stimulation, and IYCF), and counseling. A structured questionnaire containing knowledge and perception questions of 25 questions each was used. The analysis was carried out using a t-test to see the differences before and after the intervention. Meanwhile, qualitative data were analyzed manually according to the theme of the interview. Results: The mean knowledge score increased from 61.2±9.8 (pre-test) to 86.4±7.1 (post-test 3), with a mean difference of 25.2 points (p-value<0.001). The perception score increased from 58.4±10.2 to 82.4±7.4, with an average difference of 24.0 points (p-value<0.001). Meanwhile, the results of qualitative findings showed high acceptance of the program, including increased self-efficacy, emotional support, and practical guidance during home visits. Conclusions: Home visits by cadres with integrated nutrition education effectively improve mothers’ knowledge and perceptions on stunting prevention, supporting the ZNS program.

Abstract

Background: Food safety in school environments is a key factor in supporting the continuity of educational activities. However, monitoring of hygiene and sanitation practices among school food vendors remains suboptimal. Unsafe or contaminated food can pose serious health risk to students. Poor hygiene and sanitation practices by food vendors are a key contributor to foodborne illnesses in schools. Objectives: This study aimed to evaluate the hygiene and sanitation practices of school food vendors in Yogyakarta and the factors influencing the implementation of these practices. Methods: A cross-sectional study employing a quantitative observational design was conducted in nine elementary schools in Sleman, involving 60 food vendors operating in the surrounding school areas. Associations between independent variables (age, gender, educational background, years of experience, training history, knowledge level, and attitude) and hygiene and sanitation practices were assessed using the chi-square test. Logistic regression analysis was performed to examine the model and identify factors influencing the implementation of hygiene and sanitation practices among school food vendors. Results: The study found that 55% of respondents did not adequately implement hygiene and sanitation practices, particularly in the area of personal hygiene. Slightly more than half of the food vendors (51.67%) had insufficient knowledge. Education level (AOR=1.58, 95% CI=1.05-3.616) and knowledge level (AOR=6.535, 95% CI=1.652-25.86) were significantly associated with the implementation of hygiene and sanitation practices. Conclusions: Education and knowledge levels are key factors influencing hygiene and sanitation practices among school food vendors. Targeted educational interventions for all stakeholders involved in program implementation are recommended to enhance knowledge and promote good hygiene and sanitation practices.

Abstract

Background: The prevalence of malnutrition among toddlers in Surabaya is 4.8% for stunting and 6.1% for wasting. Therefore, the City Government implemented a snack distribution program targeting toddlers with growth problems. Given the importance of this nutrition intervention, an evaluation is needed to assess its implementation and effectiveness. Objectives: This study aims to evaluate the implementation of the snack distribution program for toddlers with stunting, pre-stunting, and malnutrition in Surabaya City in 2023. Methods: This qualitative evaluation involved in-depth interviews with City Health Office staff, nutritionists at community health centers (puskesmas), and mothers of target toddlers, as well as document analysis conducted from June to July 2024 at the City Health Office and five puskesmas in Surabaya City. Data was collected using structured questionnaires and relevant documents. Qualitative data was analyzed using thematic analysis. Results: The evaluation revealed several problems, such as a shortage of nutritionists and incomplete program mechanisms. Snack consumption among toddlers was inconsistent because some of them did not receive snacks or received snacks that were unfit for consumption due to the negligence of Micro, Small, and Medium Enterprises (MSMEs). In addition, snack acceptance was low due to differences in toddlers’ preferences. Five out of 15 toddlers showed improved growth indicators and a downward trend in the prevalence of stunting, pre-stunting, and malnutrition; however, the overall program implementation remained suboptimal. Conclusions: The snack distribution program for toddlers with growth problems in Surabaya City in 2023 has not yet been implemented optimally. Systematic and strategic improvement is required to improve the program’s effectiveness and results.

Abstract

Background: Obesity in children under five years of age is an emerging public health concern in Indonesia, where birth history and family characteristics may play a significant role. Objectives: This study aimed to analyze factors associated with obesity among toddlers in Central Java, Indonesia, using data from the 2022 Indonesian Nutritional Status Survey (SSGI). Methods: A cross-sectional study was conducted with a weighted sample of 21,987 toddlers (aged 0–59 months) from six cities and 29 districts in Central Java. Independent variables included toddler characteristics (sex, stunting at birth), family factors (area of residence, socioeconomic status), and maternal factors (marital status, education, employment, weight monitoring). Complex samples analysis was performed using chi-square tests for bivariate associations and multiple logistic regression for multivariate analysis, with a significance level of α=0.05. Results: Obesity prevalence was low at 1.4% (n=304). Bivariate analysis identified significant associations with stunting at birth (p-value=0.024), socioeconomic status (p-value<0.001), maternal education (p-value<0.001), and maternal employment (p-value<0.001). In the multivariate model, higher socioeconomic status (aOR=1.94, 95% CI=1.34–2.83, p-value<0.001) and maternal employment (aOR=1.42, 95% CI =1.03–1.96, p-value=0.032) were significant predictors of obesity. Stunting at birth approached significance (aOR=0.44, 95% CI=0.19–1.02, p-value=0.056). Sex, area of residence, marital status, maternal education, and weight monitoring were not significant predictors of childhood obesity. Conclusions: Higher socioeconomic status and maternal employment are key risk factors for toddler obesity in Central Java.

Abstract

Background: The commercial development of tepache beverages requires systematic mapping of sensory attributes, such as the Check-All-That-Apply (CATA) method, to ensure alignment with consumer perceptions. However, integration of emotional responses during sensory evaluation remains relatively underexplored. Emotional profiling offers complementary insights alongside conventional sensory acceptance data, offering a more holistic understanding of consumer responses to the product. Objectives: This study aimed to identify the sensory profile of tepache beverages using the CATA method and to determine emotional profiling through the Emotional Sensory Mapping (ESM). Methods: This study was conducted in two stages: Sample preparation and sensory testing. The materials used included three formulations of tepache beverage (A, B, C), mineral water, and crackers. The tools utilized were stationery, questionnaires, cups, and the XLSTAT 2020 software. Results: Correspondence Analysis showed that product A is closely correlated with acidity, product code B with citrate, and product code C with sparkle. Emotional profiling in Cochran’s Q test revealed significant difference at 5% level, with relaxing, energy, plain, and satisfied influencing consumer perceptions. Conclusions The correlation between the mouthfeel sensory attributes and preference for the tepache beverage samples was found in sweetness and richness, while the sensory attributes at swallow were lightness and richness. Emotional profile mapping indicates that consumers prefer relaxing emotional response observed in product C.

Abstract

Background: Lactic acid is a primary biomarker of fatigue, produced as a byproduct of anaerobic energy metabolism. Elevated lactic acid levels are associated with muscle fatigue and reduced performance. Consuming alkaline drinks can help speed up the reduction of lactic acid levels. Coconut water, which is rich in nutrients and electrolytes, can also be an alternative for recovery after intense physical activity. Objectives: Evaluate the effect of coconut water and alkaline water on lactic acid after high-intensity physical activity. Methods: Study participants were male university students divided into two groups: alkaline water group (K1) and coconut water group (K2), each consisting of 13 participants with similar characteristics. K1 was given 350 ml of alkaline water and K2 was given 350 ml of coconut water. Lactic acid levels were measured before and after performing a 35-meter sprint running six times. Differences in lactic acid levels between groups were analyzed using an independent t-test. Results: Pre-activity lactic acid levels were 2.55 ± 0.54 mmol/L for the K1 group and 3.2 ± 0.53 mmol/L for the K2 group. Post-intervention, the levels rose to 6.63 ± 2.54 mmol/L for K1 and 8.3 ± 3.15 mmol/L for K2. There was no statistically significant difference in the increase of lactic acid levels between groups (p-value=0.414). Non-significant results indicate that both liquids have an influence on changes in lactic acid levels. Conclusions: Both alkaline water and coconut water demonstrated similar effects in inhibiting the accumulation of lactic acid following high-intensity physical activity.

Abstract

Background: The proportion of stunted toddlers is 19.8%, and 12.9% in Bengkulu Province and Bengkulu City, respectively. Indicating that the prevalence of stunting among toddlers remains high in Indonesia. Stunted toddlers experience impaired growth, dysfunctional immune system and are more susceptible to diseases. Maternal lack of knowledge on dietary needs reduces their role in meeting toddler nutritional requirements, exclusive breastfeeding and fish-based complementary foods. Objectives: To determine the effects of fish consumption on height of stunted toddlers. Methods: The study used a quasi-experimental non-equivalent (pretest and post-test) control group design, conducted on intervention and control groups for 2 months (8 weeks). The intervention group received the Toddler Feeding Program, while the control group received local Community Health Center Program. The sample consisted of all stunted toddlers aged 6-18 months in North Bengkulu Regency Health Office's work area. Data analysis was performed using Stata 16. For univariate analyses, variables including education, occupation, gender, age of mother and toddler, and toddler fish consumption patterns were considered. Bivariate analyses were conducted using independent t-test to assess differences in body length. Results: Consumption of freshwater, saltwater, and brackish water fish in the intervention group was higher than those in the control group (28.13±48.43 and 19.17±16.89; 23.54±26.92 and 14.79±12.47; 1.88±5.07 and 0.21±0.79). Maternal behaviors towards toddlers, including knowledge (7.83±4.62), attitudes (8.71±5.12), and actions (7.63±4.62) in the intervention group was higher than those in the control group (4.96±4.70; 5.46±5.13; 4.83±4.58). The height of stunted toddlers in the intervention group (0.71±0.59) was higher than the control group (0.46±0.48) with a Pearson coefficient of 0.002. Conclusions: This study shows that fish consumption is associated with improved growth of stunted toddlers.

Abstract

Background: The nutritional transition has led to a shift from a single nutritional problem to a double burden of malnutrition, particularly in adolescent females. Objectives: This study aims to explore the determinants of the co-occurrence of stunting and overweight-obesity in adolescent females. Methods: A cross-sectional method was applied to all female adolescents of school age (n = 360) from 4 secondary schools in Majene, West Sulawesi, Indonesia, between March and April 2021. Data were obtained through the use of a structured questionnaire, 24-hour dietary recall, and measurements of body weight and height. Analysis of the data was conducted using SPSS, NutriSurvey, and WHO AnthroPlus, with statistical significance at the 5% level (95% confidence interval). Results: The results showed that among participants aged 10–19 years, 31.7% were stunted, 14.2% were overweight, 2.2% were obese, and 2.5% experienced both stunting and overweight-obesity. Intake of protein, fat, and magnesium was significantly related to stunting and overweight-obesity (p-value = 0.004, 0.049, and 0.016, respectively). According to the multilevel regression analysis, nutritional knowledge (AOR=6.880; 95% CI: 1.189–39.804; p-value=0.031) and fiber intake (AOR=25.830; 95% CI: 1.565–426.370; p-value=0.023) were significant contributors to the co-occurence. Conclusions: Nutritional knowledge and dietary intake are significant predictors of stunting and overweight-obesity in adolescent females. Therefore, the enhancement of school-based nutritional literacy programs and the creation of a supportive school environment that promotes nutritional status are recommended.
