Abstract

Introduction: Ridge preservation is a periodontal surgical treatment that has an important role in the long-term success of dental implants. An adequate alveolar bone dimension makes future implant placement more predictable. Xenograft is being used as filler material in ridge preservation following tooth extraction, due to osteoconductive and slow-resorbing properties. Patient’s dental impressions and radiographs pre- and post-treatment ridge preservation were used to evaluate the alveolar ridge dimensions. This case report aimed to evaluate of ridge preservation treatment using xenograft material in an open wound healing approach. Patient’s main concerns: A 58-year-old male patient referred from the conservation department was undergoing root canal treatment of his right lower molars and wanted to make a fixed denture. Important clinical findings: Tooth 46 had mobility grade 2, probing pocket depth of 12 mm, and clinical attachment loss of 11 mm. Primary diagnoses: Other conditions affecting the periodontium et causa endodontic-periodontal lesions. Interventions and outcomes: Tooth 46 were extracted with minimal tissue trauma and followed with ridge preservation using xenograft. The collagen membrane was immobilized with the combination figure of eight and simple interrupted suturing technique in open wound healing approach. The purpose of ridge preservation is to prevent of alveolar bone resorption and prepare alveolar dimension for dental implant placement. Xenograft plays an important role as a scaffold in regenerating alveolar bone. Conclusion: Xenograft effectively regenerates new bone in ridge preservation to prepare an implant placement. Periodic control and long-term evaluation are needed to evaluate the success of this treatment.

Abstract

Introduction: The radiographic procedure is hazardous waste because it uses toxic materials. According to the concept of green dentistry, appropriate and efficient waste management needs to be carried out to avoid risks to human health or the environment. This study aims to determine the knowledge level of dentists in Medan concerning green dentistry in the management of radiological waste. Methods: A descriptive study was used as a study method, and purposive sampling was used to choose samples. The total sample consisted of 31 dentists in Medan who owned radiographic equipment. Data were obtained from direct questionnaires. The data were analyzed univariately and calculated in a frequency and percentage table. Result: The results showed that 100% of dentists knew about the definition of green dentistry, 83.9% about the 4R principle of green dentistry, 96.8% about the green dentistry recommendation for radiological waste management, 100% about the benefit of digital radiographic images, 93.5% about paper waste management of digital radiography, 35.5% about the silver-containing hazardous waste solution, 90.3% about how to dispose of fixer solution, 71.0% about disposing of developer solution, 93.5% about disposing of x-ray films, 71.0% about disposing of lead foil, and 45.2% about good x-ray cleaners. Conclusion: This study concludes that 87.1% of dentists in Medan had a knowledge level of green dentistry on radiological waste management in the good category, 9.7% in the fair category, and 3.2% in the less category.

Abstract

Introduction: The destruction that occurs in peri-implantitis, if left untreated, can eventually lead to dental implant failure. Autogenous growth factor is prepared by centrifugation of whole blood samples and has been widely applied in wound healing and tissue regeneration. This platelet concentrate can be used alone or in combination with other biomaterials for soft tissue healing or bone formation. CGF (concentrated growth factor) is a derivative of PRF (platelet-rich fibrin) and has the same 3-dimensional structural complex as PRF. However, it contains more growth factors than PRF and has a more rigid fibrinogen structure. Aims: The objective of this systematic review is to obtain scientific data related to the use of concentrated growth factors in peri-implantitis treatment in order to avoid dental implant failure. Method: A total of 5,339 studies were initially identified from the three databases (Wiley, Researchgate and Google Scholar) between January 2018 and July 2023. Result: Three randomised clinical trials (RCTs) were selected for analysis. Conclusion: Concentrated Growth Factor (CGF) can be employed in combination with bone graft material for the treatment of peri-implantitis, with the objective to prevent dental implant failure that may result from peri-implantitis. However, it is not the sole recommended approach.

Abstract

Introduction: Necrotizing fasciitis due to odontogenic infection is a serious, rapidly progressive infection that infects the skin and soft tissue and has a high mortality rate, requiring early diagnosis and aggressive treatment. Systemic lupus erythematosus is an autoimmune disease that can predispose to severe odontogenic infections, especially necrotizing fasciitis, and worsens immune response to surgical debridement. Case and Management: A 46 years old female patient with a history of systemic lupus erythematosus was referred to the Emergency Department at Hasan Sadikin General Hospital, Bandung with complaints of swelling accompanied by pain at the chin extended to the right lower jaw region. Clinical examination revealed necrosis of the skin, fluctuations, spontaneous drainage, and crepitations. The patient was diagnosed with necrotizing fasciitis and was underwent necrotizing debridement at submental region and tooth extraction under local anesthesia and administration of broad-spectrum empiric antibiotics, followed by wound care with modern dressings. Immunosuppressant drugs were stopped for three days post treatment. Conclusion: Management of necrotizing fasciitis with necrotizing debridement, extraction of teeth, discontinuation of immunosuppressant, and empiric administration of broad-spectrum antibiotics in patients with Systemic Lupus Erythematosus was necessary to prevent further infection to other fascia.

Abstract

Introduction: Periodontal disease, including periodontitis, ranks as the 6th complication for Diabetes Mellitus. Research has found an increase of inflammation and higher alveolar bone loss in the periodontal of diabetes mellitus patients. One of the components which exhibited multiple bio-activities like anti oxidative, anti-inflammatory, antidiabetic antimicrobial cardioprotective, neuroprotective, immunomodulatory, and anti-obesity activities is procyanidin. Procyanidin can be found in various natural ingredients and one of the most abundant compositions is found in red grape seeds. Methods: The method used in this study is a literature review from several academic databases including : Google Scholar, PubMed, Mendeley, Wiley, and the university's online library with boolean searching method. Results: The several articles used in this literature review show that the protective effect of grape seed procyanidin plays a role in attenuating the increase of MFG-E8, suppressing the increase of IL-1beta protein level, improving HOMA-IR and insulin levels in diabetes, increasing IL-10 and TGF-beta accumulation in the gingival epithelium of periodontitis condition, and MMP- and HIF-1alpha immunohistochemistry showed an increase in its levels for periodontitis with diabetes rats, but decreased as grape seed procyanidin extract (GSPE) was administered. Conclusion: The present study suggested that procyanidin in grape seed extract played a role in treating periodontitis in diabetes patients through its anti- inflammatory role and its ability to decrease alveolar bone loss. Abstract

Introduction: Dental anxiety in children aged 4-12 years is an obstacle in dental and oral health care. Anxiety management such as distraction is needed, namely a method to shift the child's focus to other things, such as the use of Virtual Reality (VR). Virtual Reality (VR) is an audiovisual distraction media that can reduce the level of anxiety and pain and unpleasant situations by displaying three-dimensional images along with VR glasses and their voices on the headset. Aim: The purpose of this study was to examine the advantages and disadvantages of Virtual Reality (VR) as a distraction in dental management in children aged 4-12 years, cases of dental management using VR and, the effectiveness of Virtual Reality (VR) as a distraction in dental management in children aged 4-12. Method: The literatures in this review was obtained through searches of the PubMed, Science Direct, and Google Scholar databases using specific keywords along with inclusion and exclusion criteria. Result: The findings of the review of the five articles indicated that virtual reality (VR) was an effective distraction for reducing anxiety and pain levels in children aged 4-12 years undergoing dental treatment. The results of other reviews did not provide specific age range for its use, the ideal duration for its use, the management of adverse effects following its use, or solutions for children who are unable to use it. Conclusion: Virtual reality represents a promising approach for the management of dental management in children aged 4-12 years.

Abstract

Introduction: Radiographic repetition analysis is a systematic process to record the results of rejected or repeated images by determining the cause of image repetition to improve program effectiveness and quality of service in radiology installations. One method to assess the quality results of radiology services is to calculate the percentage of repeated x-rays in each month. In accordance with the Decree of the Indonesia Minister of Health Number 129 of 2008 concerning the minimum service standards of hospitals, the maximum permissible limit for errors and the repetition rate of radiology X-ray services is ≤ 2%. Objective: This study aims to determine the percentage of repetition of periapical radiographic photographs at Baiturrahmah Dental Hospital Education Center (RSGMP Baiturrahmah). Methods: This study employed a quantitative, descriptive observational methodology. A total of 454 periapical X-ray data points were collected over the period from July to October 2023 using the saturated sampling technique. The research data were analysed using Excel formulas and presented in tabular and graphical formats. Results: The highest repetition result was observed in August at 10.5%, while the lowest X-ray repetition was recorded in September at 0%. The average repetition from July to October 2023 was 6.8%. Conclusion: The highest repetition rate was observed in pediatric patients, at 14.17%, due to improper film size selection. The mandibular posterior teeth exhibited the greatest repetition rate, at 61.30%. The overall score in this research is classified as high

Abstract

Introduction: Oral squamous cell carcinoma (OSCC) is the sixth most common cancer worldwide and the most prevalent form of oral cancer. The five-year survival rate for OSCC varies considerably depending on the stage of the disease, ranging from 40% to 60%. The majority of cases are diagnosed at an advanced stage, resulting in a significantly reduced life expectancy. Therefore, there is a clear need for effective strategies to detect cancer at an early stage. Aim: The objective of this article is to identify the potential of matrix metalloproteinase (MMP) as a salivary biomarker for the early detection of oral squamous cell carcinoma (OSCC). Review: Saliva is considered a potential source of biomarkers for oral cancer due to its continuous contact with cancerous lesions in the oral cavity and the various enzymes, hormones, antibodies, antimicrobial constituents, and cytokines it contains. MMP is an extracellular endopeptidase enzyme present in saliva and associated with the carcinogenesis process. It has been identified as a salivary biomarker for the early detection of OSCC. The levels of several MMP proteins in the saliva of OSCC patients have been found to be elevated, including MMP-1, MMP-2, MMP-3, MMP-7, MMP-8, MMP-9, MMP-10, MMP-11, MMP-12, and MMP-13. Conclusion: The increased levels of salivary MMP, which were most specifically found in OSCC patients, included MMP-1, MMP-2, MMP-3, MMP-9, MMP-10, MMP-12, and MMP-13. This suggests that MMP may be a potential salivary biomarker for the early detection of OSCC

Abstract

Introduction: Dental and oral health problems are a major problem in school-aged children. Nearly a third of Indonesia's population has dental and oral health problems. Efforts that can increase knowledge about dental and oral health are through health promotion using audio-visual media, counseling using creative media encourage students to study hard and optimally received the lesson. Objective: To determine the influence of health promotion using audio visual media in increasing dental and oral health knowledge of SDI students. Method: This type of research was a quasi-experiment with a pre-test and post-test group design and analysis using the Wilxocon test with a sample size about 48 students. Results: Based on the results of research entitled The Influence of Health Promotion Using Audio Visual Media in Increasing Knowledge of Dental and Oral Health at SDI Kampus 1 UNHAS. Based on a comparison using the Wilcoxon test, a p-value of 0.000 was obtained, namely less than 0.05 (p value <0.05) . This shows that the treatment given, namely Audio Visual Media, increases students' knowledge about dental and oral health after it is applied. Conclusion: There is any influence of increasing knowledge on health promotion using audio-visual media.

Abstract

Introduction: Teeth with furcation involvement lesions present a significant challenge for clinicians regarding treatment options. Various methods can be employed to restore mutilated teeth for a longer period of function, including resective surgery (hemisection, bicuspidization and root amputation/root resection). Case and Management: This case series presents clinical and radiographic data collected from two patients who were referred to the dental and oral hospital at the University of Sumatera Utara. The patients were examined and treated by a dental conservation resident, who diagnosed a periodontal case in the patient with a chief complaint of pain and swelling associated with class II and III furcation involvement, as well as endo perio lesions on the radiograph of the first lower left and right mandibular molars. The class II and III furcation was complicated with endodontic involvement. Patients were provided with endodontic treatment in conjunction with the initial stage of periodontal treatment, which includes dental health education, root scaling and debridement. Following three months of endodontic treatment, resective surgery, including hemisection and bicuspidation, were performed. Radiographic analysis revealed that the teeth exhibited satisfactory regeneration following resective surgery, with the teeth becoming the supporting teeth for the fixed denture. Conclusion: It is not always necessary to extract teeth with conditions involving furcation. However, a multidisciplinary approach is essential to ensure effective treatment of the lesion.
