Abstract

Introduction: Oral frailty is a geriatric syndrome characterised by a decline in oral function associated with a decline in cognitive and physical function. Virtual reality hypnosis (VRH) is a current method that combines the benefits of virtual reality (VR) technology and hypnosis. Aim: This case report aims to describe the role of VRH in the management of elderly patients with oral frailty. Case and Management: A 65-year-old male patient came to the Oral Medicine Department with a chief complaint of dry mouth for the past three months. The patient often felt stressed because of his health problems. Intraoral examination showed frothy saliva, white plaque on the buccal and lateral mucosa of the tongue, missing teeth, and poor oral hygiene. Some additional examinations performed included the Indonesian Version of Summated Xerostomia Inventory (SXI-ID), Clinical Oral Dryness Score (CODS), sialometry, Depression Anxiety Stress Scale (DASS-21), and D-E-N-T-A-L oral frailty questionnaire. The test results showed that the patient had oral frailty. Case Management: Pharmacological therapy consisted of chlorine dioxide mouthwash, sucralfate syrup, and probiotic caplets. Non-pharmacological therapy consisted of instructions to maintain good oral hygiene, adopt a healthy lifestyle, and VRH. The relaxing effect of VRH was evidenced by the recording of increased alpha waves on quantitative electroencephalography (qEEG) during the procedure, and the results of SXI-ID, CODS, and DASS-21 at the last visit were normal. Oral frailty resolved after two weeks of treatment. The patient was also referred for oral care from the Departments of Periodontia, Prosthodontia, and Oral Surgery. Conclusion: VRH has an important role especially in the management of stress-induced xerostomia and pain in elderly patients with oral frailty.

Abstract

Introduction: Early identification of oral squamous cell carcinoma (OSCC) is a crucial factor influencing the disease's prognosis. The elevated levels of IL-6 and IL-8 in saliva are a result of their overproduction in the cancer microenvironment. The production of salivary cytokines, particularly IL-6 and IL-8, has a role in cancer development. These cytokines may also be used as biomarkers. The use of saliva as a biomarker is quite interesting because it is in direct contact with cancer cells, easy to access, non-invasive, and cost-effective. Aim: The aim of this literature review was to assess the potential of salivary IL-6 and IL-8 as biomarkers for OSCC early diagnosis. Review: The initiation of carcinogenesis in the oral cavity is actively repressed by inflammation and cell-mediated immunity. IL-6 and IL-8 are two important molecules involved in stromal-to-cancer cell communication. These cytokines play a major part in the growth, progression, metastasis, and recurrence of OSCC. Several studies have found that OSCC patients have higher levels of salivary IL-6 and IL-8 than healthy people and patients with an oral potentially malignant disorder (OPMD). Conclusion: Salivary IL-6 and IL-8 are very promising potential biomarkers for the early diagnosis of OSCC. Further research is still needed to validate and use these biomarkers clinically.

Abstract

Background. Particles of calcium carbonate (CaCO3) have been applied in a number of fields, including as dental biomaterial. Because CaCO3 particles have three distinct polymorphs and complex crystallization behavior, it is difficult to systematically adjust their physical properties for desired applications. Objective. This investigation aims to examine the influence of the addition of the 0.02 M HCl additive on the phase and size of the CaCO3 particles synthesized with the fine bubble diffuser method. Methods. The type of research is descriptive explorative. There were two sample groups in this study, and each group had three replications (n = 3). Group 1 added 0.02 M of HCl as an addition to the as-prepared material, while Group 2 added no HCl at all. The phase of CaCO3 was evaluated by using the FTIR (Thermo Scientific Nicolet iS5), while the particle size was measured via DLS (Horiba Scientifica SZ-100 Nanopartica) Result. The results showed the sample of CaCO3 without any additive posesses Amorphous Calcium Carbonate (ACC) and calcite. Meanwhile, sample of CaCO3 with addition of HCL 0.02 M showed ACC, calcites and vaterite phase. Particle size of CaCO3 without additve is larger than another one. Conclusion. In this study, the addition of the additive HCl 0.02 resulted in a decrease in the pH of the solution resulting in a phase change from clacite to vaterite as well as decreasing the particle size of CaCO3.

Abstract

Introduction: Bell’s palsy (BP) is an acute weakness or paralysis of the peripheral fascial nerve with an incidence of approximately 20–30 per 100,000 individuals. Bell’s palsy is characterized by a droopy appearance on one side of the face and the inability to open or close the eyes on one side. This condition can affect limitations in daily activities such as eating, drinking, and speaking, as well as the oral health of the sufferer. Patients with Bell’s palsy can have a high caries risk due to decreased saliva production and angular cheilitis. The purpose of this case report is to determine the relationship between the oral manifestations experienced by patients with Bell’s palsy and appropriate management. Case and Management: A 54-year-old woman came with complaints of mouth discomfort. Two weeks earlier, the patient felt that her mouth was left-sided and had received treatment from a neurologist, currently it has improved but still feels numb. There was no history of dental disease or allergies. Extraoral examination: the face was symmetrical, and there were brownish macules, multiple, about 1-2 mm in size, painless. Intraoral examination: The lips were dry, and there was a brownish macule on the lower lip, an irregular shape, a size of about 1cm, no pain, and a deviation of about 1-2 mm to the left on the lower lip. The white plaque on the entire surface of the tongue dorsum could be scraped and did not leave red areas. There was a longitudinal gap in the center of the tongue dorsum and an indentation on the lateral tongue. The patient was instructed to maintain oral hygiene, prescribed povidone iodine 1% as an antiseptic, vaseline album to treat dry lips, neurobion tablets, and consultation with a neurologist. Conclusion: The patient's clinical condition is mild, but oral care for BP patients is very important to maintain oral hygiene

Abstract

Background: Macro-sized and inhomogeneous distribution of ceramic filer particles make it difficult to obtain smooth surfaces of dental composite after polishing. Objective. This study aims to synthesize alumina silica (Al2O3-SiO2) fillers using sol gel method with the additives agent chitosan 5%, 10% and polyethylene glycol (PEG) 5% to produce micro particle size and evenly distributed particle as filler dental composite. Methods. The type of research is descriptive explorative. The sol-gel process was utilized to synthesize filler particles of Al2O3-SiO2. TEOS was dissolved in ethanol and mixed with acetic acid as catalyts. Al2(NO3)3 was added into the solution and mixed homogenously. Subsequently, the additive agents (Chitosan 5%, chitosan 10% and PEG) were mixed into the mixture for 30 minutes. Drying the samples for 48 hours at 60°C in the oven. The Dynamic light scattering (DLS) SZ-1 was used to evaluate the size, particle distribution and zeta potential. Result. The results showed that the addition of chitosan 10% produced a smaller size of Al2O3-SiO2 compared to other samples. A homogeneous particle distribution is shown in the sample with PEG 5%. Meanwhile, zeta potential values of the filler particles Al2O3-SiO2 with the addition of chitosan 10% shows the biggest value. Conclusion: The additive agent of chitosan 10% can modify the surface of filler Al2O3-SiO2 in order to inhibit particle growth more effectively but better particle distribution is shown in samples with PEG 5% due to the lower viscosity than chitosan thus it is easily homogenized in the solution

Abstract

Introduction: Trigeminal neuralgia (TN) is the most frequent form of craniofacial neuropathic pain, with a wide range of severity. Trigeminal neuralgia classically has been associated with neurovascular compression in the prepontine cistern at the nerve root entry zone or in Meckel’s cave due to an abnormal vessel, tumor, or cyst. Arachnoid cysts of the posterior cranial fossa are still less commonly compared to those superior to the constituent part of the posterior cranial fossa meninges. However, the arachnoid cyst in conjunction with the impacted third molar may cause compression of the trigeminal ganglion with similar clinical symptoms. The aim of this report is to explain the management of the first division of trigeminal neuralgia with an arachnoid cyst perplexed by impacted third molars. Case report: A case of a 27-year-old female with throbbing pain on top of her head, forehead, temple, and eyes several times. Extensive clinical and radiologic examinations were performed and revealed an arachnoid cyst and impacted third molars. Impacted third molars were removed. The patient declined the surgery for the arachnoid cyst and was treated symptomatically with amitriptyline 25 mg once daily until recently. The definitive diagnosis was the first division of trigeminal neuralgia and an arachnoid cyst. Conclusions A fundamental aspect of the diagnosis of trigeminal neuralgia is to examine and exclude all possible underlying etiologies and structural pathologies, which can refer the pain to a location remote from the stimulus.

Abstract

Introduction: Periodontitis is a chronic infection of periodontal tissues caused by interplay between periodontopatogen bacterial and the immune system. In periodontitis condition, inflammatory cells are found in large numbers and will release proinflammatory cytokines. Non-steroidal anti-inflammatory (NSAID) is one of a class of drugs used in periodontitis cases treatment. However, long treatment using NSAIDs may give side effects such as gastrointestinal disorder. Ambon banana peel (Musa paradisiaca L),a medicinal plant contains flavanoid, alkonoid, tanin and saponin which has anti-inflammatory effect due to its ability in inhibiting inflammatory cell migration. This research aimed to study the effect of the Ambon banana peel extract as an antiinflammation on the number of inflammatory cells infiltration in the periodontitis induced rats. Methods : This research uses experimental laboratory with Post Test Only Control Group Design. Silk ligature was placed in subgingival areas at the lower anterior teeth of male Wistar rats for 14 days to induce periodontitis. After 14 days, the ligation was released and the rats from each group were administered orally either with 25%, 50% concentrations extract and aquades (control negative). The rats were then decapitated 1, 5 and 7 days after the treatment. The serially sections were stained with hematoxylin eosin to examine the number of inflammatory cell infiltration. Results :Two-way Anova showed significant differences (p <0.05) among groups, indicating that Ambon banana peel extract affected the number of neutrofil cells infiltration. Conclusion: There is an effect of Ambon banana peel (Musa paradisiaca L) extract to reduce inflammatory cells infiltration.

Abstract

Introduction: The elderly population is increasing both globally and nationally, including in Dumai City, Riau Province. The older they get, the more health problems they complain about, and xerostomia is one of the most common issues. There are only a few studies that examine the problem of xerostomia in the elderly. Therefore, the purpose of this study is to examine how xerostomia relates to gender, age, and overall health among elderly people in Dumai City, Riau Province. Methods: This study used a cross-sectional design approach. The survey included data on gender, age, and subjective assessment of xerostomia using the Summated Xerostomia Inventory in Indonesian (SXI-ID) questionnaire, consisting of 5 questions with answer options: never, rarely, sometimes, often, and always. In addition, the standard xerostomia question, "How often does the mouth feel dry?" has four answer options: never, sometimes, often, and always. Sampling was done by the accidental sampling technique; 247 respondents met the requirements and participated in the study completely. Results: The study showed that more respondents of the male gender (18.6 ± 2.7) suffered from xerostomia than women (17.4 ± 4.4), which was statistically significant (p<0.05). The results of the Spearman Rank correlation test between the age variable and the xerostomia condition showed statistically significant results (p<0.05), namely that the older the age, the greater the potential for xerostomia in the elderly. Conclusion: The incidence of xerostomia is more prevalent in men and as individuals age. Promotive and preventive efforts for the oral health of the elderly need to be strengthened to minimize oral health problems due to xerostomia.

Abstract

Introduction: Stunting is a condition in which the length or height is less than the age. One of the causes is a lack of nutritional intake for a long time in the first 1000 days of life (HPK), which is a period of nutritional fulfillment that affects head growth. The process of head growth requires adequate nutrition. Disorders of brain development due to stunted head growth mean that there are differences in head shape that can be measured using the cephalic index. Aim: The purpose of this study was to determine the relationship between stunting and head index. Method: This type of research is an observational analytic study with a cross-sectional research design. The sample in this study consisted of two groups, and the size was calculated using an unpaired categorical analytic formula. The minimum sample size for two groups, namely stunted and normal children, was 50 children. Result: The results of this study showed a relationship between stunting conditions and head index in children aged 10–12 years with a value of p < 0.05. Conclusion: Inadequate nutrition results in deficient craniofacial growth and development.
