Abstract

Japan has the world's largest elderly population, with a family culture that entrusts elderly care to nursing homes, or the so-called Roujin Home. In 2020, there were around 11,575 Roujin Houses in Japan. A common problem that arises in patients with auditory hallucinations is sensory perception disorders. The purpose of this study is to reveal the impact of therapy or reprimands in the healing process of auditory hallucinations in elderly patients. This research was conducted on June 20-22, 2023 at Iryou Houjin Aiwa Deikea, using observation instruments and interviews with patients and therapists. The study focused on one patient with auditory hallucinations. The results showed that the intervention given for 3 days did not provide significant progress, due to a lack of support and motivation from the family, which caused the patient to be reluctant to follow the reprimanding therapy. Limitations of this study include limited duration of intervention and lack of emotion supports from the family. These findings provide important insights into the importance of family support in improve the effectiveness of auditory hallucination therapy.

Abstract

TB (Tuberculosis) patients in Indonesia are the 2nd highest in the world after India. The large number of TB patients in Indonesia is due to the lack of success in TB treatment due to patient non-compliance with medication regularly. Factors that affect non-compliance are low knowledge of TB patients and lack of family support. This study aims to determine the level of family knowledge and support, medication adherence, and determine the relationship between the level of knowledge and family support to medication adherence in TB patients at the Kroya Health Center, Cilacap Regency in 2024. This study uses a non-experimental method with cross-sectional data collection conducted in April-May 2024. The study population is all TB patients totaling 130 respondents who underwent treatment at the Kroya Health Center, Cilacap Regency from November 2023 to May 2024, and the research sample was 56 respondents. The conclusion of this study is that the knowledge level of TB respondents is the highest at 37.5% and 55.4%, and the adherence to taking medication at the Kroya Health Center is 30.4% and 57.1%. In this study, the highest TB patients were at a moderate level of knowledge (55.4%); family support is not supportive (96.4%) and medication adherence is low (30.4%) to moderate (57.1%). Testing with the chi-square method there was a relationship between the level of knowledge and medication adherence in TB patients (p-value 0.000), but there was no relationship between family support and medication adherence (p-value 0.268) in patients at the Kroya Health Center, Cilacap Regency with a r value of 0.541. The limitation of this study is that there is no follow-up on the respondents such as monitoring TB drugs so that the existing data is only data taken when filling out the questionnaire.

Abstract

Primary clinics play a crucial role in improving public health through healthcare services, disease prevention, and education. However, the challenges in accreditation preparation include the staff's lack of technical understanding in completing applications, limited facilities such as computers, and insufficient support from clinic leadership and monitoring from the Health Office. This study aims to analyze factors related to the accreditation status of primary clinics in Pekanbaru City in 2024. The population consisted of 198 people, with a sample size of 198 respondents. Data analysis was conducted using univariate, bivariate, and multivariate analyse. There is a relationship between activity standardization (p= 0.000), completeness of clinic facilities (p= 0.000), activity coordination (p= 0.016), leadership support (p= 0.013) and attitude (p= 0.005) with the accreditation status of the Pratama Clinic. There is no relationship between knowledge factors (p= 0.596), competence (p= 0.319), work team support (p= 0.237), ease of access to information technology systems (p= 0.074), motivation (p= 0.826) and support from the Health Office (p= 0.367) with the accreditation status of the Pratama Clinic. The most dominant variable related to the accreditation status of the Pratama Clinic in Pekanbaru City in 2024 is the activity standardization variable (POR= 11.572). Suggestion: The Pratama Clinic needs to routinely evaluate and update operational procedures to ensure compliance with national accreditation standards. In addition, the establishment of an internal monitoring system and additional training on ASPAK applications, DFO, and managerial skills are essential to maintain compliance and improve operational effectiveness.

Abstract

Health workers play a key role in handling outbreaks, with performance influenced by factors such as ability, discipline, experience, perception, motivation, leadership support, and appreciation. Despite efforts, basic immunization coverage in recent years has not met the national target of 95%, underscoring the need for optimal health worker performance to achieve coverage goals, control rubella outbreaks, and protect vulnerable groups. This study, conducted at Sentajo and Perhentian Luas Health Centers from January to May 2024, analyzed factors affecting health worker performance in managing rubella outbreaks using a sample of 85 respondents and multivariate regression analysis. The findings indicated that awards (r = 0.377, P = 0.000), motivation (r = 0.336, P = 0.002), ability (r = 0.345, P = 0.000), and work experience (r = 0.302, P = 0.005) significantly influenced performance, with awards having the strongest impact. Conversely, leadership support, discipline, and perception had no significant effect on performance. Based on these results, the Kuantan Singingi District Health Office is advised to implement effective reward policies, such as performance bonuses and certificates of achievement, to boost health worker motivation and performance in handling rubella outbreaks. Enhancing health worker skills and rewarding good performance at the health centers can also help improve immunization rates and community protection against rubella. This approach could serve as a model for other health institutions aiming to increase immunization coverage and disease prevention efforts.

Abstract

The Triple Elimination Program aims to prevent the transmission of HIV, syphilis, and hepatitis B from mother to child. In Rokan Hilir Regency, screening coverage remains low: hepatitis B (42.2%), HIV (38%), and syphilis (48.3%). Bantaian Health Center has the lowest coverage, while Bagan Batu Health Center has the highest. A thorough evaluation is needed to improve program performance using the Malcolm Baldrige approach. This study aims to evaluate the performance of the triple elimination program for pregnant women at the Rokan Hilir District Health Office in 2024, employing a mixed-method explanatory sequential design. The sample consisted of 40 respondents and 9 stakeholders as informants. Triangulation was applied to sources, methods, and data. The results show that most respondents rated program leadership as high (57.5%), while strategic planning (52.5%), patient focus (62.5%), measurement and knowledge management (60%), team focus (55%), process focus (57.5%), and performance results (60%) were rated low. Qualitatively, program leadership demonstrated strong commitment. Program planning is based on technical guidelines and BOK funds, with limitations in APBD budget allocations. Education and collaboration with community leaders have been implemented. However, the program has not focused on patients and is not widely recognized by the community. Staff at Bantaian and Bagan Batu Health Centers enthusiastically implement the program despite limited human resources, introducing innovations and conducting regular evaluations. The program is developed through coordination meetings, data analysis, and cadre training. Key factors for achieving targets include case monitoring, health education, cross-sector collaboration, and periodic evaluations. Health centers need to expand education and socialization on the importance of triple elimination screening, supported by integrated coordination between the Health Office and related agencies (OPD).

Abstract

The Riau Provincial Health Service Health Training Center UPT Institute has an important role in improving the Training Institute using the Learning Organization method. This research uses the Watkins & Marsick Approach model which includes seven dimensions of Learning Organization. At the individual level there are two dimensions, namely Continuous Learning and Inquiry and Dialogue. At the team/group level it is reflected in Team Learning. At the organizational level there are four dimensions, namely Embedded System, Empowerment, System Connection, and Strategic Leadership.  There were 8 informants. Triagulation is used in sources, methods and data. The research results show that the three dimensions of the Learning Organization have been running, with several obstacles. The other four dimensions, namely Organizational Culture, Embedded System, Empowerment, and System Connection are still not optimal. The main obstacles include ineffective communication, low motivation, limited understanding of technology, and lack of infrastructure support. Strategic Leadership alsoneeds to be improved to provide better guidance and resources. This research highlights the importance of improving communication, motivation, and understanding of technology to achieve better organizational performance. Apart from that, the implementation of the Embedded System, Empowerment, System Connection, and Strategic Leadership also still requires improvement.  Overall, this research concludes that the UPT Health Training Center has the potential to become a more effective Learning Organization, requiring more comprehensive efforts in increasing human resource capacity, building an organizational culture that supports learning, and aligning leadership strategies with Learning Organization principles. Recommendations for UPT Bapelkes Riau Province to strengthen continuous learning, inclusive dialogue, team learning, and technology-based systems for efficiency.

Abstract

Hypertension is a significant non-communicable disease, with a global prevalence of 29.4% in 2021. This research aims to explore the relationship of age, gender, and family caregiver burden on family coping skills in caring for hypertensive patients with stroke complications. Using a quantitative research design, we selected 76 caregivers at Dr. Moewardi Surakarta Hospital through purposive sampling. Data were collected by use of the Zarit Burden Interview (ZBI) and Family Crisis Oriented Personal Evaluation Scales (F-COPES) questionnaires. The findings show that there is no significant relationship between age and coping ability (p = 0.280). However, a significant association between gender and coping strategies was found, with 34.2% of males using adaptive coping compared to only 5.3% of females (p < 0.001). In addition, caregiver burden had a significant effect on coping ability, with 61.8% of caregivers who experienced heavy burden tending to use maladaptive coping. These findings emphasize the importance of social support and gender-tailored interventions to improve caregiver well-being and patient care

Abstract

Surgery is an action carried out by opening or exposing a part of the body through an incision and ending with stitching the wound. This surgical action can cause anxiety in the patient so that the patient feels uncomfortable and even feels threatened. This may influence a person's spiritual level. Spirituality is something important for every individual in their relationship with God Almighty. This research was conducted to determine the effect of the patient's level of spirituality on the pre-operative anxiety level of patients in the Dahlia ward at RSUD dr. R. Goeteng Taroenadibrata Purbalingga. The design of this research is quantitative with a cross-sectional approach. The samples used were pre-operative patients in the Dahlia ward at RSUD dr. R. Goeteng Taroenadibrata Purbalingga with 60 respondents using Purposive Sampling techniques. The research instrument used a spirituality level questionnaire and an anxiety level questionnaire. Data analysis uses the Chi-Square Test. The research results showed (1) the majority of pre-operative respondents in the Dahlia ward were aged 26-45 years, female, self-employed, had at least elementary school education, and the type of general surgery; (2) the majority of preoperative patients' spirituality level is moderate and the anxiety level of surgical patients is not anxious; (3) there is an influence of the patient's spirituality level on the anxiety level of pre-operative patients in the Dahlia ward at Dr. RSUD. R. Goeteng Taroenadibrata Purbalngga with a p-value of 0.025. The urgency of this study lies in the importance of understanding the influence of spirituality on anxiety in the context of preoperative care, because unmanaged anxiety can impact the success of surgery and patient recovery. This study contributes to health science by providing evidence that spirituality can be one approach to reducing preoperative patient anxiety. The level of spirituality has a significant effect on the level of preoperative patient anxiety. Therefore, it is recommended that health workers integrate a spiritual approach into preoperative patient care, such as providing spiritual support, counseling sessions, or facilities for worship. This strategy is expected to increase the patient's psychological comfort, thereby supporting the success of medical procedures and the recovery process.

Abstract

High-quality healthcare services are crucial for ensuring safety, accessibility, and effectiveness, which ultimately enhance patient satisfaction and support hospital accreditation. This research sought to examine the connection between service quality and patient satisfaction in the Outpatient Unit of RSGM-P UNSRAT. A quantitative cross-sectional design was applied from January to September 2024. The population included patients receiving treatment at RSGM-P UNSRAT, with a sample of 200 patients selected through purposive sampling based on inclusion criteria such as being ≥18 years old and having received at least one treatment. Data were collected by using questionnaire and analysis through univariate, bivariate, and multivariate techniques. The univariate analysis indicated that over 50% of patients were satisfied with the service quality. Bivariate analysis showed significant associations between patient satisfaction and all five service quality dimensions—reliability, Tangibles, assurance, responsiveness, and empathy (p<0.05). Multivariate analysis revealed that assurance was the most dominant factor influencing satisfaction, with an odds ratio of 5.334. The study concludes that service quality is significantly related to patient satisfaction, with assurance being the most influential factor. It recommends regular patient satisfaction surveys to identify improvement areas and suggests further research into other factors affecting service quality.

Abstract

Stunting is a chronic nutritional issue in children, characterized by shorter stature than their peers, affecting their growth and development. This study aims to analyze the relationship between access to healthcare services and the incidence of stunting among toddlers in East Bolaang Mongondow Regency. The research employs a mixed-methods approach, using a cross-sectional observational analytical design for quantitative aspects and qualitative analysis for supporting factors. The study sample includes 976 toddlers, with secondary data from the 2022 Indonesian Nutrition Status Survey (SSGI). Variables examined include access to healthcare facilities, Maternal and Child Health (MCH) book ownership, and immunization services. Data analysis revealed significant relationships between visits to healthcare facilities (p=0.000) and immunization services (p=0.000) with stunting incidence, although the correlations were weak (c=0.127; c=0.114). However, no significant relationship was found between ownership of MCH books and stunting. Immunization services emerged as the most influential factor in stunting incidence. Significant barriers included distance, transportation costs, and parents' lack of knowledge and awareness. Recommended interventions include intensive health promotion programs to raise community awareness about the importance of immunization and regular healthcare visits, as well as expanding access to immunization by increasing schedules at health centres and integrated service posts (posyandu) located closer to communities.
