Reporting hospital is an organization tool that aims to be able to generate reports in a quick, precise and accurate. This type of reporting consists of internal and external reports. From the observations which the author did to the internal hospital report Islam Ibnu Sina Pekanbaru, still found error calculation by nurses in Inpatient census room and Out patient so the summation becomes out of sync. With the rekapan of an error report is not accurate and the information needed to be less precise. Internal reporting utilized by the hospital especially the Director as well as the Foundation. As for the research purposes for causing Internal Report making process and utilization of medical record Information in hospital Islam Ibnu Sina Pekanbaru 2015.The design of this research is descriptive research with qualitative approach. Descriptive research is a method of research conducted with the goal to create a picture or descriptive of a situation objectively conducted in April-May 2016 with 3 Informant, Deputy Director, head of Unit, medical record and medical record Clerk.The research of data processing for medical record in hospital Islam Ibnu Sina be it Assembling, Coding, Indexing, and reporting already done with optimal. The process of making the report and the information already put to good use by the Director or such a Foundation report MOR. And for the utilization of medical record information used for decision making research and education.The conclusion to the Medical Record data processing already carried out with optimal. The process of creating reports and information were already well underway, and the utilization of medical record information already utilized by the Board of Directors for decision making. His medical record officers must quickly in input and merekap every day, room nurses should carefully in making the census, and the hospital must be responsible for protecting information.

The production process design is the form ones creativity in the form of a paper form or electronic form in the form of visual communication which has the function and aesthetic value to convey information or a message to everyone who has arranged the format so as to comply with certain requirements. Electronic form is a space that is displayed on the computer screen that is used to capture the data to be processed in an electronic data processing. The method used is descriptive qualitative approach is also called non statistik data processing. Data analysis techniques used in this research is the technique of inductive thinking process; that is to say, in testing hypotheses starts from the data collected and summarized, this technique is usually used to analyze the data obtained from observation and interviews Results showed Desai electronic form in the hospital sassy yellow pekanbaru not run well, hospitals are still using manual design, while at the present time other hospitals already use electronic medical records to facilitate the work, reductions loss of data and a double storage. The conclusion from this study that the factors that make hospitals sassy yellow pekanbaru not use an electronic medical record that there is no concern for the hospital to work in the system of medical records.

Based on Act No. 29 years 2004 about medical practice as stated in articles 45paragraphs (1) said that informed consent is approval of medical action which is givenby the patient or his immediate family after obtaining a complete explanation aboutmedical or dentist action that will be performed on that patient. This research aims tofind out the relationship between the completeness information with an understanding ofthe approval of medical action in public hospital with class C at Pekanbaru. Dataanalysis was carried out in two stages: univariate and bivariate analysis. Based on theresults of research found that from 194 respondents there were 120 respondents (61.9%)who had a low level of understanding, there were 119 (63.1%) respondents who receivedincomplete information about approval of medical action. Based on the result ofbivariate analysis found that there was a significant relationship between completenessinformation (p value 0,001 or 2,857) with an understanding of the approval of medicalaction. It is recommended that the hospital with class C should conduct re-evaluate toknow whether all medical personnel who perform surgical action or other invasiveaction are in accordance with the procedure applied and given a complete information,so, if things happen that are not desirable after surgery, the patient or family is expectedto receive it because before the surgery is done, the doctor has given an explanation tothe patient.Keyword: Completeness Information, Understanding, Approval of Medical Action

Background: e-Puskesmas as a Health Information System for Public Health Center based on information technology, can present information accurately, quickly and reliably, so that the information presented can be used for decision making. From the initial survey, the implementation of the e-Puskesmas application experienced problems in the field, namely network constraints, applications that were sometimes not connected to Healthcare and Social Security Agency (BPJS), officers had not been able to feel the benefits of implementing the application, there was no decree to determine the person in charge of the e-Puskesmas and reporting was still being made. Objective this study aims to analyze the implementation of the e-Puskesmas application in Public Health Centers throughout Siak Regency. Methods: Qualitative research uses a case study approach with in-depth interviews and observation. The informants in this study were the person in charge of the e-Puskesmas, entry officers and the Head of the Public Health Center, totaling 15 people. Results: The implementation of the e-Puskesmas application in terms of human resources (HR), organization and technology has not run optimally. Conclusion: There are obstacles in its implementation. Therefore, it is necessary to have a joint commitment between the Dinas Kesehatan and the Puskesmas to overcome the existing obstacles

The Indonesia Sehat program is implemented to improve the degree of public health through health efforts and community empowerment supported by financial protection and equitable health services. The implementation of the Healthy Indonesia program is organized through a family approach. The purpose of this study was to analyze community satisfaction in implementing the elements of a healthy Indonesia program in the Pekanbaru City Health Center Work Area. This research is descriptive correlation with cross sectional analytic design. The research sample of 346 respondents with a purposive sampling method. The results of the univariate analysis showed the value of the community satisfaction index in the implementation of the Healthy Indonesia Program in the Work Area of the Pekanbaru City Health Office, which was 3.014 with a conversion value of 75.36. This means that the quality of service provided to patients falls into category B (good). Statistically, there is a relationship between PISPK service elements and community satisfaction in the working area of Peknbaru City Health Center in 2019. It is recommended that institutions improve the quality of human resources through training programs and seminars in order to increase the competency and capacity of officers.

The implementation of the Non-Communicable Disease Control Program carried out in Posbindu PTM is a concrete manifestation of community participation in implementing early detection, monitoring and early follow-up of Non-Communicable Disease risk factors independently and continuously. Rokan IV Koto I Community Health Center has implemented PTM control in an integrated manner but the scope of Non-Communicable Disease prevention and eradication efforts is still low at 53,9%. The purpose of the study was to determine the implementation of the control program for non-communicable diseases in Posbindu PTM Rokan IV Koto I Community Health Center seen from input indicators and processes of the Non-Communicable Disease Control Program. This type of research is qualitative. 8 informants consisted of midwives midwives in charge of posbindu, mobilizing midwives, monitoring midwives, counselor doctors, recording cadres, Head of Sub-Division of Administration, community leaders, BPJS. The tringulation used is source tringulation, methods and data. The results of the study show that inputs (human resources, incentives, facilities and facilities) have been done well even though the use of Posbindu PTM facilities and equipment is not yet available in every village. The process (identification of potential PTM problems, implementation of activities and program innovations, recording and reporting, early risk follow-up and referral of PTM) has gone well. BASMI PELAKOR 1 innovation was able to increase the coverage of community visits to Posbindu PTM to 57,4% which previously had been in 2018 at 53,92%. Suggestion for Community Health Center to facilitate the formation of partnerships in each village through CSR (Corporate Social Responsibility) to buy their own tools, deliberation with the community to determine the Posbindu PTM schedule that is mutually agreed upon, involve community participation in implementing BASMI PELAKOR. Ergonomi adalah ilmu yang mempunyai peranan penting dalam meningkatkan faktor keselamatan kerja dan kesehatan kerja. Berdasarkan hasil penelitian di RSUD Siak terdapat keluhan dari petugas ruangan filling di RSUD Siak yaitu sulitnya petugas untuk melakukan aktifitas dan lamanya petugas dalam melakukan aktifitas, akses petugas sempit, tidak adanya tangga, penyusunan dirak terbuka tidak rapi. Adapun tujuan dari penilitian ini adalah untuk mengetahui letak ruang filling,fasilitas fisik, jenis dan ukuran rak, luas ruangan filling dan jarak antar rak.Metode penelitian ini adalah bersifat deskriptif dengan pendekatan kualitatif yaitu mendapatkan gambaran faktor-faktor apa saja yang mempengaruhi ergonomi ruangan filling terhadap akses petugas rekam medis di RSUD Siak tahun 2018.Teknik analisa data yang digunakan dalam penelitian ini adalah teknik analisa deskriptif dengan metode kualitatif. Berdasarkan hasil penelitian tentang faktor ergonomi ruangan filling terhadap akses petugas adalah adalah jenis dan ukuran rak terbuka, jarak antar rak terbuka, letak dan luas ruangan filling masih belum ergonomi dan kelengkapan fasilitas fisik ruangan filling masih belum lengkap. Kesimpulan dari penelitian ini adalah terdapatnya faktor-faktor yang mempengaruhi ergonomi di RSUD Siak terhadap akses petugas rekam medis adalah letak dan luas ruangan yang sempit, fasilitas fisik yang belum lengkap, menggunakan rak roll o’pack agar jenis, ukuran dan jarak antar rak sesuai dengan ergonomi oleh sebab itu untuk mendukung kelancaran dalam sistem kerja yang ergonomi di ruang filling berkas rekam medis, hendaknya pihak manajemen rumah sakit melakukan pengembangan terhadap letak dan luas ruangan khususnya dibagian filling dan memperhatikan kelengkapan fasilitas fisik ruangan filling.
